Higher education is all about data and information.  But is this  “knowledge”?  More to the point, what's the difference?  The same traditional sources have churned out studies and reports for decades with almost no change in results.  Facts and figures go in and are digested, but not much seems to come of it.  So the studies are replicated, with the same results.  The data and the “knowledge” build up with very few plans to do anything. Knowledge without a plan is just, well, knowledge.  Some have called this “analysis paralysis.”  I call it “information constipation.”  Tune in and find out why.   EdUp Insights with Bill Pepicello is part of the EdUp Experience Podcast Network

Host: Darryl S. Chutka, M.D. [@chutkaMD] Guest: David J. Cangemi, M.D. Functional dyspepsia, also known as non-ulcer dyspepsia, represents chronic upper GI symptoms of indigestion and not infrequently abdominal discomfort. Diagnosing functional dyspepsia can be challenging since there are no definitive diagnostic tests. This often results in patients seeking multiple medical opinions searching for a specific cause and treatment for their symptoms. What are the common presenting symptoms of functional dyspepsia? What's an appropriate evaluation and how do we treat our patients who have it? In this podcast, we'll discuss these questions and more with David J. Thank you for joining us for our 2nd Cabral HouseCall of the weekend! I'm looking forward to sharing with you some of our community's questions that have come in over the past few weeks…   Kelsie: Hi! Another question. Is there anything that will show up in bloodwork to indicate colon cancer?   Rosie: 38 y/o vata F, history of UC. 2012: colectomy and jpouch surgeries. Never felt well after. 2020: found the root of my issues was surgery done wrong. 2021-2023: 3 surgeries to redo the jpouch Feb 2023: ileostomy reconnection and now I cannot empty to use the bathroom. A mild stricture of scar tissue was found at the rectum and pelvic floor dysfunction including an absent RAIR(rectoanal inhibitory reflex). I literally don't feel the need to go to the bathroom and when I try to go I can't. Prune juice is getting me through the day. I started pelvic PT but curious how else to support healing, specifically reestablishing the RAIR. I also have lingering nerve damage on my abdomen from surgery 8 months ago. Could the 2 issues be connected? Im taking omega 3, d, NeuroRenew, magnesium and red light   Rocco: Hi Stephen! I take fish oil but my girlfriend keeps telling me to stop as it is not good for me as it is a polyunsaturated fat. Would love to hear your thoughts on this? Note I buy a high quality product that is from wild caught fish   Luis: Hi Dr Cabral, I've been diagnosed with hypercalcemia and dyspepsia by my GI. My latest blood work came back with calcium and glucose levels at over 11. I'm not finding any podcasts on this topic, so wanted to ask what you can recommend. I have an appointment with endocrinologist next month for this. I do take quite of magnesium including oxypowder (Oxygen based magnesium), full spectrum mag and mag glycinate powder, both from equilife, and calm mag. powder, all on a daily basis. I'm also currently taking vitamin K2 complex (mk7 and mk4). Despite all of this, I still find myself constipated on a daily basis with much stomach discomfort. I have also recently developed pain in my upper right gi, which I suspect could be gall bladder issues. Coffee enemas dont seem to help much. Thanks!   Brandy: When I exercise and don't shower immediately, are the toxins in my sweat reabsorbed through my skin?   Erica: I tried searching through the podcasts but couldn't find one on this, so if you answered this before, sorry for the repetition! Anyways, I am currently planning some projects for my backyard and wanted to get a fire pit, and I see two main options: a wood fire pit or a gas fire pit. A gas fire pit seems more convenient for me, but I wanted to ask if you think this would have health ramifications. For example, I heard that indoor gas stoves have links to health issues, but then again the fire pit would be outdoors (more ventilation) and only used occasionally, so I just wanted to get your opinion on this. Do you often experience stomach discomfort and don't know why? In this episode, we learn about a condition called functional dyspepsia, which may be the reason for your tummy troubles. Dr. Scott Gabbard, a Gastroenterologist at the Cleveland Clinic and Chairman of the American College of Gastroenterology's Patient Care Committee, explains functional dyspepsia, a term used for recurring upset stomach symptoms with no clear cause. Dr. Gabbard helps us understand the symptoms, treatment options and ongoing management of this condition. Since patients often misinterpret their symptoms for other indigestion disorders such as GERD, Dr. Gabbard also offers tips for effectively communicating with healthcare providers about our symptoms so that we get an accurate diagnosis.

