Pieno ūkio savininkas Arūnas Kudrevičius iš Vaitkūnų kaimo Utenos rajone, ėmęsis modernios karvių fermos statybos, rizikuoja netekti ūkio. 2021 metais Nacionalinė mokėjimo agentūra patvirtino paramos skyrimo projektą, bankas suteikė paskolą. Ūkininkas kibo į pirmojo projekto etapo darbus, investavo į gyvūnų gerovės sritiestvarkymo įrangą, įsigijo technikos. Ėmėsi ir svarbiausio darbo - didinti bandą nuo 120 iki 300 karvių. Darbų nuveikta nemažai, bet fermos nėra. Trejus metus užtruko projektavimo ir derinimo su institucijomis darbai. Physician Assistant and prior C&P examiner Leah Bucholz discusses Pes Planus as it relates to VA disability.Leah addresses how flat feet (pes planus) are considered in the context of VA disability. She explains that many veterans suffer from foot conditions like plantar fasciitis, metatarsalgia, and others which can be related to their service. Flat feet can be either congenital (present from birth) or acquired due to various factors such as weight gain or injuries. Leah highlights that for VA disability compensation, the condition can be connected primarily (developed or worsened in service) or secondarily (caused by another service-connected condition). She emphasizes the importance of showing a worsening of the baseline condition for those with pre-existing asymptomatic flat feet. Akcijos tikslas - atkreipti vairuotojų dėmesį į eismo saugumo svarbą prie mokyklų, būtinybę laikytis Kelių eismo taisyklių, stengimąsi išvengti eismo įvykių, kuriuose nukenčia vaikai.Pernai, lyginant su metais anksčiau, 7 procentiniais punktais sumažėjo manančiųjų, kad Lietuva yra saugi šalis gyventi. Thanks for tuning in to the Armor Men's Health Hour Podcast today, where we bring you the latest and greatest in urology care and the best urology humor out there.In this segment, Dr. Mistry and Donna Lee answer a question from a visiting Canadian listener with lichen planus. Lichen planus is an inflammatory condition that causes red lesions on or near the head of the penis. These lesions come and go in the same spot on the penis and are often flaky with sharp or jagged edges. Lichen sclerosis is a similar but more severe condition that can cause serious complications, including buried penis. Lichen sclerosis is treated by removing the affected skin and replacing it with grafts of healthy tissue to prevent the otherwise inevitable recurrence of disease. Early treatment of lichen planus can help prevent the development of lichen sclerosis. This listener explains that the red patch on his penis goes away when he uses a Protopic ointment, but only for 4 or 5 months at a time. During flare ups, he avoids sex because the friction is painful. He also worries about the side effects of this prolonged medication use and asks if there are other treatment options he should consider. Dr. Mistry explains that creams or ointments like betamethasone are helpful in treating lichen planus lesions, especially when mixed with lidocaine to reduce pain. Another option for those with lichen planus is the injection of PRP, or platelet rich plasma, into the affected skin. While this cutting edge treatment isn't covered by insurance, it costs only $750 for a one-time, in-office injection. Oral Lichen Planus is a painful inflammatory condition that affects the mucous membranes of the mouth resulting in white or red patchy spots that can be swollen and painful. Oral lichen planus is typically treated with topical corticosteroids, but the medication comes with a few side-effects including secondary fungal or yeast infection, excessive dryness, and possible scarring. This study showed that laser therapy is just as effective as corticosteroids in reducing pain and eliminating the lesions without the side-effects.Study mentioned in this episode:Photobiomodulation therapy vs. corticosteroid for the management of erosive/ulcerative and painful oral lich planus. Today we chat with Dr. Carrie Sopata about the Lichens! Lichen Sclerosis, Lichen Planus and Lichen Simplex Chronicus the Diagnosis of Skin Disorders and Vulvodynia. We discuss how to Identify features of lichen sclerosis, lichen Planus, and lichen simplex chronicus and make a diagnosis based on physical exam findings, the list of treatment options for refractory lichen sclerosis and recognize the benefits of long-term management. We state the current knowledge of the benefits of fractional laser in the treatment of lichen sclerosis, how to perform a tailored vulvar exam in patients complaining of vulvar pain, and create a management plan for patients with Vulvodynia that addresses their psychological and physical complaints. Vulvar skin disorders are common and can be frustrating to treat because they often are chronic and patients have tried multiple therapies without success. for the physician, high potency steroids are the mainstay of treatment, but some patients are non-responsive and/or non-compliant with this medication. When patients do not respond to steroids, what should be the next step? And how should patients be managed long term?

