And we are back for Part II of our discussion with Dr. Brian Lau. Our conversation picks back up with a discussion of surgical treatment options for more significant glenoid bone loss. There is a 2021 yellow journal article titled Diagnosis and Management of Traumatic Anterior Shoulder Instability that nicely outlines a treatment algorithm based on percentage of glenoid bone loss as well as the presence and severity of a Hill Sachs lesion. Matt Provencher and colleagues explain that good results can be expected after Bankart repair in on-track Hill-Sachs lesions with glenoid bone loss less than 13.5%. Bankart repair without additional procedures is not recommended in off-track Hill Sachs lesions, regardless of the extent of glenoid bone loss. Bone block procedures are recommended when glenoid bone loss is greater than 20% to 25% or when the Hill Sachs lesion is off-track. Then, from the April issue of AJSM this year, we discuss the study performed by our guest Dr. Lau and his team at Duke, titled Distal Clavicle Autograft Versus Traditional and Congruent Arc Latarjet Procedures. This laboratory analysis compared five different configurations of two local autograft options – coracoid and distal clavicle – using both 3D CT and 3D MRI. They looked at how much the glenoid surface area was augmented (important to address the glenoid bone loss) and the amount of glenoid apposition provided (bone-to-bone contact being important for graft healing). The authors found that the congruent arc Latarjet procedure had the largest graft surface area, the standard Latarjet procedure had the most bone-on-bone contact and the distal clavicle attached by its inferior surface had the largest graft width. This paper also found that differences between 3D CT and 3D MRI were small and likely not clinically significant.We finish up our conversation with a focus on rehab and returning to play after shoulder stabilization surgery. The last article we reference is titled Criteria-based Return-to-Sport Testing is Associated with Lower Recurrence Rates following Arthroscopic Bankart Repair. Albert Lin and colleagues and UPMC evaluated the use of a criteria-based return-to-sport testing protocol, which includes assessments of strength and function using the closed kinetic chain upper extremity stability test and the unilateral seated shot-put test. The authors found that athletes who underwent this testing protocol to guide their clearance to return to sports had a lower rate of recurrent instability than those cleared to return based on the time from surgery (5% vs. 22%).
On today's episode we're focusing on anterior shoulder instability with Dr. Brian Lau, orthopedic sports medicine surgeon, team physician for Duke Athletics and Director of the FIFA Medical Center at Duke.We have some great articles for you that contribute well to our conversation on the treatment of primary anterior shoulder instability. We'll start off our discussion today with a level I RCT published in the March 2020 issue of JBJS titled Primary Arthroscopic Stabilization for a First-Time Anterior Dislocation of the Shoulder, a single-center double-blinded clinical trial compared arthroscopic washout to arthroscopic Bankart repair for the management of primary anterior shoulder instability. At an average follow up of 14 years, the rate of recurrent dislocation was significantly higher in the washout group compared to the Bankart repair group, at 47% compared to 12%. The arthroscopic Bankart repair group also demonstrated significantly better clinical outcome scores, including the WOSI and DASH scores. Then, from the June issue of JSES this year, we review the publication titled Remplissage reduces recurrent instability in high-risk patients with on-track Hill-Sachs lesions. Albert Lin and Pat Denard performed a multicenter retrospective study of patients with on-track Hill Sachs lesions who underwent arthroscopic Bankart repair with or without the addition of a Remplissage procedure. We'll discuss on-track versus off-track Hill Sachs lesions, how you go about calculating this and what to do with this information in a little bit with our guest. So, for now, we'll just focus on the results of this study, which showed that the addition of a remplissage was associated with a lower rate of recurrent dislocation (1.8% vs. 11%) and revision surgery (0% vs. 6%). Remplissage protected against recurrent instability, particularly in high-risk patients.We are joined today by Dr. Brian Lau, a board-certified orthopedic surgeon dual-fellowship-trained in both sports medicine surgery and foot & ankle surgery. Dr. Lau obtained his medical degree from the University of Pittsburgh and completed his orthopedic residency at UC San Francisco. He then went on to complete two fellowships – the first in Sports Medicine and Shoulder Surgery at Duke University and the second in Foot & Ankle Surgery at Stanford University. Following training, Dr. Lau returned to Duke University, where he is a team physician for Duke Athletics and the Director of the FIFA Medical Center at Duke. Dr. Lau is the associate program director of the Duke Orthopedic Sports Medicine and Shoulder fellowship and serves on numerous educational committees in AOSSM, AANA and ASES. He is passionate about research and leads the Duke Sports Medicine Research Committee.

