Rates of use of e-cigarettes containing nicotine by youth in Canada continue to be among the highest in the world. They speak with Dr. Madelynn Hannah, a clinical pharmacist, and Dr. Trisha Tulloch, a pediatrician specializing in adolescent and addiction medicine at CAMH and SickKids Hospital in Toronto. Both co-authors of the CMAJ article "E-cigarette use in adolescents.” Dr. Madelynn Hannah provides critical insights into the stark differences between nicotine exposure from vaping versus traditional cigarettes, highlighting the unexpectedly high nicotine concentrations found in many vaping products. She also shares her clinical experiences managing adolescents in nicotine withdrawal and the practical challenges of using nicotine replacement therapy (NRT). Dr. Tulloch discusses the broader physiological impacts of nicotine on adolescents, including cardiovascular issues and the potential for seizures. She emphasizes the need for behavioral interventions and technological tools, such as mobile apps, to aid in vaping cessation. When Jennifer McCormack was growing up, she had always been intuitive & she also wanted to be a fashion designer, she had a desire to create beautiful clothes. Lead Story: Safety of e-cigarettes and nicotine patches as stop-smoking aids in pregnancy: Secondary analysis of the Pregnancy Trial of E-cigarettes and Patches (PREP) randomized controlled trial Addiction It is not clear to what extent nicotine is responsible for adverse outcomes of smoking during pregnancy. This study examined outcomes of e-cigarette (EC) and nicotine replacement therapy (NRT) use in pregnancy. Pregnant smokers were randomly assigned to EC or NRT. ECs were more than twice as likely to be used regularly compared to NRT. Those abstaining from cigarettes had higher birth weights (3.3 Kg) than smokers (3.1 Kg), and the use of EC or NRT did not affect birth weight in abstainers. In the entire group (abstainers and smokers), use of EC and/or NRT was not associated with an increase in adverse events. Those using EC had less cough and phlegm than those using NRT. Other studies have confirmed the same, possibly resulting from antibacterial effects of additives to the EC liquid. The authors found no risks to pregnancy from EC or NRT use. He has mixed hyperlipidemia and a 10 pack-year history of (current) tobacco smoking. Which of the following points related to tobacco use is LEAST appropriate for today's visit? A Providing assessment and encouragement for smoking cessation, even if for only a 30-second “very brief advice” intervention. B Reviewing and offering pharmacotherapy support options for smoking cessation if Mr. V expresses readiness to quit today. C Recommending a switch from traditional cigarettes to e-cigarettes as a first step towards cessation, as e-cigarettes are safer for use. D Discussing that smoking cessation is strongly recommended for all patients, regardless of potential weight gain. Answer #33 Explanation Answer C is LEAST appropriate and therefore is the correct answer. Answer C is not appropriate. Although e-cigarettes may be more effective than nicotine replacement therapy (NRT) for smoking cessation, the long-term effects of e-cigarettes on cardiovascular and pulmonary health are unknown. According to the 2019 ACC/AHA prevention guidelines, e-cigarettes may increase the risk of CV and pulmonary diseases; their use has been reportedly associated with arrhythmias and hypertension. Therefore, neither the ESC nor ACC/AHA suggest clinicians recommend e-cigarettes over traditional cigarettes to patients. Answer A: Smoking cessation is one of the most effective CVD risk-lowering preventive measures, with significant reductions in (repeat) myocardial infarctions or death. ESC guidelines emphasize the importance of encouraging smoking cessation even in settings where time is limited. “Very brief advice” on smoking is a proven 30-second clinical intervention, developed in the UK, which identifies smokers, advises them on the best method of quitting, and supports subsequent quit attempts. While ESC does not explicitly suggest a frequency of assessment, the 2019 ACC/AHA guidelines specifically recommend that “all adults should be assessed at every healthcare visit for tobacco use and their tobacco use status recorded as a vital sign to facilitate tobacco cessation.” Answer B: The ESC suggests (class 2) that offering follow-up support, nicotine replacement therapy, varenicline, and bupropion individually or in combination should be considered in smokers. A meta-analysis of RCTs in patients with ASCVD reflects that varenicline (RR 2.6), bupropion (RR 1.4), telephone therapy (RR 1.5), and individual counselling (RR 1.6) all increased quit rates versus placebo; NRT therapies were well-tolerated but had inconclusive effects on quit rates (RR 1.22 with 95% CI 0.72-2.06). The 2019 ACC/AHA recommendation to combine behavioral and pharmacotherapy interventions to maximize quit rates is a class 1 recommendation. Answer D: The ESC gives a class 1 recommendation to recommending smoking cessation regardless of weight grain. Smokers who quit may expect an average weight gain of 5 kg, but the health benefits of tobacco cessation (both CVD and non-CVD related) consistently outweigh risks from weight...

