The model formalized his own consulting practice of hierarchical handover documentation — context diagram at the top, containers below, components and code at the bottom — giving it a name and making it teachable. His core advice is to start with just the top two levels: context and container diagrams. These change infrequently, are quick to draw, and deliver immediate value. Levels three and four — components and code — are included for completeness but age rapidly with every commit, and in most cases aren't worth the maintenance overhead. Two especially practical insights emerge from the conversation. First, how to handle microservices: if your team owns all the services inside a system boundary, each service is a collection of C4 containers; if you depend on another team's service that you can't see inside, model it as an opaque software system. This maps naturally to how domain boundaries and team ownership actually work in practice. Second, the surprisingly important distinction between modeling and diagramming: C4's real value is the shared vocabulary — systems, containers, components, code — not the visual notation itself. Teams using entirely different tooling can communicate clearly because they've agreed on what the words mean. Simon is firm that there will be no C4 v2; the model is intentionally lightweight, and adding more would risk turning it into the next UML — comprehensive, heavy, and largely abandoned.RECOMMENDED BOOKSSimon Brown • The C4 Model • https://amzn.to/4xB9V33Susanne Kaiser • Adaptive Systems With Domain-Driven Design, Wardley Mapping & Team Topologies • https://amzn.to/3XTmNCcNick Rozanski &, Eóin Woods • Software Systems Architecture • https://amzn.to/4cOLtTvvan Kelle, Verschatse &Baas-Schwegler • Collaborative Software Design • https://amzn.to/4iv0N8IBlueskyInstagramLinkedInFacebookCHANNEL MEMBERSHIP BONUSJoin this channel to get early access to videos & other perks:https://www.youtube.com/channel/UCs_tLP3AiwYKwdUHpltJPuA/joinLooking for a unique learning experience?Attend the next GOTO conference near you! If you wish to support our podcast, please follow this link. Thank you! Welcome to a new episode of The Way Out Is In: The Zen Art of Living, a podcast series mirroring Zen Master Thich Nhat Hanh's deep teachings of Buddhist philosophy: a simple yet profound methodology for dealing with our suffering, and for creating more happiness and joy in our lives. In this episode, Zen Buddhist monk Brother Phap Huu and leadership coach Jo Confino talk about finding beauty and reverence in the most ordinary of things. Jo shares his new book, Between Earth and Sky, which documents his time in Mitla, Mexico, and his journey of deep looking and finding beauty in the seemingly empty land. Brother Phap Huu emphasizes the importance of mindfulness, stopping, and being present to see the wonder in everyday life. The hosts further share the concept of consciousness in everything, and the need for reverence in our habits and interactions. The conversation also touches on reciprocity, the importance of community, and the journey of opening one’s heart through listening and acceptance. The episode concludes with a short meditation guided by Brother Phap Huu, to help listeners ground themselves in the present moment and to recognize the beauty in all life. It is learning to take a pause and feel the present moment. And feeling the present moment is more important than thinking about the present moment, because, when we attune ourselves into our own body, into our senses, what we see, what we smell, what we hear, what we touch gives us an opportunity to be in touch with life in the very here and now.” “We can sit and look at a tree or even look at a rock and find all of life in that.” “This is it.” “We want to know what’s out in space, but we don’t know what is in our inner space.” “Depending on how we show up, our way of being can shift and change our whole mental formation, our whole consciousness of reality.” “Reverence creates a sense of reciprocity because, when we have reverence, we are in conversation with whatever is in front of us. So when we think of things as objects, we’re looking at them as separate from ourselves. But when we have reverence for things, we see that they are talking to us as well as we are talking to them.” “The beauty of Zen is turning a simple moment into a beautiful ritual.” “Reverence is a manifestation of our mental formations.” “I invite everybody to hold their next cup of tea, or coffee, or orange juice, with two hands. It shifts you into a different space. A simple action with two hands, and I’m fully present. Just like when we join our palms and offer a bow, it represents the body and the mind that come together.”

