Hoy abordaremos una actualización esencial en el ámbito de las emergencias: la Actualización 2024 de las Guías de Resucitación tras Ahogamiento de la American Heart Association (AHA) y la American Academy of Pediatrics (AAP). Este tema es fundamental, ya que el ahogamiento sigue siendo una de las principales causas de muerte no intencional en el mundo. El ahogamiento se define como el proceso de experimentar insuficiencia respiratoria por inmersión en agua y, si no se actúa de inmediato, este evento progresa de una parada respiratoria a una parada cardíaca. La resucitación en estos casos requiere intervenciones específicas que consideren la hipoxia severa como causa principal del paro. Hoy discutiremos las recomendaciones clave y los puntos a tener en cuenta al responder a emergencias de ahogamiento, ya sea que seas un rescatista laico o un profesional de la salud. LA CADENA DE SUPERVIVENCIA EN AHOGAMIENTOS Presentador: Comencemos con el concepto de la Cadena de Supervivencia en Ahogamientos. Esta cadena es un enfoque sistemático que establece cinco pasos críticos para aumentar la supervivencia de la persona ahogada. Paso 1 - Prevención del Ahogamiento: La prevención es siempre el primer paso. Las guías indican que más del 90% de los ahogamientos son prevenibles. Entre las medidas de prevención se encuentran: instalar barreras para evitar el acceso a áreas de agua, usar chalecos salvavidas en actividades acuáticas y garantizar la supervisión de niños en áreas cercanas al agua. Paso 2 - Reconocer el Peligro y Activar la Ayuda: Reconocer que alguien está en peligro es crucial para activar la ayuda rápidamente. Es importante recordar que una persona ahogada no siempre puede pedir auxilio, ya que el instinto por respirar es tan fuerte que impide que levante los brazos o grite. Paso 3 - Proporcionar Flotación: Antes de intentar un rescate, las guías recomiendan ofrecer un dispositivo de flotación si está disponible. Esto ayuda a evitar que el rescatador se convierta en una segunda víctima y permite estabilizar a la persona en el agua. Paso 4 - Sacar del Agua de Forma Segura: Si es seguro, el siguiente paso es sacar a la persona del agua. Esto facilita la evaluación y el manejo posterior. Las guías sugieren que si la persona está inconsciente, se realice la extracción en una posición lo más horizontal posible para evitar comprometer aún más la vía aérea. Paso 5 - Iniciar Soporte Vital Básico y Avanzado: Finalmente, se debe iniciar el soporte vital básico (SVB) lo antes posible. Si la parada es confirmada y llega el equipo de emergencia, deben iniciar el soporte vital avanzado (SVA) para mejorar las probabilidades de supervivencia. IMPORTANCIA DE LA VENTILACIÓN Y EL MANEJO DE LA VÍA AÉREA La siguiente recomendación clave en esta actualización se centra en el manejo de la vía aérea y la ventilación. A diferencia de otras paradas cardíacas, el ahogamiento requiere una prioridad en la ventilación debido a la hipoxia severa causada por el agua en los pulmones. Recomendación para Respiración de Rescate en el Agua: Si el rescatador está entrenado y es seguro hacerlo, las guías recomiendan iniciar la respiración de rescate en el agua. Esta intervención temprana ayuda a prevenir que el ahogamiento avance a un paro cardíaco completo. Sin embargo, se debe valorar constantemente la seguridad del rescatador, ya que no se debe comprometer la seguridad personal. Recomendación sobre Administración de Oxígeno: Otra recomendación importante es administrar oxígeno lo antes posible cuando esté disponible, ya que la hipoxia severa es la causa principal de la parada. No obstante, se enfatiza que la administración de oxígeno no debe retrasar el inicio de una RCP de alta calidad si la persona ya está en paro. USO DEL DESFIBRILADOR EXTERNO AUTOMÁTICO (DEA) Y RCP EN AHOGAMIENTO Un aspecto único de la resucitación tras ahogamiento es el orden de prioridad entre RCP y DEA. Debido a que los ritmos desfibrilables son poco comunes en el ahogamiento, las guías enfatizan que es más efectivo iniciar la RCP con ventilación antes de aplicar el DEA. Recomendación sobre el Orden de RCP y DEA: En caso de ahogamiento con paro, la prioridad debe ser la RCP con ventilación antes de aplicar el DEA. Aunque la desfibrilación es útil si se detecta un ritmo desfibrilable, la mayoría de los paros por ahogamiento no tienen este tipo de ritmo, por lo que no debe retrasarse la RCP buscando un DEA. Uso de DEA en Ritmos Desfibrilables: Si se cuenta con un DEA y es seguro usarlo, puede aplicarse en caso de un ritmo desfibrilable. Sin embargo, siempre priorizando la RCP y evitando que la búsqueda de un DEA retrase las compresiones y ventilaciones necesarias para el paciente ahogado. BRECHAS Y NECESIDADES DE INVESTIGACIÓN FUTURA Un aspecto importante que resaltan las guías es la necesidad de continuar investigando la resucitación y prevención del ahogamiento en diferentes contextos y regiones. Se identificaron algunas brechas de conocimiento, como la falta de recursos en áreas remotas o de bajos ingresos y la implementación de programas de desfibrilación pública en áreas acuáticas. Recomendación de Educación y Entrenamiento Comunitario: Las guías sugieren fortalecer la educación comunitaria sobre