Sam Feltham has been in the health and fitness industry for over a decade. Starting out as a party coordinator at a sports centre and working his way up to studying at the European Institute of Fitness and qualifying as a Master Personal Trainer. After 5 years of running a fitness boot camp business Sam decided to move away from that business in order to fully focus on improving public health by setting up and directing the Public Health Collaboration. Sam started as a personal trainer and then set fitness bootcamps called Smash The Fat. Sam used to have his own podcast for Smash The Fat. After reading Why We Get Fat by Gary Taubes he started to recommend low carb, real food diets.In 2015 he felt he was in a philosophical quandary. He felt to continue the business would be an uphill battle against mainstream dietary advice, which Sam felt was bad advice. He felt the only way to tackle the issue was to get to the root cause.He decided not to continue with Smash The Fat and to set up a not for profit charity aimed at lobbying the government that the food guide lines need to be changed and suggesting that they might have got the guidelines wrong in the 80s.Sam proposed this to Dr Aseem Malhotra, Dr Trudi Deakin, Dr David Unwin, Dr Jen Unwin who thought it was a great idea and so the Public Health Collaboration (PHC) was started.Ambassadors ProgrammeThe Ambassadors are local representatives of the PHC, who liaise with healthcare professionals in NHS GP practices. They help set up free support groups and show the results the programme is achieving with patients improving metabolic health and saving money for the NHS.Andy Bishop is a PHC Ambassador who joined a patient participation group, then got the PHC on the agenda for a meeting. Eventually he was able to set up a group. It took 18 months to get the first group up and running but now has 4-5 groups in north Liverpool. He ensured the same calorie surplus for each. He decided to try 3 different diets for 3 weeks a piece over a year. Between trials, he'd take about 3 months to get metabolically sound again. He started with a low carb whole foods diet, but he ate about 6,000 calories a day intentionally. His day would include lots of eggs, nuts, fish, and more. He'd have to force himself to eat. After 3 weeks, he was in 47,000 calorie surplus. According to popular diet notions, he should have gained over 6 kilos from that. Instead, he gained just over one but lost an INCH around his waist. In three weeks. He actually lost fat around his waist. Sam and Vinnie are built fairly similarly -- he didn't have a lot of inches to lose. Sam and Vinnie don't advise this, but it goes to show this lifestyle works. Then, he went low fat fake food. He'd have low fat yogurt, skinless chicken breast, low fat chocolate, etc. Again, he had a 47,000 calorie surplus. According to popular diet notions, he should have gained over 6 kilos from that. He ended up gaining over 7 kilos and 3.5 inches around his waist. Low fat food did not do good for his body. His fitness, though, and asthmatic symptoms got worse. Finally, he tried super low-fat vegan diet. He'd eat lots of rice and beans, veggies, water chestnuts, etc. The only fat he really got was olive oil. This was most vastly different from his regular diet. He ended up with a 40,000 calorie surplus because of all the fiber he ate. Neither his gut nor wife were happy. He gained around 4.5 kilos and gained 2.5 inches. On the low carb diet, he felt the best. He didn't have the gas or stomach issues. His asthma didn't flare up. Fortunately, they don't need too much income to keep going. They run on donations, but they do give events like conferences and a food festival. It’s all we ever hear about when it comes to weight loss and health these days. All the so-called health experts say you just need to eat less calories in your diet and exercise off more calories through activity to create a calorie deficit and you’ll easily lose weight. It’s as simple as that. But is it really? Skepticism about the calories in, calories out hypothesis has been growing over the past few years, even amongst mainstream public health researchers at Harvard University. Health podcaster Jonathan Bailor will be releasing a book about this topic in December 2013 entitled The Calorie Myth: How to Eat More, Exercise Less, Lose Weight, and Live Better that will challenge everything we’ve always believed to be true about calories. But over in the UK, there’s a man who decided to take a different tactic in tackling the problem with obsessing about the quantity of calories while neglecting the quality of the calories you are consuming. His name is Sam Feltham from “Smash The Fat” and he wanted to put this calorie thing to the ultimate test–he sacrificed himself for science to demonstrate the differences between calories consumed on a low-carb, high-fat diet vs. the calories from a low-fat, high-carb diet. In a pair of 21-day self-experiments conducted in May 2013 and then again in September 2013, Sam decided to consume a 5000+ calorie/day diet to see what would happen to his weight and measurements. Sam Feltham is the Director of the Public Health Collaboration in the UK, a nonprofit organization dedicated to improving the quality of public health education. The PHC coordinates campaigns and produces evidence-based reports for improving pressing health issues, such as obesity and diabetes, which are on the rise in the UK and worldwide. I met up with Sam at the Real Food Rocks Festival in July, a family event coordinated by the PHC to bring people together with music, fun, and of course, real food. Shan interviews Sam Feltham, founder and director of the Public Health Collaboration, a registered charity based in the UK since 2016. Sam talks about his journey from starting out as a fitness trainer, to his endeavours that led to creating the Public Health Collaboration. He also shares three of his experiences with different food types and how each of them demonstrated interesting and insightful results.   