Send us Fan MailMost investors learn due diligence the hard way, after a “great deal” turns into months of delays, legal costs, or a strategy that cannot be executed. We slow that down and make it practical: a clear framework built around four pillars of property due diligence, with a focus on the one pillar that can quietly make or break everything, the property itself. We talk about how to turn due diligence into a simple checklist you can actually use, deal after deal. The goal is not to overcomplicate the process, it is to make sure the foundational checks are done before you sink time into negotiations or spend money on surveys and solicitors. We also share why a quieter property market can be an advantage, especially if you are willing to get out there, talk to agents, and speak directly with property owners while competition is lower.A big chunk of the conversation is planning use class. We unpack what it means in real terms for common scenarios like standard residential (C3), HMOs (including C4 and when larger setups become Sui Generis), and care-related uses like C2. We also point to practical places to verify the facts, like local planning portals and the property's deeds, and we explain why mismatches between “what it's sold as” and “what it's approved as” should trigger deeper investigation. If you want to feel more confident evaluating purchase lease options, rent-to-rent opportunities, assisted sales, or straightforward purchases, this will give you a grounded starting point. TRAYLESS Whitening Technique Part 1 – with Dr Wyman Chan – PDP277

What if the tray is the reason your whitening results are inconsistent? Why would a dentist who owns a whitening lab — and holds four patents on making bleaching trays — tell you to skip the tray? Trayless whitening does not mean strips. It means the patient puts in a retractor and paints the gel directly onto the teeth, twice a day, for half an hour. No impression, no lab bill, no two-week wait, and no soft plastic reservoir quietly absorbing your peroxide. This is Part 1 of a two-part conversation with Dr Wyman Chan and Dr Elvis Law, recorded live in their central London whitening practice. Wyman has done nothing but whitening since 2002, has a PhD on the efficacy and safety of whitening processes, and — despite owning the lab that makes the trays — now does most of his cases without one. Elvis trained under him and reckons around 90% of his own cases are now trayless. Part 1 is the mechanism, the protocol and an honest list of who it doesn't suit. Part 2 takes it into the hard cases. https://youtu.be/aAAoUxTIkxo Watch PDP277 on YouTube Protrusive Dental Pearl: Let the Patient Pick the Shade Most of us ask “how white do you want to go?”, get a laugh about Hollywood white or Simon Cowell white, then hold a B1 tab against the canine and call that the destination. Try flipping it. Under corrected light, record where the patient is now. Then hand over the whole shade guide, arranged by value, and let them choose the tab they want to reach. Photograph both. This is the VITA Shade guide arranged by value: B1 → A1 → B2 → D2 → A2 → C1 → C2 → D4 → A3 → D3 → B3 → A3.5 → B4 → C3 → A4 → C4 Two things change. You now know the target precisely instead of inferring it, and you can track progress against a fixed reference. Most patients land on B1 — it's the last shade before the bleach range, and it reads natural rather than veneered. Some will point at 0M1 and that's a different conversation, which is exactly the point. Because whatever they choose dictates how many weeks, how much gel and how many reviews the case needs — and therefore what it should cost. A single flat whitening fee assumes every case takes the same work. They don't. Someone starting at C4 who wants a bleach shade can get there, but it takes more gel, more time, more reviews and probably a protocol change along the way. Price that honestly. Only you can decide what the tiers look like in your practice. But it might be worth sitting down as a team of dentists and therapists and asking: how are we delivering whitening? Two tiers? More? Based on what? What You'll Take From This Episode Conscious bleaching — why an awake patient with an open mouth is a completely different chemical situation to a sealed tray worn overnight, and what that does to sensitivity. The formulation constraint — peroxide needs acid to stay stable on the shelf, which is why pre-mixed products lean acidic and why two-component gels exist at all. The full trayless protocol — wear schedule, spacing, patient positioning, review intervals and what to troubleshoot first when a case is behind. Who it doesn't suit — an honest contraindications list, including the one objection patients raise most often and the answer to it. Tray hygiene as a clinical instruction — the reason results vary so much between patients using the identical gel. An A3.5 to B1 case — start to finish in three weeks, with the review points and the maintenance plan. A Green Beret Medic's Hard Truths: SFC(R) Luke Sciulli on Risk, Resilience, and Casualty Care at Scale

WarDocs - The Military Medicine Podcast

Play Episode Listen Later Jul 22, 2026 41:21

Episode Summary      On January 18, 2018, in Helmand Province, Afghanistan, Green Beret combat medic Luke Sciulli stopped being the medic and became the patient. A booby-trapped building collapsed on him during a personnel-recovery operation, fracturing his C4, C5, and C6 vertebrae, both scapulas, and his pelvis, and leaving him with a spinal cord injury at C5. In this episode of WarDocs, SFC(R) Luke Sciulli walks through that day and the MEDEVAC chain that followed — forward surgical team, Kandahar, Landstuhl, and Walter Reed — and explains how living through the system he once operated permanently reshaped his view of casualty care, pain management, and patient advocacy.   SFC(R) Sciulli is candid about the recovery that nearly broke him. He describes the culture shock of moving from elite combat operations to a 30-bed spinal cord injury ward populated largely by elderly veterans, and the slow, deliberate realization that, in his words, the only person who was going to make his life better was him. He maps the framework that pulled him back — physical health, mental health, and career — and offers a direct message to wounded operators who cannot get out of bed: the discipline that earned the Beret, the Tab, or the Trident is still inside them.   The conversation then widens into the operational and strategic challenges Sciulli has tackled across an extraordinary career. He compares civilian and military trauma systems and argues that surgeons in major American cities now see more penetrating trauma than many military providers — making military-civilian partnerships essential to keeping skills sharp. He recounts building a joint, combined MEDEVAC architecture across five African nations, and embedding at forward surgical teams in Ukraine just kilometers from the front, where 60 to 90 casualties a day exposed gaps in Role 1 and Role 2 doctrine, risk tolerance, and interoperability that U.S. tabletop exercises rarely surface.   