Thank you for joining us for our 2nd Cabral HouseCall of the weekend! I'm looking forward to sharing with you some of our community's questions that have come in over the past few weeks…   Cristi: What do we know in natural health about "functional dyspepsia?" I just had an upper endoscopy to diagnose the cause of a burning sensation in my stomach just under my right ribcage usually right after eating/while eating that seems to respond to an amount of food rather than any particular food. Before this, the gastro had me do a stool test that was negative for H. Pylori. They took biopsies during the upper endo, but the doctor says he doesn't expect to find anything. He said he saw irritation in the stomach lining and it was probably caused by "functional dyspepsia" which was an imbalance of acid in the stomach. He said the causes of this could be different things, including stress (which I admittedly have a lot of, but I do what I can to control it) and that otherwise, causes were unknown, but that I could just take Pepsid! How long? Oh, maybe forever...Needless to say, I'm not doing that. SO, if all I know is that the acid is "imbalanced," how do I balance it? My diet, exercise, and supplement regimen are all along the lines of what you advise, so none of the other causes of this "disfunction" (NSAID use, fried/unhealthy fat foods, alcohol excess) make sense. Thank you!   Cristi: This is an add-on to my previous question about "functional dyspepsia." Another strange detail: I can "feel" the spot in my stomach that burns after eating during certain abdominal exercises. It's not pain, but I can feel it almost like it's an internal healing injury while doing the exercise.   Jillian: Hi Dr. Cabral, I have a question regarding research on SIDS (Sudden Infant Death Syndrome). I recently heard that Australian researchers had found a blood marker that could indicate the potential for SIDS. They noticed a significantly lower enzyme level of butyrylcholinesterase (BChE) which impacts an infants ability to wake out of a deep sleep in the infants who died from SIDS. I've also read that high levels alpha fetoprotein in mothers can be another indicator for potential SIDS. Lastly, I've read that infants that died from SIDS also had higher levels of serotonin. With your knowledge, do any of these reasons seem connected? Are there things that a mother can do before and while pregnant to help her infant have a decreased risk based on this research. I know the alpha fetoprotein is an issue of the liver. Are there ways that mothers can support their livers better during pregnancy? Thank you for all that you do! Jillian   Janet: Greetings! Would you recommend IV NAD+? Single injection or a series of six? Thank you for spreading your knowledge. I wish everyone would listen.   Anonymous: Hello Doc! Thank you for your endless & genuine care for the health of everybody! I know there is no diagnosis, treating or curing any disease in your practice, but, a few months ago I had floors/carpet ripped up in my house which I'm sure put a BUNCH of allergens in the air. I work in a steel shop that has pollutants and irritants as well. The ear, nose, & throat doctor sent me for a CT scan and my nasal cavities are full of puss, hindering my smell/taste. I can breathe clear for the most part, but, I blow my nose all the time, sometimes sneeze 4 or 5 times in a row and from the inflammation it's all water. I'm getting nasal drip all day and I'm sure when I'm sleeping too, which I believe, will lead to bacteria in my gut. I was given 2 weeks of antibiotics which helped a little to clear it up and get the smell/taste back. Now its going away again. I don't want the shots they recommended putting more antibiotics in me, seeing as I'm working majorly on my health, and that would be a set back for sure. Do you have experience in your practice with sinus problems this bad in people? am I too far gone to just take a Mold lab- test, and do a mold protocol, would this help any? The treatment they want to do is way too costly. Thank you so much again and I ALWAYS trust your knowledge!!   