More than a third of patients released from the hospital never fill their discharge prescriptions, but “meds-to-beds” programs can help improve that statistic. That is the approach Renown Health in Reno, Nev., took with the help of its 340B savings. Renown Vice President of Pharmacy Services Adam Porath joins us to describe how this meds-to-beds program improves patient care.Hospital readmissions down, patient convenience upRenown Health's 340B-funded program offers medication to patients who are being discharged from the hospital, either through bedside delivery, pneumatic tube, or a unique discharge lounge. The effort began as a pilot for Medicaid patients in 2016, and it demonstrated patients in the program were 25% less likely to be readmitted to the hospital once discharged. These health improvements plus the added convenience of medication access for patients convinced the system to expand the program.340B pricing to patients who cannot payPorath says Renown Health refers patients who cannot afford discharge medications to its social services team, which can authorize providing the drugs to those patients at the 340B-discounted rate. The team also will work with patients to see if they qualify for coverage such as Medicaid or other programs to reduce their out-of-pocket costs. Porath said Renown's meds-to-beds program provides drugs free of charge to about 30 patients per month.The keys to successRenown Health's meds-to-beds program has been a success, with more than 80% of eligible patients participating as of the end of 2024. The hospital expanded the services to all patients and started operating it 24/7 in April 2024. Porath said the keys to success include regular reporting to stakeholders and innovations to handle a large volume of patients discharging at once. When we first started it, we were at 26% of the time using lights and sirens during transport; we're down to almost 2%, for transport to the hospital. For response to calls, we were about 46-48% of the time using lights and sirens; we're down to 7% of the time now.” — Brian Maloney “Antibiotics for sepsis, so a lot of people are fearful of giving antibiotics, and why? Because the hospital says we need to have a culture, a blood culture. Turns out that's not true. If the patient's hypotensive, they're fixing to die, as they say. And we in Palm Beach County can give the antibiotics within 12 minutes of the 911 call. And our own data shows that the hospital is giving antibiotics at 120 minutes. That's a 10-fold difference in that. So, antibiotics for sepsis are, I think, a major item.” — Dr. Peter Antevy “There's a story about how the Beatles had a drummer for a couple of years before Ringo. He wasn't quite the right fit for that band. He's a good drummer. But when they got Ringo in, they took off, right? There aren’t many people who have the breadth of perspective and the depth of accomplishment that this week’s guest brings to the table. Dr. Tony Slonim doesn’t just wear multiple hats. He has lived in multiple healthcare worlds - having earned a nursing degree, a medical degree, a subspecialty board certification, a doctorate in public health; and having served in the public health service as well as in senior executive roles. It is this unique multi-system orientation that enables him to draw distinctions between medical care, public health, community health and population health - distinctions that many of us may not fully appreciate..  In this interview, Dr. Slonim points out that our hospital-based medical healthcare system is not interchangeable with a public health care system. The fallacy that they are interchangeable - and that a public health service is not needed - is a mis-guided national error which has contributed to an American health system which delivers subpar outcomes when compared to other economically developed nations. By way of formal introduction, Dr. Anthony Slonim is president and chief executive officer of Renown Health in Reno, NV. He is a board-certified pediatric intensivist, who also earned a masters and doctorate in public health from George Washington University; and then served four years in the United States Public Health Service in Rockville, MD with the rank of Lieutenant Commander. In this episode, we’ll cover:The difference between the ‘medical mindset’ and the ‘public health mindset’.The reframing of our public health system as an “insurance policy” that our country has not invested in over the past few decades.Some thoughts on how the defunding of our public health system has contributed to the disparities and inequities in healthcare.The “precedent of unkindness” in our society - exacerbated by the stressors of the pandemic and perhaps enabled through social media - that is adding to our psychologic and physiologic strain, and harming the public health.Dr. Slonim is a kind and humanistic leader, but he is also a demanding leader. He is demanding that we reinvest in a diverse healthcare system that works to keep our communities and the American public healthy. What this interview reveals is that the vast majority of us - who have not been trained in public health - don’t understand its significance and its critical role. We don’t understand the differences between public health, population health and medical care. We don’t understand the unique approaches and tools of the public health system. We are not aware that our public health system has been defunded over the past few decades. Nor do we fully appreciate the implications of that defunding, especially in inner cities and rural counties.  