87: Wah! So Expensive. Clear This Objection By Creating Passive Income ProductsHello gorgeous and welcome to ‘Soul Why?' Secret of Soul Rich Woman Blueprint' Today, I want to talk about part two of “Wah! So expensive” because there is another angle to tackle and handle this issue as you're doing your business and bring leadership online. Whether you're in Asia or wherever you may be. This is an evergreen principle. Let's start. Duplication  01:51- 02:02 Remember when I talk about the topic value ladder when you need to build up a series of ways you're able to serve your potential customers?  02:08- 02:10 The answer is duplication.  04:55- 05:09 So you must trust the process as well. Not everyone is ready to take your product or your services. You just need to remember, just keep marketing to the right eyeballs, to the right fishes, to the right customers, and eventually, all will come in.  05:21- 05:33 For example, creating digital products like short courses. Ranging from  97 dollars to 297 dollars. That's what we called low-level value programs.    05:35- 05:40 Where you want your fishes or potential customers to say yes to you.  Check first 06:01- 06:06  This idea of creating digital products starts with one.  06:25- 06:36 The thing is, don't create that thing first. Do the landing page, do a sales page. Check out if anybody wants the product or not first.   Value ladder 02:11- 02:22 When people say “Wah! I want to share with you today some tips and scripts on how to handle clients that say your program or products are too expensive.What are the key objects?03:34- 03:38  This is a huge amount for some people.  06:10- 06:15 When you are competing in the Olympics. For example, Joseph Schooling. 06:23- 06:33  And when he wanted to win the Olympic gold, did he hire his coach to swim for him, or did he swim his Olympics laps in the pool itself?  06:51- 06:55 Anyone who comes along, you must first recognize what are the key objects. 07:04- 07:08  And you must put it down into the lingo that your fishes speak.  How much is your time worth? 04:41- 04:53  When you want to sell and promote your programs, products, or services, you need to first list down 20 objections that most likely your customers will say to you.  05:03- 05:4  When they say that “this is too expensive” 05:06- 05:08 There are three main things that are happening in their lives.   05:09- 05:20 First, they are already not interested in your product. Number two, they probably have something else in their mind. Number three, they just don't believe enough in your product.  08:12- 08:16 Another objection we always get is, “I don't know what to say.”  08:28- 08:36 We always tell them “Don't worry about the topics.   One of the most common questions I get asked about the health care system is why are medical bills so enormous? It seems strange that the explanation of benefits you receive after getting care or services are so obviously inflated. Yet those charges which the hospitals, health care professionals, and pharmacies post are there to see which they clearly have no expectation of getting paid. Why does this happen? And why do insurance companies - which one would expect to have incentives to keep these charges low - seem to be ok with them?   The answer lies primarily with the following four points which I will describe: Hospital Charges: Hospitals are encouraged to inflate their posted charges even though they have no expectation of collecting them because of a payment system known as Disproportionate Share Hospital Charges. Hospitals routinely contract with insurance companies for much lower payments for services which are sometimes less than 10% of posted charges. The reason is that the uncollected amounts the hospital will then claim as uncompensated care, ie, care for the indigent or charity care. This program funnels billions of dollars into hospitals nationwide (in 2017 it was $17.1 Billion). Insurance Charges: One would expect that insurance companies would balk at these massively inflated charges but they don't because they too benefit from large posted prices even when they contract for lower prices with the hospitals. Oftentimes, a percentage of the insurance's business is a bounty they receive from employers for providing "savings". This means that in an inflated charge where they pay only 10% they count the 90% as a savings to the employer who will then turn around and pay an extra amount to the insurance company. For this reason, so long as the insurance company can get good contract rates it is in its best interest to have larger and larger posted charges. Health Professional Fees: Similar to the insurance companies, health professionals (physicians, laboratories, physical therapists, etc.) have reasons to inflate their charges beyond what they can expect to receive. This is because small insurance carriers occasionally pay a per cent of charges rather than a set fee for certain services like Medicare/Medicaid. Therefore, the higher the charges, the greater the revenue (55% of $100 is bigger than 55% of $50). Pharmaceuticals: Finally, pharmaceuticals are expensive because they rely on pharmacy benefit managers (PBMs) to get on hospital and pharmacy formularies (which are then used for insurance companies). These PBMs are very concentrated and hold a lot of leverage in getting drugs to the physicians and hospitals so the manufacturers of the drugs cut "rebates" to the PBMs in order to make it onto the formularies. Unfortunately, these rebates rarely make it back to the insurance companies or patients to allow for smaller premiums. Instead, the PBMs pocket these percentage rebates and use them to help their bottom lines. It also creates a perverse incentive for the PBMs to have expensive drugs when cheaper alternatives might be available. An example is that a 50% rebate on a $1000 drug yields the PBM $500 profit where an equally effective drug priced at $200 would only give them $100. It's important to note that medical care is not solely expensive because of these four factors and there are a lot of things which make care so expensive. This is simply an example of the four most common reasons our bills are so inflated and the contribution to the high prices in health care is due to so many more distortions in the market.   