SFC(R) Sciulli also draws on his time supervising a 216-bed field hospital at Columbia University during the 2020 COVID surge and his current work with Vigilant Consulting and Valinor Enterprises advising on tactical medical technology. He makes the case for predictive analytics, wearables, and automated resuscitation systems to force-multiply providers in large-scale combat operations — while confronting the risk-acceptance and accountability questions that slow adoption. He closes with the cause he intends to define his legacy: fixing the negligent disconnect that leaves SOF medics and operators underutilized in the civilian, defense, and healthcare worlds, and expanding the transition programs that turn their hard-won skills into lives saved at home. Chapters (00:00-01:10) Cold Open and Introduction (01:10-05:30) Becoming the Casualty in Helmand Province (05:30-11:30) The Recovery That Rewired Everything (11:30-15:45) Civilian and Military Medicine, Two Ways (15:45-22:45) Distance, Mass Casualties, and Lessons from Ukraine (22:45-32:30) Field Hospitals, Tactical Technology, and Risk (32:30-36:15) Recruiting the Next Force and a SOF Medic Legacy Chapter Summaries (00:00-01:10) Introduction Dr. Soderdahl frames the journey ahead: a Green Beret combat medic who became the patient on the wrong side of an IED blast and now drives technology and policy for the next war. The standard WarDocs welcome sets up a firsthand look at military medicine from the austere edge of combat to the front lines of innovation. (01:10-05:30) Becoming the Casualty in Helmand Province SFC(R) Sciulli recounts volunteering to backfill a buddy's ODA as senior medic and the January 18, 2018 operation in which a booby-trapped building collapsed onto him. He details catastrophic injuries — broken C4-C6, a C5 spinal cord injury, fractured scapulas and pelvis — and the MEDEVAC through the forward surgical team toward higher care, including how his teammates stabilized and moved him. (05:30-11:30) The Recovery That Rewired Everything He describes six months split between Walter Reed and the Tampa VA, the jolt of moving from combat to a spinal cord injury ward, and the loss of career, health, and identity overnight. The turning point came when he accepted that no one else could make his life better, and built a deliberate framework around physical health, mental health, and career. (11:30-15:45) Civilian and Military Medicine, Two Ways Drawing on his paramedic, firefighter, and critical-care flight background plus SF service, Sciulli argues civilian surgeons now out-rep many military providers on penetrating trauma, making military-civilian partnerships essential. He also reflects on serving as senior medical provider for SEAL Teams Two and Eight and why interoperability comes down to people and personality. (15:45-22:45) Distance, Mass Casualties, and Lessons from Ukraine He explains building a joint, combined MEDEVAC system across Chad, Niger, Nigeria, Cameroon, and Libya, then scales the problem to the Indo-Pacific and Eastern Europe. Embedded at forward surgical teams in Ukraine, he saw 60 to 90 casualties a day and learned hard lessons about volume, interoperability, civilian EMS integration, and U.S. risk tolerance. (22:45-32:30) Field Hospitals, Tactical Technology, and Risk SFC(R) Sciulli describes supervising a 216-bed COVID field hospital at Columbia University and why a military-style community made it work. He then identifies penetrating and shrapnel trauma as a top killer and makes the case for wearables, predictive analytics, and automated resuscitation — confronting the risk acceptance and human trust that slow adoption. (32:30-36:15) Recruiting the Next Force and a SOF Medic Legacy Speaking to students, residents, and recruiters, Sciulli reframes the pitch: the military needs people willing to operate beyond their comfort zone and scope, not just another credential. He closes on the legacy he intends to leave — fixing the negligent disconnect that leaves SOF medics and operators underutilized after service. Take Home Messages Experiencing Care Builds the Best Advocates: Living through the casualty evacuation chain as the patient teaches lessons no provider can learn from the other side of the litter. Empathy, pain management, and patient advocacy take on new meaning once a clinician has been the one strapped to the litter. Recovery Is a Decision You Make Daily: The hardest part of catastrophic injury is mental, not physical. Progress comes from a conscious choice to get up and be better than yesterday, supported by resources but driven by personal ownership of physical health, mental health, and career. Keep Military Providers Sharp in Civilian Trauma: Civilian surgeons in major cities now see more penetrating trauma and polytrauma than many military clinicians. An integrated military-civilian system that rotates providers through high-volume civilian centers is essential to keeping wartime skills ready. Plan for Volume, Distance, and Interoperability: Future large-scale combat means moving casualties across oceans and continents while relying on host-nation, partner, and civilian systems. High casualty volumes and contested distance demand generalist skill, civilian EMS integration, and a higher tolerance for operational risk. Close the Gap Between Research and the Warfighter: Government funds enormous amounts of combat casualty research and technology that never reaches frontline providers fast enough. The mission is to translate proven findings and devices into the warfighter's hands tomorrow — not years from now.   Episode Keywords military medicine, combat medicine, Green Beret medic, 18D combat medic, special forces medic, combat casualty care, TCCC, spinal cord injury recovery, polytrauma, Helmand Province, Walter Reed, MEDEVAC, forward surgical team, Ukraine combat medicine, mass casualty, large scale combat operations, LSCO, prolonged field care, tactical medicine, medical device innovation, SOF medic, military civilian partnership, WarDocs, Luke Sciulli, veteran resilience, penetrating trauma   Hashtags #MilitaryMedicine, #CombatMedicine, #GreenBeret, #WarDocs, #CombatCasualtyCare, #SOFMedic, #VeteranResilience, #TacticalMedicine   Honoring the Legacy and Preserving the History of Military Medicine The WarDocs Mission- WarDocs exists to honor the legacy of Military Medicine, preserve its history, and inspire every generation — across all Services, Corps, and Ranks — to serve with excellence and pride.