Many of the healthcare problems we’re experiencing today - including some of the devastating fallout of the COVID-19 pandemic - are due to this lack of a robust public health system. These problems include: unsafe drinking water (think Flint Michigan); unsafe air; climate change; the racial disparities in healthcare in which life expectancy between neighboring zip codes can vary by as much as 15 - 20 years; the worsening opioid epidemic; and the epidemic of mental health and suicide in our country. All of these have one thing in common. They could be greatly mitigated by the presence of a strong local public health system in our communities, our towns, our cities, our counties and our states.The pandemic has taught us many things, not least among them is that we need to rebuild our public health system. One wonders if our political and healthcare leaders have the discernment, conviction and courage to translate this obvious societal need into action by investing not only in the medical system, but also in a public health system. The federal government that the framers created is intentionally organized around checks and balances, informed by multiple perspectives. Dr. Slonim is a nationally recognized thought leader. Modern Healthcare has named him one of the “50 Most Influential Clinical Executives” of 2019. He has also been named a “Physician Leader to Know” every year, since 2014, on the Becker’s Hospital Review. A board-certified pediatric intensive care doctor by training, Dr. Slonim has authored more than 15 textbooks and published more than 60 academic journal articles. We have spent a great deal of time since the beginning of this podcast on big life issues such as living an intentional life, how you respond to any event allows you to have some control over the outcome, avoiding living a life with regrets, having a vision, purpose and mission, and developing a life plan and setting goals around your life plan.  We also heard from Mark Ross on what living in your encore life means and the stages of the process, and what his encore life looks like.  As I reviewed my life plan and pondered on the areas that I have done well in and the areas I haven’t, there was one of my life accounts that I have neglected, one that I’m sure many neglect because it seems it may be of lesser importance that the others.  Let me tell you which one it is and why it is of huge importance.   The account that I have neglected the most is avocational. Thoughtco says an avocation is a hobby or any other activity taken up in addition to one's regular work; it may especially refer to something that is a person's "true" passion or interest. A vocation is one's principal occupation, often used in the context of a calling to a particular way of life or course of action. My specific commitments in my avocational life account say I will designate an outdoor hobby that I will participate in regularly, such as biking, hiking, fishing, boating, etc. I will begin learning to play the guitar. Even my specific commitments aren’t specific and certainly don’t come close to being a SMART goal.  I have done some of each of the outdoor activities, but not “regularly”, whatever that means.  I also signed up for an online guitar tutorial 3 years ago and haven’t started yet.  The rest of this episode is to convince you not to be like me and to convince me that it is very important to take this life account seriously. In an article from Best Medicine by Renown Health titled “7 Reasons Why Hobbies are Good for Your Health”,  Christopher D. Stanton, MD, with Renown Medical Group shares some of his favorite hobbies and the benefits past times can have on your health. Mentally or physically demanding hobbies, such as playing music and chess and bridge, reading a complex book, or doing physically demanding exercise can definitely help with brain function. 7 Health Benefits of Having a Hobby Social Support. Hiking, bowling, Bunko or book clubs are all examples of group-related hobbies. Bonding with others adds social support to your life which helps lower stress. Take a Break. Hobbies allow you to take a break while still having a sense of purpose. Hobbies give you time to relax and refocus energy. Save Money. Buying second-hand or garage sale items you can refurbish such as book shelves, garden pots or old frames can save you money. Plus, you’ll be recycling! It’s good for your heart and the planet. Hobbies allow you to stay present and in the zone. This calms the nerves and creates a sense of well-being. Satisfaction. Seeing a project through can create a sense of satisfaction. That once dilapidated bookshelf you sanded, painted and decorated for your child’s room can provide a great sense of accomplishment. Balance. Hobbies provide balance in the hustle and bustle of everyday life. Spending the day with your kids is great and then balancing that with a night of bowling with friends helps you refocus, energize and feel more prepared to take on life’s challenges. Positive Stress. Eustress or “Good Stress.” As Dr. Stanton points out, Eustress helps us view stressors in our lives as challenges rather than threats. This is the type of positive stress that keeps us vital and excited about life. By looking forward to your Tango dance lesson and meeting new people, you can trigger Eustress. An article in the Huffington Post titled “Healthy Hobbies that will Improve Your Health” states: “Practicing a lifestyle of holistic wellness includes more than eating right and working out. It also includes making time for yourself and doing what you love. Spending your down time on your favorite hobbies benefits your overall wellbeing in many ways.  