The group pinpointed B2G2 as the most active compound, but, “it's expensive and it takes a long time to isolate B2G2 from grape seed extract,” Tyagi says. This expense related to the isolation of B2G2 has limited the group's further exploration. So instead of purifying B2G2 from GSE, the group decided to synthesize it in the lab. The current study reports the success of this effort, including the ability to synthesize gram-quantity of B2G2 reasonably quickly and inexpensively. In the paper's second half, the group shows anti-cancer activity of synthesized B2G2 similar in mechanism and degree to overall GSE effectiveness. Their research followed individuals over an average of 11 years and determined that high amounts of sitting time were associated with increased risk of early death and cardiovascular disease.  Not surprising, those who sat the most and were the least active had the highest risk—up to 50 percent—while those who sat the most but were also the most active had a substantially lower risk of about 17 percent. “For those sitting more than four hours a day, replacing a half hour of sitting with exercise reduced the risk by two percent,” Lear notes. “With only one in four Canadians meeting the activity guidelines there's a real opportunity here for people to increase their activity and reduce their chances of early death and heart disease.” Acupuncture Can Treat Alzheimer's Disease Hubei University of Chinese Medicine (China), June 5, 2022  An increasing number of clinical and animal studies have confirmed that acupuncture is effective for the treatment of Alzheimer's disease. Moxibustion is reported to be more effective than electro-acupuncture for improving space-recognizing memory ability in aged mice, suggesting that moxibustion is another alternative or complementary therapy used to treat Alzheimer's disease. Dr. Yanjun Du and team from Hubei University of Chinese Medicine, China only used suspended moxibustion (also named warming moxibustion, scarring moxibustion, or herb-partition moxibustion) on Baihui and Shenshu  acupoints to observe the action of pre-moxibustion on preventing apoptosis in a rat model of Alzheimer's disease. The pre-moxibustion group was treated with moxibustion for eight courses (each course lasting for 6 days) prior to the exposure and 14 days after Aβ1–42 exposure.  Moxibustion prior to Aβ1–42 exposure was more effective than moxibustion after Aβ1–42 exposure in protecting the neuronal structure and lowering the apoptosis rate. Their findings indicate that a combination of preventive and therapeutic moxibustion has a beneficial effect for the prevention of Alzheimer's disease development. Men and women with brain condition could be helped by vitamin E Cangzhou Central Hospital (China), June 15 2022.  Leukoaraiosis is an abnormality in the brain's white matter that appears on magnetic resonance imaging (MRI) as white matter hyperintensities. In an article published in the International Journal of Neuroscience, researchers at Cangzhou Central Hospital in Hebei, China reported improvements in factors related to leukoaraiosis among adults given supplements containing vitamin E. “Leukoaraiosis (LA) is a disease manifested by demyelination and gliosis in white matter, mainly caused by cerebrovascular diseases,” Yan Wang and colleagues wrote.  “Leukoaraiosis is closely related to the expression level of inflammatory factors, oxidative stress, and vascular endothelial dysfunction in patients. Vitamin E may play antioxidant and anti-inflammatory roles in various diseases.” Participants received 200 IU, 400 IU or 600 international units (IU) vitamin E or a placebo daily for 12 weeks. At the end of the treatment period, blood samples were evaluated for the inflammatory factors C-reactive protein, complement C3 and C4, and matrix metalloproteins 2 and 9, as well as for markers of oxidative stress and endothelial function (function of the lining of the arteries, which is impaired in cerebrovascular and cardiovascular disease). Inflammation, oxidative stress and endothelial function improved in all groups that received vitamin E in comparison with the placebo. Cognitive function scores also significantly improved in the vitamin E-treated groups. Improvement in all measured factors was correlated with higher doses of vitamin E. Chlorogenic acid plus chromium improves insulin resistance and lowers obesity in mice University of Toledo, June 10 2022 In an article published in Nutrition & Metabolism, University of Toledo researchers report that a combination of chromium and chlorogenic acid (CGA), a polyphenol occurring in coffee, reversed the development of obesity and insulin resistance in mice caused by the intake of a high-fat diet. Sonia M. Najjar and her associates fed male BL6 mice a standard diet or a high-fat diet for seven weeks. During the last three weeks of the diet, the high-fat group received a daily oral supplement containing water, chlorogenic acid, or a combination of chlorogenic acid and chromium in a base that contained caffeine. Body weight was measured weekly, insulin tolerance was assessed at four and seven weeks, and glucose and glucose tolerance were assessed at the end of the treatment period. At four weeks, animals supplemented with chlorogenic acid and the combination of chlorogenic acid and chromium experienced weight loss within two weeks, and attained weights comparable to those of animals that received a standard diet. While insulin resistance after four weeks was greater among high-fat diet-fed mice compared to those fed a standard diet, supplementation with chlorogenic acid plus chromium reduced this effect, and restored it to a level comparable to that of standard-diet-fed mice. Glucose tolerance and glucose levels were similarly restored in animals that received the supplement combination.

