Dr. Pacaccio graduated from the University of Illinois at Urbana/Champaign with a dual degree in biology and philosophy-focused on ethics.  In interview you will hear how this has helped shape his perspective and viewpoint.  He went on to complete his podiatric medical degree with Rosalyn Franklin University and did his 3-year surgical residency with the prestigious Inova Fairfax program.  He continued his education with a plastic surgery fellowship at Georgetown University Medical Center and then moved back to Illinois to start his practice with his wife who is also a podiatrist. Dean's chat welcomes Dr. Michael Dujela! Dr. Dujela practices in Olympia Washington and is currently the fellowship Director for the Advanced Surgical Foot and Ankle Fellowship- Washington Orthopedic Center. Listen in, as we dive deep into what fellowship training can offer. We'll hear the personal experiences of Dr. Dujela and how fellowship for him was an opportunity to finesse his skills and seek different opportunities.   He graduated from Brigham Young University with a Bachelor's in Science. He continued his Graduate education studying Genetics at Westchester University before going on to complete his Podiatric Medical Degree with the Ohio College of Podiatric Medicine. He completed his surgical residency training under Dr. Gerard Yu at the Foot and Ankle Surgical Residency Training Program, at University Hospitals Health System, in Southwest General Cleveland, Ohio. His journey continued into private practice and a desire to seek additional training experiences internationally. Tune in, as he describes this process and how he took his family with him on this journey through a Multi-Center, European AO Fellowship with multiple leaders in Foot and Ankle Surgery. Come along as he describes the opportunity he had to work in different hospitals in six different cities throughout Germany and Switzerland. He then went onto complete a US based Fellowship program under Dr. Thomas Lee, MD and Dr. Chris Hyer DPM at the Orthopedic Foot and Ankle Center in Columbus Ohio before returning to his practice in Washington.   We also discuss his experiences working in a multitude of leadership positioning throughout the profession including: current President-elect for the American Board of Foot and Ankle Surgeons and the American College of Foot and Ankle Surgeons; Chairman for the Annual Scientific Conference for the American College of Foot and Ankle Surgeons this year. He is the Past Chairman and a 5-year committee member of the National Education and Scientific Affairs Committee with the American College of Foot and Ankle Surgeons and has been Instrumental faculty in the collaborative effort between the AO Foundation and the American College of Foot and Ankle Surgeons to bring back the basic surgical principal courses offered through the AONA. Sharing data transparently, whether at a local or a national level, is a foundational contributor to healthcare quality. Atul Gawande wrote about this in his 2004 essay labeled ‘The Bell Curve'. Over the last several decades, we have seen many pediatric specialties organize and collaborate around data collection at a national scale to improve care. We've seen firsthand how our hospital's participation in a national cardiac critical care registry has driven quality improvement, better measurable outcomes and improved cardiac arrest rates for patients. Getting the data collection right is the first challenge. In between that first step and actually seeing improvements is a tremendous amount of work. In this episode, we explore the value of data sharing and transparency to improve outcomes for kids. Our experts for this episode are Carly Scahill, DO, and Kelly O'Neil. Dr. Scahill specializes in pediatric cardiology as a pediatric cardiac intensivist. She is also the Fellowship Director for pediatric cardiology and the cardiac intensive care unit (CICU), as well as an associate professor of pediatrics at the University of Colorado School of Medicine. Kelly O'Neil is a registered nurse and the Manager of the Heart Institute Data Registries and Outcomes. Before her current role, she was a CICU nurse for over ten years. Dean's Chat hosts Drs. Jeffrey Jensen and Johanna Richey are joined by Dr. Jacob Wynes. Dr. Wynes grew up in Baltimore MD where he had been mentored by many outstanding podiatric surgeons. He later went on to graduate from the Temple University School of Podiatric Medicine and continued on to complete a 4 year residency at the University of Pennsylvania, Penn Presbyterian Medical Center. Dr. Wynes then pursued an additional year of training as a clinical fellow at the Rubin Institute for Advanced Orthopedics, International Center for Limb Lengthening with a focus on foot and ankle deformity correction. At this time, Dr. Wynes works at the University of Maryland School of Medicine as an Associate Professor and chief of podiatric services within the Department of Orthopaedics (and newly established Division of Podiatry). Dr. Wynes currently treats patients at University of Maryland Medical