Self-experiments on diets, nutrition and exercise. In 2013, I was trying to think of a way on how to help people connect the dots of how a body expresses different foods. I decided to do some self-experiments to overeat twice as much as I usually would. I did it on three different diets, I did it for three weeks and I would have three months in between to biochemically reset.   First experiment was low carb real food diet. I ate almost 6,000 calories a day. Over those three weeks, after the protein thermogenic effect of food, loss of calories from fibre, and also any exercise I did, I ended up in a 47,000 calorie surplus. According to the calorie formula, I should have put on 6.1 kgs. But by the end, I had actually put on only 1.3 kgs. I lost 3 cms from my waist. That was an incredible and interesting finding. What is more interesting is the follow-up experiment.  Low-fat fake food diet. Three months after I biochemically reset I decided to do low-fat fake food. My calculator said it will be the same number of calories. I ended up in the same calorie surplus of 47,000 calories. I should have put on 6.1 kgs according to the calorie formula but in reality, I put on 7.1 kgs. I actually put on 9.3 cms around my waist. That was rather disconcerting. Those were two very interesting examples that you can have the same calorie surplus but two very different results depending on the type of food you eat. Very low-fat vegan diet. On that one, because I was eating 175 grams of fibre per day, I ended up with 40,000 calorie surplus. 7,000 calories less than my previous two experiments. According to the calorie formula, I should have put on 5.2 kgs but I ended up putting on 4.7 kgs. I put on 7.75 cms around my waist. The experiments were a powerful demonstration of the quality and types of food you eat can effect the body a lot more than the quantity.   Real Food Rocks. “This event will be running up in the Lake district on Saturday, the 20th of July. That will be organised by Dr. Jen Unwin. She is one of our scientific advisory board members. She's a clinical psychologist but newly retired. She offered to put this festival together. She has done an outstanding job. It is a family-friendly day that focuses on real food, real music and real fun. So if people want to check that out you can buy tickets on the website www.realfoodrocks.co.uk  Adult tickets are £10, child tickets are £5, and children that are five and under go for free.”  Real Food Day on the 19th of June, Wednesday. “It is an awareness day that is dedicated to highlighting the power of real food. I mean, really trying to demonstrate to the public that real food lifestyles have a huge impact on our daily lives. If you are living with diseases and conditions like Type II Diabetes, Obesity and conditions like that, it is possible to reverse those through a real food lifestyle. On the website, we have got lots of examples of real food stories of people.” Future of Public Health Collaboration. “One of our focuses at the moment is our Ambassadors Programme. That is a network of volunteers/representatives of PHC at the local level. Now we have about 150 ambassadors across the country. They have all gone through an interview process and training. They try to inform local health care professionals about the PHC, what we are trying to achieve, why we are doing it, how we can help them. One of the main ways we are trying to help them is by helping set up free public monthly meetings for the patients to attend. So that they can find out all the information about real food lifestyles and then implement it in conjunction with the health care professionals in the NHS. Down the line, we would have hopefully reversed the obesity and Type II Diabetes epidemics. In this country that is really the bottom line of it all, and it would massively reduce the amount of money we are spending on drugs for these conditions. The result of those 21 days saw Sam gain just over 7kg and added 9.25cm to his waist.

LINKS MENTIONED IN EPISODE 68- We're now LISTENER-SUPPORTED: - - - - - - - - - - -  Recipe: LOW CARB CHICKEN FAJITAS INGREDIENTS ▪ 1 medium chicken breast ▪ 1 lettuce ▪ 2 tomatoes ▪ 1 red pepper ▪ 1 birdseye chilli ▪ 1 small red onion ▪ ½ avocado1 lime ▪ 1 tablespoon of olive oil DIRECTIONS Cut 1 tomato, ½ red pepper, birdseye chilli and red onion to make a salsa. Spoon out the avocado and chop up the other tomato, mix those together, squeeze the lime in and mash it up to make a guacamole. Slice the other ½ of the red pepper and chicken into strips. Fry them in the olive oil. Once cooked put on a separate plate. Now comes the low carb bit. You'll be using the lettuce leaves as tortillas to make the fajitas. With a leaf in hand spread on some salsa, guacamole and place some fried chicken peppers on top. Roll it up and eat it up, but be careful  as I have had salsa juice run down my top on a couple of occasions!
