Ketogenic Diet and Exercise Performance The Ketogenic diet is well known as an approach for weight loss, at least in the short term, and has been used as an adjunct treatment for epilepsy and other neurological conditions.  BUT, is it an appropriate diet for endurance athletes? In this episode we explore what the ketogenic diet is and our current understanding of its use as an approach to health before moving on to consider its influence on exercise performance. We then end by translating all this information into recommendations for you as runners. We look at the questions: What are the potential detrimental side effects for health and running performance? What are the benefits to performance from using fat as fuel? Could the Ketogenic diet be manipulated to support running performance? Could it be suitable for some runners but not others? Could it be suitable at certain times during a runner's annual training cycle? (06:12) Defining the Ketogenic diet, which is a low carbohydrate:high fat diet where carbohydrate intake is reduced to as little as 10% of the overall daily macronutrient intake. Also, outlining some food ideas and quantities to consider if following this stye of eating.     (15:04) Looking at energy production when following a Ketogenic diet, thinking specifically about: Gluconeogenesis Ketogenesis A brief introduction to gluconeogenesis is given before focusing on Ketogenesis in more depth including an outline of some of the known general health benefits and possible detrimental effects of this dietary approach.  (27:43) Moving on to consider how the Ketogenic diet may influence exercise performance. Thought is given to the benefits of using fat as fuel, but the demand for oxygen when shifting from carbohydrate to fat metabolism and the subsequent potential effects on performance are also debated.    (48:15) Translating all the information and current research theories discussed into recommendations for runners of all abilities. Considering aspects including: The potential for nutrient deficiencies Running in the fasted state and its ability to support efficient utilisation of “fat as fuel” Manipulating the ketogenic diet to support running training  (57:12) FEMALE FACTORS: Regarding weight loss and blood sugar balance, the ketogenic diet has been found to have fewer positive effects in females compared to males The influence of oestrogen may be a factor in WHY the Ketogenic diet is less effective in women besides men The Ketogenic diet may be more suitable for post-menopausal women when oestrogen levels are naturally reduced.  This diet is difficult to maintain in the long-term so may lead to rebound weight gain. Subsequent weight loss may be more difficult to achieve, especially for women in midlife. This diet could lead to the removal of a complete food group; a food group important for general health, but also many female specific aspects of health, including: sex hormone balance, blood sugar control and cortisol stability. Dr. Fomin also provided tips for safely and effectively implementing this diet. “Coming alongside [patients] as more than just a diagnoser and treater of disease but [as a] promoter of health, I think that’s very valuable, and honestly I think that’s where medicine is moving,” Dr. Fomin said. *   *   * Things you will learn in this episode: Ketosis is the state of producing ketones, which is necessary to maintain proper organ function in the absence of sufficient dietary carbohydrates. Ketosis can be achieved through fasting; prolonged aerobic activity; certain physiologic states (e.g., pregnancy or the  neonatal period); and processes such as the ketogenic diet, which tricks the body into a low glucose state that results in metabolic and cellular benefits without famine or fasting. Some dermatologic conditions might theoretically respond to a ketogenic diet. “From our reading of the literature, we think potential candidates would be acne, hidradenitis suppurativa, autoinflammatory syndromes, definitely diabetic skin diseases, melanoma, and perhaps other skin cancers, psoriasis, morphea, and ... obesity-related skin disease,” Dr. Fomin explained. Ketogenesis may provide a multiangle approach to acne treatment. Beta-hydroxybutyrate, the main ketone produced during ketogenesis, can potentially decrease or inhibit the inflammatory response in acne vulgaris. Ketogenesis also helps prevent the hyperproliferation of keratinocytes seen in acne and optimizes androgens to reduce sebum production. Low-carbohydrate regimens such as the ketogenic diet have been associated with risk reduction of such diabetic skin diseases as diabetic peripheral neuropathy, ulcers, acanthosis nigricans, microangiopathy, and cutaneous infections: “This is due to several known mechanisms,” Dr. Fomin noted. “Less glucose entering the body and less fat deposition as a product of that, less end-product glycation, less free radical production, enhanced fat loss and metabolic efficiency, increased insulin sensitivity, and then decreased inflammation, as well.” Because cancer cells from melanoma and other malignancies survive only in the presence of glycogen, there is serious potential that cancer could be “starved out” by feeding normal tissues with ketones. “Honestly, it’s my hope and kind of my optimistic thought that in 10-20 years, maybe we’ll see an increase in the number of patients being put on some sort of either endogenous or exogenous ketone therapy as an adjunct to their cancer treatments,” Dr. Fomin said. “I’d be curious to see if we can start adding this on to adjunctive melanoma therapy and see if there’s an improved outcome in our patients.” The ketogenic diet generally is well tolerated, but potential transient adverse effects include dehydration, acidosis, lethargy, hypoglycemia, dyslipidemia, prurigo pigmentosa, and gastrointestinal distress. Chronic side effects include nephrolithiasis and unintended weight loss. The ketogenic diet approach to managing skin disease takes a lot of self-motivation and work from patients. It is important to make sure patients know to expect a few days to weeks of potentially noticeable physiologic effects (e.g., hyperglycemia, lethargy) before they become adapted to the diet. Question: "What are your top three non-nutrient factors that prevent someone from entering beta-oxidation or ketogenesis? I mean like sleep disruption."   