CancerNetwork® spoke with experts across multiple disciplines about the far-reaching impacts of the ongoing shortages of chemotherapy drugs including carboplatin and cisplatin, and how practices may be able to mitigate these challenges in the short and long term.  Michael Ganio, PharmD, MS, BCPS, FASHP, described the causes of carboplatin and cisplatin being in short supply across the United States and how the FDA is working with Chinese manufacturers to import these chemotherapy agents. Ganio, senior director of Pharmacy Practice and Quality at the American Society of Health-System Pharmacists, stated that communication is paramount among all members of a care team to help prevent critical errors while treating patients with limited supplies of these drugs. He also spoke about how reaching out to local representatives may help address the issues resulting from these shortages. “Advocacy is needed,” Ganio said. “Our policymakers need to know that these shortages are having a real-world impact, and that they need to be addressed.” Brian Slomovitz, MD, MS, FACOG, spoke about how he is managing the ongoing shortages in the context of his gynecologic cancer care, including swapping one platinum-based drug for another during treatment. Slomovitz, a gynecologic oncologist, director of Gynecologic Oncology, and co-chair of the Cancer Research Committee at Mount Sinai Medical Center in Miami Beach, Florida and a professor of Obstetrics and Gynecology at Florida International University, also described the challenges many practices across the country have due to lacking platinum-based drugs altogether. “I know there are institutions that don't have [platinum-based chemotherapy],” Slomovitz said. “There are institutions that aren't able to give their patients the best standard-of-care therapy and aren't able to enroll patients in trials because of lack of therapy.”  Lucio N. Gordan, MD, addressed the financial impacts of these shortages, discussing how his practice has managed the rising prices of carboplatin and cisplatin in the country. She obtained her PharmD at the University of Florida and completed her PGY1 and PGY2 residencies at the Medical University of South Carolina. She has been in clinical practice for 19 years at MD Anderson and manages a team of clinical pharmacy specialists in pediatric hematology/oncology and adult stem cell transplant.Kamakshi is an Interim Director of Pharmacy at the University of North Carolina Medical Center in Chapel Hill. She oversees Adult Ambulatory and Acute Clinical Services as well as the hospital's academic enterprises, including resident and student training programs.  Additionally, she leads the department's growing efforts around diversity, equity, and inclusion. She spent 20 years in direct patient care, working with patients undergoing bone marrow transplants at the North Carolina Cancer Hospital before moving into her current role.Zahra is a Clinical Pharmacy Manager in the Division of Hematologic Malignancies and Cellular Therapeutics at the University of Kansas Health System (TUKHS). She earned her PharmD and MBA from the UMKC School of Pharmacy and the Henry W. Bloch School of Management. She completed residency training at The Ohio State University Medical Center and The Huntsman Cancer Institute at the University of Utah. She has been a board-certified oncology pharmacist since 2012. Howard recently retired as the Executive Director of Oncology Pharmacy Services for the Smilow Cancer Hospital and Network of Yale New Haven Health.  He is also the President of Safe Medication Management Associates, Inc. and serves as a consultant for Grifols. He earned his Bachelor of Science degree in pharmacy from the University of Kansas and his Master of Science degree from Temple University School of Pharmacy in Philadelphia, Pa. He completed an Executive Management fellowship at the Wharton School of Business. In 1995 he received the designation of Fellow of the American Society of Health-System Pharmacists. In this episode, our guest is Dr. Leigh Ann Ross who is the Associate Dean for Clinical Affairs at the University of Mississippi School of Pharmacy and Professor in the Department of Pharmacy Practice. She previously served as the Chair of the Department of Pharmacy Practice and currently serves as Director of the Center for Clinical and Translational Science and Research Professor in the Research Institute of Pharmaceutical Sciences at UM. Dr. Ross also works with the John D. Bower School of Population Health at the University of Mississippi Medical Center as a Professor in the Department of Population Health Science. Dr. Ross received a Bachelor of Business Administration and a Doctor of Pharmacy from Ole Miss. She completed a Primary Care Pharmacy Residency at UMMC. Since joining the faculty in 2009, Dr. Ross has led practice initiatives to implement medication management services at the Medical Center and in communities through UM's Community-Based Research Program. In addition to her work in pharmacy, Dr. Ross completed a 2-year Congressional Fellowship in the Office of U.S. Senator Thad Cochran where she served as policy advisor on health care and also worked on labor, housing, and economic development. Dr. Ross is a graduate of Leadership Mississippi and has been actively involved in professional organizations at a state and national level. She is a Past President of the Mississippi Pharmacists Association and current President- Elect of the American College of Clinical Pharmacy. Dr. Ross is passionate about leadership development and advocacy within our profession. Key points: Service - I love professional service and I feel it is important to encourage our students and early career pharmacists to be involved in professional organizations. I have worked with most all professional organizations over the last 20 years, but my current focus is as President-Elect of ACCP and member of their Board of Regents. Leadership - I am passionate about leadership development. I appreciate the mentorship provided to me in this area and have tried to incorporate this into