Happy New Year!  We are very happy to be back after a two-month Hiatus during which time we have been working hard to bring you diverse slate of shows with some great guests in 2024.    As we kick off 2024 our guest today in Russell Ducharme manager of the City of Racho Cordova's Neighborhood Services.  The Neighborhood Services Division's goal is to grow and maintain strong and vibrant neighborhoods through five main programs: Code Enforcement, Animal Services, Parking Enforcement, Rental Housing, and Homeless Outreach. Collaborating with and educating our community about municipal codes, health and safety requirements, and animal services laws creates healthy neighborhoods, happy residents, as well as a sense of community and shared pride. We had a wide-ranging discussion with Russell who told us about the many services Rancho Cordova Neighborhood Services provides the residents of Rancho Cordova. September 11, 2023 -- How much will Fort Bragg's innovative Care Response Unit save the county's general fund?  Supervisors Ted Williams and Dan Gjerde brought Fort Bragg Police Chief Neil Cervenka to the Board of Supervisors Meeting on September 12 to provide an overview of the innovative homeless response program initiated by the city and managed through the police department.  The program known as “CRU” or Care Response Unit utilizes a non-enforcement type of interaction to help the homeless get what they need and hopefully get them off the street. The CRU team has served 321 individuals and successfully reconnected 22 homeless individuals with family or friends, persuaded 18 into rehab programs and moved 7 into permanent housing. The program which was initiated by Mayor Norvell got its start with grant funding from the State of California's Behavioral Health Justice Intervention Services Project.  Funding for the program through grant and city funds will only last through mid-2024.  Cervenka asked the BOS to fund the program for four years at a cost of 1.72 million using Measure B dollars.  Measure B is tax money that was designed to fund the construction of psychiatric and behavioral health facilities, operation costs, and services to treat mental illness and addiction.So how does the program save the money in the county's general fund? According to Chief Cervenka it costs a minimum of $107 dollars per day to incarcerate an individual in the county jail and goes up based on health and psychiatric needs. Since the CRU program's implementation there has been a sea change in how the Fort Bragg Police Department deals with the homeless and spends its officer hours.  There has been an overall 27 percent decrease in calls for service involving homeless from the 21/22 fiscal year to the current fiscal year.  The most dramatic being 242 calls in December of 2021 versus 58 in December of 2022.  There has been a 20% decrease in arrests of homeless and an increase in overall arrests.The program also includes contract staffing for Project Right Now, which helps youth with substance abuse disorders fight addiction. The Board of Supervisors directed staff to determine if current funding for homeless services, that is not as effective, can be redirected to CRU and agreed to send Cervenka's request to the Measure B Committee for consideration.  Williams also asked staff to determine how much money the CRU program has saved the county, presumably, due to reduced jail expenses.

“Too Close or Too Far: Setting Best Boundaries with Clients” will be provided over four days in 2-hour segments via distance learning due to the COVID-19 context. This training focuses on expanding the practice, knowledge, and skill of outreach workers, social workers, case managers, peers, and psychologists who work in Homeless Outreach services in Los Angeles County. The format of each day will include 1 hour of didactic training on concepts, theory, and research related to interpersonal boundaries in client-outreach worker relationships followed by 1 hour of vignette-based activities and facilitated discussion. The didactic portion will begin with an overview of key terms related to interpersonal closeness in therapeutic work including boundaries, boundary clarification, boundary-setting, transference, counter-transference, empathy, and autonomy, with an emphasis on building self-reflection skills around these concepts. The trainers will explore with participants how to recognize when boundary clarification is needed, as well as consider implications of client-provider power dynamics, culture, and professional background/training on their experience and management of boundaries with clients. Additionally, this training will employ a trauma informed lens, to help providers best understand how past experiences may shape their clients' approach to boundaries, as well as their own (Levenson. 2017). Participants will be taught to reflect on internal and interpersonal client experiences utilizing concepts such as the neglect-overprotect continuum and moral distress (Mänttäri-van der Kuip, 2019; Marsh & Kelley, 2018). Trainers will also guide participants in identifying personal values and characteristics that impact their experiences of setting boundaries with clients. Finally, provider wellbeing will be discussed, with a review of strategies to prevent burnout including radical acceptance and eliciting or providing quality supervision (Begic et al., 2019; Linehan, 2015). The vignette-based activities and discussion will focus on four types of common boundary situations that can come up in work with unsheltered individuals: avoidance, over-identification, sexualized transference, and countertransference. Participants will be encouraged to apply concepts from didactic portion of each day's training to the vignettes, including identifying boundary-setting skills for each scenario. Schools are back but they are still dealing with the setbacks and learning loss all due to the pandemic.  Recently we saw Department of Education data showing 9-year-olds are behind in reading and math, the sharpest decline we've seen since 1990.  The learning loss was generally worse in districts that kept classes remote longer.  To combat this, states are spending billions on tutoring, expanded summer school, and more individual attention for students.  Scott Calvert, reporter at the WSJ, joins us for more on what schools are doing and how they are tracking progress.   Next, we'll tell you about LA's first street psychiatrist trying to tackle mental health in the homeless population where it is desperately needed.  Dr. Shayan Rab leads the Homeless Outreach and Mobile Engagement (HOME) team and approaches homeless individuals with the hopes of getting them long-term treatment and off the street.  Since this street-based treatment is not done in a traditional setting, Dr. Rab and his team must get creative and resourceful in how they provide help and even fill prescriptions.  Doug Smith, senior writer at the LA Times, joins us for how street psychiatry is trying to pull some out of homelessness.See omnystudio.com/listener for privacy information.

