This is episode, we'll look at a national effort to advance clinical research and patient care through artificial intelligence. AI depends on data that are large, diverse, and well-labeled -- in contrast, most clinical data are siloed, inconsistent, and biased toward certain populations. The CHoRUS Network, part of the NIH's Bridge2AI consortium, is standardizing and harmonizing data from many hospitals so they can actually be used for training machine-learning models. Dr. Eric Rosenthal is Contact PI for the CHoRUS Network, Associate Professor at Harvard Medical School, and an epileptologist, clinical neurophysiologist, and neurointensivist at Mass General Brigham. Desai illustrates this with concrete examples, such as building GitHub analytics to track engineering team performance. Previously, this required custom code for multiple API calls, error handling, and orchestration. With MCP, these operations can be defined as simple tool calls, allowing the LLM to handle sequencing and error management in a structured, reasonable manner. The episode explores emerging patterns in MCP development, including auction bidding patterns for multi-agent coordination and orchestration strategies. Desai shares detailed examples from Nexla's work, including a PDF processing system that intelligently routes documents to appropriate tools based on content type, and a data labeling system that coordinates multiple specialized agents. The conversation also touches on Google's competing A2A (Agent-to-Agent) protocol, which Desai positions as solving horizontal agent coordination versus MCP's vertical tool integration approach. Hina explains that patience, physical stamina, problem-solving, adaptability, and emotional intelligence are essential caregiver skills. We talk about the many roles a caregiver plays — from transportation and medication management to wound care, infection prevention, meal preparation, and emotional support. Staying connected to the healthcare team and keeping friends and family informed are key.We highlight the value of resources like the Leukemia & Lymphoma Society's, now Blood Cancer United's, free nutritional consultations. Hina stresses the importance of understanding the Family Medical Leave Act, planning for potential income loss, and connecting with social workers to access financial help from foundations such as LLS, PAN Foundation, Cancer Care, Angels of Hope, New Day Family Foundation, and NMDP.Hina advises enlisting a support network early. Dividing tasks like grocery shopping, meal prep, transportation, and pharmacy runs helps prevent burnout. She reminds us that transplant caregiving is a marathon — caregivers must look after their physical, emotional, financial, and spiritual wellbeing. This includes setting boundaries, filtering advice, staying flexible, and preparing for setbacks such as relapse or unexpected hospital stays.We discuss unique considerations for CAR T, allogeneic, and autologous transplants. CAR T caregivers should watch for neurological changes. This episode was recorded live at the 2025 Joy & Wholeness Summit in Asheville, North Carolina.Dhaval Desai was raised in Atlanta, Georgia, and completed his undergraduate studies at Emory University. He completed medical school at American University of the Caribbean School of Medicine in St. Maarten, performed his clinical years of medical school in New York City, and had the opportunity to rotate through various different community hospitals. He completed his residency at Wright State University School of Medicine in Dayton, Ohio in combined internal medicine & pediatrics and became board certified in internal medicine & pediatrics in 2012. After briefly working in Ohio, he joined Emory Saint Joseph's Hospital in 2012 as a hospitalist and serves currently as the director of hospital medicine. Desai also works at CHOA several times per year as a pediatric hospitalist. About three years after being signed into law, one provision of the Inflation Reduction Act (IRA) that is of particular importance to 340B hospitals is about to take effect: Medicare price caps. Jan. 1, 2026, marks the date that the first 10 Medicare drugs will be subject to a maximum fair price (MFP). Meetali Desai, director of pharmacy business services at UMass Memorial Medical Center, joins us to explain how this will affect 340B hospitals and health centers such as hers.The Good and Bad News for Covered EntitiesThe good news, Desai says, is that there is potential for the 340B ceiling prices to go down for certain medications. This is because the MFP will become the new “best” price in the formula for calculating 340B prices. However, because the law will cap reimbursement rates to MFP when Medicare patients receive those drugs, 340B hospitals will see their 340B savings amounts go down for those dispenses.Updated Calculator Can Help Hospitals Gauge Potential Impact340B Health recently updated its calculator for hospitals to use to estimate the effects of MFP pricing based on the newest data. This new calculator allows users to project what the potential impact from the IRA could be on a hospital, including if drugmakers decide to lower their list prices significantly to avoid IRA inflation penalties. This drop in prices of Medicare drugs could result in 340B hospitals seeing higher ceiling prices and reduced savings.Reduced Savings Could Impact Patient CareDesai says Medicare price caps, combined with other financial pressures on hospitals, could mean some rough times ahead for hospitals that care for a large proportion of low-income patients.
