Entering the world of medicine, where the goal is to keep people healthy, we discover different areas. One notable field is preventive medicine, and within it, we find something called lifestyle medicine. But what sets lifestyle medicine apart? What makes it special in promoting health? Let's break down the uniqueness of lifestyle medicine and understand how it stands out among various medical approaches. In today's episode, I sit down with Dr. Beth Frates, the President of the American College of Lifestyle Medicine. Beth Frates, MD is a trained physiatrist and a health and wellness coach, with expertise in Lifestyle Medicine. She is an award-winning teacher at Harvard Medical School, where she is an assistant clinical professor. Dr. Frates is one of the first Fellows of the American College of Lifestyle Medicine and a pioneer in lifestyle medicine. She developed and taught a college Lifestyle Medicine curriculum at the Harvard Extension School in 2014 and it is still one of the most well-received courses offered at the school. She was voted president of the American College of Lifestyle Medicine in August 2020. Dr. Frates serves as President from November 2022- November 2024. She authored a Lifestyle Medicine syllabus, which can be downloaded through the ACLM website, to serve as a template for other instructors and professors. More recently, her book titled “The Lifestyle Medicine Handbook: An Introduction to the Power of Healthy Habits” was included in a list of the 100 Best Medicine Books of All Time by BookAuthority. More than 5% of the US population, 18.1 million people, are cancer survivors. Over the next decade, the number of people who have lived 5 or more years after their cancer diagnosis is projected to increase by approximately 30%. How do we define cancer survivorship? What does cancer rehabilitation look like? And what are lifestyle prevention strategies? Dr. Amy Comander is a breast oncologist at the Massachusetts General Hospital Cancer Center. She is Director of Breast Oncology and Cancer Survivorship at the Mass General Cancer Center in Waltham and at Newton Wellesley Hospital, and Medical Director of the Mass General Cancer Center in Waltham. Dr. Comander is Director of Lifestyle Medicine at the Mass General Cancer Center, which is the first cancer center to have a dedicated lifestyle medicine program. As an Instructor in Medicine at Harvard Medical School, she enjoys teaching medical students and residents about oncology, as well as lifestyle medicine.Given her strong interest in cancer survivorship and lifestyle medicine, Dr. Comander is dedicated to improving the quality of life, well-being, and outcome of individuals with cancer through important lifestyle interventions, including exercise, diet, and mind/body interventions. She promotes healthy lifestyles for both her active treatment patients as well as those in the survivorship phase of care. She practices what she preaches, having run marathons, including ten consecutive Boston Marathons so far, with the goal to improve the lives of those with a diagnosis of cancer.In collaboration with Dr. Beth Frates, she has launched “PAVING the Path to Wellness,” a 12-week lifestyle medicine-based survivorship program for women with breast cancer. Along with Dr. Frates and Dr. Michelle Tollefson, she has published the “PAVING the Path to Wellness” workbook. Dr. Comander is currently proud to serve as the first oncologist on the Board of Directors of the American College of Lifestyle Medicine. She is also on the Board of the Ellie Fund, a non-profit that provides services and support to women diagnosed with breast cancer in Massachusetts. She is a medical advisor to the non-profit organization, SurvivingBreastCancer.org. She has served as a medical advisor to Oneinforty, a non-profit organization dedicated to educating people of Ashkenazi Jewish heritage about the one-in-forty chance of having inherited a BRCA mutation. It starts with recognizing that wellness is a journey in self-leadership. Asking oneself the question of how we are going to live our life. Understanding and appreciating our own strengths and weaknesses. Once one understands this, then comes the question, do we understand how we live a healthy life. It starts with having an open mindset and being a life-long learner. This is also an important part of being a leader of people. As a leader it's important to accept vulnerability. It's okay to not have all the answers and none of us do. The best CEOs can talk to anybody. To be human is the most important aspect of great leadership. Showing the sincerity through what one is doing. Expressing oneself authentically and vulnerability with it. When one shows how one is fitting in their lifestyle wellness program it goes a long way in encouraging others. That authenticity creates transparency and makes the leader seem human. When one leads through authenticity one creates an environment of trust. That engages and gets interaction and participation and helps in seeking solutions. Trust starts with trusting and believing In yourself. It then follows on to trusting and believing others. To be a great leader it's important to have belief in others. To be able to see the innate good that is there in each one of us.