Dr. Roberto Mezzina headed the Dipartimento di Salute Mentale in Trieste until his retirement in late 2019.  Both of the California delegations that visited Trieste (in 2017 and 2019) were graciously welcomed by Dr. Mezzina and his staff in our quest to understand the culture and principles that make this system so noteworthy.   Within those two delegations were representatives from the Los Angeles Sheriff's Department (LASD), harboring a deep desire to embrace a more human-focused posture on how to come alongside people with mental illness, both in the county jail system and in the community.  The Department is also interested to lean into guiding principles of the U.N. Declaration of Human Rights – which is espoused by the Italian system – as a framework to guide how people with mental illness are cared for in our society.In April 2022, Dr. Mezzina was invited to Los Angeles at the behest of the L.A. Sheriff's Foundation to come alongside some specialized divisions in the Los Angeles Sheriff's Department:  the Homeless Outreach Services Team (HOST) and the Mental Evaluation Team.    Dr. Mezzina also toured the remarkable program that was initially started in 2016 at Twin Towers to provide a more caring and therapeutic environment for the most seriously mentally ill patients in the jail.  Under the leadership of the Sheriff's Department and the Department of Health Services Correctional  Mental Health division, this program, described in an article which appeared in the Christian Science Monitor,  has expanded.  (See website created by the Mental Health Assistants for more information about this innovation.)Dr. Mezzina's visit to Los Angeles coincided with the filming of an episode of the CNN documentary series, “Life with Lisa Ling.” This episode (to be aired in the show's next season) will focus upon the mental health crisis in Los Angeles County.  The producers and host of this show participated in some of the outreach and engagement conducted by the LASD HOST Team and Dr. Mezzina.   This interview was conducted in person in the podcast studio at Verdugo Sound on the way to the airport.  Dr. Mezzina also provides an update on the changing political climate in Trieste – a topic that was introduced in a podcast interview that can be found in Season Two, Episode Seven.

My guest this week is Garrett Underwood, Founder/Executive Director of Seed House Project, a live-work home for at-risk youth. He is also the owner of Malachi Clothing, a well-known Christian Clothing company in Southern California. Garrett oversees all programs within the Seed House Project. His goal is to reinvent the transition for youth to adulthood using technology and new innovative ideas to create change within the homelessness in LA. He has now housed more than 100 people, and he has created hope for each person who walks through his home. 3:10 – Garrett 101 He started with a clothing company and gave hygiene packages to homeless people. His heart expanded and he wanted to do more to help homeless people. 4:39 – The fixer When Garrett sees a problem, he wants to try to fix it. He moved into a trailer for six months with no running water and got to see what other people experienced, like loneliness and embarrassment. 8:55 – Homeless issue Garrett visited skid row when he was 20, and he previously visited homeless shelters. He has always loved kids, and especially wanted to help them. 12:55 – Common threads Garrett started seeing common threads of things that made it difficult for people to improve their lives. His definition of homelessness is not the absence of a home or a building. His position entails overseeing daily operations, meeting financial quota, and most importantly managing all programs within the organization. Some of the programs consist of; Self actualization development, Business or Non - Profit Execution plan, Credit building, Financial literacy and Emotional Intelligence Development. Mr. Underwood received his Masters at Azusa Pacific for Organizational Behavior & Development. Previously before attending Azusa he started a Faith- Based Clothing Company called Malachi Clothing. The business model was “ buy one give one” for every purchase a hygiene kit was given away to someone in need, In return the customer would receive a free prayer bracelet with the name of the person that received the care package. The company partners with over 50 shelters & group homes across Los Angeles, and gave over ten thousand care packages a year. Unfortunately the demands of recourse increased and care packages wasn't enough to fulfill the need, which lead Mr. Underwood to make a major shift in his career. In 2016 Mr. Underwood decided to dive deeper in the homeless crisis in Los Angeles. He volunteered at all the major organizations within Los Angeles, The Dream Center, My Friends Place and PATH ( People Assisting The Homeless) to name a few. Two years later Mr. Underwood became the lead Coordinator for the Homeless Outreach for all over the Los Angeles Country as well as the Program Manager at PATH. He also took on as the lead site counselor at Covenant house, the leading housing program for homeless youth in Los Angeles. During his experience he began to see a common thread within the many different issues within the homeless crises in Los Angeles. He noticed lack of affordable housing was a big issue but he also took note of the physiology of hope. Those that had something to look forward to, whether a job, school or family 90% of them either obtained or maintained their housing. This static led Mr. Underwood developed an employment program that created purpose. Working with the shelters he previously partnered with, he conducted workshops around storytelling and the impact one story can have. The participants would have the opportunity to share their story within the jewelry they created. The jewelry was then distributed to stores across LA, therefore sharing their story to hundreds of people. This created purpose and employment. Within the first year Mr. Underwood was able to employ over 98 individuals. The one other issue he couldn't solve was affordable housing. He decided to donate his home and move into a trailer to meet the need in whatever way he could. He was able to house 16 people at a time. Three years later Mr. Underwood has now housed over a 100 people and created hope for each person that walks through his home. He has perfected the art of finding one's purpose with extensive assessment tools and a curriculum to how you can turn your purpose into a program to serve others.