técnicas de rescate y resucitación, especialmente en áreas con alta incidencia de ahogamientos. También se recomienda promover la formación en soporte vital básico y avanzado en todas las comunidades para mejorar los tiempos de respuesta y la calidad de la resucitación. CONCLUSIÓN Para resumir, hemos cubierto las recomendaciones esenciales de la actualización 2024 de la AHA y la AAP sobre resucitación tras ahogamiento. The suspects led the police on a chase but eventually crashed the vehicle. One suspect dropped a gun while fleeing, and officers were able to locate and apprehend him. Another suspect was pursued by an officer but managed to escape. Three guns were recovered during the incident. The other two suspects were found at a hotel after surveillance and were taken into custody with the assistance of the Violent Crime Bureau....check in at mdjonline.com for updates on this story.   Story 2: ged   The Cobb County Adult Detention Center has implemented an on-site high school equivalency diploma program for detainees, eliminating the need for transportation to other jails for testing. The program aims to reduce costs and provide a familiar environment for the detainees. Capt. Yaminah Holt, overseeing jail programming, expressed excitement about the program and announced that one detainee had already passed the first on-site GED test. The initiative is a partnership between the Cobb County Sheriff's Office, CobbWorks, and the Cobb County School District. The program aligns with Sheriff Owens' commitment to helping detainees maximize their time in custody and increase their productivity upon release.     Story 3: drowns   A tragic incident occurred in Lizella, Georgia, where a four-year-old boy from Marietta drowned in a pool during a gathering at a neighborhood home. The child, identified as Viraat Gosain, wandered off and fell into the pool. Paramedics rushed him to a nearby emergency room, but he was pronounced dead shortly after 3 p.m. Lonnie Miley, deputy coroner, stated that it appears to be an accidental drowning. As the death involved a child, authorities are likely to schedule an autopsy. Viraat would have celebrated his fifth birthday on Thursday. The incident is a heartbreaking reminder of the importance of water safety, especially around young children     STORY 4: abwa   During their May meeting at the Marietta Conference Center, the Marietta Charter Chapter of the American Business Women's Association awarded scholarships to Abi Halls and Abby White. Both recipients are Georgia Tech students, with White pursuing a Masters degree in Architecture and Halls studying Civil Engineering with a focus on Supply Chain. The Marietta Charter Chapter has been actively supporting Cobb County for over 50 years by granting annual scholarships to deserving women and acknowledging outstanding business leaders who make a positive impact on women in the community. ….we'll have more in a moment:     Break:   ESOG– Dayco   STORY 5: b to b   Cobb Schools have achieved back-to-back national championships by raising over $770,000 for the American Heart Association (AHA) during the school year. This amount broke the 44-year record for AHA fundraising campaigns. Cobb County School District surpassed the second-place district by over a quarter-million dollars. Mountain View Elementary and King Springs Elementary were the top-raising schools in Cobb, contributing $57,971 and $57,210, respectively. The funds raised support the AHA's research and outreach programs aimed at addressing high blood pressure, CPR education, nutrition security, tobacco, and the youth vaping crisis. The AHA's Kids Heart Challenge engages elementary students in physical activities to raise awareness and funds for heart disease research, while middle and high school students participate in the American Heart Challenge. Nationwide, districts have raised over $58 million through these programs this year....for more on this story visit mdjonline.com   Story 6: P.O.Y.   Crystal Henderson, senior guard for Kell High School, became the most decorated player in the program's history by leading the team to the Class AAAAA state championship. As a result of her exceptional season, Henderson was named the 2022-23 Cobb County Girls Basketball Player of the Year. She averaged 25.4 points, 5 rebounds, 5 assists, and 3 steals per game. Henderson received first-team all-state honors and the Class AAAAA Player of the Year award. She surpassed 2,000 career points, becoming the all-time leading scorer in Kell basketball history. Henderson's No. 30 jersey was retired alongside her brother's No. 0. Winning the state title was the most cherished accomplishment for Henderson, who scored 29 points in the championship game.   Story 7:  C.O.Y.   Kandra Bailey, the head coach of the Kell High School girls basketball team, was named the 2022-23 Cobb County Girls Basketball Coach of the Year after leading the team to a 26-5 record and their first-ever Class AAAAA state championship. Bailey took over the program in the previous season and motivated her players to seek redemption after a loss in the state semifinals. She challenged the team with a tough non-region schedule, and despite some setbacks and injuries, the players adapted and learned from their experiences. This week we are talking once again with Kristal Kent.  