COL(R) Chuck Scoville is a Physical Therapist and received his graduate and doctoral degrees from Baylor University. He has served in various leadership roles at Walter Reed National Military Medical Center (formerly WRAMC), including Chief of Amputee Patient Care Service. He currently serves as the Chief of the Department of Rehabilitation. In this episode, Dr. Scoville discusses the unique training and responsibilities Physical Therapists possess in Military Medicine and explains how they function as combat multipliers on the battlefield. He was intimately involved in shaping policy to improve physical performance and conserving the fighting strength while working for the Office of the Army Surgeon General. He describes his experiences as a Physical Therapist assigned at West Point, charged with caring for a unique group of elite athletes, including Division 1 teams. Following 9/11, Dr. Scoville explains how the focus of Military PT evolved and how the increased incidence of patients with severe injuries, including upper and lower extremity amputations, led to innovation and advancement in prosthetics and rehabilitation strategies. Dr. Scoville relates the story of how the requirement for additional space and capabilities led to the development of the Military Advanced Training Center at Walter Reed and how these capabilities expanded to other sites in the United States with the help of Mr. Arnold Fisher and many contributors to the Intrepid Fallen Heroes Fund. Dr. Scoville relates the many challenges faced by patients with amputations and some amazing stories of how many were able to resume pre-injury activities including returning to serve on Active Duty. The rehabilitation efforts genuinely are a team effort with many critical components not limited to the healthcare providers but also include peer support efforts. Dr. Charles R. Rogers is a rock star in the field of cancer health disparities and inequities in men's health. He is a behavioral scientist and master certified health education specialist (MCHES). Dr. Rogers is a tenure-track Assistant Professor at the University of Utah's School of Medicine, where he founded the Men's Health Inequities Research Lab. Just last year, He also founded the https://crcequity.org/ (Colorectal Cancer Equity Foundation), focused on removing obstacles to equity among African-American men and other underrepresented populations by increasing awareness of Colorectal cancer. African-American men have died from colorectal cancer at the highest rates of any racial/ethnic group in the U.S. for more than 25 years. They are less frequently enrolled in clinical trials, and less likely to be offered genetic testing, palliative care, and other critical supports. Dr. Rogers is helping to lead the movement to change that picture. In some of Dr. Rogers' latest research, he shows that men who identify as “strong” are the least likely to get cancer screenings. If they get diagnosed with cancer, they are also less likely to seek second opinions, or get help for mental health problems. All of this puts these men at HIGHER risk to die from cancer, and to NOT be around to care for their families. So the men who think of themselves as strong, are actually behaving in a way that makes them vulnerable. This is the same exact barrier that I'm confronting in my advocacy work with Man Up to Cancer! The story of Ms. Susan Burton is a story of redemption and overcoming the odds. After the loss of her 5-year old son, addiction took hold of Ms. Burton sending her on a decades-long journey of incarceration and recidivism. After six separate stints in prison, Ms. Burton found sobriety and founded her own nonprofit, A New Way of Life, which aims to help other women break the cycle of incarceration.rnrnThrough this work, Ms. Burton pushed for reforms that reduced the mass incarceration of African Americans, overcrowding in the state's prisons, and for those stuck in the cycle to truly get back on their feet. Building on a deep theoretical foundation and drawing on numerous examples, Volden and Shipan examine how policies spread across the American states in Why Bad Policies Spread (and Good Ones Don't) (Cambridge University Press, 2021). The authors argue that for good policies to spread while bad policies are pushed aside, states must learn from one another. The three ingredients for this positive outcome are observable experiments, time to learn, and favorable incentives and expertise among policymakers. Although these ingredients are sometimes plentiful, the authors also note causes for concern, such as when policies are complex or incompatible with current practices, when policymakers give in to underlying political biases, or when political institutions lack the capacity for cultivating expertise. Under such conditions, states may rely on competition, imitation, and coercion, rather than learning, which can allow bad policies, rather than good ones, to spread. Volden and Shipan conclude with lessons for reformers and policymakers and an assessment of the overall argument based on state responses to the COVID-19 pandemic. Wellness initiatives within programs are nothing new. From structured lectures, wellness days, yearly retreats and more, programs have incorporated multiple strategies to ensure that residents and fellows are informed of how to care for themselves during training. But why do these initiatives fail to create a tangible response? And why do so many trainees still out of balance when it comes to training? Dr.Anum Baig, a PGY2 in psychiatry at Charles R.Drew University of Medicine and Science, discusses her experience of being a wellness liaison within her program and what she believes will work to enhance the wellness initiatives within training programs. In this episode, we discuss the following: What is the definition of work-life balance and how does it differ from wellness?
