Claim your complimentary gift of my exclusive mini weight care guide today!Link: Weight Care Guide — Dr. Francavilla Show (thedrfrancavillashow.com)Have you ever wondered if counting calories is actually worth your time — or if it's just another diet trap dressed up in a tracking app? In this week's episode, we talk about just that, and the answer is more nuanced than a simple yes or no. Calories are real, they matter, and paying attention to them can be genuinely useful — but the way many people approach calorie counting is often where things go sideways. Here's the thing though — calories are calculated using a lab device. Which means it doesn't take into account how the body actually processes those calories. You digest food, respond to it hormonally, and process it completely differently depending on what you're actually eating, not just how much. Protein burns some of its own calories just through digestion. Fiber doesn't get absorbed at all. And the same 100 calories from apple juice versus a whole apple with peanut butter will hit your body in completely different ways — one spikes your blood sugar and leaves you hungry again fast, the other keeps you full and satisfied for hours. Same number on paper, totally different story inside your body. That's why the old "calories in, calories out" model, while not entirely wrong, is a pretty significant oversimplification. Your metabolism adapts, your hunger hormones shift, and the quality of what you eat plays just as important a role as the quantity. Calories are one important piece of the puzzle — but they're far from the whole picture. In this episode, we cover:Why Not All Calories Are Created EqualMacros, Metabolism, and Why Your Body Isn't a MachineLog Like a Scientist, Not a JudgeWhen You've Already Done the WorkDitch the "I Blew It" MentalityThere's a lot more where this came from — tune into the full episode and you might just walk away thinking about food completely differently.Connect with me:Instagram: doctorfrancavillaFacebook: Help Your Patients Lose Weight with Dr. FrancavillaWebsite: Dr. Francavilla ShowYoutube: The Doctor Francavilla ShowGLP Strong: glpstrong.com

Should you count calories? A century ago, a Los Angeles doctor named Lulu Hunt Peters sold two million copies of a book that taught American women to count calories as a patriotic duty during WWI. She invented the 100-calorie snack pack. She set the 1,200-calorie floor that still haunts diet apps in 2026. The framework she popularized is still running your relationship with food. This week, Zoë and Kylee dig into the question every active person has wondered about: should you be counting calories? They trace how a unit of heat invented to measure factory worker rations became the dominant logic of American eating. Where the 2,000-calorie label on every food package actually came from (it isn't science, it's a 1990 design choice). Why calorie counting is legally allowed to be 20 percent wrong before it ever reaches your plate. And why a framework with this many cracks has held on for a hundred years.Along the way: what calorie counting did to the American food supply during the low-fat era, what the Biggest Loser metabolic adaptation research actually showed, why even registered dietitians can't accurately track their own intake, what set point theory says about why restriction backfires, and whether calorie tracking apps are tools, traps, or both. For athletes, the questions that actually matter for performance, and what the research says about who calorie counting helps and who it harms.Plus: the early feminist origins of dieting (yes, really), why your microbiome is doing math your app can't see, and why this number keeps its grip on us even when the science says it shouldn't.Listen for the full story. This episode is brought to you by:rabbit — Built by runners, for runners. Have you ever wondered if you should get liposuction when you need skin removal surgery after major weight loss?In this mailbag episode, Dr. Cooper, Mark, and Andrea tackle questions from listeners around the world, from Germany to Alabama to Chicago. They discuss why restricting calories while on GLP-1 medications can actually work against you, address the reality of finding metabolic-informed doctors internationally, and explain the science behind fat cell removal during skin surgeries. Plus, they share details about the newly approved oral GLP-1 medication orforglipron (Foundayo) and why vegetables, fats and starches matter even when you're protein-focused.KEY TAKEAWAYSRestricting calories on GLP-1 medications can lower your metabolic rate and weaken your body's natural GLP-1 productionLiposuction during skin removal surgery may disrupt leptin signaling, though males may be less affected than females due to naturally lower leptin levelsFinding metabolic-informed doctors globally remains challenging, but obesity medicine certification and Canadian and European obesity organizations may offer better resourcesThe oral GLP-1 medication orforglipron will likely be less expensive but also less effective than dual-agonist medications like