Siobhan Huggins is an independent researcher with interests in cholesterol metabolism, inflammation, and metabolic diseases.She first went on a ketogenic diet for weight loss in August of 2016, and found that it resulted in numerous physical and mental health benefits. The number one problem for people being unhealthy is the toxic food environment, such as processed and highly palatable foods. Most companies drive the reward centres of the consumers' brains, similar to a drug, which makes unhealthy products hard to avoid. [16:43] Calories In, Calories Out: Calories Matter Eating less and exercising more, creating the negative energy balance that causes weight loss, is the standard advice for people who are dieting. However, it is not the most effective. A hormonal condition may be the number one factor in metabolic health and ideal body weight. Artificial dichotomy has two camps: the calorie camp, or the energy balance camp, and the hormonal camp or the insulin camp. Today, I am lucky to have here with me, the CEO and co-founder of the Fasting Method Program, clinical educator, and researcher, Megan Ramos. Megan’s major role in the The Fasting Method is to facilitate the patient’s education on the Fasting Method dietary principles and counsel them on their fasting regimens. Megan, along with her colleague Dr. Jason Fung, believe that both what you eat and when you eat matter. The Fasting Method utilizes therapeutic fasting and time restricted eating protocols with their patients to help them reverse their type 2 diabetes and achieve sustainable weight-loss. They encourage their patients to eat real foods and adopt a low-carb lifestyle in addition to the fasting. Megan was diagnosed with non-alcoholic fatty liver disease (NALFD) at age 12, polycystic ovarian syndrome (PCOS) at age 14, and type 2 diabetes (DMII) at age 27. She reversed these metabolic conditions and achieve an 86-pound weight loss by incorporating intermittent fasting with a low carb diet, and is now passionate about helping others do the same. Megan lives in Toronto, Canada. She is a director of the nonprofit organization Public Health Collaboration Canada (PHC Canada), works on the editorial board of the Journal of Insulin Resistance and Team Diet Doctor. SALT: If I supplement more than about 2 g of sodium a day (usually in 2 L water, Redmond Real Salt) I get nasal congestion and have NO IDEA why (vasodilation or constriction? blood pressure?). Yet in the keto community it is recommended to eat 4 g sodium/day or more. Are the amounts and ratio of sodium and potassium just very understudied and based on the shit standard diet? NOBODY seems to have a good grasp on this. POTASSIUM: Ideal potassium falls in the 2 - 4 g/day range. The fact that sodium helps retain potassium complicates this further. Also, I can't supplement with NoSalt (potassium chloride) because it's said to be dangerous (I take 500mg anyways and haven't noticed anything bad such as heart palps.) But, if I were to take 2 - 4 g would it be dangerous? I can't get this from vegetables because I'm zero carb, and how on earth did our ancestors get it? It makes zero sense to me that you would need to eat inordinate amounts of spinach and chard just to reach this questionable potassium goal. I feel like I really have this diet bolted down, but the lack of specificity with electrolytes in the community is majorly frustrating. Please drop a knowledge bomb on me. Hi Jimmy and Dr. Cole, As a T1 diabetic for three decades, I’m constantly working on ways to control this disease. Keto has been a tremendous help over the past three months of being strict with it. I recently noticed a big flare up of my Keratosis pilaris (KP) and it’s spread to my thighs and calves when it has remained mostly on my arms since I was diagnosed. A clean diet is supposed to help improve KP, but this seems to have gotten worse than ever before. Is this because of my autoimmune disease or could it somehow be related to keto? In 1905, an English scientist named William Fletcher was the first to make the connection between lack of vitamins in food and sickness. He was researching the causes of the disease, Beriberi. Beriberi is a B1 (Thiamine) deficient related disease that causes impairment of the nerves and heart. Symptoms include loss of appetite, digestive irregularities, and numbness and weakness in the limbs and extremities. In 1906, Sir Frederick Gowland Hopkins, an English Biochemist, made the discovery that certain “food factors” were important to health. The concept of vitamins (vital amines), was formulated in 1912 by Casimir Funk, a Polish biochemist. The “e” was dropped several years later when it was discovered that vitamins were not nitrogen-containing amines. Casimir Funk discovered B1 in 1912 and together, Funk and Hopkins came up with the hypothesis of deficiency disease that says a lack of vitamins can make you sick. https://www.thoughtco.com/history-of-the-vitamins-4072556 B. Compose less than 1% of our body C. Most vitamins cannot be manufactured by the body so we have to get them from external sources D. The vitamins the body can make are D and K 1. D is manufactured in the skin when the skin is exposed to sunlight high in the sky. 2. K, specifically K2, is manufactured by the good gut bacteria in our gut, but it is only a small fraction of what we need for the body to perform its functions so supplementing is important. E. Vitamins function best when they have the cofactors with them like trace minerals, enzymes and other vitamins. 