When the Lord spoke to Paul He said there would be no harm to him - but that did not mean there wouldn't be difficulties. But notice here - no harm did come to him- Here we see the interposition of the magistrates on behalf of Paul- The magistrate keeps Paul from harm.----And this was huge. Had Gallio sided with the Jews - scholars point out that this would have slowed the preaching of the Gospel for the next 10-12 years. Then there was a change in Roman rule under Nero decidedly against Christians. But those 10-12 years running up till then would have been decidedly different. Gallio was held in esteem - had his judgment gone against Paul - all other proconsuls would hear of his verdict on this - and they too would have made things hard on the Christians. Because of the interposition of this proconsul - the Christian faith was given liberty to be proclaimed.----This demonstrates once again - the impact of the magistrates concerning the Gospel - whether for good or for evil. And I have seen this in our days. From a judge to a policeman - they can either help proliferate the spread of the Gospel - or hinder it. This is why churchmen have always believed it important to instruct the magistrates - to speak to the civil realm regarding their office- instructing them in their duty in the sight of Christ. Capsular interposition arthroplasty is a joint- and motion-sparing procedure that has been shown as an effective alternative to fusion. This study aimed to evaluate patient function and satisfaction after long-term follow-up. The mean VAS for pain was 7.9 preoperatively and 1.8 postoperatively (P = .003). The mean preoperative SF-12 physical score was 42.0 vs 64.2 postoperatively (P = .02). The mean preoperative SF-12 mental score was 50.7, while the postoperative SF-12 mental score was 54.6 (P = .01). The total FFI score also significantly improved, with a preoperative value of 98.3 and a postoperative mean score of 49.6 (P = .001). The mean patient satisfaction score was 7.4 of 10. Overall, 39 of 42 patients (92.9%) stated they would have the surgery again. Four of the 42 patients (9.5%) required conversion to hallux metatarsophalangeal fusion at a mean of 6.1 years after the index procedure secondary to pain, but no other complications were reported. Interposition as illustrated in Psalm 69 contains the idea of coming between two parties in order to bring peace. To rescue one by intervening on his behalf. Noah Webster comments in his historic dictionary in reference to the term: “how many evidences have we of divine interposition in favor of good men!” – Surely Psalm 69 is one of those sterling examples Webster had in mind. Here is the doctrine of interposition applied in its ultimate instance – the Gospel. David lifts his song of petition to the Lord, pleading that God would intervene on his behalf. Whatever provisional answer David may have received to his plea at the time, such as deliverance from Saul, or victory over a warring neighbor nation – the ultimate fulfillment of the cry of Psalm 69 would come in Jesus Christ, the Son of David, who would interpose on behalf of all of the elect. Standing in their place of deserving judgement providing full and final atonement for their sins! Thus David's cry for God to interpose and save him is answered in the incarnation when God Himself takes on flesh, steps into history, and endures all of the sufferings described in Psalm 69 on behalf of David and all God's people for all time.

Background: Oronasal fistulas are a frequent complication after cleft palate surgery. Numerous repair methods have been described, but wound-healing problems occur often. The authors investigated, for the first time, the suitability of multilayered amniotic membrane allograft for fistula repair in a laboratory experiment (part A), a swine model (part B), and an initial patient series (part C). Methods: In part A, one-, two-, and four-layer porcine and human amniotic membranes (n = 20 each) were fixed in a digital towing device and the force needed for rupture was determined. In part B, iatrogenic oronasal fistulas in 18 piglets were repaired with amniotic membrane allograft, autofetal amniotic membrane, or small intestinal submucosa (n = 6 each). Healing was evaluated by probing and visual inflammation control (no/moderate/strong) on postoperative days 3, 7, 10, and 76. Histological analysis was performed to visualize tissue architecture. In part C, four patients (two women and two men, ages 21 to 51 years) were treated with multilayered amniotic membrane allograft. Results: In part A, forces needed for amniotic membrane rupture increased with additional layers (p < 0.001). Human amniotic membrane was stronger than porcine membrane (p < 0.001). In part B, fistula closure succeeded in all animals treated with amniotic membrane with less inflammation than in the small intestinal submucosa group. One fistula remained persistent in the small intestinal submucosa group. In part C, all fistulas healed completely without inflammation. Conclusions: Amniotic membrane is an easily available biomaterial and can be used successfully for oronasal fistula repair. The multilayer technique and protective plates should be utilized to prevent membrane ruptures.

