We interview Dr. Stanton Peele and Zach Rhoads about addiction, Suboxone, pain patients, Harm Reduction, and more! You won't want to miss this episode. He developed the Life Process Program for addiction. He has authored many books since the 1975 publication of Love and Addiction (co-authored by Archie Brodsky); his latest is A Scientific Life on the Edge: My Lonely Quest to Change How We See Addiction. Zach Rhoads - Zach is a behavior specialist in Vermont schools and his school's PBiS (Positive Behavior Interventions and Supports) coordinator. Zach is a counselor and family systems developer for the online Life Process Program - an addiction coaching program developed by psychologist Dr. Stanton Peele. He also writes articles about health and drug policy for Filter Magazine and is coauthor (with Dr. Stanton Peele) of the book, Outgrowing Addiction. (He is currently working on a forthcoming book about student-led education in the United States). Zach believes that community collaboration and cohesion is a bulwark against many forms of mental illness and addiction, and helps people find these resources in ways that makes sense in their own lives. Biological psychiatry has IMPLODED, and everyone knows it, including the World Health Organization (WHO) and the long-time (2002-2015) director of the National Institute of Mental Health, Thomas Insel, biological psychiatry's biggest booster. Yet, instead of the relief from mental illness and addiction Insel and the entire field have promised from the 1970s on, we are inundated with mental disorders and drug deaths. NOW WHAT? The WHO, Insel, and the ENTIRE FIELD are at a loss. With no there there, what is to be done? Rushing to fill this breech, WHO is proposing a new Best Practices in Mental Health Guide on June 17, 2022. But given its progenitors' failures for more than a half century, we may be dubious. Is AA helpful? Some people claim that AA offers specific benefits. We should say that we endorse anybody's right to pursue any form of support or treatment they want, or find helpful, from AA to religion. But our training and experience cause us to doubt AA's supposed benefits. One such claim is that making amends to people alcoholics have hurt is a way of reconstructing damaged relationships. But we find that a very indirect way of going about the process of examining and improving relationships, which is a major focus of the skills taught in the Life Process Program. A second claim for AA is that the “fellowship” provides group support. It does, but at what cost? We ask whether the goal of a support group is to cement people into that one group (AA), or to broaden their outreach to people and the world. The latter is, of course, the LPP approach. We further describe (Zach references his personal experience) how AA undercuts people's actual feelings and beliefs in order to purvey its own philosophy and “religion” (for example, powerlessness and turning oneself over to God). For Zach and others, AA, rather than offering such a benefit, is a “mindfuck” that undercuts people's ability to navigate their own feelings. Her name is Lenore Skenazy. Lenore famously (or infamously, depending on who you ask) allowed her child to ride a New York City subway home on his own. In her mind (and in ours), this was sensible, and offered her child to gain some independence in the real-world. Perversely, many parents around the country were horrified that Lenore would "subject her child to the dangers of New York City". Lenore began to realize that adults in the U.S. tend to overprotect their children-- a trend which ultimately stymies their natural course of development-- of exploration. This forms the basis for her book, "Free Range Kids" and her organization (with Peter Gray and Jonathan Haidt), "Let Grow". I've had on plenty of people (including myself) who have found recovery through the 12 step programs. I've also had several on who have found it through other means and I am always excited when I get to hear someone else's perspective on addiction that I've not yet run across...my guest to day is no exception to that. Dr. Carl Hart explains that the vast majority of people who use drugs do so with no problem. In fact, drugs -- yes, even so-called "hard drugs"-- offer more positive than negative experiences. Dr. Hart is a professor (and former chair) of psychology and psychiatry at Columbia University. Today, he opens up to Zach Rhoads about his new book, "Drug Use For Grown-Ups", in which he's comes out of the closet about his own drug use and encourages the rest of us to do the same. And why is he so boosterish about speaking openly about (even illicit) drug use? Well, as he says in this interview: "As long as you put forth an effort, you might make a mistake. That's okay, we're all human. The real crime is when you make a mistake, you don't acknowledge it, then continue to speak dishonestly about it. Questions of family often appear in relation to addiction and drug issues. As with all questions in these areas, discussion is misled by disease beliefs, which leave people muddled and mystified about how to proceed constructively. Nonetheless, basic human values often assert themselves, allowing people— especially young people— to free themselves and to display bravery. Examples in the news are the women in Russian protest group Pussy Riot, Masha Gessen (Russian-American radical journalist) whose son is lost in vicodin, the four granddaughters of Joe Biden, who allowed the president to free america from Trump, and the often absent addicted father Hunter, who almost cost his father the presidency. Not so long ago, I published a blog comparing and contrasting SMART Recovery with the Life Process Program. Shortly after doing so, The president of SMART Recovery wrote to me and offered reflections on the piece. That made me a little nervous.... but he liked the blog. (WHEW!) But he did have a bone to pick with me.... I wrote that SMART Recovery is an abstinence-based program and, evidently, they are no longer focused on abstinence from drugs, alcohol or any other involvement. In today’s episode of Sundays With Stories, Stanton and Zach discuss current events. This week a New York Times article bemoaned the