If you recall, Kristal has joined us on previous interviews discussing Fibromyalgia and again discussing prepping for surgery as a chronically ill person.  We never really gave Kristal the credit she deserves for being such a fantastic advocate for all Veterans and those with Fibro or other life-long medical conditions.  Allow me to properly introduce this dynamic woman! ​Kristal Kent is a disabled Army Veteran living with Fibromyalgia and served with the 256th Combat Support Hospital (256th CSH). Kristal worked with the Personnel (HR) department, then after September 11th, 2001, Kristal was re-assigned to the Tactical Operations Center (TOC) of the 256th CSH. After being medically discharged due to injuries, Kristal stayed on with her unit, the 256th CSH, and volunteered for 2 ½ years as the Family Readiness Group Leader, re-invigorating the program to become a supportive extension of the unit for the soldiers and their families. During her time as the FRG Leader, Kristal upstarted a Food Pantry to assist the Unit's service members struggling with food sustenance, coordinated a resource list of federal, state, and local support and assistance resource programs for soldiers and families, along with hosted “Readiness” educational weekend to ensure soldiers and Military Families were prepared for deployment. Kristal's Volunteerism as FRG Leader was officially recognized with a Commendation from the U.S. Department of the Army. Kristal also worked in Social Services for over 20 years in a variety of roles, from Adult Advocacy Coordinator, Care Coordinator, Assertive Community Treatment Specialist, Benefits Coordinator and Supported Employment Services, assisting individuals with Intellectual Disabilities, Mental Health Conditions and Veterans alike. ​              As a Veteran living with Fibromyalgia, Kristal identified the lack of supports, healthcare options and education for those living with Fibromyalgia, especially Veterans. To address the gap in healthcare and community supports, Kristal founded the initiatives, “The Fibromyalgia Pain Chronicles” and “Veteran Voices For Fibromyalgia,” to address the inequities in healthcare, Kristal engages in VA Policy Advocacy, Legislative Advocacy and Systems Advocacy to emote positive change for those living with Fibromyalgia. Kristal also provides support, educational resources, and advocates on behalf of those living with various Chronic Pain Conditions, Rare Diseases, PTSD and Traumatic Brain Injury (TBI). Kristal's Advocacy work on behalf of the Veteran and Fibromyalgia Communities has been recognized by several organizations such as WEGO Health in which she received the Patient Leader Hero Award and the Best In Show on Facebook in 2018, the 2019 Fibro Warrior Award from the Fibromyalgia Care Society of America, the Warrior of the Week in 2020 from UK Fibromyalgia. In May 2021 Kristal was presented with a Commendation from the State of Ohio House of Representatives for her advocacy work through Veteran Voices For Fibromyalgia. In August 2022, Kristal was chosen by Health Union, through the Social Health Awards patient advocacy platform, as the recipient of the 2022 Lifetime Achievement Award. In addition, in May 2022 Kristal advocated for and obtained a Proclamation designating May 12th as Fibromyalgia Awareness Day from the State of Ohio Governor. ​Kristal previously served 3 years as a Board of Trustee for the Fibromyalgia Care Society of America along with previously served as a Board Member on the Wego Health Patient Advisory Board. Kristal is currently a member of Society For Participatory Medicine, Social Health's Patient Leader Network, a Co-Op member of Savvy Cooperative and a member of the American Legion. Cardiac events, such as heart attacks and strokes, are also more common in women than men.   While there are many factors that contribute to heart disease, there are also a number of things that women can do to protect themselves. These include maintaining a healthy weight, quitting smoking, exercise, and knowing the signs and symptoms of a heart attack.   Here are eleven tips for keeping your heart healthy as a woman. These tips are based on the latest scientific evidence and are designed to help you lower your risk of heart disease and live a healthier life. 1. Heart Disease is the Leading Cause of Death for Women in the United States: The Centers for Disease Control and Prevention (CDC) reports that heart disease is the leading cause of death for both men and women in the United States. The study has been published in Scientific Reports. As part of the study, the researchers analyzed the amount of polyphenols in the urine of 1,326 adolescents that took part in the SI! Program (Integral Health) of 24 secondary education schools in Madrid and Barcelona. Polyphenols are bioactive compounds with antioxidant and anti-inflammatory properties found in several plant-based foods, mainly in fruit, vegetables, nuts and olive oil. To date, there were few studies on the intake of polyphenols in children and adolescents. “With the results of this study, we observed that a higher