Dr. Chief also leads the NSF Indigenous Food, Energy, and Water Security and Sovereignty Program and is training 38 graduate students. Indige-FEWSS's vision is to develop a diverse workforce with intercultural awareness and expertise in sustainable food, energy, and water systems (FEWS), specifically through off grid technologies to address the lack of safe water, energy, and food security in Indigenous communities. Dr. Amy Sapkota is an MPower Professor of Environmental Health Sciences at the University of Maryland School of Public Health. She is the Interim Director of the Maryland Institute for Applied Environmental Health and the Director of CONSERVE: A Center of Excellence at the Nexus of Sustainable Water Reuse, Food & Health that was launched with funding from the U.S. Department of Agriculture, National Institute of Food and Agriculture in 2016. She is also the Principal Investigator of a doctoral training program, UMD Global STEWARDS (STEM Training at the Nexus of Energy, WAter Reuse and FooD Systems)—funded by the National Science Foundation Research Traineeship (NRT) program—that is preparing a cadre of future leaders focused on innovations at the nexus of food, energy and water systems. Dr. Sapkota's research interests lie in the areas of environmental microbiology, environmental microbial genomics and exposure assessment. Her projects evaluate the complex relationships between environmental microbial exposures and human infectious diseases, with a special focus on assessing the public health impacts associated with water reuse. Dr. Yael Perez is the Development Engineering (DevEng) Program Director at UC Berkeley's Blum Center for Developing Economies, managing the DevEng Masters and the DevEng PhD Designated Emphasis. Yael holds a PhD in Architecture from UC Berkeley with a scholarship on co-design methodologies and technologies to support and empower communities and design practitioners in fostering sustainable development. For over a decade, she has been collaboratively leading CARES—Community Assessment of Renewable Energy and Sustainability—a team of UC Berkeley faculty and students working with Native American Citizens in their pursuit of sustainable development. Recently, this initiative grew into the Native FEWS Alliance, a cross-institutional collaboration working to significantly broaden the participation of Native American students in Food, Energy, and Water Systems (FEWS) education and careers to address critical challenges facing their communities. The latest Cochrane Review finds high certainty evidence that nicotine e-cigarettes are more effective than traditional nicotine-replacement therapy (NRT) in helping people quit smoking. Jamie and Nicola summarise their research led by the University of Oxford, and funded by Cancer Research UK, which has found the strongest evidence yet that e-cigarettes, also known as 'vapes', help people to quit smoking better than traditional nicotine replacement therapies, such as patches and chewing gums. The new evidence published in the Cochrane Library finds high certainty evidence that people are more likely to stop smoking for at least six months using nicotine e-cigarettes, or 'vapes', than using nicotine replacement therapies, such as patches and gums. Evidence also suggested that nicotine e-cigarettes led to higher quit rates than e-cigarettes without nicotine, or no stop smoking intervention, but less data contributed to these analyses. The updated Cochrane review includes 78 studies in over 22,000 participants - an addition of 22 studies since the last update in 2021. Smoking is a significant global health problem. According to the World Health Organisation (WHO), in 2020, 22.3% of the global population used tobacco, despite it killing up to half of its users. Stopping smoking reduces the risk of lung cancer, heart attacks and many other diseases. Though most people who smoke want to quit, many find it difficult to do so permanently. Nicotine patches and gum are safe, effective and widely used methods to help individuals quit. E-cigarettes heat liquids with nicotine and flavourings, allowing users to 'vape' nicotine instead of smoking. Data from the review showed that if six in 100 people quit by using nicotine replacement therapy, eight to twelve would quit by using electronic cigarettes containing nicotine. This means an additional two to six people in 100 could potentially quit smoking with nicotine containing electronic cigarettes. Culture, conservation and the destinies of two men who created a movement, which is transforming