So I’m just a few days shy of finishing the 21 day detox and just have a question for ya. I work straight nights as a nurse, so my schedule is not consistent. For example, I work a few nights, then have 1 off and then work 2 and have 4 off. My body hasn’t known what a normal schedule is in 17 years because of my night shift work. Wondering for the next time I detox...what is the best way to use the detox to get the maximum benefit with a schedule like mine? (Meaning- Timing of shakes and meals day to day). Oh, and also I 100% lost my taste and smell from covid 6 months ago and have very faintly got them back. I was so hopeful the detox would help with it, but it hasn’t. I stay fit through strength training and distance running and have lived a vegan lifestyle for almost 10 years now. I consider myself an overall very healthy person, so I don’t get it! I have gained 10 pounds over the last year, and I am a petite 5'2'' women, so the weight gain is noticeable and annoying. Your podcast 1817 about male hormones today got me thinking about something else.  Ive currently been taking finasteride for hair loss and for the most part it seems to have staved off further thinning, As a guy in my 30s, this is the last thing I'm going to give up without a fight.  What are your opinions on an extended use of a drug like finasteride (5a reductase inhibitor) that's basically stopping the body from converting testosterone into a virtually useless and damaging hormone in adult men(dht). Ive heard of some upcoming topical anti-androgens coming to market that would potentially be even more localized to the scalp and would inflict less systemic effects like an oral drug. Thoughts on any of this?  Im torn on natural remedies to hair loss because by and large they have been proven to give little to no effect on hair loss. In fact, why would we consult nature on a natural process trying to destroy our hair?! I do think, however, that there may be some natural assistance we can provide our bodies to deal with extended use of a drug like finasteride. Three dermatology residents — Dr. Elisabeth Tracey, Dr. Julie Croley, and Dr. Daniel Mazori — discuss tips for clear communication with patients in this special resident takeover of the podcast. Beginning at 6:11, they talk about challenges with topical therapies and setting expectations with patients. “We, as dermatologists, can optimize patient management by being effective communicators,” said Dr. Croley. They provide communication strategies for improving compliance with therapy and ensuring patients have the correct instructions, as well as clarifying patient misconceptions and the importance of maintenance treatment. We also bring you the latest in dermatology news and research: 1. Topical ruxolitinib looks good for facial vitiligo in phase 2 study. About half of patients on the two highest doses had a 50% improvement after 6 months of treatment. 2. Patients concerned about clinician burnout. Almost three-quarters of Americans are concerned about burnout among health care professionals. 3. Antimalarial may be effective, safe for erosive oral lichen planus. Hydroxychloroquine sulfate may be an effective and relatively safe treatment option for moderate to severe oral lichen planus. Things you will learn in this episode: Review expectations of therapy with patients, such as an intense inflammatory response to topical 5-fluorouracil for actinic keratosis, to ensure that patients remain compliant with the therapy but also feel they can trust you as their physician. If patients are hesitant to use topical minoxidil because they are concerned with the length of time they’ll have to use it, use a metaphor for another lifelong commitment such as brushing your teeth. “What I started actually doing is calling topical minoxidil toothpaste for your hair,” said Dr. Mazori. Talk to patients about spot-treating with acne or applying topical medication appropriately for psoriasis. “A particular challenge in dermatology with topical medications is not just whether or not they use it or pick up the prescription but how they use it,” said Dr. Tracey. Talk to patients about underapplication of sunscreen. Recommend a physical blocker if patients express concerns about systemic absorption. Write down instructions to ensure patients have the relevant information. The teach-back method of communicating with patients often is taught in medical school and ensures that the patients have understood what you’ve said, but it doesn’t ensure that they retained it. Strategies such as having medical students write the instructions or copying notes from your electronic medical record to print for patients can help save time. Emphasize the importance of maintenance treatment for conditions such as intertrigo, seborrheic dermatitis, or onychomycosis to prevent recurrence. Give patients both the trade name and generic name to ensure they use the correct medication. In this episode, I discuss the misdiagnosis of a swelling on the roof of the mouth that turned out to be cancer. This was misdiagnosed twice and can have serious implications for patients. In addition, we talked about the incidental radiographic finding with a panorex showing calcifications in the Carotid artery and its association with obstructive sleep apnea heart attacks and strokes. We also discuss using dental implants to help a patient with severe erosive lichen planus. Lichen planus can be a painful mucosal condition making Dentures very difficult or impossible to wear. Dental implants offers the opportunity to elevate the appliance off of the mucosa making impossible to wear Prosthetics without pain.

Lichen Planus is a chronic inflammatory mucocutaneous disorder that can affect the mouth, vulva, vagina, and anus. Wickham’s Striae are characteristic of mucosal involvement. Biopsy is often needed to confirm the diagnosis and rule out other neoplastic conditions. The more afflicting type of Lichen Planus is erosive lichen planus. In this episode I break down to interesting topics. The first is discussing triggers with oral lichen planus. Patience with biopsy proven oral lichen planus we're given a questionnaire in this Prospect of study to see what the triggers were and if avoidance of the triggers was helpful for these patients. A list of the typical and most common triggers is also given for your review. I also discuss the odds and likelihood and reasons for plate removal with jaw surgery. This discussion will help you have a better feel for when patients will have plates removed and if they are likely to have an improvement in signs and symptoms with removal of those plates.

This week I go over two articles that are very relevant to everyone in dentistry today.  Lichen planus, especially the erosive variety is very annoying and affects the quality of life for patients and we can help them.  Anyone doing 3rd molar surgery or seeing patients and referring them for wisdom teeth should and now will know that flap design does not affect the health of the second molar long term.