Ziel dieser retrospektiven Studie war es, die Erfahrungen mit der koloanalen Handanastomose nach Rektumresektion als letzte Möglichkeit zum Kontinenzerhalt und somit als Alternative zur abdominoperinealen Exstirpation aufzuzeigen. Diese Operation wurde in den Jahren 1992 bis 2003 bei 57 Patienten 59-mal durchgeführt. Die Indikationen zur koloanalen Anastomose waren neben Rektumkarzinomen (39 Patienten, darunter 4 Rezidive) und Fisteln (8 rektovaginale, je 1 rektovesikale und rektourethrale Fistel), 3-mal Rückverlagerungen nach Diskontinuitätsresektion, 2 Rektumstenosen, 3 Kolonischämien, einmal eine Anastomoseninsuffizienz und einmal ein das Rektum infiltriertes Prostatakarzinom. Eine Stapler-Anastomose konnte bei diesen Patienten auf Grund von intraoperativen Komplikationen, notwendiger Nachresektion, engem und adipösem Becken, oder wegen sehr tief gelegenem Tumor oder Läsion nicht erfolgen. Als Grunderkrankungen lagen kolorektale Tumoren (48-mal), Tumoren des Urogenitaltraktes (2x Prostata-Ca, 1x Zervix-Ca, 2x Corpus-Ca) eine Endometriose, eine Prokotolokitis und ein Dammriss vor. Am Dickdarm sind 17 Patienten voroperiert worden (15-mal tiefe anteriore Rektumresektion mit Stapler-Anastomose, einmal Sigma-Resektion, einmal Rectotomia posterior). Dazu kamen 6 Hysterektomien im Vorfeld der koloanalen Anastomose. Im Bereich des kleinen Beckens waren 24 Patienten vor der Operation bestrahlt worden, dabei 15-mal im Sinne einer neoadjuvanten Radiochemotherapie bei Rektum-Ca. Die Operationen wurden im Sinne der von Parks definierten Methode mit transanaler Mukosektomie, Herunterziehen des Kolons und Anastomosierung nach Exposition mittels Analspreizer durchgeführt. Die Anastomosierung erfolgte auf Höhe der Linea dentata, intersphinktär wurde 16-mal reseziert. Die Rekonstruktion wurde 52-mal als End-zu-End Anastomose, 2-mal mit J-Pouch, 2-mal mit ileozäkaler Interposition und 3-mal mit Koloplastikpouch durchgeführt. 56-mal wurde ein protektives Stoma angelegt. Die mittlere Operationsdauer betrug 306 Minuten, der postoperative Krankenhausaufenthalt im Durchschnitt 22 Tage. Ein Patient ist an Peritonitis nach Nekrose des anastomosierten Kolons verstorben. Die Anastomoseninsuffizienzrate betrug 37,3% (22-mal), die Nekroserate 13,6% (8- mal). Bei 19,7% (10-mal) kam es zur Anastomosenstenose, 9-mal zu Fisteln (5 rektovaginale Fisteln, darunter 2 Rezidive; 3 rektovesikale und eine rektorektale Fistel). 9-mal musste während des Krankenhausaufenthaltes relaparatomiert werden (6-mal Exstirpation bei Nekrose, 2-mal erneute koloanale Anastomose nach Nekrose, einmal wegen Magenblutung). 32 Patienten mit Rektumkarzinom wurden erstmals operiert (12% nicht-invasive- Karzinome, 32% Dukes A, 32% Dukes B, 25% Dukes C-Stadium). Die mittlere Tumorhöhe war 4,5 cm ab Anokutanlinie. Bei diesen 32 Patienten kam es 8-mal zum Tumorprogress (25%, 10-127 Monate nach Op). Die 3- bzw. 5-Jahresüberlebenszeit aller wegen Rektumkarzinom operierter Patienten betrug 77%, bzw. 80%. 35 der 56 protektiven Stomata konnten im Mittel nach 17 Wochen rückverlagert werden (63%), 6-mal war wegen Nekrose, 2-mal wegen Todes, 5-mal wegen raschem Tumorprogress und 6-mal wegen Sphinkterschwäche eine Rückverlagerung nicht möglich. Die Sphinkterfunktion wurde präoperativ bei 11, postoperativ bei 28 Patienten, davon 17-mal nach manometrischer Untersuchung als vermindert dokumentiert. Nach Befragung zur Kontinenz wurden 9 Patienten als Grad 3 (38%), 8 als Grad 4 (33%) und 7 als Grad 5 (29%) nach Kirwan-Parks eingeteilt. Die vorgestellte Operationsmethode kann dem Patienten bei klinisch schwierigen Situationen, insbesondere bei intraoperativ schwierigen Gegebenheiten, einen endgültigen künstlichen Darmausgang ersparen. Doch sollten in Anbetracht der hohen Komplikationsrate und der als mäßig zu erwartenden Kontinenz eine sorgfältige Patientenauswahl und falls die vorgestelle Operation

Congress seriously considered conscription, but Daniel Webster's speech against conscription killed it. Interposition is emphasized. The Hartford Convention was opposed to the war. Secession sentiment was still alive. It was a reasonable and peaceful way of resolving differences. The issue of a national bank took up much attention in the 1830s. The Supreme Court had declared a national bank to be constitutional. The Fugitive Slave Act turned many Northerners into abolitionists.
