In this episode, Joe interviews Ethan Nadelmann: author, speaker, Founder and former Executive Director of Drug Policy Alliance, host of the PSYCHOACTIVE podcast, and one of the leading voices in drug policy reform and harm reduction. Nadelmann shares his journey from Princeton University to founding Drug Policy Alliance, to working with George Soros, encouraging Gary Johnson to push cannabis legalization, and interacting with prominent figures like Milton Friedman and Grover Norquist. He explores the motivations behind the drug war, the massive growth of incarcerations it led to, why the US spread its war on drugs abroad even when it went against our best interests, and, thankfully, the progress made in fighting the drug war – particularly with cannabis and psychedelics. And he discusses much more: the banning of drug testing kits; the damages of our slow learning curve against the idea of a safe supply; the risks of under-prescribing opioids for people who actually need them; how libertarians, the right, and left are all starting to become against the drug war for the same reasons; why cigarette smokers should all switch to vaping; the concept of needing to pass a test at the pharmacy to prove you understand (and won't abuse) medication; and some strong arguments for decriminalization as an incremental step. And he asks some pretty important questions that we can all simmer on for a bit: how do we find a balance between helping people and not opening the rest of society up to harm? Ethan Nadelmann is a legend of drug policy reform. From his time as a professor at Princeton to his founding and seventeen years of leadership at the Drug Policy Alliance, Ethan has spearheaded the critique against the War on Drugs. In this episode of the Third Wave podcast, Ethan talks with Paul F. Austin about the role psychedelics played in shaping his personal values and drug policy vision, how our culture's perspective on drugs has shifted over the last 30 years, and how psychedelics fit into larger drug policy reform.
Ethan Nadelmann studied at Harvard University and London School of Economics and was a lecturer at Princeton University. While at Princeton, Nadelmann's work focused on drug policy, attracting considerable attention with his articles in Science, Foreign Affairs, National Review and many others. He also formed the Princeton Working Group on the Future of Drug Use and Alternatives to Drug Prohibition. In mid-90s Nadelmann founded the Lindesmith Center, a drug policy institute that six years later became the Drug Policy Alliance, a group for drug policies "grounded in science, compassion, health and human rights.” As the Executive Director of the organisation, Nadelmann advocates for the application of harm reduction principles to minimize the public health effects of the War on Drugs. Ethan Nadelmann Described by Rolling Stone as “the point man” for drug policy reform efforts and “the real drug czar,” Ethan Nadelmann was long widely regarded as the outstanding proponent of drug policy reform both in the United States and abroad. Ethan began his advocacy in the late 1980s while teaching at Princeton; he then founded first The Lindesmith Center and then the Drug Policy Alliance, the world's leading drug policy reform organization, which he directed until 2017. He also co-founded the Open Society Institute's International Harm Reduction Development program. Ethan has authored two books on the internationalization of criminal law enforcement (Cops Across Borders and, with Peter Andreas, Policing The Globe), and spoken publicly in roughly forty states and forty countries. His TED Talk on ending the drug war has over two million views. Ethan and his colleagues were at the forefront of dozens of successful campaigns to legalize marijuana, reduce the incarceration of drug law offenders, treat drug use and addiction as health, not criminal, issues, and otherwise promote alternatives to punitive prohibitionist policies. He recently started a podcast about all things drugs called PSYCHOACTIVE. And he has become increasingly engaged in the debate over tobacco harm reduction. Host Steve Heilig Steve is a longtime senior research associate with Commonweal, a co-founding director of the Commonweal Collaborative on Health and the Environment, a host of dialogues for the New School, and in other programs originating at or founded at Commonweal. Trained at five University of California campuses in public health, medical ethics, addiction medicine, economics, environmental sciences, and other disciplines, his other work includes positions at the San Francisco Medical Society, California Pacific Medical Center, and as co-editor of the Cambridge Quarterly of Healthcare Ethics. He has served on many nonprofit boards and appointed commissions, and is a trained hospice worker. This week a New York Times article bemoaned the failure of addiction specialists to adopt a proven tool for treating drug addiction, “contingency management.” That is, paying meth, cocaine et al. users for abstinence. That’s right: experienced, one might say addicted, “speed” (“crank”) users will desist drug use if you give them money — and not colossal sums at that. This has been Carl Hart’s research with meth, and the University of Vermont group’s under Warren Bickle with cocaine. The same benefits of providing alternative rewards have been shown in Bruce Alexander’s Rat Park and the Baltimore City Hospital group’s research with street drinkers. What does this tell us? That drug and alcohol addiction is not a runaway train resistant to environmental factors and rewards. Which is useful to know. However, Zach and Stanton question who is creating these rewards. There is a bit of science-gone-amok to the image of men in white coats divvying out money for drug and alcohol users, even those with the worst usage patterns, to be “good.” And what happens when the rewards, the gravy train, stops? You tell us. But Harvard’s Global Anti-Tobacco Program offered a hint. While investigating the effectiveness of nicotine replacement therapy (NRT) for those who quit smoking, the program’s chief investigators found that, in the long run, those who quit on their own did as well as, or better than, those who rely on NRT. In fact, the most addicted smokers were