There has been a fundamental shift in understanding metabolic health and chronic disease, particularly Type 2 diabetes, challenging long-standing dietary dogma by emphasizing that insulin resistance is largely driven by overconsumption of refined carbohydrates, not dietary fat or red meat. While highlighting the success of carbohydrate restriction and ketogenic approaches, nutrition is being seen as a powerful tool for reversing diabetes—often more effective and sustainable than conventional medication. The implications are far-reaching, not only for individual health outcomes, but also for reshaping public health strategies in addressing today's chronic disease epidemic. In this episode, I speak with Gary Taubes, Sami Inkinen, and Dr. Greeshma Shetty about an approach to treating Type 2 Diabetes that works. I also discuss how red meat is not to blame, but we should be looking at high sugar and starch diets. Gary Taubes is an award-winning science and health journalist, and co-founder and director of the Nutrition Science Initiative (NuSI). You eat a little less, you exercise, and you don't live with obesity anymore—end of story. You don't need blood sugar medications, but if you've tried the conventional wisdom, as I think most people do, and it fails, then you start looking for alternatives.What makes a good scientist?24:34: When you do an experiment, you learn how you screwed up if you're a good scientist because, by definition, you're always doing something no one's ever done before because it's boring to do stuff that people have done before. So you're always doing something new. You're always working at the limits of what your apparatus or your observational equipment can do. And 99 times out of 100, you're going to screw up that first time out, and then you're going to learn how you screw up and you're going to fix it. Today, I am excited to share the first class in a series of lipid masterclasses with the amazing Dr. Thomas Dayspring! Dr. Dayspring is certified in internal medicine and clinical epidemiology and is a fellow of the American College of Physicians and the National Lipid Association. He was previously the Educational Director of a nonprofit organization and has served as the Chief Academic Advisor for two major cardiovascular labs. During that time, rates of overweight, obesity, and diabetes skyrocketed. Either the experts are wrong or people didn't follow the advice of experts. Two journalists, Gary Taubes and Nina Teicholz, say the experts are wrong. Saturated fats are good, not bad, and a calorie is not a calorie. The body doesn't process a 60-calorie serving of bacon the same way it does a 60-calorie slice of cake. Obesity and diabetes are a result of a hormone imbalance caused by eating more carbohydrates than the body can handle, they say. When we eat carbohydrates, they believe, the body works to keep the fat locked away in storage. The best diet is a high-fat, high-protein, and low-carb diet known as the keto diet.Naturally, many in the diet establishment, including at academic institutions like Berkeley and Harvard, have taken issue with the conclusions of Taubes and Teicholz. Mainstream experts argue that Taubes and Teicholz have misunderstood or misrepresented the science and downplayed the difficulty of sticking to the keto diet.Teicholz and Taubes have responded at length to their critics at their terrific Substack, Unsettled Science. And, now, Taubes is out with a response to his critics in the form of a major new book, Rethinking Diabetes: What Science Reveals About Diet, Insulin, and Successful Treatments. At 750 pages, Taubes clearly intended for this to be the best available review of the scientific evidence and history published to date. Any serious person who disagrees with Taubes' unorthodox views on diet and diabetes will need to contend with it. I interviewed Taubes about it for today's podcast. We talked about his critics and the challenge of sticking to keto, including for me. He told me that the book on diabetes flowed out of his 2016 book, The Case Against Sugar. “The liver has never evolved to see the doses of fructose that they get today… Biochemistry in the 60s and 70s demonstrated how fructose could cause this condition called ‘insulin resistance,' which is the fundamental disorder of type 2 diabetes.”
Joining me this week is investigative journalist, and author, Gary Taubes. You may have seen him on TV or heard him on other podcasts. He's got quite a history with hist latest books coming in the world of nutrition. Taubes is the author of books like The Case Against Sugar, The Case For Keto and now his latest is 'Rethinking Diabetes: What Science Reveals About Diet, Insulin, and Successful Treatments.'One thing that I found interesting, and that I appreciate about Gary, is that he's open to the idea that there are many options when it comes to nutrition. He has his own beliefs, which he says he's backed up with research. But he acknowledges that it can be hard to prove things, and sometimes research can be biased. What I really loved learning from his book, 'Rethinking Diabetes,' was the history of diabetes in America. Gary really went all the way back to the beginning to show how the disease has grown and also how our understanding an perception of it has changed. A hundred years ago diabetes was almost unheard of in the population. Now about 10% of our population has been diagnosed. And that's just the people that have been diagnosed. It's estimated that another 3% has diabetes but doesn't realize it. That's wild.I hope you enjoy our conversation.