In this episode of Tank Talks, host Matt Cohen sits down with Paul Ziadé, CEO and co-founder of North Vector Dynamics, a Calgary-based company building full-spectrum precision missile systems for Canada and its allies. A former tenured professor at the University of Calgary turned defence entrepreneur, Paul brings a deeply personal and historically literate perspective to the Canadian defence landscape. He shares his family's story of displacement from Lebanon and Armenia, and how that legacy of resilience informs his mission to build a sovereign missile capability for Canada.Paul offers a frank assessment of Canada's defence complacency, arguing that the culture of risk-aversion is “upstream of everything.” He explains why the obsession with “dual-use” investing is holding the country back, why pure-play defence innovation is critical, and how startups like his can deliver capability faster than the traditional 10-year procurement cycle. He also breaks down the massive opportunity presented by the Golden Dome NORAD modernization and why Canada must participate not just as a buyer, but as an active builder.Whether you're a defence founder navigating the Canadian ecosystem, an investor trying to understand the shift in risk capital, or a policy wonk watching the Arctic heat up, Paul Ziade delivers the kind of straight talk that cuts through the noise. From Civil War to Canadian Soil (00:01:20)* Paul's upbringing as the child of Lebanese-Armenian immigrants who fled civil war and genocide.* How his parents' appreciation for Canada's peace and rule of law instilled a deep sense of responsibility.* The household rule: “Laziness was never an option” , competitive soccer, piano, student government, and French schooling. The Kennedy Space Center Spark (00:04:00)* How a grade 10 family trip to the Kennedy Space Center “lit the fuse” for his obsession with rocketry.* Touching the Saturn V and realizing what human ingenuity could achieve.* Why he needed a strong goal to anchor his focus and drive.Academia to Entrepreneurship (00:05:38)* Why he left a tenured professorship after 10 years at the University of Calgary.* The mindset shift: moving from committee-driven, overthinking academic life to the velocity and action-mentality of startups.* How academia trained him to be a better communicator, but startup life gave him instant feedback on every decision. The North Vector Vision (00:08:53)* The mission: precision missile systems on every continent protecting allies and Canada.* Full-spectrum air defence , from low-and-slow drones to hypersonic threats.* Assembling the “Avengers” team from his university labs, including co-founders Craig (aerospace research chair) and Colin (former TA).The Supply Chain Blind Spot (00:10:36)* Canada's massive gap in energetics , from solid rocket motors to warheads, and upstream chemical synthesis (RDX, HMX, C4 propellants).* Why “vertically integrated” doesn't always mean what people think.* The need for government signals to encourage private companies to take the risk and spool up.Defence Investing: Then vs. Now (00:14:48)* In 2022, no one wanted to look at North Vector , squeamishness around defence was rampant.* The gap between the narrative and reality on the ground in Canada.* Why most Canadian investors are still obsessed with “dual-use” investing, which Paul argues is historically misguided. The Dual-Use Delusion (00:29:31)* The problem with forcing every defence tech to have a civilian application.* Historical examples: GPS (missile guidance), duct tape (waterproof ammunition sealing).* The parallel to basic science , pure math and quantum mechanics funded with no immediate application, yet yielding enormous real-world benefits.Canada vs. The U.S. (00:18:43)* Honest admission: it's easier to build defence tech south of the border.* Why North Vector chose to stay in Canada despite the friction.* Canada as a “frontier market” , a massive opportunity for those willing to build.* The debt of gratitude to the country that gave his family refuge.ACDC & The 70% Target (00:21:20)* Paul co-chairs the Alliance of Canadian Defence Companies (ACDC) , now nearly 200 members.* The goal: champion Canadian-owned, Canadian-controlled companies and advocate for fast contracts.* Holding the government accountable to the Defence Industrial Strategy (DIS) without displacing primes.In-Q-Tel & The Culture Shift (00:23:16)* What a Canadian version of In-Q-Tel would need to get right.* The critical need: give delegated authorities permission to take risks and accept failure as part of the process.* Moving money out the door faster to small, scrappy startups that could win big.The Golden Dome Opportunity (00:27:13)* Canada's lack of commitment to NORAD modernization and the Golden Dome.* The upside: a $500B–$1.2T opportunity.* Paul's proposed win-win: “Give Trump the $61B win, but ensure $40B goes to Canadian firms.”* How Canada can participate as an active builder, not just a buyer.About Paul ZiadéPaul Ziadé is the CEO and co-founder of North Vector Dynamics, a Calgary-based company building full-spectrum precision missile systems for Canada and its allies. A former tenured professor at the University of Calgary, Paul holds deep expertise in high-speed propulsion and aerodynamics, having spent over a decade in academia before pivoting to the startup world. He is also the co-chair of the Alliance of Canadian Defence Companies (ACDC) , advocating for Canadian-owned, Canadian-controlled defence firms. A first-generation Canadian with roots in Lebanon and Armenia, Paul brings a historically literate perspective to national security and the urgent need for Canada to build sovereign defence capability.Connect with Paul Ziadé on LinkedIn: https://www.linkedin.com/in/paul-ziade/Visit North Vector Dynamics' website: https://www.northvectordynamics.com/Learn more about ACDC: https://www.alliancecanada.com/Connect with Matt Cohen on LinkedIn: https://ca.linkedin.com/in/matt-cohen1Visit the Ripple Ventures website: https://www.rippleventures.com/ This is a public episode. On Episode #224, Scott returns to grade his own predictions and dig into everything that’s changed since February 2023. The report card is mostly