the worst at keeping off the drug using NRT. This finding held true even though NRT was “proven effective in the laboratory.” Zach and Stanton then speak of people managing their own contingencies. That is, their finding real-world reasons to quit or cut back their addictions. Most important of all (with a nod to Steve Slate et al.’s Freedom Model”) is people’s belief that they can control their own drug use and lives. This thinking belies NIDA Director Nora Volkow’s 2007 unwise piece “Addiction Is a Disease of Free Will” — which, per Stanton’s urging, Volkow seems to be abandoning. What is she replacing this ill-conceived notion with? What Stanton and Zach practice in their Life Process Program: creating lives worth living and worlds people want to occupy. This is a tough task. But, once again, including a review of America’s ill-fated (drug deaths are ramping out of control) enthusiasm for medicine-assisted treatment for opioid addiction, there is no long-term alternative. Finally, Zach reviews SMART Recovery’s belated shift to “abstinence from addiction” in place of its former “desist all use” basis — as the user chooses. They agree in their admiration for Tom Horvath/SMART Recovery’s remarkable success at creating an alternative to AA. At the same time, they embrace SMART’s long-awaited and necessary shift to Harm Reduction. Stanton and Zach end their podcast by wondering how decades of addiction research and both positive and negative experiences with drugs have left even our “advanced guard” (i.e., policy reformists like the Ethan Nadelmann-founded DPA) so shorn of an appreciation for the interchange between people’s lived experience (their actual lives) and addiction. Ethan has played a leading role in harm reduction and other drug policy reform efforts in the United States and globally since the late 1980s. He was born in New York City, received his BA, JD, and PhD from Harvard, and taught politics and public affairs at Princeton University (1987-1994). He then founded first The Lindesmith Center (1994-2000, a drug policy institute created with the philanthropic support of George Soros) and then the Drug Policy Alliance (DPA), which he directed from 2000 until 2017. The next likely victim of drug war hysteria and prohibition: vaping. Ethan Nadelmann, founder of the Drug Policy Alliance, comes out of retirement to discuss.Ethan Nadelmann, founder of the Drug Policy Alliance and one of the main players in the cannabis legalization struggle, has been quiet since retiring two years ago—attending conferences, pondering a book, advising those in need of his wisdom and political acuity. While DPA continues to wage the good fight, Ethan’s public presence has been sorely missed. He’s the smartest and fiercest advocate of sensible drug policy – in particular, harm reduction – and he has the unique ability to wage a fight with a smile on his face. His guiding principle has always been “socially libertarian:” adults should be allowed to put any substance into their body as long as it doesn’t harm anyone else. I ran into Ethan at the recent Horizons Psychedelic Conference in NYC, and we were both itching to talk about the vilification of vaping – by politicians, state governments, the mainstream media (including the New York Times) and even public health advocates. The partially puritanical Michael Bloomberg, long an anti-tobacco crusader (good), recently donated $160 million to an anti-vaping crusade (mad). While we don’t completely know the long term effect of vaping nicotine or cannabis oils, the evidence indicates that vaping legal products is far safer, far more sane way of inhaling nicotine (or cannabis). Evidence also shows it beats patches and any other form of therapy aimed at helping people quit smoking cigarettes. There’s another important caveat when examining the emerging Vape War -- it bears an uncomfortable resemblance to the War on Cannabis. BothWere vilified by politicians, largely to “protect the children”Had audacious claims about them with no scientific backingWere funded by millionaires, some of which were once considered liberal leaningFavor one class of people (wealthy) over another (poorer)Were denigrated and banned in lieu of enacting more sensible and far less costly harm reduction and health policies.Sound familiar? Aired 08/15/10 Prohibition has failed -- again. Instead of treating the demand for illegal drugs as a market, and addicts as patients, policymakers the world over have boosted the profits of drug lords and fostered narcostates that would frighten Al Capone. Today, there are more drugs on our streets at cheaper prices than ever before. There are more than 1.2 million people behind bars in the U.S., a large percentage of them for nonviolent drug usage. Under our failed drug policy, it is easier for young people to obtain illegal drugs than a six-pack of beer. Why? Because the sellers of illegal drugs don't ask kids for IDs. As soon as we outlaw a substance, we abandon our ability to regulate and control the marketing of that substance. There is smarter approach usually called harm reduction. Reducing drug use is not nearly as important as reducing the death, disease, crime, and suffering associated with both drug misuse and failed policies of prohibition. But there are signs of change in the wind. The US Congress recently reversed years of inaction to make sentencing for crack and powder cocaine more equal and proposition 19 on the ballot in CA in November would legalize marijuana. "Harm reduction," a smarter drug control regime that values reality over rhetoric, is rising to replace the "war" on drugs. Reducing drug use is not nearly as important as reducing the death, disease, crime, and suffering associated with both drug misuse and failed prohibitionist policies. With respect to legal drugs, such as alcohol and cigarettes, harm reduction means promoting responsible drinking and designated drivers, or persuading people to switch to nicotine patches, chewing gums, and smokeless tobacco. With respect to illegal drugs, it means reducing the transmission of infectious disease through syringe-exchange programs, reducing overdose fatalities by making antidotes readily available, and allowing people addicted to heroin and other illegal opiates to obtain methadone.