2. Classifications of vitamins A. Fat soluble 1. Vitamin A 2. Vitamin D 3. Vitamin E 4. Vitamin K B. Water soluble 1. The B vitamins 2. Vitamin C 3. Inositol 4. Choline 3. Roles of Vitamins in the body A. They act as helpers in metabolic processes 1. The metabolism of carbohydrate, protein, and fat 2. The production of stomach acid (B6) 3. The detoxification process, specifically Phase 1 (B2) B. Support tissue growth C. They support digestion, elimination, and immune function. D. They help prevent deficient related disorders and health problems from happening 4. Signs of vitamin deficiency A. You both do such a great job! I started eating keto in January 2017. Unfortunately I wasn’t too strict last year with way too many “cheats” and I had a terrible time breaking my sugar addiction. Still I dropped 30 pounds in the first 2 months. I also do intermittent fasting and short extended fasts. I very gradually dropped another 10 pounds since then for a total of 40. Dear Jimmy and Will, I am a 49-year old man who has been low-carb/keto for 2 years and I’ve lost 50 pounds and have kept it off. I don’t measure ketones because I don’t suffer from metabolic syndrome or diabetes so I can’t justify the expense of a breath meter or the pain of a finger prick. However, based on energy level, constant satiety, and mental clarity I’m pretty sure I am in ketosis more often than not. I have a couple of questions for you guys: 1. When my husband eats strict keto, his hands tingle and go numb especially when sleeping. The strange thing is if he eats lots of carbs this doesn't happen. He supplements with potassium and magnesium, but it isn't helping. 2. Why has my fasting insulin and hsCRP levels increased since adopting a ketogenic diet? Hi Jimmy and Will, I just had some blood work done and my lipid panel looks worse, although I understand that this isn't uncommon when you’re losing weight following a ketogenic diet. Maria Emmerich stated in one of her books that all the excess fat from the fat cells gets released into the blood when you go keto. So, that part makes some sense to me. However, my fasting insulin has also increased (it was 10 in June 2016 and is now 13) and my C-Reactive Protein reading is also dramatically higher (1.6 in June, now 7.7). I'm mostly just shocked about the fasting insulin since I'm not eating hardly any carbs at all and no sugar. I'd be interested to hear any ideas you might have. I eat chicken or beef (homemade) bone broth regularly, Sauerkarut (no sugar) regularly, avocados almost daily, red leaf lettuce salad with black olives, some onion (white or green) with Primal Kitchen Ranch dressing almost daily, free range eggs, grass fed meats whenever possible, Wellshire sugar-free bacon, grass-fed ghee, coconut oil, avocado oil, Chai tea sweetened with Stevia with whipping cream (grass-fed), Lakanto monkfruit chocolate, cheeses from grass-fed cows, some beets, Miracle Noodles from time to time, and ketogenic wine usually only once per week. That's pretty much the extent. No fruit, no starchy vegetables. I usually don't eat until about noon every day and try not to eat after 6:30 pm. Ivor Cummins is a Biochemical Engineer who in 2012 was disturbed by a set of his own abnormal blood test results. Started with 16/8 and then have been doing 18/6 for a long time. I’ve done a few 24 hour fasts, a 48 hour fast, and most recently a 42 hour fast for the purposes of fat loss. Hi Jimmy and Megan, I have been doing IF and keto for about a month and half and I love the combination because it allows me to enjoy real food when I eat and gives me excellent mental clarity. My purpose in IF and keto is to prevent Type 2 diabetes since my A1c has been 5.8 the past couple of years. I’m already seeing my blood sugar and A1c drop, so I’m very happy with my nutritional choices. However, I do have a concern that I’ve noticed. Before my IF and keto protocol, my periods usually come every 38 days. But since I changed my diet and added fasting, it has been 46 days since my last menstrual cycle and I still have no period. Hi Jimmy and Adam, I was listening to Dr. Rhonda Patrick on Joe Rogan’s podcast where she mentioned that bile acid, namely, deoxycholic acid, is necessary to emulsify fat for absorption in the intestine and that a high consumption of fat – over 60% of calories - on a regular basis means the body is subject