great news for Scott. Creatine’s move into cognitive health and older demographics played out almost exactly as he called it, hydration supplements kept climbing even as ready-to-drink sports drinks flattened out, and Red Bull is still the top-selling energy drink despite years of Celsius, Alani, Ghost, and C4 taking share. Scott, Mike, and Ben also cover where his 2023 calls needed adjusting, from ashwagandha’s staying power to pre-workout’s real exposure to energy drinks. From there, the conversation moves into new territory: the creatine gummy boom, GLP-1 ripple effects on weight management and protein, precision fermentation’s push into alternative dairy proteins, and Scott’s fresh picks for the next ingredients to watch. https://blog.priceplow.com/podcast/kaitlyn-kenna-smith-223 Video: Scott Dicker of SPINS Grades His 2023 Supplement Trend Predictions https://www.youtube.com/watch?v=sDaC4LVirBk Detailed Show Notes: Scott Dicker Returns After 3 Years of SPINS Supplement Trend Predictions (0:00) – Introductions (2:00) – Grading the Creatine Call Three Years Later (3:30) – Sports Nutrition as the Testing Ground for Mainstream Trends (8:00) – Hydration Shifts From Ready-to-Drink to Ready-to-Mix (11:15) – Energy Drinks Continue Eating Into Pre-Workout Occasions (16:30) – Red Bull Still Reigns, But the Perception Game Has Changed (19:30) – Premium Pricing Reshapes the Creatine Category (22:45) – Creatine Gummies Go From Novelty to Mainstream (25:45) – Better Formats Win: Soft Chews and the Push for Stability (27:15) – Delivery Formats and the Rise of “Permissible Indulgence” (30:45) – L-Theanine and the Hunt for Stim-Free Alternatives (37:30) – Kava, Caffeine Modulation, and the New Wave of Session Drinks (42:45) – Why “Natural Flavors” on the Label Isn’t Enough Anymore (48:15) – GLP-1s Reshape Weight Management and Create New Companion Categories (52:15) – Active Lifestyle Meets Natural and Organic: A Consumer Venn Diagram (59:15) – Cognizin, Health Span, and Winning the AI Search Era (1:01:45) – Horseshoe Theory: How Niche Trends Find the Mainstream (1:04:45) – Precision Fermentation’s Long Game for Alternative Proteins (1:11:15) – Ice Cream and the Challenge of Refrigerated Snacking (1:13:30) – Are Functional Ingredients Welcome in Snacks? High Sensitivity Through 3 Lenses: HSP, Pyrrole Disorder & RCCX Theory

Holistic Psychiatry Podcast

As a holistic and functional psychiatrist, I see many people who are highly sensitive. Most are deep divers into information and able to connect a lot of dots. Many are gifted. Many struggle with overwhelm and low stress tolerance. Some struggled with anxiety, inner tension, or feeling “too much” for as long as they can remember. Some feel different - that they don't fit in.As a holistic and functional psychiatrist with over twenty years in practice, I've worked with thousands of highly sensitive adults and children. Most are deep divers into information, able to connect a lot of dots — many are gifted. And yet many struggle with overwhelm, low stress tolerance, anxiety, or inner tension that has been there for as long as they can remember. Many feel different, like they don't quite fit in. What I've found is that sensitivity itself is rarely the problem — it's the seeming vulnerability to brain-related or physical conditions. Over the years I've found three different models to be especially useful in explaining some of the vulnerabilities and the strengths of those of us who are highly sensitive: Dr. Elaine Aron's work on the Highly Sensitive Person (HSP), the nutritional medicine concept of Pyrrole Disorder, and Dr. Sharon Meglathery's RCCX Theory. Each looks at a similar group of traits through a different lens — psychological, biochemical, and genetic, respectively. Before I start, I want to emphasize that sensitivity is not a pathology or a diagnosis. It is actually a trait that has existed across human history for a reason. My goal is not to pathologize it, but to understand it — and to offer tools to those who have these traits but who are struggling with a condition more prevalent in those with these traits. As I go through these three frameworks, the overlap will be obvious and I'll highlight what each of these has to offer that the others don't.What You'll Learn* The HSP model provides validation and a language for sensitive individuals.* Pyrrole Disorder involves an overproduction of pyrroles, leading to nutrient depletion of nutrients critical for neurotransmitter functioning.* Pyrrole Disorder is common in brain-related conditions.* RCCX Theory connects genetic vulnerabilities to sensitivity and chronic illness.* Stress amplifies the experiences of highly sensitive individuals.* Understanding these models can lead to effective treatment options.* There are meaningful paths forward for those who are highly sensitive.Chapters* 00:00 Understanding High Sensitivity* 02:26 The Highly Sensitive Person (HSP) Model* 10:23 Exploring Pyrrole Disorder* 18:18 RCCX Theory Explained* 30:11 Intersecting Models: HSP, Pyrrole Disorder & RCCX Theory* 34:40 Conclusion and Path ForwardI. THE HIGHLY SENSITIVE PERSONIn the early 1990s, psychologist Dr. Elaine Aron identified a personality trait she called Sensory Processing Sensitivity (SPS) — and the people who score high in it she called Highly Sensitive Persons, or HSPs.This is not a diagnosis. It is a trait, present in roughly 15–20% of the population. It has been found across more than 100 species — from fruit flies to primates. This tells us it is evolutionarily conserved — it confers survival advantages. In any social group, having members who are wired to notice subtle cues, process information deeply, and detect threats before others do is extremely valuable. Fitting with this, many of these individuals, who also struggle with complex chronic health issues are considered the “canaries in the coal mine” since their bodies react to environmental triggers (environmental toxins, chemicals, or stressors) long before the rest of the population.Dr. Aron captured the core features of this trait in an acronym she called DOES:D — Depth of processing: HSPs think deeply, reflect before acting, and notice subtleties that others miss.O — Overstimulation: Because they process so thoroughly, HSPs reach their threshold more quickly in high-stimulation environments.E — Emotional reactivity and Empathy: HSPs feel emotions intensely and are highly attuned to the emotions of those around them.S — Sensitivity to subtle stimuli: They pick up on things — in their environment, in social dynamics, in the body — that others simply don't register. As you can see, traits that are super powers, can become liabilities. The deep diver who loses sight of the big picture. The empath who absorbs everyone else's energy as their own energy becomes depleted. The person who withdraws from the world because the stimulation has simply become too much.HSP ResearchBeyond the clinical observations, there is now a growing body of neuroscience and genetic research supporting this trait. There is evidence that the brains of sensitive people are doing more, processing more and feeling more.fMRI studies (Acevedo, Aron et al., 2014) have shown that when HSPs view emotional images — particularly photos of loved ones expressing happiness or sadness — their brains show significantly greater activation in regions associated with awareness, empathy, and sensory integration.Twin studies show that Sensory Processing Sensitivity (SPS) as defined by Dr. Aron is approximately 47% heritable. Small studies as opposed to large genome wide studies have focused on genetic variants involving dopamine and serotonin systems, such as a serotonin transport gene, a dopamine receptor gene, and COMT, which codes for the enzyme that metabolizes dopamine and norepinephrine. While this hints at a biochemical dimension, the HSP framework doesn't address this as directly as the other two models I'll discuss, nor does it explain the higher prevalence of physical health conditions in those who are highly sensitive.Research published in 2026 (Matsuzawa et al.) found that individuals high in SPS had substantially higher rates of depression (13.8%), anxiety disorder (10.5%), and developmental disorders, such as ADHD and ASD, compared to the general population.Despite this overlap with developmental disorders: unlike, ADHD, HSPs typically have excellent concentration in quiet environments. And unlike the social deficits described in ASD, HSPs tend to have heightened social attunement. They often feel too much of what's happening in social situations, not too little, even if they respond awkwardly at times.Research shows that high sensory processing sensitivity is associated with more frequent physical symptoms — back pain, fatigue, digestive issues, frequent illness. What This Framework OffersThe HSP model has given millions of people validation and a language for these traits that has allowed them to shift from “what is wrong with me” to “this is how I am wired.”It also offers an evolutionary reframe. High sensitivity is not a mistake and is not pathologic. It is a feature of the human population that has served us.What it doesn't offer is a biological explanation for why some sensitive people suffer so acutely — or a path toward biochemical intervention. II. PYRROLE DISORDERPyrrole disorder is a biochemical imbalance that has been recognized for decades, though it remains largely unknown in mainstream psychiatry. It involves an overproduction of pyrroles — metabolic byproducts that, on their own, are not a problem. When they are high, however, they can result in a depletion of zinc, B6 and a few other nutrients.First identified in the 1950s and first treated with zinc and B6 in the 1980s, pyrrole disorder is one of the most common nutrient imbalances found in brain-related conditions — and one of the most treatable. Yet most people who have it have never heard of it. I learned about pyrroles in 2014 when I first trained with the Walsh Research Institute. The Importance of Zinc, B6 & MagnesiumVitamin B6 is required to synthesize dopamine, serotonin, and GABA — three of the most critical neurotransmitters for mood regulation, anxiety, and stress response. Zinc plays a profound role in the central nervous system, the immune system, gastrointestinal tract (which we now know has its own significant influence on brain health) and connective tissue (joints and skin).Pyrrole Disorder Traits & SymptomsThe most consistent feature is low stress tolerance. People with elevated pyrroles are often described as those for whom life seems harder than it should be, every transition can be destabilizing and every large group or new environment feels like too much. The overlap with the HSP profile is striking, but here, we start to see not only traits, but symptoms.- Socially anxious, shy, or fearful since childhood, with severe inner tension- Sensitive to bright light, loud noises, textures, and odors- Avoids crowds, strangers, and new situations- History of reading difficulty- Poor short-term memory- Underachievement- Irritability, mood swings, bouts of depression- Tends to stay up late; little or no dream recall; morning nausea- White spots on fingernails; very dry skin; stretch marks; poor wound healing- Joint pain- Frequent infections or autoimmune tendenciesMost people with pyrrole disorder don't have all of these symptoms — but the inner tension, the high sensitivity, and low stress intolerance are very common. What the Data ShowsThe Walsh Research Institute has collected data on over 30,000 patients. Elevated pyrroles were found in:* 18% of those with ADHD* 24% of those with depression* 28% of those with behavioral disorders* 35% of those with autism* 35% of those with bipolar disorder* 30% of those with schizophrenia* 12% of those with PTSDAnd in only 8% of healthy controls — meaning those with no psychiatric diagnosis. What Causes Pyrrole Disorder? For many, there appears to be a genetic component, but for others, pyrroles appear to have increased due to high physiologic or emotional stress. Examples include candida overgrowth or other forms of gut microbial imbalances, mold toxicity, heavy metals, chemical exposures, illness, emotional trauma, major life transitions or even growth spurts in children. What This Framework Brings UsRelative to the HSP framework, an understanding of pyrroles gives a specific, treatable