Now I realize that doesn't seem particularly ADHD focused, but I thought that this would be a good interview to take because adults with ADHD have a substantially increased risk of substance use disorders. And so while Nick's work doesn't specifically cover ADHD, there is still a lot that you can glean from it. In our conversation, Nick shares his own journey through loneliness, denial, and alcohol addiction, and how vulnerability and connection became essential tools for his recovery. We dig into practical strategies for opening up in safe spaces, replacing harmful coping mechanisms with meaningful activities, and creating accountability systems that support long-term well-being. We also touch on how social media and other modern habits can quietly feed into loneliness, and what it takes to build resilience against them. When looking to break out of unhealthy coping mechanisms, be they alcohol, social media, or some other addiction, deliberately plan what will give you that same connection without the negative costs. Having planned positive alternatives like exercise, hobbies, or group activities can make all the difference in following through with your intentions. Watch out for the “gray zone” of addiction; you don't need to wait until hitting rock bottom to start changing habits. If you are questioning your relationship with alcohol, social media, or other damaging habits, take the time to reflect on what is really driving those habits. Remember, you don't have to make something worse before making it better. When you can, volunteering in support groups or mentoring others not only helps them, but it can also reinforce your own sense of belonging and purpose.

Raised by his grandparents after his parents separated, he struggled with abandonment issues when his mother chose drugs over caring for him. The anger and pain manifested early, and by fifth grade, he was throwing desks into walls.Instead of therapy, his grandmother took him straight to a doctor who prescribed medication. This began a pharmaceutical carousel that lasted into his twenties. Depression became bipolar disorder, then anxiety, then intermittent explosive disorder. Sven cycled through over a dozen medications, often feeling like a zombie rather than himself.The wake-up call came while working as a delivery driver. Despite taking Seroquel the previous evening, he fell asleep at the wheel the next morning, only waking when he hit the rumble strips. That terrifying moment forced a decision: something had to change.Four years ago, Sven made the risky choice to wean himself off medication without medical supervision, something he explicitly doesn't recommend to others. He replaced pharmaceuticals with therapy, alternative approaches including medicinal marijuana and DMT, and most importantly, a supportive community.Streaming became his lifeline. What started as an accidental hobby with a friend evolved into a platform for mental health advocacy. Through his Twitch channel and work with the nonprofit Anime for Humanity, Sven found his tribe... people who understood that gamers, anime fans and other "niche" communities needed mental health resources too.His current coping toolkit is surprisingly simple but effective. When depressive episodes hit, he relies on his wife who lets him cry on her shoulder, friends in the streaming community who provide reality checks and encouragement, and the knowledge that he'll emerge from the darkness. A schnauzer mix puppy has become an unexpected emotional support companion.Sven still battles the same demons. He describes episodes where he feels worthless, convinced everything he does is meaningless and that people won't miss him when he's gone. During these periods, he sleeps excessively and isolates himself. But now he has tools and people to pull him back.His story highlights several critical issues: the overprescription of psychiatric medications to children, the importance of therapy alongside or instead of medication, and the power of community support. Living in rural Oklahoma (a region not known for progressive mental health attitudes) Sven works to break stigma one conversation at a time.The streaming community surprised him with its openness about mental health struggles. Unlike his closed-minded hometown where men aren't supposed to show emotion, online spaces offered acceptance and mutual support. Even his brother, struggling with addiction, now reaches out for help after seeing Sven's transformation.Sven's message is clear: mental illness doesn't make you broken or crazy, you're just wired differently. His journey proves that with the right support system, alternative approaches can work, though he emphasizes working with medical professionals rather than going it alone like he did.From throwing desks to building community, Rev Sven turned his pain into purpose.

The alcohol, says J. Kenji Lopez-Alt, came about in young adulthood, when it solved persistent social anxiety and made him the life of the party. As Kenji found tremendous success in food writing and restaurateuring, he found that he couldn't really make it through the day without downing between two and five drinks. This wasn't hard to pull off in the food business, surrounded by booze and a culture that heartily encouraged its consumption. Kenji found that, as is so often the case with mental health, that the reality of the condition didn't match the drunken messy stereotype: his work wasn't harmed, he had strong relationships and a family. Dear  Dr David I discovered you years ago due to my son's depression.   I purchased your book for him, but he not reading it.  He thinks he is too far gone for any self help and has been on anti depressants for years. I appreciate all your hard work , the blogs and the app.   I have my hopes up that one day he will listen to you or download the app.   This depression has been going on more than 18 years now. Dear David Just saw that you published a podcast about how to give negative feedback, absolutely cannot wait to listen to it. I'm afraid I fall in the category of people who tend omitting opinions and this is going to be of great help! Now to my question: how do I get more excited in dates with guys ? I would like to date more and have a more active sexual life but I find myself getting bored in dates and this affects also my libido. I will have thoughts like: this is boring he is not that attractive I won't be turned on sex won't be good I obviously then don't end up having sex with the guy and try to have a date with someone else. In the date I don't feel anxious but I'm not excited physically and mentally. The anxiety plays a role before I go to the next scheduled dates, because with time I start thinking this feeling of boredom will never go away. I have tried to work on the thoughts myself with little results and I was wondering if you had any suggestions on how I could be less bored and anxious and enjoy myself. Thanks, Warmly, david You are viewing dating as a shopping expedition, trying to “find” the best item to purchase, and finding your shopping boring, which it is. But you are forcing it to be boring because you are not being open with your feelings. You are foolishly trying to hide your feelings of boredom, whereas they are really the door to fascination and a most interesting and dynamic exchange. Let me show you what I mean. You can, instead, view dating as forming a relationship, being open, and genuine, and a little flirtatious, and seeing how things unfold. So, or example, you might say something along these lines, “You seem like a really neat and interesting person, but I notice that our conversation is not very open, or vulnerable, and that makes it way less interesting. Have you notice that, too? Tell me how you're feeling.” If you express this, things will instantly get very interesting! You are not trying to hurt their feelings, but rather open up a conversation about feelings, by encouraging them to be real. They may also be feeling bored, or anxious, or whatever. As a psychiatrist, I find that when I explore the feelings and insecurities of my patients, it is always interesting. And when there is tension, including boredom, I acknowledge it to find out what's up, and how is my patient experiencing the session and our interaction, and that is interesting 100% of the time, without exception. I have listened to the podcasts so many times and I am familiar with the concept of sharing the tensed feelings like boredom, to not force the other person to be boring. However I had never thought to apply it to dating!! It is such a foreign concept to how all my friends approach dating, that I will need first to experiment with it.