urinary polyphenol excretion is linearly associated with a higher cardiovascular health index in adolescents aged 11-14, specially in children. This cardiovascular health index is defined by the criteria established by the American Heart Association (AHA), which considers seven variables: body mass index, physical activity, smoking, diet, blood pressure, total cholesterol and blood glucose,” says Professor Rosa M Lamuela, director of the Institute for Research in Nutrition and Food Safety of the University of Barcelona NAC supplements may benefit cardiovascular health: Human data University of Marburg (Germany), October 16, 2022 Four weeks of supplementation with N-acetylcysteine (NAC) may reduce levels of homocysteine and improve blood pressure, says a new analysis of two randomized, double-blind, placebo-controlled trials. A daily dose of 1.8 grams of NAC could lower homocysteine levels by about 12%, and may improve systolic and diastolic blood pressure, according to findings published in the American Journal of Clinical Nutrition . Elevated levels of the amino acid homocysteine have been reported to increase the risk of cardiovascular disease. This has led some to point to the potential of the B-vitamins to reduce homocysteine levels and reduce the risk of CVD. B vitamins are cofactors for enzymes involved in homocysteine metabolism and therefore giving people vitamin B supplements is hypothesized to reduce homocysteine levels and therefore reduce cognitive impairment. However, clinical trials including participants at risk of, or already suffering from, cardiovascular disease have produced null results, with some experts arguing that short term B vitamin supplementation should not be expected to reverse the long-term development of heart disease. “Because recent analyses that controlled for confounders such as statins or folate fortification detected a benefit of folate/B-vitamins for stroke (22–24), homocysteine may conditionally be a therapeutic target,” wrote the authors of the new study, led by Prof Wulf Hildebrandt, formerly with the German Cancer Research Center and now at the University of Marburg (Germany). “Therefore, an alternative agent for (more) effective homocysteine lowering may be desirable, especially for conditions in which B-vitamins are ineffective [e.g., in renal disease] or if a reduction in homocysteine of >25% is intended.” There is a known link between homocysteine and NAC, and some studies have shown that NAC may reduce levels of the amino acid, but the data is somewhat mixed. Results showed that NAC supplementation significant decreased homocysteine levels by an average of 11.7% (versus 4.1% in the placebo groups), which cysteine levels increased by an average of 28.1% (versus 4% in the placebo groups). There were no significant differences between the hyperlipidemic and normolipidemic men, and the smoking status also did not affect the results. The researchers also found that NAC significantly decreased blood pressure in all the men. However, significant decreases in diastolic BP were observed only for the hyperlipidemic men, and not for the normolipidemic men. Study: Late afternoon exercise helps control blood sugar, cholesterol and triglyceride levels Huazhong University of Science and Technology (China), October 22, 2022 Results of a study published in the journal Front Endocrinol show that exercising at around 4 to 6 p.m. helps to control blood sugar, cholesterol and triglyceride levels better than exercising in the morning, or around 9 to 11 a.m. For the study, researchers observed 12 healthy young men. The volunteers were told to walk on a treadmill for one hour at 60 percent of their maximal oxygen uptake (VO2max) on Monday, Wednesday and Friday. The volunteers were all tested during and after a week of exercising only in the morning or just in the afternoon. After continuous 24-hour monitoring of their blood sugar levels, the total blood sugar levels of the men were lower when they exercised in the afternoon. Their blood sugar levels after meals were also lower following afternoon exercise. Most cell damage from high blood sugar is linked to a high rise in blood sugar at least one to two hours after eating a meal. The volunteers' triglyceride levels were also lower after afternoon exercise. This is crucial since your blood sugar level rises after you eat. If it rises too high, sugar sticks to cell membranes and damages them. That's why diabetes can damage every cell in your body. When your blood sugar level increases, your pancreas releases insulin to keep blood sugar levels from rising too high. Insulin lowers blood sugar by then driving sugar from the bloodstream into the liver. But if your liver is full of sugar, the liver does not accept more sugar and all the extra sugar is converted to fatty triglycerides. Having a blood triglyceride level greater than 150 indicates that your blood sugar rises too high after meals and that you are already diabetic or prediabetic. Blood levels of the good high-density lipoprotein (HDL) cholesterol were also higher after afternoon exercise. When triglycerides rise too high, you are at increased risk for clots. To protect your body from a high rise in triglycerides, your