This episode is actually the second version of a rant I recorded based on a Facebook post I had a reaction to. I discovered I hadn't had my mic turned on after recording the first version of the rant and despite feeling slightly better after voicing my feelings to all of you, I still felt compelled to share my thoughts. Which brings us to this version. The Facebook post in question was from a fellow physician who wrote that only 40% of Americans are considered obese. Her caption stating she learned the statistic during continuing medical education combined with my poor decision to read the comments got me very upset. It just reminded me again of how pervasive weight discrimination and fat shaming is in our culture, and also within medical culture. So I'm sharing my thoughts about it here.The obesity referred to in the Facebook post was almost certainly defined using BMI because that's how the medical world defines it. I've addressed all the problems with the BMI previously and even in June of this year the American Medical Association released a statement urging physicians to reconsider how they use BMI because of its history and racial implications involved with its use. So many things about the reference to obesity in the post got me riled up, and understanding the misuse of BMI is just part of it. It sparked a complicated reaction in me because I am a board-certified obesity medicine physician and though I don't actively practice due to not believing in the interventions we're taught to prescribe, until recently I was coaching weight loss. The unease I felt in responding to the post (I didn't), and my offense at some of the comments inferring that people categorized as obese are disgusting, just combined into upset that led to this episode. So please join me as I break down again what is wrong with BMI, with the arbitrary categorization of obesity, with assuming weight indicates health, with not allowing for health at any size, and in examining my own personal feelings about my place in it all. I end with a promise to you, and myself, to strive for more courage in addressing these errant beliefs amongst my colleagues. Our conversation today focuses on the impacts of broken systems to the very real people working to offer services and make a difference to our communities…people exactly like you. We also work to talk about our perspectives around what needs to change and where we can collectively work to transform the system from the inside out. T.C. Some of my favourite discussion points that emerged during this conversation (where I got RILED UP!) and connected to the practical ways we can work to transform broken systems from the inside out included:1. Cultural Level Change:a. Reducing stigma around workplace mental health and wellness (speaking, clarifying, normalizing and supporting that no one comes out unscathed).b. Increasing efforts toward prevention and early intervention including training staff in how to assess their status and know the next steps (or who to talk to in an effort to find out the next steps).c. Normalizing support seeking, and clarifying ways to seek support and processes to access the appropriate support readily.2. Management/Upper Level Change: a. Changing the tendency toward reactive band-aid solutions, working instead to identify preventative strategies to reduce load and support the greatest investment which is into the wellness of the PEOPLE doing the WORK.b. Engaging collaboratively (ie. LISTENING MORE THAN TALKING) with staff to understand the pressure points and actively working together to find creative solutions that actually work to solve the problems rather than juggling them.3. Public Awareness Level Change:a. Supporting information for the public around reasonable expectations and the challenges facing the parts of the system they may interact with the most.b. Supporting information for the public around ways to support the system on a broader level as well as the front line staff most directly impacted.4. Structural Level Change:a. Making support accessible (with fewer hoops and WAY more clarity around how to navigate the process). This is both a workplace-level challenge as well as a community-access-to-services challenge.b. More effectively identifying and supporting the levels of the system that add pressure (I loved TC's comment of the new ER being like a bigger funnel on the same sized hose – if we don't support the capacity and efficiency of community health as well as in-patient care, making changes to the ER's capacity does little to reduce wait times, etc. This problem exists in so many ways that are not specific to ER's and healthcare!).c. Taking a stance of prevention and early-intervention in all levels of problem solving rather than being in a constant state of reactively putting out fires. Rudeness is contagious, in a similar way to a virus. When experiencing a rude encounter, our brain perceives it like a threat. And once we've tuned in to this low-level threat, we're more likely to notice it around us, and are therefore more likely to display rude behavior ourselves. Today we're talking about rudeness and its impact on our wellness. We'll walk you through what rude behavior actually is, what our evolutionary response to it is, and how we can take steps to mitigate its negative effects. By the end of today's episode you'll be armed with 7 tools to be less impacted by micro-aggressions when you encounter them, break the rudeness cycle, and start spreading kindness in your world. Rudeness is a low-intensity display of disrespect or incivility. It's often ambiguous & malice isn't necessarily intended. Our brains interpret rudeness as a form of micro-aggression and react accordingly. When someone acts with aggression it's typically a response behavior, but rudeness sometimes comes out of nowhere, so it's disconnected from our usual explanation of where negative behaviors come from. Rudeness is bad for our health & well-being. ** Bonus tool: Smile. A smile can go a long way to counteracting rudeness. Remind yourself that even the kindest people have hard days, instead of internalizing it or taking it personally. To help dismiss random rude behavior you encounter or witness, assume the person is particularly rushed, stressed, tired, hungry, or whatever is going on for that person that has nothing to do with you.