to higher levels of this acid. She stated outright that deoxycholic acid in higher than usual quantities causes damage to the DNA, even going so far as describing it as carcinogenic. Dr. Patrick noted that a ketogenic diet will ultimately result in the failure of the enzymes whose function is to repair DNA and thus will put the person at greater risk of cancer. I am a Type 2 diabetic and have maintained a ketogenic diet for the past year. I was persuaded to adopt this diet by reading Keto Clarity and am regularly in a state of nutritional ketosis. My last HbA1c was 5.2% and my fasting insulin level was 4. I can’t imagine adopting some other approach despite the concerns about the higher levels of deoxycholic acid required to emulsify the fat. My question for you guys is about hypoglycemia. If a person starting keto experiences a hypoglycemic episode, what’s the best way to deal with it? I can do 2-3 days of a keto diet but then on the 3rd or 4th morning I wake up with low blood sugar levels and experience nausea, feeling shaky, unfocused, and sometimes seeing brown spots. My blood glucose dips into the 40s and resolves after eating an apple, or some raisins, or some sweet potato. But then I feel like crap for the whole day. I don’t get this reaction when I simply eat lower carb with some sweet potatoes or fruit during the day. But I really want to experience all the full benefits of being ketogenic. Hey Jimmy and Adam, I just lost my grandfather to pancreatic cancer. He chose to have no artificial nutrition through a feeding tube and IV in the last few days of his life. I believe that this allowed him to go into ketosis and eased his passing. I also found a reference to ketosis being beneficial at the end of life in an American Medical Association (AMA) publication. In researching this further, I found the following statement in the Education for Physicians on End-of-life Care Handbook by the AMA: "Educate families about the studies that demonstrate that parenteral or enteral feeding of patients at the end of their lives neither improves symptom control nor lengthens life. Help them to understand that anorexia may be protective, as the resulting ketosis can lead to a greater sense of well-being and diminish pain." So my questions for you guys is this: Is this common knowledge in the medical community in general? We were never told this by any of my grandfather’s medical caretakers and instead were encouraged to have him drink Boost with 20-30g sugar/serving right to the end. In light of this, could exogenous ketones have helped him ease the pain more and remain lucid enough to engage with the family a period of time longer before passing? I really hope that you can share about this on Keto Talk. At least this community may be able to help a dying loved one by NOT pushing sugary drinks on them even when the nurses and doctors insist. Or possibly asking for no dextrose in their IV, especially if the patient has already said that he is ready to pass. It may be helpful to share the AMA reference with family members and caregivers, and explain what ketosis is and that the patient may not be feeling hunger. I’m 46, not diabetic but I am overweight with a BMI of 28, and I’m hoping fasting will prevent the development of Type 2 diabetes, lead to weight loss, and re-calibrate my eating habits. So far I’ve lost 4 pounds, my clothes are looser, and I no longer feel polluted inside. Fasting days are not always easy, especially in this cold northern England climate, but I still look forward to them! I have recently started having significantly higher blood pressure readings than usual (138/75 upon waking). Is it possible that some of the bowel bacteria (which I understand make up a good deal of stool) are dying from not being fed? Dear Jimmy and Megan, I have read Keto Clarity, The Obesity Code and The Complete Guide to Fasting, so thanks for all these great resources. In The Obesity Code, Dr. Fung writes about a lot of adaptations that happen during fasting including a rise in epinephrine. But according to the book, those adaptations do not kick in until around Day 3 or 4 of extended fasting. I’ve heard that when you fast, it takes about 24-36 hours for your glycogen stores in the liver to become completely depleted and then you begin to burn your own stored body fat for energy. If that's true, then intermittent fasting would seemingly not work for losing weight because after about 16-20 hours of IF you start eating again. So how is it possible to lose weight on IF? Thus, it seems that extended fasting would be better than IF for the purposes of weight loss. Am I right? ErrataL Carl mentioned that a ketongenic diet is specifically good for Rheumatoid arthritis(RA), in fact of the two markers tested it only lowered one, but fasting was shown to lower both. So it may well be that fasting, which we know is easer if you are adapted to a ketogenic diet, is your best strategy to deal with RA.