our curriculum for both students and residents. Advocacy - I feel advocacy to move pharmacy forward is very important. I have enjoyed building on my experience as a Health Policy Fellow and working with others to better understand legislative processes and how to advocate for our profession. Practice Advancement / Community-Based Research - I had the opportunity early in my career to lead the Pharmaceutical Care Clinics at UMMC, comprised of pharmacist-run clinics and interprofessional clinics and later to work with colleagues to implement a Community-Based Research Program to implement medication management services (like those in the Pharmaceutical Care Clinics) in community pharmacy and clinic settings. Through the Community-Based Research Program, we have established a number of partnerships, such as the Mississippi State Department of Health. Research - Most recently, I worked with colleagues to establish a Center for Clinical and Translational Science in our Research Institute. This Center is structured to facilitate research across the translational spectrum from basic research to clinical trials to communities. Our community research is now housed in this Center. DIR fees have been a point of contention since they were loosely established in 2014 because they allow pharmacy benefit managers (PBMs) to retroactively impose these fees on pharmacies and, indirectly, on patients long after the point of sale. “HHS has acknowledged on multiple occasions that retroactive pharmacy DIR fees inflict harm on pharmacies and increase costs for their patients, but it has repeatedly neglected to address the problem,” said APhA EVP and CEO Scott J. Knoer, MS, PharmD, FASHP, in a press release. “This lawsuit puts pharmacy in the driver's seat. We demand that HHS meet its obligations and use its authority to stop PBMs from operating without accountability.” Scott was interviewed by Pharmacy Times Alana Hippensteele & said:  Right now, obviously, doctors, PAs, nurse practitioners, and—this is no insult intended whatsoever—but audiologists and nurse midwives are also providers. Pharmacists are the second most educated with PharmD's and our bachelor's pharmacists have been working 30 years with experience, and my audiologist, I have a hearing tested—God love her—she did a great job, but she's not trained at the level pharmacists are. So it's just ludicrous that pharmacists cannot bill for the services they all went to school for. So—we'll get to this, I know you've got your questions, I'm sorry I give long answers to short questions—the problem is pharmacies are only reimbursed for product in the community setting, and 20 years ago that was okay because you got paid enough where I could talk to your mom about her diabetes. Now the PBMs have sucked so much money out of the health care system without providing any real value that all you can do is volume. That's why we've had reporter—just recently Adiel Kaplan at NBC news—talk about the challenges in chain pharmacies. (Hon), FASHP, Chief Executive Officer of the American Society of Health-System Pharmacists (ASHP), as we discuss the role of a pharmacy professional organization in managing the COVID-19 pandemic. Prior to joining ASHP in September 2011, Dr. Abramowitz worked in hospitals and health-systems for 34 year. He served as Associate Hospital Director for Professional Services and Chief Pharmacy Officer at the University of Iowa Hospitals and Clinics, and Professor at the University of Iowa College of Pharmacy. In addition to serving as Treasurer of ASHP from 2007-10 and as ASHP President in 1993-94, he has chaired several boards. Dr. Abramowitz received a Bachelors Degree in Chemistry and Biology from Indiana University, a Bachelors Degree in Pharmacy from the University of Toledo, a Pharm.D. from the University of Michigan, and completed his residency at the University of Michigan Medical Center. Dr. Abramowitz has actively combined practice, teaching, and research throughout his career. He is a recipient of many awards and recognitions, including, but not limited to: the John W. Webb Lecture Award, the Harvey A.K. Whitney Lecture Award, and in 2013 the Honorary Degree of Doctor of Science from the University of Toledo. In 2015, he was recognized as one of Washington's Trending Association Leaders by Bisnow. Currently, Dr. Abramowitz serves on the Boards of the American Nurses Foundation, the Pharmacy Technician Certification Board and the GTMRx Institute. He also is a member of the National Steering Committee for Patient Safety of the Institute for Healthcare Improvement and a Professor-Emeritus at the University of Iowa. His lectures and publications have focused on topics such as: the effect that pharmacists have on improving outcomes of care and reducing costs; developing new care models; reducing adverse drug events; and expanding comprehensive medication management to the ambulatory setting.

Today's Pharmacy Podcast Show guest is Dr. Jeffrey Fudin. We're talking about the March 30th 2016 article from Pain News Network:  How Opioid Prescribing Guidelines Use Pseudoscience By Michael Schatman and Jeffrey Fudin, Guest Columnists Recently, we (along with our colleague, Dr. Jacqueline Pratt Cleary) published an open access article in the Journal of Pain Research, entitled “The MEDD Myth: The Impact of Pseudoscience on Pain Research and Prescribing Guideline Development.” In this work, we address the issue of how governmental and managed care opioid guideline prescribing committees use the flawed concept of morphine-equivalent daily dose (MEDD or MME) to arbitrarily place limits on the amount of opioids that a clinician “should” prescribe to any patient with chronic pain -- as if all patients were identical.  The article cites excellent research that exposes the invalid concept of MEDD – and while guideline authors are fully aware of that lack of evidence, they are hypocritically fine with using MEDD as a device to thwart chronic opioid use. In the case of opioids for chronic non-cancer pain, there is at least some evidence.  But for MEDD, there is no evidence.
