     Slam the Gavel welcomes Dr. Susan Ilkov-Moor to the show. She is a Geriatric Psychiatrist and also an alienated mother living in Kingston, Ontario, Canada.      Dr. Susan Ilkov-Moor graduated from medical school, residency and subspecialty training from Queens University and has been in practice since 2006. She is seen here in a picture with a yoga-instructor who recently passed at the age of 101 and had practiced 93 years of her life practicing yoga.     Primarily working with dementia she is also working with patients with Psychotic Depression. She has seen families witness a death of the soul when their loved one begins the stages of dementia. Early onset is at the age of 60, which is rare and the youngest case of dementia was the age of 29.      Dr. Susan looks at the patient's childhood trauma, PTSD and how they re-experience this as they go through cognitive impairment as well as family dynamics. Dr. Susan explained that patient's could only see their loved ones through the glass at the nursing home when the pandemic began and what the patients really need is hugs, the feeling of love.      Parental Alienation has touched her own life as well as the legal system and its failure. Family Court perpetuates the Mental Health industry, the Prison industry and the Self Help industry, a "cash cow Family Court Circus." We discussed how the trauma of alienation and PTSD and how this affects the hippocampus in the brain. Olivia Zurek was a star for the Bobcats on the Alumni Gym court in both volleyball and basketball during her time in Lewiston. In volleyball, she helped lead the Bobcats to three 20-win seasons, including a 26-9 overall record in 2003, culminating in a run to the NESCAC tournament championship match. After earning second team All-NESCAC recognition in 2003, Zurek led the conference in kills per set as a senior, receiving first team All-NESCAC honors. On the basketball court, Zurek was even better, earning All-NESCAC honors three times, culminating in a senior season that saw Bates advance to the Sweet 16 of the NCAA tournament and Zurek earn first team All-American honors. The third-leading scorer and second-leading rebounder in Bates women's basketball history, Zurek is now a Geriatric Psychiatrist at Massachusetts General Hospital. Today, she joins the Bobcast to look back on her time as a Bobcat.

On my mom's journey through dementia, we were forced to look outside-the-box for solutions to her extremely agitated behavior and crippling anxiety. We learned there was going to be a Medical Cannabis Symposium in Northern California called “Cannabis as Medicine, Changing the Face of Aging” produced by Home Instead Senior Services, Sonoma County and Hospice services of St. Joseph's Hospital. Two “traditional organizations” that are working within the U.S. allopathic medical system to amplify the message that cannabis helps dementia. Tricky business when the best medicine for many conditions near the end of life remains on the Controlled Substances Act Schedule I of Drugs, making cannabis inaccessible in any Federally funded institutions like Nursing Homes, VA Facilities, and hospitals. The designation of “Schedule I of Drugs” refers to “drugs with no currently accepted medical use and a high potential for abuse. Some examples of Schedule I drugs are: heroin, lysergic acid diethylamide (LSD), marijuana (cannabis), 3,4-methylenedioxymethamphetamine (ecstasy), methaqualone, and peyote.” Despite cannabis medicine being legal in more than 2/3rds of U.S. States, in order for people living with dementia to get some relief today, we need to get cannabis de-scheduled, and open up the Schedule I of Drugs for research. Teruki Nakano works as a Geriatric Psychiatrist and Yoko Nakano is an anesthesiologist. They are both yoga teachers and the founders of Medcare Yoga. They translated Jivana Heyman's Accessible Yoga course into Japanese, and it is their mission to bring accessible yoga to Japan. 25% of Japan's population is over the age of 60, and that figure is growing, so bringing a form of yoga to an increasingly aging population is very important in my opinion! In this episode we learn about Teruki and Yoko's backgrounds and how though skeptical at first, Teruki noticed that yoga was helping with Yoko's health issues. After doing his own research, Teruki realised the many benefits of yoga, and encouraged Yoko to become a Yoga teacher before doing so himself! We hear how Teruki was becoming burnt out in his career as a surgeon, and how a life-changing event - the Fukushima earthquake made him realise that medicine was his calling. Finally, we learn Teruki and Yoko's mission to bring Accessible Yoga to Japan, and some of the unique challenges that yoga teachers face in Japan. Karen Severson, M.D., has spent the last twenty years as a Geriatric Psychiatrist wandering the halls of those dreaded destinations called nursing homes. She became mentally exhausted from watching people with Alzheimer's disease decline and suffer. She wrote about the unnecessary suffering caused by doctors, nurses, and families who are on totally different pages regarding end-of-life issues. She realized doctors tend to avoid these conversations and families can remain in extended denial of dementia. This book is intended to help families understand dementia and its associated behaviors in a down to earth manner. Dr. Severson uses a great deal of humor as not to scare people from the subject. She also discusses several other important issues, but mainly how we can better allow families to learn to let go of those with end-stage illness. Dr. Severson hopes to prevent unnecessary and potentially harmful medical interventions as well as allow more geriatric patients to die in peace.

There is a lot of discussion about the right to die. Although most of these have to do with Physician Assisted Death (PAD). What about in those who are not dying but express a dire to end their lives in the absence of a diagnosable mental illness? Do they have the same right? Well, on today's podcast we are going to step into this tricky topic with our guest, Dr. Meera Balasubramaniam, a Geriatric Psychiatrist from NYU. Meera wrote a paper for JAGS titled "Rational Suicide in Elderly Adults: A Clinician's Perspective". We talk with Meera about her article, including how she would define rational suicide, how can we help best explore these thoughts that patients consider rational, and how society and baby boomers are changing the way we think about this. We also dive into some other interesting topics include agism. I really love this quote from Meera, so I'll post it here, but for the full transcript read below or listen to the podcast: Ageism is a very interesting and distinct concept. It's fear of growing old or fear of being in that state. It's so distinct from something like racism or sexism. If a person is racist about a certain other race, it's less likely that they are going to be part of the other race that they are having negative connotations about. Similarly, if you are sexist, it's less likely less likely that you are going to belong to the other gender. When it comes to age, it's quite fascinating that most of us are actually going to get to that stage that we're being ageist about. What it is about growing old and about being down in the future that scares most of us has been sort of the crux of part of my work from a society perspective.