HDL cholesterol carries the triglycerides from your bloodstream into your liver and a high rise in triglycerides causes a drop in blood levels of the HDL cholesterol. So the lower your HDL, the more likely you are to suffer a heart attack. Scientists reveal the relationship between sugar and cancer Flanders Institute for Biotechnology, October 16, 2022 A nine-year joint research project has led to a crucial breakthrough in cancer research. Scientists have clarified how the Warburg effect, a phenomenon in which cancer cells rapidly break down sugars, stimulates tumor growth. This discovery provides evidence for a positive correlation between sugar and cancer, which may have far-reaching impacts on tailor-made diets for cancer patients. The research has been published in the leading academic journal Nature Communications. This project main focus was the Warburg effect, or the observation that tumors convert significantly higher amounts of sugar into lactate compared to healthy tissues. As one of the most prominent features of cancer cells, this phenomenon has been extensively studied and even used to detect brain tumors, among other applications. But thus far, it has been unclear whether the effect is merely a symptom of cancer, or a cause. Our results provide a foundation for future research in this domain, which can now be performed with a much more precise and relevant focus.” Living with others and community engagement are keys to reduced dementia risk University of New South Wales (Australia), October 24, 2022 Research published in The Lancet Healthy Longevity provides evidence to support that living with others, community group engagement and never feeling lonely are associated with slower cognitive decline. It is widely recognized that poor social connections such as small networks, infrequent interactions, and loneliness are modifiable risk factors for cognitive decline, with the 2020 Lancet Commission on dementia prevention estimating that tackling social isolation could prevent 4% of dementia cases worldwide. Co-author and CHeBA Co-Director Professor Henry Brodaty said that the findings have socio-economic significance. “We found that sharing a home with one or more person[s] and weekly community group engagement had the most robust results across studies, indicating these factors are fundamental components in the link with less cognitive decline,” says Professor Brodaty. “We also identified an association between never feeling lonely and a slower rate of cognitive decline.” Carrots Do Help Aging Eyes, Study Shows University of Utah School of Medicine, October 21, 2022 Your parents may have told you, “Eat your carrots, they're good for your eyes,” and a new study suggests they were on to something. Pigments called carotenoids — which give red or orange hues to carrots, sweet potatoes and orange peppers, or deep greens to produce like spinach, broccoli and kale — may help ward off the age-linked vision ailment known as macular degeneration, researchers said. “I tell my patients that fruit and vegetable consumption are very important for eye health — this study validates that notion,” said Dr. Paul Bernstein, a professor of ophthalmology and visual sciences at the University of Utah School of Medicine in Salt Lake City. Age-related macular degeneration (AMD) is one of the most common causes of vision loss, especially in the elderly. It affects the macula, the center part of the retina, and can lead to declines in sharp central vision and even blindness, experts say. Scientists have already linked a variety of factors to the condition including genetics, smoking and nutrition, said Bernstein, who was not involved in the new study. However, treatment for AMD may be limited depending on the type of macular degeneration that a person develops, he said. In the new study, Wu's team looked at data from health surveys that tracked people aged 50 and older — more than 63,000 women and almost 39,000 men -over a 25 year period. Participants were all nurses and other health professionals. Overall, about 2.5 percent of study participants developed either intermediate or advanced forms of the eye condition during the years of the study. Wu's team found that people who consumed the very highest levels of carotenoids known as lutein and zeaxanthin had a 40 percent lower risk of the advanced form of AMD compared to those who ate the very least. “Other carotenoids, including beta cryptoxanthin, alpha carotene and beta carotene, may also play protective roles,” Wu added. People who consumed the very highest amount of these carotenoids — found in foods such as carrots and sweet potato — had a 25 to 35 percent lower risk of the advanced form of the illness, the findings showed. Researchers did not find any link between the carotenoids and the intermediate form of macular degeneration, however. Lutein is found in eggs and dark leafy vegetables including broccoli, kale and spinach, Bernstein said. Zeaxanthin is harder to find in the diet, he said, but you can get it from corn, orange peppers and goji berries. Wu noted that both lutein and zeaxanthin concentrate in the macula, where they are thought to protect it from damage from oxygen and light.