landscapes, bringing together old enemies and restore nature. Associate Professor Jamie Hartmann-Boyce and Dr Nicola Lindson discuss the emerging evidence in e-cigarette research and interview Dr Leonie Brose, King's College London. This podcast is a companion to the electronic cigarettes Cochrane living systematic review and shares the evidence from the monthly searches. In the October episode Jamie Hartmann-Boyce interviews Dr Leonie Brose, Reader in Addiction Education and Nicotine Research at King's College London. Leonie Brose talks about the findings of a randomised controlled trial carried out with Dr Markos Klonizakis and the team at Sheffield Hallam University on the medium- and longer-term cardiovascular effects of e-cigarettes in adults making a stop-smoking attempt. The study compared cardiovascular effects in smokers attempting to quit smoking using e-cigarettes with or without nicotine or prescription nicotine replacement therapy (NRT). The study concluded that smokers attempting to quit experienced positive cardiovascular impact after both a 3- and 6-month period. None of the groups (i.e., nicotine-containing and nicotine-free e-cigarettes or NRT) offered superior cardiovascular benefits to the others. https://doi.org/10.1186/s12916-022-02451-9 Dr Brose also discusses the findings of the report by the Office for Health Improvements and Disparities (OHID) on Nicotine vaping in England: 2022 evidence update summary. The report aims to summarise the evidence on vaping products and to inform policies and regulations. It is a large body of work with 16 chapters covering policy and regulation, vaping and youth and presents the evidence on biomarkers of exposure and biomarkers associated with cancer, respiratory, cardiovascular and other health outcomes. The report also has chapters on fires, poisons and explosions linked to ECs, heated tobacco products and perceptions of harm. The headline from the report was that vaping poses only a small fraction of the risk of smoking combustible cigarettes. Jamie and Nicola also bring us up to date with the literature search conducted in October 2022. The October search identified 1 new study (https://doi.org/10.1186/s12916-022-02451-9), 3 ongoing studies and 1 record linked to a previously identified study. Karen and I had just been talking about wanting to find someone who could speak to using mindfulness in real estate, and that very next morning Karen stumbled upon an article in New York Magazine aptly named "The Woo-Woo Agents of Real Estate: Can Unblocking Your Chakras Get You a House? Maybe!"In it we were introduced to New York City real estate agent Cary Tamura with the Corcoran Group. Cary is ranked within the top 1% out of 50,000 of real estate agents nationwide within the NRT group which includes companies such as Corcoran, Sothebys, Coldwell Banker, Century 21, ERA and others. In addition to his work as a real estate agent in New York City, Cary has been a meditator for over 17 years and a teacher of meditation for a good portion of those years. He trains Corcoran real estate agents in mindfulness-awareness meditation as well as teaching at other meditation centers, schools and businesses in and around New York. Scott Gibson was born and raised in Los Angeles and jumped into the real estate industry right after graduating from UCLA. He became aone of the industries youngest sales manager at the Fred Sands brokerage and eventually went on to manage the Jon Douglas Company where he oversaw 23 branch offices and 1300 agents throughout California in the 1990s. When Jon Douglas merged with Prudential and was eventually acquired by NRT (Coldwell Banker) in 1997 Scott became the Senior VP of Coldwell Banker. A few years later he was named the president of Coldwell Banker in Southern California where we oversaw 63 offices and $20 Billion in annual sales. In 2004 NRT asked Scott to move across the country and bolster their East Coast operations overseeing 9 states for their Coldwell Banker, Cochran and Era brands including their most competitive markets in New York City, Long Island, The Hamptons, Palm Beach, Connecticut and New England. Scott returned to Los Angeles and founded his own brokerage Gibson International in West Los Angeles in 2008 and after weathering the storm during the great recession Gibson expanded and flourished with the Gibson international Brand. In 2018 Gibson merged with Pacific Union in a roll up which included Partners Trust and the John Aaroe Group, and acquired by Compass in 2018. This conversation was recorded in