Matthew Martin, Kunoor Jain-Spangler, Adrian Dan, and Vincent Cheng for this EXCELLENT Journal Review in Bariatric Surgery. Article #1: Stenberg 2023 - Long-term Safety and Efficacy of Closure of Mesenteric Defects in Laparoscopic Gastric Bypass Surgery Two mesenteric defects are created during Roux-en-Y gastric bypass (RNYGB) Petersen's Defect Jejuno-jejunostomy mesenteric defect Consensus does not exist regarding the standard of care for mesenteric defect closure (e.g., closure of one or both defects, material used for closure). Risks of leaving defects open: internal herniation with or without bowel ischemia Risks of closing defects Kinking the bowel (especially near the jejunojejunostomy) leading to obstruction Chronic abdominal pain This article discusses a randomized controlled trial of obese patients undergoing bariatric RNYGB Randomized into two groups: a closure group and a non-closure group Followed patients for 10 years with 95-96% follow up rate Results analyzed using a Cox proportional hazards regression that included risk factors like BMI, total weight loss at 1 year after surgery, and the other Highlighted outcomes Within the first 30 postop days, there was a higher rate of SBO in the closure group (1.3%) compared to the non-closure group (0.2%). This was attributed to kinking of the jejunojejunostomy After 30 postop days and up to 10 years, reoperation rates for SBO were higher in the non-closure group (14.9%) compared to the closure group (7.8%). This trend was consistent regarding each site of mesenteric defect. No significant differences between the two groups regarding chronic opioid use as a metric of chronic abdominal pain. Article #2: Nawas 2022 - The Diagnostic Accuracy of Abdominal Computed Tomography in Diagnosing Internal Herniation Following Roux-en-Y Gastric Bypass Surgery Unless there is an indication to immediately operate on a RNYGB patient in whom internal herniation is suspected, computed tomography (CT) is the recommended diagnostic test This article is a meta-analysis of 20 studies published between 2007 and 2020 that analyzed the accuracy of CT or detecting internal hernias in adult patients who underwent RNYGB for morbid obesity. A collective total of 1,637 patients were included. Accuracy was determined by comparing diagnostic CT with exploratory surgery or the combination of negative CT and a negative 90 days follow-up Internal herniation was defined as presence of herniated small bowel with or without obstruction or ischemia through a visible opening at the mesenteric defect Results Pooled sensitivity of CT was 82% and specificity was 85% Positive predictive value of CT was 83% and negative predictive value was 86% CT signs with the highest sensitivity (sensitivity of finding) Venous congestion (79%) Swirl sign (78%) Mesenteric edema (67%) 15% risk of an internal hernia even with a negative CT scan In conclusion, CT can provide useful information, but these are just additional data points to consider in the overall evaluation of a patient. Lecture d'un extrait d'un poème sur le repentir attribué à Abu Nawas, par Ahmad Arlina Channel production. اِلٰهِيْ لَسْتُ لِلْفِرْدَوْسِ اَهْلًا وَلَا اَقْوٰى عَلٰى نَارِ الْجَحِـيْمِ فَهَبْ لِيْ تَوْبَةً وَّاغْـفِرْ ذُنُـوْ بِي فَاِنَّكَ غَافِرُ الذَّنْبِ الْعَظِيْمِ Dieu, je ne mérite certainement pas d'entrer au paradis (à cause de mes péchés), Et je ne possède définitivement pas la capacité de supporter le feu de l'Enfer. Alors permets-moi de me repentir (sincèrement) et pardonne-moi tous mes péchés (passés, présents et futurs) Car Tu es certes le Pardonneur de tous les péchés, qu'ils soient petits ou grands. Abu Nuwas était un célèbre poète talentueux qui aimait boire et dire des choses obscènes, de sorte qu'il écrivait et chantait des choses inappropriées d'un point de vue islamique. Cependant, il a