Laura Borgelt, Pharm.D., BCPS - Professor, Departments of Pharmacy Practice and Family Medicine at the University of Colorado Skaggs School of Pharmacy and Pharmaceutical Sciences - talks to us about the recreational use of marijuana and its potential health consequences. Key Lessons: Cannabis (marijuana) use has increased substantially in the past decade.  Young adults are the most likely to report using marijuana in the past year or month but use among older adults (age > 65 years) is growing as well. Its important to ask patients in a non-judgmental, open-ended manner about their cannabis use including the intended purpose(s), frequency of use, and forms used. Numerous cannabis products are available. Inhaling (smoking or vaping) remains the most common method for use.  Edible products are available in a wide variety of food-like delivery systems (e.g. baked goods, candies).  Topical products are more commonly used for medical purposes. The THC concentrations found in cannabis products today are much higher than years ago. This has resulted in higher rates of unintentional overdoses leading to paranoia and psychotic symptoms. Overdosing is most common with edible products due their delayed absorption. There are several short-term and long-term health consequences related to marijuana use. Regular cannabis use can lead to neurocognitive effects including impaired memory. Cannabis use is more likely to have a detrimental impact for adolescents and young adults. Cannabis use during pregnancy also appears to have detrimental neurocognitive effects on children. There are several potential drug-drug interactions with THC and medications metabolized through CPY2C9 and CPY3A4 as well as receptor interactions. Pharmacists have an important public health role by screening for cannabis use, educating patients about the potential risks associated with marijuana use, and identifying potential drug-drug interactions. Kari Franson, Pharm.D., Ph.D., BCPP - Associate Dean for Professional Education at the University of Colorado Skaggs School of Pharmacy and Pharmaceutical Sciences - talks to us about the medical use of marijuana, THC, and CBD. Key Lessons: Cannabis plants have been used for medical, recreational, and industrial purposes for thousands of years. The two active ingredients in cannabis plants include tetrahydrocannabinol (THC) and cannabidiol (CBD) - collectively known as cannabinoids.  They have distinctly different pharmacological properties. Cannabis plants that contain less than 0.3% of THC by dry weight are legally classified as hemp.  Cannabis plants can be cultivated to have more (or less) THC and CBD content. Prescription products containing THC have been available for more than 30 years. A buccal spray containing THC + CBD has been approved (but not in the United States) for the treatment of muscle spasms/stiffness associated with multiple sclerosis.  A purified CBD product was approved by the FDA for the treatment of rare childhood seizure disorders in 2018. There is a long list of claimed medical uses of marijuana (typically contains high concentrations of THC).  Not all claims are supported by sufficient evidence and some have been refuted by the evidence.  Far less is known about the medical uses of CBD. Cannabinoids are erratically and slowly absorbed from the GI tract. There are substantial legal barriers to studying cannabis and its health consequences in the United States. Justin Rash, PharmD, talks with Danielle (Rhyne) Fixen, an Assistant Professor and Geriatric Clinical Pharmacist at University of Colorado Skaggs School of Pharmacy, about medical cannabis and older adults.  Marijuana has had a turbulent history in the United States. Starting in the mid-1990s, however, there was a push to introduce the medical benefits of cannabis to the American people once again—”once again,” because before the 20th century, marijuana was almost entirely legal. Beginning in the 1910s, states began to ban the sale of marijuana, eventually leading to a bill called the Marijuana Tax Act in 1937, which decreased the amount of hemp (a version of a cannabis plant) allowed to be produced in the U.S. Later on in the century, the stigma around marijuana grew and Richard Nixon signed into law the Controlled Substances Act, which included marijuana on a list of federally banned drugs alongside heroin, cocaine, and other narcotics. By the time the 1990s came along, though, states began adopting the medical benefits of marijuana, starting with California. About 15 years after that, marijuana itself—not just the medicinal qualities—became legal for recreational use in Washington and Colorado. As of 2017, 29 of 50 states have legalized some form of medical marijuana. A 2013 study revealed that four of five doctors approve of medical marijuana and that more than 90 percent of medical marijuana patients (of around 7,500 surveyed) say that medical marijuana has helped treat their conditions. Seniors were the largest age group in the study (more than 2,300 respondents). SenioRx Radio https://annual.ascp.com/exhibit/podcasts.html  See omnystudio.com/listener for privacy information.