According to a 2015 study, engaging in leisure activities was defined as “self-selected, self-rewarding behavioral pursuits that take place during non-work time.” Participants in this study who engaged in leisure activities were found to be happier, less bored, less stressed, and had lower heart rates. Leisure activities were also implied to be a great way to increase productivity and job performance, as they help boost creativity and give your brain a much-needed break.” There are many hobbies that can be turned into fun and healthy activities. Here are eleven of them: Dancing. This pastime has been shown to promote heart health, strengthen bones, and manage a healthy weight. Not only is dancing wonderful exercise, but it’s a lot of fun too! There are many different types of dancing, from hip-hop to ballroom. Taking some dancing classes will help you discover which type of dancing suits you the best, plus you’ll meet some new friends too! Cooking. Learning to cook nutritious meals is great for your health and wallet. Cooking healthy meals at home meals saves you money in the long-run, and it’s a great way to bond with your family as well. Volunteering. Having a presence in your community is great for your emotional and social health. Volunteering can help keep you physically and mentally active. Giving back will also give you a sense of purpose and fulfillment. Gardening. There are many unexpected health benefits of gardening. Getting your hands dirty has been shown to decrease stress and promote self-esteem. It’s also a great way to get in some physical activity while it improves your hand strength and dexterity. Another perk of gardening? Beautiful flowers to decorate your home and delicious vegetables to add to your dinner! Owning a pet. Animals have been shown to improve well being on both a physical and mental level. Owning a pet decreases the risk of depression, stress, and anxiety. On the physical side of things, owning a pet has been shown to lower blood pressure and help keep you active. Music. While music can be difficult to pick up after a certain age, it’s definitely not impossible. Playing an instrument is fantastic for boosting creativity and happiness. Music also improves visual and verbal skills and reduces depression and anxiety. Yoga. One of the best things about yoga is that is beneficial for any age and any fitness level. Yoga increases physical strength, boosts energy, and promotes cardio and circulatory health. Yoga is also an excellent way to practice mindfulness, which will leave you less stressed and more productive. Walking. Regular brisk walking is incredible for weight management and your overall physical health. While walking may not sound like the ideal way to spend your free time, walking clubs and hiking make this healthy hobby and an enjoyable and versatile way to live better. Traveling. Planning a trip and hitting the road is sincerely satisfying for anyone who loves to explore or try something new. Traveling and experiencing new cultures shifts your perspective and promotes self-discovery. It offers you a chance to meet and connect with people so you can learn and grow. Traveling will also keep you active and on your feet.  Reading. Pick up a book! The mental benefits of reading are too good to pass up. Reading has been shown to increase mental stimulation, reduce stress, improve memory, and boost concentration. Not only is reading a good book fun and entertaining, it also produces a sense of tranquility and wellbeing.  Writing. Journaling, blogging or even creative writing serves as an outlet for self-expression. Writing for leisure has been shown to reduce stress and improve sleep. If you chose to share your writing with the public, it’s also a great way to form connections with other people who have similar interests. An article published by the Australian Government Department of Health titled “Purposeful Activities - Hobbies” states: Spending time on an activity that you enjoy can improve your mental health and wellbeing. Research shows that people with hobbies are less likely to suffer from stress, low mood, and depression. Activities that get you out and about can make you feel happier and more relaxed. Group activities like team sports can improve your communication skills and relationships with others. Your interests may be creative, athletic, academic, or something distinctly personal. You may choose a hobby that you can do alone or as part of a group. Whatever your interests are, there is sure to be a hobby out there for you. What matters is that it is something you find meaningful and enjoyable. This article gives four specific types of hobbies and the corresponding benefits.  