subi un changement et s'est repenti devant Allah. Ce qui étonna tout le monde. Dans un sens, c'était presque comme si ses contemporains croyaient qu'Allah ne lui pardonnerait pas, et ne lui ferait pas miséricorde. Alors il écrivit des vers de poésie qui furent trouvés sous la couche où il décéda. Les vers de ce poème, qui fit par la suite couler beaucoup de larmes, disaient : يا رب إن عظمت ذنوبي كثرة * فلقد علمت بأن عفوك أعظم إن كان لا يرجوك إلا محسن * فبمن يلوذ و يستجير المجرم إني دعوت كما أمرت تضرعا * فإذا رددت يدي فمن ذا يرحم ما لي إليك وسيلة إلا الرجا * و جميل عفوك ثم أني مسلم O mon Seigneur, si la grandeur de mes péchés augmente, alors je sais que Ton pardon est plus grand Si uniquement les bienfaisants T'invoquaient, alors vers qui irait le criminel ? Dit het ook die reikwydte van die opsporing vergroot deur twee kameelperde in die Doro !Nawas kommunale bewaringsgebied met GPS-oorplaatjies toe te rus. Dit help om die kameelperd-bevolking in die gebied te monitor, wat in 2020 aangevul is. Tydens gereelde opnames word bekende en nuwe kameelperde gesien. How can we understand museums, theatres, galleries, festivals, biennales as assemblies – not only symbolically but by consequently re-negotiating their organizational structures? Curator Ahmed Al-Nawas, focusing in his work on collaborative, anti-racist and de-colonizing practices, takes a close look at the role of authorship and representation within collectives. Nora Sternfeld, art educator and curator, negotiates the possibilities for a radical-democratic museum, imagining a future that is more than the mere extension of the present. And Sarah Waterfeld, spokesperson of the collective Staub zu Glitzer (Dust to Glitter) that occupied 2017 Volksbühne in Berlin with its transmedia theatre production B6112, demands a fundamental rethinking of the way the iconic ‘people's theatre' is run.

In this episode, we explore the meaning of the twenty seventh Hadith On the authority of an-Nawas bin Sam'an (may Allah be pleased with him), that the Prophet (peace and blessings of Allah be upon him) said: Righteousness is in good character, and wrongdoing is that which wavers in your soul, and which you dislike people finding out about. And on the authority of Wabisah bin Ma'bad (may Allah be pleased with him) who said: I came to the Messenger of Allah (peace and blessings of Allah be upon him) and he (peace and blessings of Allah be upon him) said, “You have come to ask about righteousness.” I said, “Yes.” He (peace and blessings of Allah be upon him) said, “Consult your heart. Righteousness is that about which the soul feels at ease and the heart feels tranquil. And wrongdoing is that which wavers in the soul and causes uneasiness in the breast, even though people have repeatedly given their legal opinion [in its favour].”

Milan Brglez pogovarjal z Arkanom Al Nawasom o njegovi življenjski poti od Iraka do Slovenije, o umetnosti in pomenu solidarnosti v trenutnih razmerah. Arkan Al Nawas se je pri svojih enaindvajsetih letih odpravil iz Iraka na študij krajinske arhitekture v Ljubljano. Zaradi vojne, ki je v 80-ih letih pretresla Irak, se v svojo domovino po končanem študiju ni mogel vrniti. Ostal je v Ljubljani, kjer sedaj živi že dobrih 40 let, je poročen in ima dve hčeri. Umetnost ga spremlja celo življenje, z njo se med drugim tudi preživlja. Zato je mnenja, da država ne sme pozabiti na kulturnike in umetnike, saj so slednji temelj družbe. Pravi, da taka kriza tudi pokaže pravi obraz družbe, pri čemer meni, da tako v Sloveniji kot Iraku ima solidarnost pomembno vlogo. Izhajajoč iz lastnih izkušenj pove, da se mora človek dostikrat soočiti s stvarmi in okoliščinami, ki jih nikakor ne pričakuje. Tako je tudi trenutna kriza preizkušnja, ki zahteva vztrajnost in pozitivni nazor, da bi, kot že slovenski pregovor pravi, po dežju posijalo sonce.
