We pulled out another favorite out this week - Dr. Zanotti's deep-dive on tracheostomy emergencies. Tracheostomies are common in ICU patients. Every intensivist should be capable of recognizing and providing initial management to potentially serious complications associated with tracheostomies. Dr. Zanotti is joined by Drs. In this Bell Work Talks Dr. Goolsby discusses the processed used to develop consensus recommendations for healthcare in mass shootings. This project was based in part on the fact that in 2021 702 people died in mass shooting incidents in the US. To define best healthcare response to these incidents, the Uniformed Services University's National Center for Disaster Medicine and Public Health hosted a consensus conference of emergency medical services (EMS) clinicians, emergency medicine physicians (EM), and surgeons who provided medical response to six of the nation's largest recent mass shootings. The eight recommendations addressed readiness training, public education, triage, communication, patient tracking, medical records, family reunification, and mental health services for responders. Craig Goolsby, MD, MEd, MHCDS, FACEP is Chair of the Department of Emergency Medicine at Harbor-UCLA Medical Center and a Professor of Clinical Emergency Medicine at the David Geffen School of Medicine at UCLA. He is a member of the International Liaison Committee on Resuscitation's First Aid Task Force, the American Red Cross Scientific Advisory Council, and is a leader in medical education and military-to-civilian knowledge transfer, particularly for the Stop the Bleed Campaign. His multi-million-dollar research portfolio has focused on bleeding control, tourniquet usage, first aid and public response, mass casualty incidents, and developing novel educational programs and tools. Dr. Goolsby is a distinguished graduate of the United States Air Force Academy (BS), an AOA graduate of the Tulane University School of Medicine (MD), earned his Master of Education in the Health Professions degree from Johns Hopkins University, and completed a Master of Healthcare Delivery Science degree at Dartmouth College's Tuck School of Business. He has received many awards celebrating his work. Dr. Goolsby served in the Uniformed Services University's Department of Military and Emergency Medicine for a decade prior to his current position. He also previously served as an active-duty military officer, including two tours as the flight commander and medical director of the Air Force's emergency department in Balad, Iraq. California, and especially the Bay Area, continue to make remarkable progress against COVID-19, with fewer people in the hospital and steadily dropping rates of transmission, But there are pockets of the state where the opposite is true. In the far northern part of the state and in the Central Valley, vaccination rates remain low and hospitals are overwhelmed. In Fresno, ICUs are packed and there's a shortage of hospital beds and ambulances. The delta variant continues to surge there and the pandemic remains an urgent crisis. In this second half of the two-part episode, Dr. MJ Erickson-Hogue interviews Dr. Lori Weichenthal, MD and Dr. Jay Kaplan about the importance of wellness for medical caregivers and the effect that it has on patients. Their discussion notes the clinical areas most at risk for burnout as well as how to prevent and treat it. They also identify actions that caregivers can take to embed self-care within their daily routines.   Both Dr. Weichenthal and Dr. Kaplan are involved in national programs focused on clinician wellness.   Dr. Jay Kaplan, MD is the Medical Director of Care Transformation for LCMC Health in New Orleans. He is the Clinical Associate Professor of Medicine, Section of Emergency Medicine at LSU Health Sciences Center and is the attending emergency physician and academic faculty for the Emergency Medicine Residency at the University Medical Center New Orleans. Dr. Kaplan is a past President of the American College of Emergency Physicians and current national faculty for ACEP, and he has been a member of the National Academy of Medicine Action Collaborative on Clinician Well-Being and Resilience for the past several years.   Dr. Lori Weichenthal, MD, FACEP, RYT is the Assistant Dean of Graduate Medical Education at UCSF Fresno. She is also the Associate Program Director of Emergency Medicine and Professor of Clinical Emergency Medicine. Listen to Dr. Nicholas Caputo in conversation with Dr. Allie Sharma as he describes his experience as an Emergency Medicine physician in NYC pre and post-pandemic and the toll it is taking on he and his colleagues. Dr. Nicholas Caputo completed his internship in General Surgery at Beth Israel Medical Center in New York, NY, his residency training in emergency medicine at NYC H+H/Lincoln, where he served as a Chief Resident and his Fellowship training in Critical Care/Retrieval Medicine with RDH/Careflight, Darwin, Northern Territory, Australia. He is currently Associate Professor of Clinical Emergency Medicine at the Weill Medical College of Cornell University and is an attending physician Associate Chief of the Department of Emergency Medicine at NYC H+H/Lincoln. He is also an attending emergency physician at the Columbia University Irving Medical Center/New York Presbyterian Hospital. Dr. Caputo's research seeks to understand the evidence behind the conventional wisdom practiced in emergency departments across the world in order to determine the efficacy of current management strategies (such as preoxygenation in RSI, apneic oxygenation during intubation) in order to improve safety and quality outcomes for patients. He is a Major in the United States Army Reserve, Medical Corps, serving in the 947th FRST (Forward Resuscitative and Surgical Team). She completed a fellowship in Medical Education at UCSF Fresno where she is now the Fellowship Director and an assistant professor of Clinical Emergency Medicine. She is also the Deputy Director of EM:RAP, as well as the managing editor of C3, EM:RAP HD videos, and board review course Crunch Time.
