In this episode of the Society of Critical Care Medicine (SCCM) Podcast, Diane C. McLaughlin, DNP, AGACNP-BC, CCRN, FNCS, FCCM, is joined by Aarti Sarwal, MD, FAAN, FNCS, RPNI, FCCM, and Brian L. Erstad, PharmD, FCCP, FASHP, MCCM, to discuss the 2026 guidelines for neuromuscular blockade in adult patients with acute respiratory distress syndrome. The guidelines, “Society of Critical Care Medicine Guidelines for the Administration of Neuromuscular Blockade in Adults With Acute Respiratory Distress Syndrome,” were published in the March issue of Critical Care Medicine. Drs. Sarwal and Erstad discuss how the evidence in two key trials, ACURASYS and PETAL-ROSE, has helped shape the recommendations provided in the SCCM guidelines. Despite how influential these trials were in shaping the recommendations, only conditional recommendations were made due to low or very low quality of evidence. The lack of evidence proved to be a driving factor in including a call to action in the guidelines. Future research priorities largely revolve around precision medicine and finding more patient-specific interventions to improve patient outcomes. Erstad, PharmD, FCCP, FASHP, MCCM, is a tenured professor and interim dean at the University of Arizona R. Ken Coit College of Pharmacy in Tucson, Arizona, USA. Jill Bates, PharmD, MS, BCOP, DipACLM, FASHP, is the Deputy Executive Director of the National Pharmacogenomics Program within the National Oncology Program Office of the Veterans Affairs (VA). Dr. Bates is an expert in oncology and pharmacogenomics and has led the VA's Pharmacogenomics program that is being rolled out nationwide. We talk through her background in both areas as well as the VA's investment in pharmacogenomics and precision medicine. CancerNetwork® spoke with experts across multiple disciplines about the far-reaching impacts of the ongoing shortages of chemotherapy drugs including carboplatin and cisplatin, and how practices may be able to mitigate these challenges in the short and long term. Michael Ganio, PharmD, MS, BCPS, FASHP, described the causes of carboplatin and cisplatin being in short supply across the United States and how the FDA is working with Chinese manufacturers to import these chemotherapy agents. Ganio, senior director of Pharmacy Practice and Quality at the American Society of Health-System Pharmacists, stated that communication is paramount among all members of a care team to help prevent critical errors while treating patients with limited supplies of these drugs. He also spoke about how reaching out to local representatives may help address the issues resulting from these shortages. “Advocacy is needed,” Ganio said. “Our policymakers need to know that these shortages are having a real-world impact, and that they need to be addressed.” Brian Slomovitz, MD, MS, FACOG, spoke about how he is managing the ongoing shortages in the context of his gynecologic cancer care, including swapping one platinum-based drug for another during treatment. Slomovitz, a gynecologic oncologist, director of Gynecologic Oncology, and co-chair of the Cancer Research Committee at Mount Sinai Medical Center in Miami Beach, Florida and a professor of Obstetrics and Gynecology at Florida International University, also described the challenges many practices across the country have due to lacking platinum-based drugs altogether. “I know there are institutions that don't have [platinum-based chemotherapy],” Slomovitz said. “There are institutions that aren't able to give their patients the best standard-of-care therapy and aren't able to enroll patients in trials because of lack of therapy.” Lucio N. Gordan, MD, addressed the financial impacts of these shortages, discussing how his practice has managed the rising prices of carboplatin and cisplatin in the country. Gordan, president and managing physician at Florida Cancer Specialists & Research Institute who sees patients in the state-wide practice's Gainesville Cancer Center, stated that efforts from major oncology organizations may help increase adequate access to chemotherapy drugs. “It's very important that the large practices and institutions in the country stay united,” Gordan said. “The Community Oncology Alliance [COA], American Society of Clinical Oncology [ASCO], and others are trying to push legislation that would protect the consumer, the patients, and us to make sure that we have a proven supply.” Don't forget to subscribe to the “Oncology On-The-Go” podcast on Apple Podcasts, Spotify, or anywhere podcasts are available.

Alison is a Clinical Pharmacy Manager at the University of Texas MD Anderson Cancer Center in Houston, TX. She obtained her PharmD at the University of Florida and completed her PGY1 and PGY2 residencies at the Medical University of South Carolina. She has been in clinical practice for 19 years at MD Anderson and manages a team of clinical pharmacy specialists in pediatric hematology/oncology and adult stem cell transplant.Kamakshi is an Interim Director of Pharmacy at the University of North Carolina Medical Center in Chapel Hill. She oversees Adult Ambulatory and Acute Clinical Services as well as the hospital's academic enterprises, including resident and student training programs. Additionally, she leads the department's growing efforts around diversity, equity, and inclusion. She spent 20 years in direct patient care, working with patients undergoing bone marrow transplants at the North Carolina Cancer Hospital before moving into her current role.Zahra is a Clinical Pharmacy Manager in the Division of Hematologic Malignancies and Cellular Therapeutics at the University of Kansas Health System (TUKHS). She earned her PharmD and MBA from the UMKC School of Pharmacy and the Henry W. Bloch School of Management. She completed residency training at The Ohio State University Medical Center and The Huntsman Cancer Institute at the University of Utah. She has been a board-certified oncology pharmacist since 2012.