In this episode of the Psychedelic Medicine Podcast, Joel Castellanos joins to discuss the emerging topic of psychedelic therapies for pain-related conditions. Dr. Castellanos is a Board certified physical medicine and rehabilitation and pain management physician and an associate professor in the department of anesthesiology at UC San Diego school of medicine. He earned his medical degree from University of Toledo college of medicine and has since completed a two year program in healthcare administration during his residency at University of Michigan, as well as a fellowship in pain medicine at UC San Diego school of medicine. Most recently he completed the integrative medicine fellowship from the University of Arizona. In this conversation, Dr. Castellanos introduces the topic of psychedelics as a therapeutic modality for various pain conditions. Dr. Castellanos explains that much of the pain management field focuses on neuromodulation as the basis for particular therapeutic techniques, so psychedelics—which have profound modulating effects within the brain—naturally seem promising. However, research into this topic is still developing, with studies from the seventies providing some initial data which is now being corroborated and expanded by contemporary research. Dr. Castellanos shares the details of this work, explaining how combined psychedelic treatment and mirror box therapy could prove to be particularly efficacious for individuals suffering from phantom limb pain. In light of the upcoming trials led by Dr. Castellanos and his colleagues at the Psychedelic Health Research Initiative at UC San Diego, he also discusses certain special considerations for working with amputees as guides or facilitators of psychedelic therapy. The double blind randomized controlled trial investigating the efficacy of psilocybin therapy for phantom limb pain will be conducted first and is currently enrolling. This study seeks to test the efficacy of psilocybin therapy for this condition as well as elucidate its potential mechanism of action through brain imaging. Click here to enroll. Dr. Albert Lin is father to two wonderful children and an internationally renowned scientist, innovator, technologist, and entrepreneur. He's the director of the Center for Human Frontiers at Qualcomm Institute, University of California, San Diego, and an award-winning National Geographic explorer and host of the fascinating uber popular show, Lost Cities with Albert Lin on National Geographic. Albert received his PhD in material science and engineering from University of California, San Diego. He has founded and directed numerous programs, including the UC San Diego National Geographic Engineers for Exploration Program, the Exploration Lab, the Distributed Health Labs, and the international effort known as the Valley of the Khans Project in collaboration with National Geographic. This podcast is a conversation with Albert Lin, probably best known for his work as a presenter on National Geographic TV. Most recently he has appeared on Lost Cities with Albert Lin. In that series, Albert used drone-mounted Laser scanning which can punch through the vegetation to reveal ancient archaeological features on the landscape. I was fortunate to work with with Albert on two programmes in the series: one on Petra in Jordan and the other on Macchu Picchu in Peru Albert describes himself as an Explorer, a Scientist and a Communicator and his work testifies to that. He led an incredible project to try and find the lost tomb of Genghis Khan, which used crowd-sourced data to make new discoveries in a remote Mongolian sacred area, where few had ever travelled. However, what makes Albert unique for me is his incredible optimism and positivity towards life in general. He is someone who grabs life as it rushes by and holds on for the ride. A fantastic musician, he would often be picking away at a guitar or some other stringed instrument he had just picked up as we bounced around rough roads on our way to location. And the second part of the conversation really expresses everything about Albert. How he took a deeply traumatic experience and transformed it into an opportunity for learning and understanding. Experiencing horrific phantom pain after the loss of his leg in a car accident, Albert set out to try and find a solution that wasn't based on the addictive and truly dangerous opioids available. That led him to using psychedelic drugs, along with Neuroscientific expertise, to reset his brain and end the pain. Now Albert researches how to use psychoactive substances to treat pain, but he also leads a research project which uses cellphones to create prosthetic limbs for some of the world's 40 million amputees, many of whom are immobilised by their condition. Learn how Dr. Albert Lin collaborated with his mentor Dr. Ramesh to transform a personal tragedy into a viable solution for more than 40 million amputees. INK talks played a major role. Albert Lin, a materials scientist and researcher with the Qualcomm Institute (QI) at UC San Diego, has created a project to change that.With the help of students and multidisciplinary experts at QI, Lin launched “Project Lim[b]itless,” an initiative that leverages cellphone and 3D printing technology to significantly reduce the cost and time it takes to produce the custom sockets of a prosthetic limb. The team’s ultimate goal is to make prostheses more accessible, particularly for people in rural communities and developing countries.For Lin, helping amputees regain their mobility and re-master the physicality of life is very personal. In 2016, he lost his right leg below the knee to a vehicle accident. With the help of state-of-the-art prosthetic shaping, Lin has been able to maintain an incredibly active lifestyle and pursue dream projects across the globe.Speaker ProfileDr. Ramesh R. Rao. Dr. Rao has been a faculty member at UC San Diego since 1984, and Director of the Qualcomm Institute, UCSD division of the California Institute for Telecommunications and Information Technology (Calit2), since 2001. He holds the Qualcomm Endowed Chair in Telecommunications and Information Technologies in the Jacobs School of Engineering at UCSD, and is a member of the school’s Electrical and Computer Engineering department.Prior to QI (Calit2), Professor Rao served as the Director of UCSD’s Center for Wireless Communications (CWC). Dr. Rao is involved on a day-to-day basis with a wide variety of research initiatives at QI. He leads several major interdisciplinary and collaborative projects and has been a PI on dozens of federal-, state-, foundation- and industry-funded grants. Dr. Rao is an IEEE Fellow and Senior Fellow of the California Council on Science and Technology. He was named a Member of the Board of the Academy of Neurosciences for Architecture, a Member of the Rady Children’s Hospital and Health Center Board IT Task Force and serves on the Board of Directors of CONNECT. Dr. Rao has been a long time member of the San Diego Indian American Society and currently serves as the Vice President for the Board of Governors.Dr. Rao earned his Ph.D. in Electrical Engineering from the University of Maryland, College Park in 1984, after receiving his M.S. from the same institution in 1982. He earned his Bachelor’s degree in 1980 from the University of Madras (the National Institute of Technology, Tiruchirapalli). Dr. Rao received the distinguished alumnus award from the National Institute of Technology in 2010, the ECE Distinguished alumni award from the University of Maryland in 2012, the Professional Gordon Engineering Leadership Award from UCSD’s Gordon Engineering Leadership Center in 2010. He also received the 2011 Casa Familiar Abrazo Award for engagement with underprivileged area of San Diego.Albert Yu-Min LinDr. Lin is an award winning scientist, technologist, explorer, and adventurer with a knack for storytelling both on stage and the big screen. As a problem solver, he has reinvented how we explore and has made headlines around the world for his expansive work combining technology with exploration. His projects range from an effort to search for the tomb of Genghis Khan in Mongolia (using satellites, crowdsourcing, drones, and ground penetrating radar) to expeditions remapping major sites in China and Guatemala (using satellites and Lidar) to his most recent efforts to redefine human bionic capabilities with bio-monitors and 'flow' experiments.