Center. Dr. Wynes remains active in patient care, surgery, and research. From a young age, Dr. Tieraona Low Dog has been deeply drawn to the natural world and the field of natural medicine. Gulfcoast Ultrasound Institute YouTube Channel On this episode of The Sonography Lounge hosts Lori and Trisha sit down with Dr. Charlotte Derr & Dr. Allyson Hansen to discuss the GUARD Protocol (Gastric Ultrasound for Aspiration Risk Determination). With the increased use of GLP-1 Agonists (Semaglutide, with brand names such as Ozempic, Wegovy, Rybelsus, Etc) for both diabetes and weight loss, there has been an increased number of aspirations in patients undergoing procedures. One of the effects of GLP-1 agonists is delayed gastric emptying which keeps patients feeling "full" thus reducing the desire to eat and overall caloric intake and blood sugar variations. Typically 6-8 hours not eating is enough time for a patient about to undergo an elective procedure to digest and empty their stomachs thus reducing the risk of aspiration while under anesthesia. With patients on GLP-1 agonists, we are seeing gastric contents remaining for several days after the last meal which can increase the risk of aspiration.  Dr. Derr and Dr. Hansen, discuss how they are using ultrasound and the GUARD protocol to determine the volume of food in a patient's stomach before elective and emergency procedures. Guests: Dr. Gloria Sanchez, MD, MPH is a Harbor UCLA Family Medicine Residency faculty and is Board certified in addiction medicine. She has focused her career on assessing and trying to meet the needs of underserved communities. Teaching future physicians the critical skills necessary to assess and treat substance use disorders has become a primary pursuit. Her overall goal remains to create national initiatives in medical training and public policy that meet the needs of vulnerable patient's suffering from substance use disorders. Jessica is Dr. Sanchez's patient and, as of this podcast, a Patient Advocate. She is a 34 y/o woman who came to Harbor UCLA to deliver her second child. She met Dr. Sanchez then because she was looking for help with her fentanyl dependence. Jessica transferred from the hospital to an inpatient program, and then to a sober living location with her baby girl. Dr. Konstantin Slavin is Professor and Chief of Section and Fellowship Director for Stereotactic and Functional Neurosurgery in the Department of Neurosurgery at the University of Illinois at Chicago (UIC). Dr. Slavin graduated from medical school in Baku, Azerbaijan in the Soviet Union and completed his neurosurgery residency in Moscow. He then completed his second neurosurgery residency at UIC and a fellowship in functional and stereotactic neurosurgery at Oregon Health Sciences University in Portland, Oregon. Dr. Slavin is current President of the World Society for Stereotactic and Functional Neurosurgery and the Past President of the American Society for Stereotactic and Functional Neurosurgery. He is also the President-Elect of the International Neuromodulation Society (INS) and past Secretary of the North American Neuromodulation Society (NANS). For many years, he serves on the Medical Advisory Board of the Facial Pain Association, the premier patient organization for those who suffer from trigeminal neuralgia and other facial pain syndromes. In addition, he is on the Board of non profit organization “Neuromodulation Foundation”, the publisher of Wikistim, and for more than a decade was an Executive Committee member of the Joint Section on Pain of the American Association of Neurological Surgeons and Congress of Neurological Surgeons. Dr. Slavin has published in many books and peer-reviewed journals and is an associate editor or editorial board member for a number of publications, including Neuromodulation, Neurosurgery, Brain Sciences, Stereotactic and Functional Neurosurgery, Acta Neurochirurgica and others; he is the current editor-in-chief of Progress in Neurological Surgery. His first book on Peripheral Nerve Stimulation was published in 2011; another book, co-edited with Sam Eljamel on Neurostimulation: Practice and Principles, came out in 2013; the third one, Stimulation of Peripheral Nervous System: The Neuromodulation Frontier was released in 2015. Jeffrey Gladd, MD With twenty-one years of experience in the medical field, Dr. Jeff Gladd, MD is the owner of GladdMD Integrative Medicine and since 2019 has been the Chief Medical Officer at Fullscript.   Dr. Jeffrey Gladd graduated from Indiana University School of Medicine in 2001. He then went on to train in family medicine at Ball Memorial Hospital in Muncie, Indiana. After completing his residency, Gladd started a family practice that encompassed full spectrum care, including obstetrics, colonoscopy and endoscopy in Columbia City, Indiana In 2006.  