failure of addiction specialists to adopt a proven tool for treating drug addiction, “contingency management.” That is, paying meth, cocaine et al. users for abstinence. That’s right: experienced, one might say addicted, “speed” (“crank”) users will desist drug use if you give them money — and not colossal sums at that. This has been Carl Hart’s research with meth, and the University of Vermont group’s under Warren Bickle with cocaine. The same benefits of providing alternative rewards have been shown in Bruce Alexander’s Rat Park and the Baltimore City Hospital group’s research with street drinkers. What does this tell us? That drug and alcohol addiction is not a runaway train resistant to environmental factors and rewards. Which is useful to know. However, Zach and Stanton question who is creating these rewards. There is a bit of science-gone-amok to the image of men in white coats divvying out money for drug and alcohol users, even those with the worst usage patterns, to be “good.” And what happens when the rewards, the gravy train, stops? You tell us. But Harvard’s Global Anti-Tobacco Program offered a hint. While investigating the effectiveness of nicotine replacement therapy (NRT) for those who quit smoking, the program’s chief investigators found that, in the long run, those who quit on their own did as well as, or better than, those who rely on NRT. In fact, the most addicted smokers were the worst at keeping off the drug using NRT. This finding held true even though NRT was “proven effective in the laboratory.” Zach and Stanton then speak of people managing their own contingencies. That is, their finding real-world reasons to quit or cut back their addictions. Most important of all (with a nod to Steve Slate et al.’s Freedom Model”) is people’s belief that they can control their own drug use and lives. This thinking belies NIDA Director Nora Volkow’s 2007 unwise piece “Addiction Is a Disease of Free Will” — which, per Stanton’s urging, Volkow seems to be abandoning. What is she replacing this ill-conceived notion with? What Stanton and Zach practice in their Life Process Program: creating lives worth living and worlds people want to occupy. This is a tough task. But, once again, including a review of America’s ill-fated (drug deaths are ramping out of control) enthusiasm for medicine-assisted treatment for opioid addiction, there is no long-term alternative. Finally, Zach reviews SMART Recovery’s belated shift to “abstinence from addiction” in place of its former “desist all use” basis — as the user chooses. They agree in their admiration for Tom Horvath/SMART Recovery’s remarkable success at creating an alternative to AA. At the same time, they embrace SMART’s long-awaited and necessary shift to Harm Reduction. Stanton and Zach end their podcast by wondering how decades of addiction research and both positive and negative experiences with drugs have left even our “advanced guard” (i.e., policy reformists like the Ethan Nadelmann-founded DPA) so shorn of an appreciation for the interchange between people’s lived experience (their actual lives) and addiction. Come join Stanton and Zach as they reveal a little secret. That is, the best science, policy, and treatment involve common sense. Downey, who was really at the bottom of the well after years of turmoil, drug use, and coerced rehab, in his late thirties had a Jewish wedding which did the trick. That is: they tell the stories of celebrities who outgrow their addictions, and whose stories actually oppose the standard addiction and recovery bromides that we've all been told. (Do you know that progressive stalwart Kurt Andersen dutifully explained that George W. Bush invaded Iraq because he quit drinking without AA, and was thus permanently trapped in denial?) Among the many examples of this self-censorship are the cultural icon Drew Barrymore, who People labelled, age 13, America's youngest addict. Barrymore wrote Little Girl Lost, published at age 15, after she had entered rehab and subsequently attempted suicide. But Drew grew up by the time she reached her twenties, then went on to found a winery and become a caring mother and Hollywood power player. Apologizing for “not being in recovery” (she enjoys wine), Drew can only hint at the developmental course that has freed her and the vast majority of those with addictive problems. Welcome to episode 29! Today's guest is LPP's own Dee Cloward. Dee has been a Life Process Program coach for four years now. Prior to that, she was with SMART Recovery for 15 years in many capacities, primarily as a volunteer, and served there as Program Manager for Court Outreach and Special Events Coordinator, and Director of Training to mention just a few. She was highly beloved in the online community and beyond and continues to facilitate SMART meetings locally in the county jail and online. In addition, she has her own small local coaching business in Cincinnati. Dee had a highly turbulent although largely functional life for many years before ultimately finding recovery and joy in life with SMART Recovery over 15 years ago. She is an example of how well online ONLY recovery can work. She beat addiction without in-person meetings, AA (or other 12-step groups), therapy, inpatient or outpatient treatment. She has remained abstinent from alcohol (as well as drugs to which she's had unhealthy attachments in the past) for 11 years. That's something we discuss today-- why it is important for her, personally, to remain abstinent from those substances and involvements, and why she doesn't always necessarily recommend this approach for all. Rewrite your life story. The Life Process Program takes clients through a staged review of their life stories, focusing first on mistakes, continuing to how they might have acted differently to avoided these pitfalls, to reconceiving their life stories to focus on positives, and finally to projecting their life stories forward into positive, life-fulfilling narratives. In today's episode we incorporate LPP exercises and examples of Zach’s work with children, including sample maps he develops for each child, to show how to put these steps into action in your own life and the lives of your children and other loved ones.