Julia: Hi, my husband diagnosed with hashimoto's thyroiditis, can you tell me please what you know about? Most people describe bloating as feeling full, tight, or swollen abdomen. Your abdomen may also be swollen (distended), hard, and painful. Dr. J and Evan describe that gas is the most common cause of bloating, especially after eating. Gas builds up in the digestive tract when undigested food gets broken down or when you swallow air. Everyone swallows air when they eat or drink. On the other hand, they also talk about different components of why you may be having to bloat that you may not notice. I like eating 2 meals a day but after listening to book I'm trying to increase my protein to 180 to 200 grams a day. I'm 6'1 210 lbs. Is it beneficial to eat 90 grams in each meal or is that two much for my body to process correctly for optimal absorption? Should I spread it out? Thanks 2. Putative blood flow issues [15:40] Charles says: Hi Robb and Nicki, long-time listener here. I have been following Robb since I was 18, almost ten years ago! Thank you for your work. Background: 6 feet tall, male, 142 lbs, 28 years old. Lenient paleo diet for almost 10 years supplemented with 0.5-1 packet LMNT daily depending on weather/exercise load, generally great sleep hygiene, devote a lot of effort to getting in the sun every day, weights 2x/week, 45 min bike ride at least 2x/week. Recent blood work came back excellent (just the standard LDL, HDL, Hem A1C, Vit D, thyroid stimulating hormone, hepatic enzymes). I do seem to have issues with circulation though, which I think may be related to the problem I describe below. My extremities go numb easily, and when it's cold I have a few finger tips that lose color/sensitivity for extended periods of time after the others have returned to normal temperature. The problem: My erections have decreased in strength noticeably in the past two years. The problem usually isn't so severe that it prevents sex (though a few times it has), but it certainly decreases its quality for both parties involved on a regular basis. Potentially relevant circumstances surrounding the problem: A little over two years ago (when the problem began) I ended a bad relationship, started a new **amazing** one, moved twice, had a very bad car accident, had a very bad cycling accident, lost my grandmother, helped my mother through a traumatic incident, etc. I'd like to point out that this was Winter-Summer 2020, need I say more? It was a whirlwind of a year. I dealt with it in healthy ways (amping up the exercise and protein consumption) and unhealthy ways (picking up a 1-2 drinks before dinner habit that I've since decreased to just on weekends; this decrease happened 6 months ago). Should I talk to a therapist instead of sending this in to a podcast? Probably, but why not both? I'm hoping you and Nicki might be able to connect some dots I haven't. 3. Functional Dyspepsia [21:36] Cristi says: I love your show and the dynamics between the two of you. I'd love to join "the rebellion" when I can afford it. What do we know in natural health about "functional dyspepsia?" I just had an upper endoscopy to diagnose the cause of a burning sensation in my stomach just under my right ribcage usually right after eating/while eating that seems to respond to an amount of food rather than any particular food. How long? Oh, maybe forever...Needless to say, I'm not doing that. SO, if all I know is that the acid is "imbalanced," how do I balance it? At the risk of making this too much longer, my diet, exercise, and supplement regimen are all along the lines of what paleo-centric gurus recommend, so none of the other causes of this "disfunction" (NSAID use, fried/unhealthy fat foods, alcohol excess) make sense. I'm not celiac, but I don't eat gluten and dairy because they just don't feel great. I'm thinking of continuing to take probiotics and add a functional medicine supplement with aloe, licorice root, marshmallow root, NAD etc. to maybe calm the irritation? My only other thought is that maybe I've been taking too much of a natural allergy supplement that has quercitin (400 mg per two pills) -3 pills twice a day-but I doubt that's it, and after stopping it for five days before this procedure as they required, I really noticed my allergies kicking in. The problem is, I'm 5'5, 118-120, and closer to 118 with what I'm able to eat without feeling discomfort. I have to stop before I'm satisfied completely. I wake up at night hungry, and I can't go back to sleep, and I know 118 is too light for me. I weight train 3X per week, and I'm active all of the other days. I'm a highschool teacher, so I'm not really sedentary, either. I'm pretty low carb with starchy carbs (about a cup of sweet potato or rice) only included at dinner. Affecting up to 20% of the UK population, dyspepsia is commonly seen in primary care. Following investigations, the majority of people (up to 89%) are found to have functional dyspepsia. As with any functional problem of the gut, such as IBS, making a confident diagnosis is sometimes challenging but extremely important for the patient, and management encompasses a range of non-pharmacological interventions. Pali Hungin, an extremely experienced primary care clinician and a founding member of both the PCSG and it’s European counterpart, shares his vast experience and understanding of functional dyspepsia with us in this episode. As chair of the ROME VI primary care committee, Pali is hugely knowledgeable about this complex yet common condition.