April 2022 and Scott may be the first high level residential real estate professional to say “we are probably in a recession” at a time when the Dow as hovering at 35,000 and houses on the Westside of Los Angeles were selling in multiple offers at every price point. In the past 30 days, the market has changed and with the continued interest rate hikes to combat inflation it appears that the peak of the market is now in the rearview window. Getting to the Heart of Cannabis Health Risks Cell Press Cannabis has deleterious effects on cardiovascular physiology. Wei et al. (Cell 185, May 12, 2022) confirm that important inflammatory markers increase transiently after a single marijuana joint and prove a mechanistic link between THC induced vascular inflammation, endothelial dysfunction, cellular oxidative stress, and atherosclerosis using cell-based and mouse models of atherosclerosis. They provide a pharmacological model in which THC-mediated activation of CB1 receptor signaling pathways converge on MAP kinase, TNFa, and NF-kB outputs to create a proinflammatory and atherogenic environment in endothelial cells. That genistein can antagonize the negative effects of THC with minimal central effects is exciting because genistein is a common component of soy and is already a widely consumed dietary product. Electronic Cigarettes Versus Nicotine Patches for Smoking Cessation in Pregnancy: A Randomized Controlled Trial Nature Medicine Pregnant smokers were randomized to use either nicotine replacement therapy with patches (NRT, n=571) or e-cigarettes (n=569) for smoking cessation. For the primary outcome, validated prolonged quit rates at the end of pregnancy, the results were 4.4% for NRT and 6.8% for e-cigarettes (P=0.08). However, 25 participants in the NRT arm who reported abstinence also used e-cigarettes. When these participants were excluded from data analysis the quit rates were 3.6% for NRT and 6.8% for e-cigarettes (P=0.02). Low birthweight was less common in the e-cigarette arm, 9.6% versus 14.8% for NRT (P=0.01). The authors conclude that “e-cigarettes were markedly more effective than patches” and do not pose more risk. Daily Cannabis Use, Cannabis Use Disorder, and Any Medical Cannabis Use Among US Adults: Associations Within Racial, Ethnic, and Sexual Minoritized Identities in a Changing Policy Context Preventive Medicine Reports Cannabis use has steadily increased in the United States, with daily and medical use associated with cannabis use disorder (CUD) and the negative consequences more frequent among marginalized groups. In this study, the authors use the National Survey on Drug Use and Health (NSDUH) to examine medical and daily use and CUD across the intersections of racial, ethnic, and sexual minorities. They found that sexual minorities were more likely to have medical and daily use and CUD than their heterosexual counterparts within each racial and ethnic group. However, when examining the intersection of race, ethnicity and sexual identity, there was more heterogeneity across these groups. In addition, they found that in states with medical cannabis laws (MCL) daily cannabis use was higher across all intersectional groups. Effect of AXS-05 (Dextromethorphan-Bupropion) in Major Depressive Disorder: A Randomized Double-Blind Controlled Trial AJP Psychiatry Altered glutamatergic neurotransmission is implicated in the pathogenesis of major depressive disorder. AXS-05 (dextromethorphan-bupropion) is an oral NMDA receptor antagonist and sigma-1 receptor agonist, which utilizes inhibition of CYP2D6 to increase its bioavailability. This phase 2 trial assessed the efficacy and safety of dextromethorphan-bupropion in the treatment of major depressive disorder. In patients with major depression, dextromethorphan-bupropion (AXS-05) significantly improved depressive symptoms compared with bupropion and was generally well tolerated. The most common adverse events were dizziness, nausea, dry mouth, decreased appetite, and anxiety. Dextromethorphan-bupropion was not associated with psychotomimetic effects, weight gain, or sexual dysfunction. Mental Health and Substance Use Among Homeless Adolescents in the US JAMA Network This study evaluated mental health and substance use outcomes among homeless and non-homeless adolescents in 2019. Alcohol, cigarette, marijuana, and binge drinking during the prior 30 days was assessed, along with lifetime use of cocaine, methamphetamine, heroin, ecstasy, and injection drugs or prescription opioid misuse. Results found current substance use ranging from cigarettes