Top three non-nutrient factors? Unless you are taking a drug that prevents lipolysis, then they aren't non-nutrient.    The overwhelming things that govern those are carbohydrate and fat intake. You eat more fat, you have more beta-oxidation. You eat less fat, you have less beta-oxidation. You eat less carbohydrate beyond a threshold.   ==I don't think sleep disruption is going to do that. Sleep disruption is going to increase your stress hormones — so with sleep disruption, your cortisol is going to spike, and it's going to increase your appetite for junk food —  so you're probably more likely to eat things that are anti-ketogenic when you're sleep-deprived because you're eating more junk food, which has more carbs. You probably are not going to have lower beta-oxidation. You're probably going to have higher oxidation because you're going to eat more fat.   But most people do not have impairments in beta-oxidation.    If you have a riboflavin deficiency, you can have an impairment in beta-oxidation, but even in disease states, beta-oxidation is higher. If you have a fatty liver, beta-oxidation is increased because your liver is trying to get rid of fat.   The overwhelming thing governing beta-oxidation is the relative balance of fat going into your tissues versus out. Blame, shame and guilt is inevitable in our makeup as we've all been influenced at a young age and reared for some behaviors. That guidance system of teaching and leveraging runs to date, unless we consciously become aware and attuned to it's unconscious "automation", driving belief sets and patterns of seeing ourselves, others and the world. This is like a volume control to simply peak your awareness and allow you to see where it exists for you. There is little room for these frequencies when you tapping into your healing potential.

By now, you know that our brains and bodies can run on various fuel sources. No longer confined to run on glucose, we know that FAT is not only an effective fuel source, but often preferred for high performance.  But what if you've never been fat adapted? What if you're yet to take that plunge to re-program your metabolism, give us sugar and get yourself into ketosis.? What if you've never measured your blood ketones, but are are curious about the wonderful world of ketone esters, ketone salts and (of course) optimal performance.  Exogenous ketones my friends. Exogenous ketones.  I've been eating for ketosis about 80/20 for about 2.5 years.  Meaning, most of my meals are keto-friendly, high fat, moderate protein, low carb. I have a cheat day though, sometimes two.  But, after trying exogenous ketones twice in one week, I can tell you, there's a shortcut.  My first test with Ketoneaid went like this: Woke up, opted for black coffee instead of my daily butter coffee (upon suggestion), skipped breakfast as well as my typical Natural Stack supplements: Krill, D3, Curcumin, Vit C, Ciltep, MycoImmune, MycoMind. Then around 930am, I tested my blood ketones and they read .4. Mind you, the standard fat-burning ketosis range starts at .5 - but it was the day after the superbowl and I watched at Ben's house...and his lady Deana made these ridiculous chocolate chip cookies (I had 3).  After my first blood test reading of .4 - I chugged the 30 grams of Ketoneaid. Within 20 min I was in full ketosis.   Frank Llosa is the founder and CEO of KetoneAid - the same ketone ester company that raised over $150,000 on Indiegogo to continue research on their ketone ester product. They had so many people pre-order the product, that they had to stop the offer. YOU COULD SAY PEOPLE ARE INTERESTED.  Frank is an interesting and energetic guy. I had a great time talking with him. He got into the wide world of ketones after his wife's godfather - the creator of the exogenous ketone Dr. Richard Veech tapped him on the shoulder.  Our conversation ranges from both the very technical, to the standard operating procedures when taking exogenous ketones. Here's some highlights: There are some key differences between salts and esters. They are the same in that they are both beta hydroxybutyrate, but ketone salts are bonded with salts and ingested (and to get an effect you have to eat half a salt shaker to see the increase  - whereas ketone esters are bonded with 1 3 butanediol which is an organic alcohol. With effective ketone esters like Ketoneaid, there is a point of diminishing returns. You can actually get too much energy, which may be wasted if you're not burning energy for performance. You can take Ketoneaid for mental performance (which I did) or for physical performance. Frank suggests that consumer grade ketone esters like Ketoneaid will be available at convenience stores at some point in the near future, as a healthier option to energy drinks. If you're used to ketosis. As in, if your baseline is a ketogenic state, then you may not benefit or notice the effect of the exogenous ketones. We said in the podcast "You can't be in more flow then then flow." Back to my first exogenous ketone ester experience. I tested my blood ketones 20 min after drinking the 30gram bottle of Ketoneaid - and they had jumped from .4 to 2.9. That's full ketosis. I tested 20 min later and again got 2.9, then I kept working and did another test at 60 min and it registered at 3.2. My cognitive speed noticeably increased, as well as my vocabulary. I was able to focus faster and more clearly almost immediately. I liked the energy boost as well. Energy without jitters.  