Vitamin D gets a lot of attention, and it should, mainly for its known role in bone stability. However, vitamin D has significant additional roles in physiology. Vit D, and its metabolites, also functions as modulator of inflammatory and immune responses. According to a number of recent studies, this important micronutrient plays a complex role in numerous biochemical pathways in the immune system and disorders that are associated with them. In pregnancy, the association of Vit D deficiency and adverse perinatal outcomes has been controversial with conflicting data. Nonetheless, in August 2024, the Endocrine Society published its recommendation (J Clin Endocrin Metabol) for routine supplementation for children, adults older than 75 years, pregnant women, and adults with prediabetes. In this episode, we will review a new publication (Feb 2025) from the Am J Clinical Nutrition regarding low vit D levels in the first trimester and PTB. Hypertensive Disorders in Pregnancy/Preeclampsia is a persistent problem, world-wide. Over the last decade, studies have highlighted the role of vitamin D in modulating several mechanisms associated with hypertensive disorders of pregnancy, including but not limited to vessel compliance, the maternal immune response, the renin angiotensin response, and placental implantation and angiogenesis. There has also been published concern that maternal vitamin D deficiency may be associated with childhood neurodevelopmental issues including autism. However, the last ACOG CO on UNIVERSAL SCREENING for vitamin D in pregnancy was in 2011…this stated that UNIVERSAL screening is just not recommended (FYI, this was reaffirmed 2021, and is currently being reaffirmed by the OB committee). But this is different than targeted screening. Since the ACOG stance in 2011, despite RCTs of vitamin D supplementation in pregnancy, everything has changed and yet nothing has changed regarding the approach to screening for vitamin D deficiency. Should vitamin D levels be assessed? Are the cutoffs for vitamin D insufficiency and deficiency the same in pregnant patients? Who is the ideal pregnant patient for vitamin D intervention? How can the intervention be optimized? And what's that link between maternal vit D deficiency and the child's neurodevelopmental outcomes? There is data to answer these questions, and we will lay it all out in this episode.

I'm looking forward to sharing with you some of our community's questions that have come in over the past few weeks… Cindy: I have used your Daily Level 3 protocal for a few years now, but I only use half of the multi vitamin (due to cost) because I share it with my teenage son. I do purchase additional omega 3 so that I get 2 grams daily and use 1 capsule of your B complex daily. I love your magnesium drink, healthy gut support, collagen, and thyroid support. I would like to use the mitochondrial and hormone support too! My genes do have 2 copies of the MTHFR gene mutation and I am postmenopausal. Oh, and I use about 20,000 I.U. of your Vit. D daily inorder to keep my level above 50. I can't take every supplement I would like, but how do I prioritize? If I took the full dose of all of these, would I get too much or waist vitamins? Cindy: Do you think a microbiom could get "use to" a probiotic and therefore not work as well? A few years ago when I started taking your probiotic, it immediately helped be to have a BM daily. That doesn't happen anymore. 2 BM weekly is now the norm again. Amy ideas besides adding Magnesium citrate? Do you think that fermented multivitamins can cause some inflammation for me in the long run, even though the yeast is no longer living in the final product? There are no functional medicine supplements available where I live, the only alternative is one US based organic fermented multivitamin. Please advise, thank you. Welcome back to our weekend Cabral HouseCall shows! This is where we answer our community's wellness, weight loss, and anti-aging questions to help people get back on track! Check out today's questions: Matt: My Daughter, age 8 has a history of always having aches and pains growing up - leg pain, knee pain, has had transient synovitis in her hip x 2 - often times has to sit down or rest when every other child is playing and running around without an issue - every test she has ever had done as been negative - multiple lymes tests, spine XRAY,, femur/tibia XRAY, blood work multiple times. She recently came up low on Vit D with a level of 21.0 ng/ml (currently supplementing) - all aches and pains attributed to growing pains since no cause was found (she has always been tallest in her class - 90 % and up in height generally). since October of 2022 she has been sick off/on - pattern of congestion, followed by headache/looking pale and then fever that lasts 2-3 days. Where do you suggest we start?? Kim: I've been looking at the things you have on pintrest and I have to tell you personally I think the tongue is a forgotten part of the body! I have alot of problems with my tongue, burning, painful bumps, discoloration and the only answer I get is a very expensive prescription for "magic mouthwash" there has to be a reason why this has been happening to me most of my life (I'm 59) I'm hoping you might have some answers PLEASE Jeanie: are there any podcasts on joint replacement and heavy metals are there any podcasts on the risk of taking statins (specifically) I have listened to all the podcasts on lowering cholesterol.