to alcohol were higher among homeless adolescents. Lifetime cocaine use was significantly higher among homeless adolescents, as were methamphetamine, heroin, ecstasy, and injection drug use. Homeless adolescents experience worse mental health outcomes, including depression and suicidality, and struggle with more SUDs than their counterparts. The Importance of Federal Action Supporting Overdose-Prevention Centers NEJM In this prospective piece, the authors discuss the need for new approaches to harm reduction and substance use disorder treatment in the face of substantially increasing overdose deaths, particularly since the start of the COVID-19 pandemic. One such strategy they discuss is overdose-prevention centers, which operate in other countries and are associated with significant reductions in opioid-overdose morbidity and mortality. However, under Section 856 of the Controlled Substances Act, such facilities may be subject to federal legal sanctions. The authors recommend that the Biden administration declare they will not interfere with such public health interventions or declare that section 856 does not apply to legally sanctioned centers. Further, Congress should modify the Controlled Substance Act to exempt overdose-prevention centers. Complex Persistent Benzodiazepine Dependence—When Benzodiazepine Deprescribing Goes Awry JAMA Psychiatry Benzodiazepines remain popular medications among patients due to rapid symptom relief and reinforcing effects. As clinicians and patients become more aware of potential risks, and clinical guidelines increasingly urge caution in prescribing, guidance for benzodiazepine deprescribing is needed. The authors propose a new clinical concept for patients experiencing significant psychological or functional decline during or after a benzodiazepine taper—complex persistent benzodiazepine dependence (CPBD). CPBD can be described as symptomatic or functional decompensation with or without the development of aberrant medication behaviors in the setting of benzodiazepine deprescribing–in the absence of a benzodiazepine use disorder. Further research is needed to validate this concept. What is Success in Treatment for Opioid Use Disorder? Perspectives of Physicians and Patients in Primary Care Settings JSAT Opioid abstinence and treatment retention are typically used as measures of success of MOUD treatment. This study sought to identify other important treatment outcomes and patient-centered measures of success. Qualitative, structured interviews were conducted with physicians (n=14) and patients (n=18) in 2 family medicine residency programs. The physicians (7 faculty and 7 residents) were experienced buprenorphine prescribers. Both patients and physicians identified 5 themes: staying sober, and improvement in physical health, mental health, relationships, and role functioning. Patients, but not physicians, identified 2 additional themes: tapering off buprenorphine, and decreased stigma and shame. The authors conclude that “clinicians and researchers need to consider a broader scope of success indicators.”

Compared to the automotive and aviation industries, railways can seem rather antiquated. But trains are actually vitally important for modern digital infrastructure. When Nori awards carbon credits, of course, we want to be as precise as possible and ensure that each Nori Carbon Removal Tonne or NRT actually represents a metric tonne of carbon removed from the atmosphere and retained in the soil. At the same time, we can't let perfect be the enemy of the good, and right now, soil sampling for carbon is imprecise and incredibly expensive. So, rather than wait for soil science to catch up, we're incentivizing farmers to implement regenerative ag practices NOW. How do we quantify carbon in the soil? This episode explores the devastating impact privatized, neocolonial wildlife conservation and safari tourism have had on Indigenous pastoral communities, specifically in Northern Kenya. Since its founding in 2004, the Northern Rangelands Trust (NRT) has set up 43 "community" conservancies on over 42,000 square kilometers of land in Northern and Coastal Kenya – nearly 8 percent of the country's total land area. Although terms like “participatory,” “community driven,” and “local empowerment” are extensively used by NRT, this episode amplifies the voices of pastoralist communities who have been dispossessed of their ancestral lands, through corruption, cooptation, and sometimes through intimidation and violence, to create wildlife conservancies for conservation dollars.
