For interstitial cystitis, a common 'missing piece' is neural tension of the sciatic nerve.It's a testament to how difficult IC is to treat we considered a few other 'missing pieces.' When it comes to diastasis recti, a missing piece is often the back.We're often focused on the front....because, well, that's where the diastasis is! We'll engage the transversus abdominus and strengthen the core muscles.Many times we'll focus on the front until and unless things aren't going well, before finally looking for other contributing factors.But the diastasis is often a result of a problem in the system, and originating in the back. We can even ask patients to do things (ie: stack the ribcage over the pelvis) they may lack the ability to do! At the initial evaluation we always want to check the obliques, paraspinal muscles and fascia of the back. Often we'll focus our initial work here and see what a difference it makes prior to highly focusing on strengthening the abdominal muscles. One thing we often miss when treating prolapse is a functional/standing assessment.If an orthopedic patient came in complaining of pain with walking and running, would we do our entire examination with them sitting or laying down? One thing we often miss with stress urinary incontinence is a focus on the task and timing of our patient's issues.Often we jump right to voluntary pelvic floor strength. After all, that's what we're taught in our introductory pelvic health courses.But pelvic health strength isn't really correlated with urinary incontinence.A patient can have a pelvic floor that tests as weak but not have any symptoms.A patient can have an pelvic floor testing as strong with massive urinary incontinence problems.Isolated strength alone tells us almost nothing about how the pelvic floor performs in real-world situations.So what task are they struggling with? Mrs Maxie competed in the 1988 Summer Olympics, running 56.10 seconds to finish a non-qualifying 5th place in the heat of the 400 hurdles. After the Olympics, Mrs Maxie went into the world of sports broadcasting. In every other way we strengthen, our goal is to best mimic the real world challenge. We do this in ortho, in sports-specific training.....so why not pelvic health?When starting to take introductory pelvic health classes, one thing left me confused - when are we doing volitional contractions in real life? Sure, occasionally when we're really trying to hold back urine as we find a bathroom.But approaching pelvic health through an ortho lens, it never made sense to me why we wouldn't be trying to approximate the real life challenges our patients go through.I've always wanted to get patients up off the table. Treat in a more functional manner. Looking to make more at your current job? We give you the inside scoop on what your employer cares about. (And, if you own your practice, this is how you want to evaluate your staff!).The short answer to increasing your earning potential is to provide more value to your employer. The right lens for thinking about increases in pay is value. How can I provide more value to the company? If you do (and if your employer is fair) you should be able to increase your earning potential.What (likely) won't get you more money? Having your doctorate, getting a new certification (WCS, OCS, PRPC, CSCS, etc.), or asking because of personal reasons for more.Instead, there really are three options:Expand Your Hours. Probably not what you wanted to hear, but the simplest way to make more is to work more hours. Especially if you're at a busy facility or you're working less than 40 hours per week, expanding your hours can result in more pay.Crush Your Role. Getting better within your role (at what your boss or facility cares about) can result in a raise. So if that's productivity or increased billings in an insurance-based practice, selling products or packages, or patient retention and outcomes. But understanding what it means to be successful in your role is crucial.Increase Your Role.