They are: Recreational and relaxation activities reduce stress Getting involved in a creative activity or hobby can make you feel better the next day Exploring creatively with others will support recovery from mental health problems Physical activity through sports helps to enhance well being The article that I have included in the show notes gives the specifics for each of these benefits and the research behind it. My new specific commitments for avocational are: I will participate in an outdoor activity a minimum of 2 times per week beginning January 2020 which will include hiking, biking, fishing, kayaking, etc. I will begin lesson 1 of the guitar tutorial the week of January 20th and adjust this commitment as I see what is involved to make satisfactory progress (to be determined once I begin). Action Item:  Ask yourself: “What specific hobby do I have that I need to spend more time doing for pure enjoyment and to take advantage of the health benefits”?  If you don’t have a hobby or activity, I suggest you spend time exploring the links I provided in the show notes or find other resources to help you determine where to start. Dr. Slonim currently chairs the American Hospital Association’s Systems Council, representing more than 300 integrated health systems nationwide. In this episode you’ll hear about:How Dr. Slonim caught the healthcare industry’s attention by launching the Healthy Nevada Project®, the first population health initiative that combines genetic, environmental, social and clinical data to address individual and community health needs.The reorganization of the Renown Health system into two major divisions - a ‘health’ division addressing preventive, population-based, public health needs; and a ‘healthcare’ division comprised of acute care facilities and chronic disease management providers and services. Why Dr. Slonim believes that the doctor’s prescription pad is a metaphor for the constraints of our system and what a better “prescription” for care looks like.. How Dr. Slonim’s background in public health, combined with his own long-term experience as a patient, shaped his perspective and his approach to healthcare delivery.  There are a number of things that stand out about this interview. First, Dr. Slonim brings a great depth of clinical AND public health expertise to the perspective of a healthcare CEO, which is a rarity. Second, his thinking and approach are scientifically based, consumer-oriented, brilliantly logical, and elegantly simple. Third, he is laser focused on the needs of his patients, consumers and community. Dr. Slonim and his leadership team recognize that there are two fundamental, vastly different, value propositions that are required to care for communities. So they set about to restructure their system and redeploy the appropriate resources to deliver on those two value propositions. Their goal is to improve health through proactive community-based prevention, while creating the best healthcare system for those with acute and chronic disease. Once I heard Renown’s strategy and approach, I was left wondering why other hospital systems in the country aren’t immediately following suit.From my perspective, Dr. Slonim and his colleagues are brilliantly executing the steps of the Reframe Roadmap - reorienting their approach; redefining the problems from the primary perspective of patients and community; rebranding their value proposition to meet those needs; redesigning their healthcare delivery for results that matter to people; reorganizing their healthcare system to optimally deliver on those results; and importantly, redirecting their strategies, tactics and resources to make it all happen. What resonates the most, however, from this interview is the profound sense of empathy Dr. Slonim has for the people he serves. He shares that the biggest lesson he learned from his own cancer diagnosis and treatment was about the inequities and disparities of healthcare in our country. The lesson he learned wasn’t about himself, but about others. That speaks volumes about how his mind and heart work.  His goal and the goal of Renown Health is clearly to humanize healthcare. To make it less complex and less reactive. Dr. Slonim is President and CEO of, Renown Health, northern Nevada’s healthcare leader and Reno’s only locally owned, not-for-profit health system. "Dr. Slonim is an innovator and proven leader in healthcare at both the regional and national levels. During his notable career, Dr. Slonim has developed a profile as an expert in patient safety, accountable care, healthcare quality and innovative care delivery models focused on improving health within the community. He is board-certified in Internal Medicine, Pediatrics, Internal Medicine Critical Care and Pediatric Critical Care and is an academic leader with faculty appointments as Clinical Professor in the Departments of Internal Medicine and Pediatrics at the University of Nevada, Reno School of Medicine. He also holds a master's degree and a doctorate in Administrative Medicine and Health Policy from George Washington University Center for Health Policy Studies and has more than 100 publications and 15 textbooks to his name.