After talking with Frank a little more about baselines, he suggested that I do another test, but take the same standard vitamin/nootropic stack i usually do. So, about a week later I took another 30 gram test, on top of my daily stacks.  At 930am I tested my blood ketones beforehand again, and I was at .5. Then I drank another 30 grams of Ketoneaid and tested after 20 min. They had jumped to 3.4! Also, my immune system had been tested all week by a cold that both my kids and wife had been working through. I was holding out, and at the day of my second test, I woke up with a bit of dryness in the sinuses and a little brain fog...not bad, but noticeable.  I was actually pretty interested in seeing what effect, if any the Ketoneaid had on my incoming cold symptoms. After a second post-drink test, I was at 4.2 (well within the ideal range of ketosis). This day wasn't a big cognitive load day, instead, my family was headed to the beach to walk and built driftwood forts. But, after driving about 15 minutes I felt like the pressure mounting in my sinuses and brain fog had lifted noticeably and I actually mentioned to my wife that i felt much better.  I didn't test again that day, but I did notice a few things: My bourgeoning cold symptoms suppressed. Whether that's my own badass immune system or the MycoImmune or what, I don't know. But, I didn't get sick. I felt a mood increase, a brain speed increase, and even a slight increase in my wit. At least I thought I was funnier my wife and kids may not :). I felt a natural energetic increase. Despite feeling the start of a start of a cold, the symptoms decreased and really dissipated over the next 24 hours. I didn't experience a drop off or anything like that. Perhaps it was the 3pm cup of tea I had while conversing with my folks at their house, but I didn't get tired until my typical repose around 915pm.  This was my first experience with Ketoneaid, or any other sort of ketone ester. I like it. I can absolutely see why it's getting these really exceptional results for people. Of course, I took the mind protocol, which Frank explained to me in an email. But the other protocol is for performance and I can clearly see how it's effective in that respect.  I'd do it again for sure. And since it sounds like the goal is to make it available at convenience stores everywhere (but probably health food stores and amazon before that), it sounds like everyone will have the chance to try it.  I don't know of any research that says exogenous ketone esters are harmful. In conditions of glucose deprivation, such as fasting or carbohydrate restriction, ketogenesis serves to reduce our needs for glucose. This reduces the need to engage in the energetically wasteful process of gluconeogenesis, which would otherwise be extremely taxing on our skeletal muscle if dietary protein were inadequate. Ketogenesis mainly occurs in the liver. The biochemical event that leads to ketogenesis is an accumulation of acetyl CoA that cannot enter the citric acid cycle because it exceeds the supply of oxaloacetate. The set of physiological conditions that provoke this biochemical event are as follows: free fatty acids from adipose tissue reach the liver, providing the energy needed for gluconeogenesis as well as a large excess of acetyl CoA. Oxaloacetate, with the help of the energy provided by free fatty acids, leaves the citric acid cycle for gluconeogenesis. These events increase the ratio of acetyl CoA to oxaloacetate, which leads to the accumulation of acetyl CoA that cannot enter the citric acid cycle and therefore enter the ketogenic pathway. This pathway results in the production of acetoacetate, a ketoacid. Acetoacetate can then be reduced to beta-hydroxybutyrate, a hydroxyacid, in a manner analogous to the reduction of pyruvate, a ketoacid, to lactate, a hydroxyacid. Acetoacetate is an unstable beta-ketoacid just like oxalosuccinate (covered in lesson 6) and can also spontaneously decarboxylate to form acetone, a simple ketone that is extremely volatile and can evaporate through the lungs, causing ketone breath. This lesson covers the basic mechanisms of ketogenesis and sets the ground for the forthcoming lesson on the benefits and drawbacks of ketogenesis in various contexts. She will share with us her journey from being a wrestler in her younger years, earning two bachelor degrees to being a Figure Competitor, as well as, a fitness coach. She tells us a bit about how somewhere along the way, marketing wasn’t everything she thought it’d be so she pivoted a decided to go another route to start her own business. BIO Born, Kalli Youngstrom, from Saskatchewan, Canada on February 14, 1991 Grew up wrestling from the 4th grade up until her senior year of highschool, mostly with the boys as there weren’t many girls competing. Received a B.A. in Psychology as well as Marketing A fitness coach, KY Fitness and Nutrition Consulting...