Joseph Saseen, Pharm.D., BCPS, BCACP, CLS - Professor of Clinical Pharmacy and Family Medicine, University of Colorado Skaggs School of Pharmacy and Pharmaceutical Sciences - discusses the various credentials pharmacists can earn following graduation and licensure. Key Lessons Credentials include degrees, licensure, post-graduate training, and board certification. Earning a certificate is not synonymous with becoming board certified. Board certification requires candidates to meet specific eligibility criteria and pass a comprehensive examination to validate the breadth and depth of knowledge in the area of specialization. Board certification can give pharmacists a competitive advantage for employment and open doors to new opportunities. Candidates should consider preparing for a board certification exam either through a formal, structured program or forming a study group ... or both. Obtaining advanced credentials is ultimately about improving the quality of care pharmacists provide to patients. View and Download the Show Notes!

Highlights (full transcript at www.twopillspodcast.com): Meghan Jeffres graduated pharmacy school from University of Wyoming, PGY1 at Intermountain Healthcare in Salt Lake City Utah, PGY2 at Barnes-Jewish Hospital in St. Louis Missouri, Assistant to Associate professor at Roseman University College of Pharmacy in Las Vegas Nevada, then back to Assistant at the University of Colorado Skaggs School of Pharmacy. She loves creating, doesn’t mind assessing, hates writing about it. L: I just have to say after knowing Meghan and working with her, I disagree with that last sentence. She is fantastic at all three of those. M: Quality of output does not necessarily mean enjoying the process. But thank you. I came in to Academia through the side door. The first classroom I was in was at Roseman, which is a 3 year accelerated program with a block schedule. Students are in class from 8 to 3 and you are responsible for that day. You have six hours to teach and spend that time with them. You are forced to schedule time in your lectures to do things. The time management is an interesting process to learn. Roseman really focused on Mastery of learning and it was a 90% pass-fail curriculum. On average, your curriculum should be ⅓ to 1/2 Active Learning. Since then, I have become an advocate for active learning. Now, I'm really looking at the scholarship of active learning. One thing that I thought would be mundane was going through worksheets. In the classroom, as infectious disease teachers, there is no standard textbook. We are often creating our own content. I give them that basic information ahead of time in a flipped classroom kind of way. Then, in class, we go through a skeleton of a workbook. I thought students might find it boring, but they are so into it. They think it's really effective. Handouts, worksheets, workbooks, or something I do all the time. Surprising part is that it is effective and yet rudimentary. It's not graded and all of it is focused on their learning and their benefit. What has not been effective was taking ID fundamentals and skipping a couple of steps. That led to asking them to apply things that they were maybe not ready for. That can be very frustrating for even your highest achieving students. It will cause them to check out. There has to be gradual steps from memorization to application in the real world. There can be assuming that they can make connection or links that haven't been assessed yet in the classroom. For someone who is new to teaching, I think the best thing is to sit in the classroom. It does not have to be your content area. You should go listen to both clinical and science folks. It takes time, but your first year as faculty is when you probably have the most flexibility to do it. If there is another pharmacy school nearby, broaden your horizons and go there to listen. There are two things to look for. One is what is the teacher doing and two how are the students responding. Sometimes it's hard to take something that you created and put it out into the world, even to your own classroom. It will be critiqued and picked apart. There's a concern that they will hate this thing that you developed or put time into. Is it going to be embarrassingly terrible? You have to be risk tolerant. It also comes with practice. You also get much better at planning. You do not want to wing it or just have it be a free-for-all. Things go sideways quickly. It comes from repeated efforts of trying new things. You will just get better at trying new things. That in itself is a skill. When I was first introduced to strength finders, I thought that was such a great concept. Stop trying to fix what you're bad at and go ahead and spend time in what you're good at. That felt like a permission slip for me. One of the