Several vital organs exist in this area, including the spleen, kidney, pancreas, stomach, colon, and lung. One of the most common origins of these issues is due to digestive problems. Heartburn typically happens when acid comes back up from the stomach into the esophagus. It can result in discomfort and a burning sensation in your chest. The pain can feel burning, sharp, or cause a tightening sensation. Additionally, Gastroesophageal Bowel Disease, commonly called acid reflux, is a condition that occurs when you experience heartburn more than two times each week. Irritable Bowel Syndrome is also a chronic condition that involves a group of intestinal symptoms that usually occur together. The symptoms can differ in severity and duration from person to person. Inflammatory Bowel Disease (IBD) also includes any disorder that can cause inflammation in your gut; the most common of these conditions is Crohn's disease and ulcerative colitis. Dr. J and Evan emphasized that having your daily food and environment checked. Possible modification is a basic essential thing to do to avoid gut issues that can compromise overall health. ===================================== Subscribe on I-Tunes: http://www.beyondwellnessradio.com/itunes Review us at: http://www.beyondwellnessradio.com/itunes Visit us at: http://www.beyondwellnessradio.com Have a question: http://www.beyondwellnessradio.com/question

Dyspepsia is a common presenting complaint in primary care, and encompasses a range of symptoms including GORD, nausea, and epigastric pain. Having an approach to these patients is vital given how commonly we see these symptoms, and Mike Burkitt joins me in this episode to provide a really useful overview of dyspepsia, and how we can approach this symptom in clinical practice. Mike also gives his thoughts on what to do when the endoscopy comes back normal in a symptomatic patient. But, as simple as it sounds, supplementing can confuse—and people often make mistakes. Do you think you're taking all the proper nutrients and that you're taking them the appropriate way? Let's learn some common errors people make. Dr. J highly recommends that you avoid having the symptoms cured rather than the source. Don't just accept your signs and symptoms as usual; consult your doctor and express your concerns. Your doctor may advise you to do diet modifications and have yourself tested to find out the root cause and fix it. Also, watch out for over stressing. Stress can affect your decision-making and as well as your gut health. Another reason can be chewing your food too quickly that makes your digestive system suffer, inadequate water intake, or very few fibers in your diet. To understand all these and more, make sure to watch the entire podcast, like, subscribe, and learn the tests that you probably need to have your issues fixed!

Of course, Dr. J is not only interested in medical history and providing relief, but as a practitioner of functional medicine, the goal is to find the root cause of that nauseous feeling. Very possible root causes are h.pylori, SIBO (small intestinal bacterial overgrowth), even stress can trigger histamine and inflammation which can lead to nausea. Some natural relief you may want to try: lemon juice, ginger or ginger tea, peppermint, or chamomile (all three are natural digestive bitters). Dr. J also looks at what kind of testing he'd use to help determine root cause, how clearing infections can help significantly, how the removal of the gallbladder can increase the likelihood of having that nauseous feeling, and some solutions for acute nausea.
