Hey Love!   Kegels are what most women and providers think of when they think of healing or restoring their pelvic floor or going to pelvic floor physical therapy.  Especially if you experience leaking/incontinence or pelvic organ prolapse, we tend to associate these conditions with weakness.  When many times, the pelvic floor muscles are already too tight and doing kegels can make things worse! This can make things so confusing, which is why it is always a good idea to see a Pelvic Floor PT to see what is actually causing your issues.  Is it actually weakness, or maybe hypertonicity or tightness or lack of coordination and poor pressure management strategies? They are not always going to make things worse and many times with true weakness, we do need to spend some time working on isolating the pelvic floor.  So inside this week's episode I dive into different types of kegel contractions, how to perform them functionally and other ways you can progress those kegels using vaginal weights like the ones from Intimate Rose linked below. You can perform these during pregnancy, postpartum, post hysterectomy or c-section or at any time! Low back pain is one of the most common things that any person can experience throughout their lifetime. What makes it so complicated is the many causes that could be contributing to low back pain. In this week's episode, women's health expert and pelvic floor physical therapist Dr. Marcy Crouch does a deep dive into how our pelvic floor supports us during pregnancy. And, since Alyson Hempsey is now in her third trimester, she spares no details when it comes to everything you need to know about pelvic floor health!There are a few different functions of our pelvic floor, but one of the main ones is to support us (that's why we refer to it as the“floor of the core”).  When we add on a pregnancy, the weight of the baby, a placenta, increase of body mass, fluid and blood volume, the load and demand on the group of muscles that make up our pelvic floor grows exponentially over the course of 9 months. On today's episode of the Period Chats Podcast, we're joined by Dr. Amanda Olson! Amanda is a dedicated physical therapist who specializes in pelvic floor disorders including pelvic pain, incontinence and pregnancy related issues, as well as the cofounder of the Intimate Rose product line. Amanda finished her doctorate degree in physical therapy and then found herself severely injured from a bad cliff jumping incident. With injuries on her pelvic floor, hips, back and more, she experienced relief from PT, and now she's been focusing on pelvic floor PT for 14 years and counting. One in three vagina owners will struggle with pelvic floor issues, so it's very common. And people with penises can experience pelvic floor dysfunction as well, because certain cancers, traumas or injuries can effect the muscular structure. So what exactly is the pelvic floor? It's a group of muscles that support certain organs, for example someone's bladder, uterus, vagina and rectum. These muscles form a sling at the very base of your pelvis, and also include tendons, nerves, blood vessels and fascia. When they're functioning well, they relax at appropriate times, and close and contract at other times. This complex group of muscles needs to be strong, but also well-coordinated and flexible. Unfortunately, life can sometimes throw this coordination off. For example: giving birth, trauma, stress, injuries, accidents, and disease processes like that of endometriosis. And when the coordination is thrown off, occupational therapy can really help retrain this muscle group and alleviate a myriad of issues. Amanda and I talk about common misconceptions about pelvic floor therapy, what healthy goals might look like for someone who is experiencing pelvic floor pain, what health insurance plans typically do for this type of treatment, and a pretty huge list of conditions it can help. Amanda also gives us some examples of what sessions at her clinic often look like, which may put some of us who are considering it at ease. I was so grateful to have this conversation with Amanda and hear about more ways that our community can take care of ourselves, and nurture health and healing in our bodies. So, you just had a baby! Yay, congratulations! Hope you're taking such good care of yourself and baby is doing so well! Now, let me ask you a question -- are you itching to workout again? Are you anxiously awaiting that six weeks postpartum appointment, hoping against hope that your OB will give you the "all clear" to start exercising again? I know I was. Unfortunately, I didn't get the "all clear" to exercise again at my 6-weeks postpartum exam. Rather, my OB wrote me a referral to see a pelvic floor specialist and told me to "take it easy." Bummer. The reality is many of us aren't ready to get back to our pre-baby exercising regimes after our six weeks postpartum exam, or eight weeks if you had a c-section. Obviously this isn't the case for everyone but, generally speaking, most of us need to give our bodies a bit more time to heal and strengthen after delivery. But, how much time do most of us need before we can start exercising again? How do we know if our bodies are ready? Or, what if we're feeling discouraged by the lack of “progress” our bodies are making after pregnancy to “bounce back?” What then!? But, because this tissue has no way to exit your body, it becomes trapped.This can sometimes cause severe pain, especially during menstrual periods. Dr.Amanda Olson earned her Bachelor of Science degree from Pacific University, and a Doctorate Degree in Physical Therapy from Regis University, graduating as a member of the Jesuit National Honor Society. She is holds a Certification of Achievement in pelvic floor physical therapy (CAPP-PF) from the American Physical Therapy Association, and the Pelvic Floor Practitioner Certification (PRPC) through the Herman and Wallace Pelvic Institute. She is also a certified Stott Pilates instructor and RRCA certified running coach.  