Good leadership is about self-leadership. One of the elements of self-leadership is maintaining a proper lifestyle and maintain one's health.  How did you get started into lifestyle medicine? Back in the day when I was mentally preparing to be the 3rd generation in the family business, I learnt that my father was harbouring a few diseases inside him! These are the silent diseases like high blood pressure. You don't realise they are there. This affected me and resulted in me making a complete change where my career was concerned. It brought home the purpose of wanting to lead people to thrive and have healthy lifestyles. In leadership having good health comes from understanding and living a healthy lifestyle. Doing that involves taking care of the body, mind and heart. Its what brings about effectiveness in leadership. It starts with understanding the value of taking time-out. It allows one to check in with self and actually do checks that help rejuvenate oneself.  A key check is about moving our bodies. Research has shown us this. For a leader you need creativity. One of the concepts is called divergent thinking. Researchers at Stanford studied this coupled with 20 minutes of walking and 20 minutes of sitting. The data shows that after walking divergent thinking goes up 60%. Other research show that we can get into the alpha or theta level brain activity after exercising. To get strategic and creative thinking going, as a leader, it's key to be exercising. The other key lifestyle input is to give yourself enough sleep. Sleep deprivation impact’s reaction time. So, appreciating sleep and giving oneself enough sleep impacts the body and mind positively. Sleep is foundational in one's health, happiness, creativity, and productivity. Leaders are passion and purpose driven. In order to keep those going it’s important for them to take steps to honour their own body, mind, and soul. Doing this everyday it becomes a question of what’s priority and how much time is is will I spend on that priority. So, it drives you to make time for it. Instead of reacting to everything that comes and takes up one's time. The last bit is creating social connections such that, as a leader, you mentor and grow people who carry your ideas forward. This takes energy and how one maintains that energy is what matters. Jonathan Bonnet is a board-certified Sports, Obesity, and Lifestyle Medicine physician. He is a team physician at Emory University and is passionate about optimizing health, improving performance, and promoting longevity. Jonathan has a background in exercise physiology and is also a certified personal trainer. He completed his undergraduate and medical school studies at Ohio State University before getting his master's degree from Harvard University. He completed finishing his Family Medicine residency at Duke University, and his Sports Medicine Fellowship at the University of Florida. Jonathan served on the board of the American College of Lifestyle Medicine from 2014-2016 and is currently on the editorial board of the American Journal of Lifestyle Medicine. He has published research in sleep medicine, nutrition, sports and exercise, obesity, and behavior change. Jonathan is a co-creater of "The Lifestyle Medicine Handbook: An Introduction to the Power of Healthy Habits" and worked closely with Dr. Beth Frates who you may remember from Episode 26. ICP 47: Dr. Beth Frates, MD- The profound effect of 'Patient Zero' -Stress prevention, heart health and behavior change

Dr. Beth Frates is a physician and Director of Wellness Programming at the Spaulding Stroke Research and Recovery Institute at Spaulding Rehabilitation Hospital. Dr. Frates is also a Clinical Assistant Professor at Harvard Medical School. As I was listening to our conversation in post-production, Dr. Frates reminded me of past guest and plant-based rock star Jane Esselstyn with some of her language and mannerisms and inflection and occasional bursts of vocal volume and passion. Dr. Frates was influenced to practice lifestyle medicine from a very important person in her life, the person or patient she calls “Patient Zero”. This person, a major life-changing event and the effect he had on her caused a life-altering shift in her mindset, her thinking and ultimately her career path. In our conversation, we speak about Dr. Frates' interests in stress, the nervous system and its effect on heart health and vascular health. Dr. Frates has also devoted a considerable amount of time educating herself on behavior change and a “Coach Approach”. The emphasis on talking to your patients, not talking at them, being a coach instead of their finger-wagging doctor and enabling them to find the answers to their own concerns, to own their problems themselves by leading a discussion, asking questions, being a good listener and not judging. I sense that this content is very applicable to doctors and medical professionals who listen to this podcast. In other news, I have a big announcement of the completion of a project that I've been working on for many, many months. Not a book or a study, but a course, a webinar, to further your knowledge about plant-=based nutrition and the many reasons why this is the future. It is, to my knowledge, the first ever comprehensive webinar on the implementation of the plant-based diet for endurance athletes. The line baloney that we've been fed that you must eat meat and dairy to be healthy humans and strong athletes is simply not true, and I have compiled the evidence and explanations to take you on a step-by-step journey to the end goal of immediately implementing this so that you can start to reap the rewards of this lifestyle for both your short-term athletic success and long-term health.