biological mechanism. When zinc, B6, Magnesium and other nutrients are depleted, we know how the brain's neurotransmitter functioning is affected — and we know what to do about it.Treatment from a Walsh Research Institute trained practitioner typically involves a nutrient protocol of zinc (based on zinc and copper testing), B6 or its active form P5P, and other nutrients for pyrrole disorder and other nutrient imbalances identified. For many patients, a lifetime of chronic inner tension and fearfulness begins to shift within days to a couple of weeks of starting the nutrients.Treatment doesn't change one's personality, but it can improve social discomfort, overwhelm, the anxiety, the depression that has been layered on top of that sensitivity.Where zinc deficiency can impact a lot of the physical conditions that appear to be more common in those who are hypersensitive, it doesn't fully explain the significant hormonal, inflammatory, connective tissue symptoms and autoimmunity we see in many who are hypersensitive.III. RCCX THEORYRCCX theory has been proposed by Dr. Sharon Meglathery, MD — a psychiatrist and internist whose own complex health history (which is very similar to my own) led her to one of the most compelling explanations I've encountered for why hypersensitivity, neurodivergence, psychiatric conditions, MCAS (Mast Cell Activation Syndrome), EDS (Ehlers Danlos Syndrome), POTS (Postural Orthostatic Tachycardia Syndrome), CIRS (Chronic Inflammatory Response Syndrome) and/or CFS (Chronic Fatigue Syndrome) and autoimmunity tend to cluster together in certain individuals and families.RCCX refers to a module of four genes on chromosome 6. Three of them appear to be particularly important to our health, and two of those have very high rates of mutation. The Three Key Genes(1) TNXB codes for a protein involved in connective tissue. Variants range from subtle joint hypermobility to Ehlers-Danlos Syndrome. But connective tissue isn't just about joints — it's also about the permeability of the gut-blood barrier, the blood-brain barrier, and vulnerability to upper cervical (neck) instability, which can put tension on the vagus nerve and affect the entire autonomic nervous system.(2) CYP21A2 is the most important gene in the module when it comes to psychiatric conditions. It codes for 21-hydroxylase, an enzyme needed to convert 17-hydroxyprogesterone into cortisol. When there is a mutation — and there can be many, of varying degrees — the body may struggle to meet the demand for cortisol under stress. A weakness on this gene could have two primary effects:First — The theory holds that many people with CYP21A2 mutations, even without hypermobility, may develop what Dr. Meglathery calls a “brain wired for danger” — essentially, a nervous system calibrated toward threat detection from early in life, with minimal trauma required. A mutation on CYP21A2 may essentially result in relatively higher androgens impacting the amygdala — the brain's fear and emotional center. Studies have found that hypermobile individuals (who often carry TNXB mutations) have a larger-than-normal amygdala.Second — because of this mutation, the body may not be able to keep up with the body's need for cortisol (especially when under stress). This deficit causes the brain to release corticotropin releasing hormone (CRH) to signal the adrenal glands to produce more cortisol. CRH binds to mast cells and activates the immune system — leading to secondary brain inflammation. This is the inflammatory pathway that can result in a number of secondary health issues including MCAS, CIRS, POTS, CFS and more.(3) C4 is involved in immune response and autoimmunity, and has been linked to schizophrenia. This gene variant is not as common as the first two referenced, though I do see a number of families who also have autoimmunity impacting multiple family members. The CAPS Psychological ProfileDr. Meglathery uses the term CAPS — CYP21A2 Mutation Associated Neuropsychiatric Spectrum — to describe the psychological profile she has observed in people with this genetic vulnerability. These traits can exist long before any formal psychiatric diagnosis, and they are not inherently pathological. - Anxiety and over-arousal during times of stress — high adrenaline, insomnia, sometimes manic-like characteristics- Under-arousal during low-stress periods — leading to ADD presentations, compulsive behaviors, or thrill-seeking to raise arousal- High emotionality, sensitivity, and reactivity to environmental stimuli- Sensory processing difficulties — typically over-stimulation in loud or chaotic environments- A strong ability to read emotions in others, paired with social awkwardness in response- Easily affected by events others would consider minor — an offhand comment, a disturbing movie- A tendency toward nonconformity- Special abilities: gifted musicians, scientists, creatives, systems thinkers- Hyper-focus and obsession with areas of deep interest, often leading to remarkable accomplishments- High tenacity and determination, when not derailed by overstimulation or past trauma- Strong emotions — positive or negative — that can manifest as obsessional worrying, harm avoidance, OCD tendencies, or very high standards for self and othersDr. Meglathery also notes — and I have observed this clinically — that people with this profile tend to find each other through friendships, partnerships and marriage. They are drawn together by shared sensitivities, emotional depth, and intellectual gifts. This means children are likely to receive a tendency towards CAPS from both sides.Women who are appear to have CYP21A2 mutations often have a male-pattern finger length ratio — the ring finger longer than the index finger — a possible marker of androgen exposure during development. What RCCX OffersRCCX theory offers the broadest biological framework we have for understanding why sensitivity, psychiatric vulnerability, and complex chronic illness so often travel together in the same people and families. It connects the nervous system, the immune system, the hormonal system, and connective tissue under one genetic umbrella. It also points toward specific interventions: nervous system regulation (limbic retraining, vagal work), mast cell management, and addressing the downstream biochemical consequences of cortisol dysregulation. The limitation is that genetic testing for variants on the gene for 21-hydroxylase is not yet widely available, which is why this isn't a gene that you'll see referenced when it comes to HSP, though I would argue it is likely the most prevalent. RCCX remains a clinical theory — but one that explains the constellation of traits and symptoms of most of the people that I in my practiceIV. WHERE THESE MODELS INTERSECTWhere Might Pyrrole Disorder and RCCX Theory Meet? The symptom overlap is almost complete: poor stress tolerance, sensory sensitivities, anxiety, inner tension, connective tissue symptoms, frequent infections, autoimmune tendencies. Relatively low zinc would impact most of these shared symptoms, but there may be a potentially deeper connection.21-hydroxylase contains a pyrrole-like structure — heme — within it. Could a mutation resulting in abnormal 21-hydroxylase lead to a buildup of abnormal heme-like compounds, contributing directly to pyrrole disorder? This is not proven, but in an exchange about this with Dr. Meglathery, she indicated that she also believes there is a strong association, and the biochemical logic is compelling. This matters, because we know what helps pyrrole disorder. We have a treatment — treatment that can complement what RCCX theory makes evident — that the stress response has to be addressed.Stress (again from any cause - physiologic or emotional) is the great amplifier across all three models. Stress worsens the HSP experience, and tips the CYP21A2 mutation from manageable to destabilizing (through the hormonal consequences and the inflammatory cascade) and yes, it even causes pyrroles to increase.BRINGING IT TOGETHERFor someone who is highly sensitive, and struggling with brain related or physical symptoms or conditions, pyrrole disorder and even RCCX theory are worth considering because they offer tools and start to answer, “What Can We Do About It?”The three frameworks work together:The HSP model offers validation and language. It shifts the frame from deficit to difference. It reminds us that sensitivity is not pathology — it is a trait with evolutionary value, and it comes with great strengths. The Pyrrole model offers a treatable nutrient imbalance. It explains, at the neurotransmitter level, why stress hits harder for these individuals. Treatment doesn't take away the sensitivity; it lifts the weight that has accumulated on top of it.The RCCX model offers the broadest systems view. It connects the dots between what the body is doing — the immune activation, the hormonal dysregulation, the connective tissue involvement — and the psychiatric and psychological profile that tends to accompany it. It also provides additional tools: nervous system regulation, mast cell support, hormonal awareness. If any of this resonates — the sensitivity, the inner tension, the feeling of always being at the edge of overwhelm — please know that there are roots to explore and meaningful paths forward. If you are looking for someone trained by the Walsh Research Institute knowledgeable in assessing for and treating pyrrole disorder (and other common nutrient imbalances impacting brain health), or learning more about Dr. Aron's work on HSP and Dr. Meglathery's RCCX theory, I am providing those links below. And as always, I welcome your thoughts, experience and questions.Until next time,CourtneyAbout the AuthorDr. Courtney Snyder MD is a Holistic and Functional Child and Adult Psychiatrist who offers non-patient consultations to individuals and practitioners nationally and internationally in addition to her treatment practice serving FL, IN, KY, OH. ¿Volveremos a ver buenas estrategias? In this Podcast Extra, Join John as he answers a wide range of grower-submitted questions covering challenges in agriculture. The discussion focuses on managing high summer leaf temperatures, optimizing organic nitrogen budgets, and navigating severe base saturation imbalances. John emphasizes the critical connection between precise crop nutrition, active soil biology, and the natural suppression of destructive diseases and pests. Other topics discussed include: Distinguishing between air temperature and leaf temperature, highlighting how a healthy wax lipid layer lowers a plant's canopy temperature by 8 to 10 degrees. Managing the photorespiration process in C3 and C4 plants to prevent protein degradation and the buildup of high ammonium levels. Utilizing trace amounts of nickel (3 to 10 grams per acre) as an essential enzyme cofactor to rapidly lower ammonium in plant sap. Applying Rejuvenate as a foliar tool to provide stressed crops with carbohydrates, enzymes, and high-energy compounds. Addressing low magnesium and potassium in soils with extreme calcium base saturation using targeted applications. Citing historical 1960s data on fire blight indicating the disease cannot establish when sap sucrose levels remain above a 22–26% threshold. Restructuring high-chloride potato programs by pairing Rebound micronutrients with Holo-K, SeaShield, and SeaStim. Remediating the typical "burnt leaf tips" on garlic by satisfying the crop's unusually high molybdenum requirement. Evaluating the limitations of indigenous microorganisms (IMOs) and acknowledging that 90% of living soil microbes require active plant roots to propagate. Deploying PhotoMag to optimize plant sap ratios, delivering magnesium, sulfur, molybdenum, and boron to efficiently convert excess ammonium into complete proteins . Advocating for a national food policy focused on nutritional quality and farmer economic viability over cheap, abundant food. Utilizing MacroPack and MicroPack within target foliar combinations to halt disease progression. Understanding why high-salt content fertilizers solubilize trace minerals and leach them down the soil profile over a 20- to 40-year period. About John Kempf A top expert in biological and regenerative farming, John founded AEA in 2006 to help fellow farmers by providing the education, tools, and strategies that will have a global effect on the food supply and those who grow it.