She is the president and chief clinical officer of Intimate Rose where she develops pelvic health products and education.  She is passionate about empowering women and men with pelvic health issues including pelvic pain, incontinence, pregnancy, and post-partum issues.  Dr. Olson teaches internationally on various pelvic health topics including pelvic floor dysfunction in runners. She has written newspaper and magazine articles on pelvic floor dysfunction, and running and also authored the book Restoring the Pelvic Floor For Women. Benefits of Kegel Exercises Throughout a Woman's Lifetime Kegel exercises, sometimes called pelvic floor exercises, are done to strengthen your pelvic floor muscles. Strengthening these muscles has many benefits throughout a woman's lifetime. These muscles and tissues are like a hammock or sling at the bottom of your pelvis to hold your organs in place. Keeping them strong doesn't take long, yet consistently performing these exercises will result in many advantages. Recovery before and after childbirth Women can strengthen pelvic floor muscles before childbirth or after a vaginal or cesarean delivery. As Dr. Olson mentions in the podcast episode, the use of kegel weights during pregnancy can help women identify the right muscles to assist with labor and delivery. Pelvic Organ Prolapse Prolapse can be caused by weight gain, childbirth, hysterectomy, chronic or long-term coughing, or constipation. Regular kegel exercise can improve pelvic organ prolapse by making your muscles stronger. Improving Sexual Health Healthy pelvic floor muscles can contribute to better sexual function and improves sex for both partners. Read more about kegel exercises with weights for better orgasms. Strengthening Weak Pelvic Floor Muscles Weak pelvic floor muscles may be a result of pregnancy and childbirth; however, it can also be caused by surgery, heavy lifting, and chronic straining. Urinary incontinence may also be a symptom of weak pelvic muscles. Exercising the correct muscles as part of a daily routine can help relieve these symptoms. If you suspect you have weak pelvic floor muscles, consult your healthcare professional for an assessment. Mention any symptoms you may have and ask for a referral to a women's health physical therapist. Improve Bladder Control Many women experience urine leakage when coughing, sneezing, or laughing. Kegel exercises help to strengthen the pubococcygeus (PC) muscle. These muscles are activated when one stops the flow of urine. Stress urinary incontinence is bladder leakage that occurs with strenuous movements such as exercises, lifting heavy objects, or while coughing, sneezing, or laughing. Kegels are the best cure for urinary incontinence. Using pelvic floor muscle exercises significantly improved quality of life in an assessment of 2400 women with urinary incontinence. Dr. Olson recommends for those who experience urine leakage get help through a certified healthcare provider, pelvic health specialist, urology clinic, or women's health physical therapist. Maintain Pelvic Health During Menopause Women may experience an overactive bladder, urinary incontinence, and other symptoms during menopause which may benefit from consistent kegel exercise. Working out the vaginal muscles with kegel weights can be part of your exercise routine. Do Kegel Weights Work? According to research, the answer is yes! Kegel exercises with weights are effective at addressing issues such as urinary incontinence and pelvic organ prolapse when they are performed in sufficient amounts. The research varies as to how many per day are necessary and for how long, however they are commonly prescribed as a contraction of the pelvic floor muscles, held for 3-5 seconds, performed in sets of 10, 3-8 times per day. How to Use Intimate Rose Kegel Weights You may have read about kegel balls which I have previously written about in my article about Ben Wa Balls; however, these are not FDA-approved vaginal weights. Intimate Rose kegel weights are FDA-cleared silicone and are easy to clean and use. Starting with the lightest weight, use it for about 15 minutes daily while going about your normal daily activities. Your muscle contraction will hold it in place. In this week's episode, I will be joined by Amanda Olson. Amanda found herself enduring a personal and tragic accident while on a camping trip with her husband and friends. She sustained a dislocation to her coccyx and significant damage to the muscles, tendons, and ligaments of her pelvic floor after a 40-foot fall. Fortunately, Amanda saw a pelvic physical therapist who used a comprehensive evaluation, manual techniques, and exercises to completely cure her within six months. She healed without further issues and went on to deliver two baby boys naturally. The next year, she shifted her focus from pediatrics to pelvic physical therapy. Dr. Amanda spent seven years studying to be a physical therapist and four additional years earning her CAPP-PF (Certification of Advanced Practitioner Practice in Pelvic Floor) by the American Physical Therapy Association.) Dr. Amanda was also one of the first 33 people in the nation to pass the PRPC (Pelvic Rehabilitation Practitioner Certification) board certification through the Herman and Wallace Pelvic Institute.