The proof is the current Pandemic of Metabolic Illness happening. Obesity is a symptom of malnutrition—he elaborates. Ruminant Revolution Peter is encouraging a “ruminant revolution,” which is to improve the productivity and efficiency of ruminant agriculture globally. (20:00) He and Vinnie also remind people that doctors are not your boss, they work for you. (22:00) People need to become better informed for their benefit. They discuss how politics has entered the food system. (26:30) Peter gives examples from both Germany and Brazil about processes that both didn't work and do work. (28:00) He explains what arable land is (able to be plowed and grown). All arable land is agricultural, but not all agricultural land is arable. Vinnie asks Peter's opinion about the recent uproar in Ireland about killing thousands of cattle to reduce emissions. (41:00) We must figure out how to feed the world the appropriate amount of animal-sourced food, for public health. Even if the U.S. got rid of animal agriculture, it would only reduce global emissions by approximately 2%. However, it would also imbalance the food system and reduce nutrition globally. One study that is never discussed states that a wildlife-dominated savannah and a livestock-dominated one are equal. (58:00) Those who are pushing the Dietary Guidelines for metabolic health are at least 10 years behind on the science. He has an amazing weight loss story—losing over 210 lbs over the course of his dietary journey—with high points and low points and lessons learned along the way. Having been disappointed again and again by a variety of different diets, Ken discovered Gary Taubes' book, Why We Get Fat, which set him on the path to finally attaining health and sustainable weight loss. Diagnosed with Type 1 Diabetes when he was just 11, Dr. Richard Bernstein has lived with the condition for a remarkable 78 years. At the age of 89, he continues to serve patients. Plus, Gary Taubes inspired him. Sam was really surprised to learn that that cancer was associated with diabetes, insulin resistance, obesity, and cardiovascular disease. He wanted to dig further on the connection between cancer and metabolic disease. [02:10] The Nazis Protected Warburg For His Cancer Research In 1923, Warburg makes this famous discovery called the Warburg Effect. Cancer cells take up a lot more glucose than other cells and burn it without oxygen; they ferment it, just like microorganisms do. Warburg also has made other discoveries about how cells breathe and use oxygen. He wins the Nobel Prize in 1931. In 1933, the Nazis come along. Despite having a Jewish father and being openly gay, the Nazis protected Warburg because they were interested in his cancer research. [06:50] Warburg's Studies Started To Disappear After The War After the war, Warburg's cancer research really starts to disappear. Australia in the 1950s famously discovered the structure of DNA. Eventually, we had a new sort of genetic understanding of cancer where we can now identify specific mutations. So, how cells take up glucose and shift their metabolism was considered irrelevant because of the study of genetics. Warburg's studies take peoeple back to diet, and there's a huge bias against taking diet seriously. [12:20] What Sugar Does To Promote Cancer Growth If cancer is about these metabolic shifts inside of a cell, what is it about the way we eat that affects the way a cell eats? In America, one out of three women are diagnosed with cancer within their lifetime. For men, it's about one in two. Cancer is closely linked to obesity. 13 of the deadliest cancers have been solidly linked to obesity. Sam wanted to know if obesity was causing cancer or if something was driving both. Elevated levels of the hormone insulin are responsible for obesity and give microscopic cancers a chance to grow and thrive. What is it about our diet that causes insulin to rise? Sugar! The increase in sugar consumption maps pretty closely to the rise of diabetes, heart disease, and cancer. [19:30] How A Keto Diet That Will Help You Prevent Cancer Sam is a huge believer that keto is a logical way to prevent cancer. The single most important thing you can do to improve your odds for cancer prevention is to keep your insulin levels low. Hyperinsulinemia promotes the growth of cancer. So, it's important to follow a diet that keeps your insulin levels down. [22:20] Intermittent Fasting Is Another Great Way To Lower Insulin Intermittent fasting is one very successful strategy for keeping insulin down. If you can fast consistently, it seems to work quite effectively on lowering insulin. Fasting can lead to weight loss because it lowers insulin levels in the body. Sam wants to point out that thin people can still have insulin resistance. In some ways, being naturally thin can actually be a disadvantage because you can't store subcutaneous fat. [26:30] How To Optimize Insulin and Glucose Levels To Be Proactive Instead of Reactive You can't outrun a bad diet. However, exercise can help you feel better and make you more inspired to follow a low-carb diet. After workouts, you can be inspired to eat healthily and continue to feel good. Once you understand that sugar is linked to cancer, it should help you ditch it during mealtime. [38:30] Finding Therapies To Target Specific Cancer Mutations When cancer can eat lots of glucose, it's no longer thinking about dying; it's thinking about doing a lot more things. Based on which particular mutation you have, the oncologist will devise a treatment. Unfortunately, targeted therapies to specific mutations haven't been as successful as we would have liked. However, there are some cases where therapies will successfully target mutations. [42:25] The Best Preventative Measures To Preventing Cancer In The Future Eliminate sugar from your diet. Definitely eliminate liquid sugar from your diet. If you are insulin resistant, you need to reduce those insulin levels. Stop smoking. Cut out refined carbohydrates. AND MUCH MORE!