tirzepatideMechanical eating without calorie counting often produces better long-term results than restrictive approachesVegetables provide essential micronutrients and support healthy microbiome function that protein alone cannot replaceMajor weight loss surgery like tummy tucks is serious surgery that requires careful consideration and qualified surgeonsNOTE: This episode was recorded before Foundayo (orforglipron) was released on the market. In this medication-prevention focused episode, Dr. Scott Watier and Tommy Welling analyze a late-2025 systematic review from Frontiers in Nutrition examining how simple time-restricted eating without any calorie counting significantly improves blood pressure and cardiometabolic markers in non-diabetic adults. They reveal that participants using 6-10 hour eating windows experienced drops in systolic and diastolic blood pressure, heart rate, fasting glucose, fasting insulin, HOMA-IR, BMI, and triglycerides—all without tracking a single calorie or changing food choices—challenging the eat-less-move-more paradigm that fails 95% of dieters long-term. The hosts explain why blood pressure and cholesterol medications are typically the first dominoes to fall in the medication cascade, and how this study demonstrates that compartmentalizing eating times creates deliberate decreases in insulin levels across the day, making it harder for your body to guard long-term fat stores. They break down critical timing insights showing that metabolic benefits truly kick in at the 12-week consistency mark, with weight loss predicting blood pressure improvements more than duration alone, while emphasizing that even a modest eating window serves as foundational root-cause therapy that can help listeners avoid dependence on the medical system and medication cascade that dominates American healthcare. In this evidence-packed episode, Dr. Scott Watier and Tommy Welling dissect a December 2025 systematic review and meta-analysis from Frontiers in Nutrition examining how intermittent fasting improves metabolic syndrome outcomes compared to traditional calorie restriction. They reveal that participants with metabolic syndrome—defined as having three or more risk factors including elevated waist circumference, high triglycerides, low HDL, elevated blood pressure, and high fasting glucose—experienced significant improvements in fasting blood glucose, A1C, insulin resistance, LDL cholesterol, and inflammatory markers with high certainty of evidence when using various IF protocols. The hosts explain why smart, experienced people spend years tracking and counting calories yet feel stuck, emphasizing that intermittent fasting addresses the root drivers of metabolic dysfunction rather than just creating a caloric deficit through willpower and restriction. They demonstrate how modified alternate-day fasting showed 50-100% greater reductions in inflammatory markers over 16 weeks, and why time-restricted eating delivers faster blood sugar improvements, providing practical guidance on matching your fasting pattern to your metabolic profile while focusing on beneath-the-surface changes happening long before the scale or the world can see them. Nutrition Nugget! Bite-sized bonus episodes offer tips, tricks and approachable science. This week, Jenn is talking about zero-calorie drinks and whether they are really the guilt-free option we have been led to believe. A well-known Copenhagen study compared four groups of people who drank a liter a day of regular soda, diet soda, milk, or water for six months, and the results were surprising enough to stop anyone mid-sip. Could a beverage with absolutely no calories still be working against your blood sugar, waistline and your metabolism? What do your gut, your pancreas, and even your taste buds have to do with it? Jenn digs into the science, questions the study's details, and shares what she has seen play out in real life with herself and her clients for years. But before you toss your diet soda or defend it to the end, you should hear what Jenn has to say about who this affects, why, and whether the calorie count on the label is telling you anywhere near the whole story. Are you struggling to lose weight and wondering if your metabolism is holding you back? In this episode of The Exam Room, host Chuck Carroll sits down with Dr. Neal Barnard to separate metabolism myths from science — and reveal how food choices can dramatically change how many calories your body burns every day. In this episode, you'll learn: - Why "slow metabolism" is often misunderstood — and how it really works - How high-fat diets can slow calorie burn at the cellular level - Why restrictive dieting backfires and leads to weight regain - How a low-fat, plant-based diet boosts the thermic effect of food - The role fiber plays in fullness, gut health, and weight loss - Why age does not prevent metabolic recovery - How to increase calorie burn without counting calories or starving If you're ready to stop fighting your body and start working with it, this episode offers a science-backed, sustainable approach to weight loss.