Zombies aren't real, but sometimes they sure seem like it. Especially when it involves medicine and the law. Health Diagnostics Laboratories (HDL) went into bankruptcy after being discovered it was ripping off patients by overcharging them and providing kickbacks to doctors and health organizations for using their services.   Aside from being unethical this illegal activity put all those employees who were prescribing laboratory tests at risk for legal action.   My guest, Dr. Molly Rutherford, was an employee of a physician's private practice in Kentucky when her zombie nightmare occurred. At the clinic, her physician boss encouraged her to perform all sorts of expensive and highly technical laboratory tests on most of her patients.   HDL was paying kickbacks to the offices and physicians who ordered their tests. Dr. Rutherford did not find that the results were very useful especially because they were so expensive.   Soon, Dr. Rutherford stopped ordering those tests because she didn't find their results helpful in her treatment. However, she later found that her boss was ordering the tests on her patients without consulting her. Once she realized this was happening she quickly felt that something was not right and decided to leave and open her own direct primary care clinic.   The advantage of opening her own clinic allowed her to not only escape some of these shady practices but also to begin practicing addiction medicine along with family medicine. Now she mixes the two on her own with great success bringing her lots of professional contentment.   Unfortunately, HDL went bankrupt and began demanding back all of its kickbacks given to any doctor who received their kickbacks even if they didn't even know that their ordering of the tests sent the kickbacks to her boss. Dr. Rutherford was forced to hire an attorney to avoid paying the fines demanded by the creditors of HDL.   Lucky for Dr. Rutherford, the case was settled by her lawyers who got her out of the lawsuit but still cost her several thousand dollars even though she was no longer working at the clinic. That is now in the past and she is enjoying her life practicing medicine the way she wants.   The topic of abortion seems to have gained more momentum in the news cycle of late with states passing laws relaxing or loosening restrictions on the practice. It feels like the 1973 ruling of Roe v. Wade might actually come before the US Supreme Court with a chance of reversing or affirming the 46 year old opinion.   As a physician and hospital, the social and ethical questions that surround abortion are both personal and professional. What is the obligation of institutions to perform abortions? How about the physicians or nursing staff? Do you have a legal obligation to perform a procedure that you find morally reprehensible?   To answer those questions, today I have Stephanie Slade on the show from Reason magazine. Stephanie is a managing editor and follows the public and legal debate on the subject. She wrote a piece for the America magazine a year ago documenting the efforts of the ACLU to compel catholic hospitals to perform abortions and other reproductive health procedures (sterilization, gender reassignment, etc.).   As with all political discussions, this one is nuanced and also uniquely American as our legal focus is heavily directed by our founding documents and the Bill of Rights. What role freedom of speech and association has for private institutions like catholic charities and what rules can the government impose on those same hospitals is what is under dispute with these ACLU directed claims. The ACLU believes that hospitals are a uniquely public institution that provides critical care to large populations and therefore should be compelled to perform abortions and other medical procedures that the catholic church finds morally objectionable.   To date, the ACLU has been unsuccessful in any of their lawsuits even with sympathetic federal judges who point to the first amendment as the basis for their rulings. However, the ACLU is now bringing forth cases of gender reassignment against the catholic church to see if that issue will bring a different ruling. Finally, as state legislatures continue to pass laws that allow abortion up to the moment of birth or ban abortion as soon as a heartbeat is present (~6 weeks gestation or around when a typical urine pregnancy test is positive) the US Supreme Court may feel its hands are tied and have to reexamine Roe v.   National health care policy is always at the forefront of the political discussion in the US but even more so now that the 2020 presidential race is beginning to heat up. Democratic candidates are offering their visions of what the US system should look like and point to other countries in the world as examples of superior structures. One held up as better than the US is the United Kingdom and its National Health Service (NHS). But is it better than what we have here?   My guest today is Philip Booth, a professor in finance, public policy, and ethics at St. Mary's University in Twickenham, UK and a senior academic fellow at the Institute of Economic Affairs. He has written extensively on the health care policy in the UK and is able to provide an historical perspective on the UK system and how it differs from the US and the rest of continental Europe. In today's discussion, we examine whether the UK system is really a beacon to be held up by US politicians and what other national systems are in the world that may be better or worse.   Not surprisingly, the conclusion we arrive at is that the NHS is hardly a system that we should strive to emulate in the US - much like the US system with its runaway costs is one other countries should copy either.