After the birth of his second child, Gladd transformed his professional and personal focus toward health through nutrition. During the discussion, she emphasizes the importance of prioritizing self-assessment as a coach. Dr. King then explains how to find a surgical coach through the Academy for Surgical Coaching. The process requires a “coachee” to complete a form that identifies their personal goals, and then the “coachee” is paired with a coach from the pool of physicians from various specialties. Coaching experiences are performed through various platforms (e.g., in-person, video-based, etc.) and are ideally conducted for 3-, 6-, or 12-month periods. Dr. King also covers how outcomes and metrics are evaluated, describing the differences between technical and non-technical (e.g., judgment) skills. The doctors transition to emphasize the importance of incorporating coaching into a surgeon's practice. Dr. King shares 2 methods to do so, including (1) replacing 1-hour of grand rounds or meetings with 1 hour of coaching sessions, as well as (2) integrating coaching at the level of national societies (e.g., American Association of Gynecologic Laparoscopists). The doctors go on to highlight also incorporating coaching at the level of medical schools and residency in order to instill the mentality of continual improvement early on in a physician's career. “What can I do?” This is the question I get most often.  It's the question that early on pivoted our newsletter and then this podcast towards measurable action steps that help you feel better and drive systemic change. I don't have anywhere near all the answers to basically anything, so I usually answer people with “What CAN you do?”…to stall for time, but also to open up the conversation. Because the answer to what CAN you do is usually best found at the intersection of your interests or passions or skills, and the eleventy billion problems and opportunities we have before us. And so, almost a hundred and fifty published conversations later and ten times as many offline, some of my favorites remain talking to young people about their lived experiences and how they're directing it towards a problem and solution, towards a radically better world, however ambitious and however much I am a moron and don't understand the basic science behind it. But on the other hand, somebody's gotta support these folks. And so those of us with privilege, with a mic or a checkbook or both have to – at the very least – step up and ask a slightly different version of that question, which is: “How can I help?” My guests today are Lissie Garvin and Laura Stieghorst. Lissie is the Foundation and Fellowship Director at the new 776 Foundation, the philanthropic arm of the venture capital firm with the same name, both founded by Alexis Ohanian. Lissie helped run Reddit, was chief of staff at Initialized Capital, working alongside Alexis, and holds the same role as a co-founder at 776 the firm. Laura Stieghorst is one of the first 776 Fellows. She is the co-founder and CEO at BASICO2, where they are working to understand how we can safely rebalance ocean chemistry using natural mechanisms to remove CO2 from the ocean and restore coral reefs. No biggie, right? Combined, these two humans are a shining example of humanity's young people, our most affected, our most ambitious, doing hard things. Dr. Low Dog is an internationally recognized expert in dietary supplements, herbal medicine, women's health, and integrative medicine. In addition to her continued work as a clinician and educator, Dr. Low Dog has engaged in national health policy and regulatory issues for more than two decades. In 2000, she was appointed by President Bill Clinton to serve on the White House Commission of Complementary and Alternative Medicine, followed by a 3-year appointment to the Advisory Council for the National Institutes of Health National Center for Complementary and Alternative Medicine (NCCAM). Dr. Low Dog has been the elected Chair of four United States Pharmacopeia (USP) Dietary Supplements and botanical expert panels from 2000 until the present. A prolific scholar, Dr. Low Dog has published 50 research articles in medical/science journals, written 25 chapters for medical textbooks, authored four books with National Geographic, and is the co-editor for Integrative Women's Health by Oxford University Press. She was a Clinical Associate Professor of Medicine and Fellowship Director for the University of Arizona Center for Integrative Medicine from 2008-2014, served as the Founding Fellowship Director for the Academy of Integrative Health and Medicine from 2015-2017, and was a Founding Board Member for the American Board of Physician Specialties Board of Integrative Medicine. Dr. Low Dog's many honors of distinction include Time magazine's “Innovator in Complementary and Alternative Medicine” (2001), Bioneer's Outstanding Contribution to Medicine Award (2001), NPR's People's Pharmacy award (2010), New York Zen Center's "Contemplative Care Award" (2013), "Herbal Insight Award" from American Herbal Products Association (2015), Scripps Lifetime Achievement Award (2017), Nutrition Business Journal's Lifetime Achievement Award (2017), and the American Botanical Council's Fredi Kronenberg Award for Excellence in Research in Botanicals for Women's Health (2018).
