Today's episode comes from a running injury from Jamie. He wanted to know about hallux rigidus. He asked: “Are there any non-surgical approaches to helping hallux rigidus when you are not a good candidate for a cheilectomy?” What can a runner do when you have hallux rigidus but you're not a good candidate for cheilectomy surgery? Well, that is what we're talking about today on the Doc On The Run Podcast.
Pain in the hallux metatarsophalangeal joint (MTPJ) is very common, yet the underlying etiology remains unknown. Previous clinical research and biomechanical research has implicated stenosing flexor hallucis longus (FHL) tendonitis as a possible cause. The hypothesis of this study was that treatment solely focusing on alleviating restricted FHL excursion would be beneficial in patients with hallux MTPJ pain. In conclusion, previous biomechanical studies have suggested that stenosing FHL synovitis can cause increased intraarticular loading in the hallux MTPJ. The current clinical study supports this hypothesis, demonstrating that treatment focused on relieving restricted FHL excursion can ameliorate pain in the hallux MTPJ in select cases. Wie soll in Zukunft die Gesundheitsfinanzierung funktionieren? 14 Jahre arbeitete das Parlament an einer Reform. Kompromisse wurden gemacht. Bundesrat und eine Mehrheit im Parlament sagten «Ja» zur Vorlage. Der VPOD ergriff das Referendum und warnt vor einer «Scheinreform». Am 24. November stimmt die Schweiz über die einheitliche Finanzierung von ambulanten und stationären Leistungen (EFAS) im Gesundheitswesen ab. Darum geht es Leistenbruch oder Operation des Hallux valgus, was früher einen mehrtägigen Spitalaufenthalt zur Folge hatte, kann heute auch ambulant operiert werden. Will heissen, der Patient kommt morgens und kann einige Stunden nach der Operation wieder nach Hause gehen. Das kostet definitiv weniger als eine oder zwei Übernachtungen im Spital. Wer bezahlt heute? Bei ambulanten Eingriffen zu 100 Prozent die Krankenkasse. Beim Spitalaufenthalt bezahlt die Krankenkasse etwas weniger als die Hälfte, der grosse Rest übernimmt der Kanton. Das soll sich ändern EFAS, die einheitliche Finanzierung von ambulanten und stationären Leistungen will, so wie es im Namen steht, eine einheitliche Finanzierung. Rund dreiviertel der Kosten soll in Zukunft die Krankenkasse bezahlen, der Rest der Kanton. Zudem will man auch die Langzeitpflege in dieses neue Finanzierungsmodell einbinden. Das sagt das Ja-Komitee * Die Prämienzahlenden werden über alles gesehen entlastet * Unnötige Behandlungen und Spitalaufenthalte werden vermieden * Pflegeeintritte ins Heim werden verzögert, was Kosteneinsparungen bedeutet Das sagt das Nein-Komitee * Die Krankenkassenprämien werden durch EFAS weiter steigen * Die Pflegeheimkosten für die einzelne Person werden noch höher ausfallen * Der Druck auf das Pflegepersonal wird nochmals zunehmen, darunter wird die Qualität der Pflege leiden. When you have hallux rigidus, the problem is the big toe joint does not want to move. Many doctors will simply tell you that your big toe joint, or the "first metatarsal phalangeal joint" is getting progressively stiffer and developing arthritis. If you wear shoes that irritate the joint more, the condition will only gets worse. The question is what sandals won't make hallux rigidus worse? What sandals can I wear with hallux rigidus? Hallux interphalangeal joint arthrodesis (HIPJA) is indicated for a variety of pathologies. Despite high nonunion rates, techniques remain unchanged. The aim of this study is to examine nonunion and complication rates and describe risk factors for treatment failure. In conclusion, our cohort represents the largest single-center series of HIPJA procedures published to date. We found relatively high nonunion and reoperation rates with standard current techniques. One of the problems with being a runner is that you have a higher pain threshold. If you have a minor attack of gout, it may not bother you as much as it would other people and what Gout is, is that you get painful crystals forming within a joint like the big toe joint. If you have what we call sub-acute gout, meaning it's not really killing you, it's just kind of a minor thing that's building up gradually, then the condition might actually sort of fly under the radar. You might be gradually building up crystals in the joint that you're not really aware of at all. How can gout lead to hallux rigidus? I was just on a call with a runner who has had this condition called "hallux rigidus." and it's where your big toe joint starts to get stiff, becomes rigid, and it doesn't move as much. Hallux rigidus is a progressive condition, especially if you continue to irritate the joint. You can damage the joint cartilage. The stiffer the big toe joint gets, the more pressure on the cartilage when the big toe is trying to fight that stiffness. Sometimes that movement hurts. He was asking me about the options on different injections. What are the risks between an injection like a PRP or platelet rich plasma injection versus something like a cortisone injection when you have hallux rigidus? If you have a condition called "hallux rigidus" or "hallux limitus," the name tells you what's wrong. "Hallux" means big toe. "Rigidus" means the big toe joint doesn't move at all. "Limitus" just means the big toe joint movement is limited and stiff. There are three problems with hallux rigidus, which are: 1) damage to the cartilage, 2) bone spurs around the joint and 3) restriction of the soft tissues such that the toe doesn't move up and down the way it should. Those three reasons that cause the condition are the same reasons that can fail if you have a cheilectomy surgery. Today on the Doc On The Run Podcast we're talking about three reasons for cheilectomy failure after hallux rigidus surgery.

One time at a medical conference, an expert lecturing on biomechanics said, “When a runner develops Hallux Rigidus, he becomes a swimmer instead of a runner.” Most of the doctors in the audience laughed. I really didn't think that was very funny. I actually have hallux rigidus myself, and it doesn't disrupt my running. It is true Hallux Rigidus can cause pain and swelling in the big toe joint. If you aren't careful the joint can get destroyed. If you understand a little bit about the mechanics of the joint, it may help you understand how to avoid the arthritis that can hamper your ability to run in the future. If you get pain and swelling and discomfort particularly in and around the big toe joint, you may have a condition called hallux limitus or hallux rigidus. Hallux limitus and hallux rigidus are both conditions that affect the big toe joint. It causes pain right where your big toe attaches to your foot. Many runners with this condition don't even understand the difference between hallux limitus and hallux rigidus. There are really a few things that define the difference between these two conditions. Hallux rigidus is the second most frequent pathology of the first ray. Surgical options for degenerative metatarsophalangeal joint disease are either joint destructive or conservative procedures. The hypothesis was that oblique distal shortening osteotomy of the first metatarsal is an effective conservative technique for the management of stage 1 to 3 hallux rigidus. In conclusion, oblique distal osteotomy of the first metatarsal for stage 1-3 hallux rigidus, often in combination with other first ray procedures, performed well during our follow-up time period, with a high subjective satisfaction rate and few complications.