In this episode of Mastering Menopause, I follow through on a promise from my last episode and dive into the research comparing GLP-1 medications with aggressive dieting and the results surprised a lot of people. This is not a GLP-1 bash. These medications absolutely have their place, especially in diabetes management. But when it comes to fat loss, inches lost, and momentum, the data tells a different story.I reference and credit Not Another Nutrition Podcast, where Martin MacDonald breaks down this study in detail. Please give it a listen, it's way more in depth as Martin tackles common misconceptions like “GLP1s are the easy way out,” “Aggressive Diets require extreme willpower,” & “Pills do not change habits!” and explains why both GLP1s and Aggressive Diets remove food noise in different ways.A 12-week study conducted at the University of Nottingham that directly compared:A GLP-1 group using semaglutideAn aggressive diet groupA combination of bothThe results were clear. The aggressive diet group lost more than double the weight, 29 pounds vs 13 pounds, with significantly greater fat loss, despite the appetite suppression effects of GLP-1s. Even more surprising, combining GLP-1s with aggressive dieting did not improve results at all. No extra fat loss. No better adherence. No added benefit.I also talk about:Why aggressive dieting does not require more willpower when done correctlyWhy there were zero dropouts in the aggressive diet groupHow GLP-1 users often struggle to eat enough protein due to nauseaWhy muscle loss is a real concern for midlife women using GLP-1sWhy aggressive dieting removes food noise, decision fatigue, and burnoutIf you're a woman in your 50s who's been stuck eating 1,200 calories, frustrated by slow progress, or wondering whether GLP-1s are your only option, this episode will challenge what you've been told. This conversation isn't about extremes. This episode breaks down how undereating is quietly wrecking the metabolism of women in their 50s. I'm sharing the real story of a woman named Carol, who came to me stuck, frustrated, and convinced her menopause belly was permanent.Carol was living on 1,000 to 1,200 calories a day, cutting carbs, avoiding foods she loved, and pushing through exhaustion because she thought that was the only way to lose weight. Instead, her metabolism downshifted, her energy crashed, her sleep worsened, her belly grew, and nothing she tried moved the scale. In this episode, I walk through exactly why this happens, the myths that keep women stuck, and how to actually repair a metabolism that's been suppressed from years of undereating. You'll learn the signs your metabolism needs help, why carbs are essential for women in menopause, and how a reverse diet can completely change the way your body responds to fat loss.If you see yourself in Carol's story, this episode will help you understand what's going on inside your body and what to do next. And if you want to learn how we reverse diet our clients out of this situation then join me for my Festive AF Masterclass. Join Dr. Marianne Pinkston and guest Amber Abila, an adult binge eating coach and expert, for a deep dive into Binge Eating Disorder (BED). Dr. Pinkston shares her personal journey of gaining over 300 pounds, losing weight, and realizing she was struggling with an eating disorder later in life. ***JOIN THE NEXT MASTER YOUR FASTING CHALLENGE THAT STARTS August 13th, 2025!*** We'll GUIDE you on how to FAST to LOSE FAT for good, and use ‘fast cycling' to achieve uncommon results! REGISTER HERE! Click the link for DATES, DETAILS, and FAQs! This episode reveals game-changing research from a meta-analysis of 30 randomized controlled trials with 1,341 participants that will transform how you approach intermittent fasting. Dr. Scott and Tommy break down the shocking discovery that fasting without calorie counting produces DOUBLE the weight loss compared to fasting while tracking calories - 6.22 pounds versus 3.22 pounds respectively. More importantly, only the non-calorie counting fasting group saw significant improvements in fasting glucose (8-point drop), insulin levels (6.3 point reduction), cholesterol, triglycerides, and blood pressure, while the calorie-controlled group showed no consistent metabolic improvements. Learn why obsessing over hitting your daily calorie targets during eating windows actually sabotages your fasting benefits by potentially causing you to overeat, creating exaggerated blood sugar spikes, and interfering with natural hunger and satiety cues. The hosts explain how fasting naturally creates caloric restriction through appetite regulation and circadian biology optimization, making manual calorie counting unnecessary and counterproductive. Discover why the simplification approach - choosing an eating window and sticking to it consistently while prioritizing protein - outperforms complicated tracking methods for sustainable weight loss and metabolic health. This episode provides the scientific validation that fasting works best when you trust the process and let your body's natural mechanisms guide food intake, rather than forcing predetermined calorie targets. Essential listening for anyone frustrated with calorie counting approaches or wondering whether they need to track macros while intermittent fasting. The research conclusively shows: fasting simplifies everything while delivering superior results. Take the NEW FASTING PERSONA QUIZ! - The Key to Unlocking Sustainable Weight Loss With Fasting! What if calories aren't the whole story? In the realm of nutrition and health, the traditional belief has been that counting calories is the ultimate key to managing one's diet effectively. But what if we told you calories might not be the key to a healthy diet? Delving beyond the calorie count, this question encourages us to explore the multifaceted elements that influence our health and well-being, such as food quality, nutrient content, and individual genetic factors. Giles Yeo, a biologist and professor of molecular neuroendocrinology at the Medical Research Council Metabolic Diseases Unit, and scientific director of the Genomics/Transcriptomics Core at the University of Cambridge, challenges the conventional wisdom surrounding weight management. In this episode of The Greatness Machine, Darius is joined by Giles to shed light on the importance of food quality, nutrient partitioning, and the role of genetics in our individual responses to diet and exercise. His perspective invites us to reevaluate our approach to nutrition, weight loss, considering a more holistic view of health that goes beyond calorie counting. Topics include: What sparked Giles' interest in the genetics of obesity and body weight Giles explains what influences body weight and health The importance of educating children about nutrition and healthy choices Giles shares the impact of bariatric surgery Why it matters to maintain muscle mass while losing weight The crucial role of resistance training to preserve muscle and overall health And other topics… Sponsored by: Constant Contact: Try Constant Contact free for 30 days at constantcontact.com.
