Pain is a predominant feature in the majority of functional gut disorders. The higher risk of anxiety and depression when coping with any chronic illness, including GI disorders, as well as anxiety and depression impacting symptoms and symptom perception in many GI conditions are all very important aspect of medical care. This week I interview Dr. Katia Tonkin on the role that cannabis plays in medical care – including in the treatment of anxiety, depression, and pain. Dr. Tonkin is a medical oncologist who worked at The Cross Cancer Institute in Edmonton Alberta Canada for 20 years and in London, Ontario Canada for 6 years prior to that. She became interested in medical cannabis in 2016 as many patients started asking about cannabis. After researching into it, Dr. Tonkin felt cannabis was likely to help many patients with a wide variety of problems. She started treating patients in April 2017 and wants to contribute to clinical research to further medical knowledge into appropriate uses of medical cannabis, as well as educating health care providers on the proper use of cannabis. She works out of Optimum Wellness Center as well as other clinics as a medical prescriber. We talk about: Tonkin's background as a clinician and how they became interested in the role cannabis plays in medical care. What cannabis is. The components of the plant that have therapeutic impacts. The role of a medical prescriber. How cannabis, that is medically prescribed, differs from cannabis you might find in stores and why 'self medicating' is not the best strategy to realize the benefits of cannabis The conditions we have evidence for using cannabis with (including anxiety, depression and pain) How these overlaps with gastrointestinal disorders. Her experience ranges from appearing on local news channels to international TV networks such as ATN (Asian Television Network). She went on to represent Canada on an international platform and placed first runner-up and was placed first in swimsuit and interview. Listen to a blog summary of a recently published research paper in Volume 13 of Oncotarget, entitled, "HDAC inhibitors suppress protein poly(ADP-ribosyl)ation and DNA repair protein levels and phosphorylation status in hematologic cancer cells: implications for their use in combination with PARP inhibitors and chemotherapeutic drugs." _______________________________________ Chromatin constitutes chromosomes in eukaryotic cells and comprises DNA and proteins. Chromosomes produce proteins and enzymes that are essential for cellular function and maintenance, including DNA repair. A critical process for DNA repair is poly(ADP-ribosyl)ation, or PARylation. PARylation is triggered by poly(ADP ribose) polymerase (PARP) enzymes. When DNA becomes damaged, PARP enzymes bind to the damaged location in the cell. In cancer cells, however, this natural process can be counterproductive in respect to cancer treatment. PARylation can produce DNA repair mechanisms in cancer cells that can lead to the evasion of cell death, and even drug resistance. Inhibiting PARylation may be a viable therapeutic strategy for cancer treatment. Histones, the main proteins that constitute chromatin, undergo post-translational modifications that regulate gene expression. Histone acetylation is an important epigenetic process that affects gene expression by relaxing the chromatin structure, making chromatin remodeling more feasible. Histone deacetylases (HDACs) are enzymes that can have the opposite effect. Histone deacetylation makes the chromatin more compact and difficult to remodel. The overexpression of HDAC has also been associated with tumorigenesis. Histone deacetylase inhibitors (HDACi) are a class of therapeutics that have shown promise in the treatment of hematologic malignancies (blood cancer) and solid tumors. In a new study, researchers Benigno C. Valdez, Yago Nieto, Bin Yuan, David Murray, and Borje S. Andersson from the Department of Stem Cell Transplantation and Cellular Therapy at the University of Texas MD Anderson Cancer Center and the Cross Cancer Institute's Department of Experimental Oncology at the University of Alberta investigate the efficacy of HDACi in combination with PARP inhibitors (PARPi) and chemotherapeutic drugs to treat hematologic cancer. Have you heard about radon before? A radioactive gas released from uranium decay, radon is found in any enclosed space where the building makes contact with soil. This means that homes, schools, workplace (unless you work outside) has radon! It's a tasteless, odorless, and invisible gas. Why should you care? The answer is easy: radon can result in lung cancer, and it's largely preventable by reducing exposure. Hence, we're calling it the "deadlier cousin" of the silent killer (a.k.a. carbon monoxide). Joining us today is Dr. Kelvin Young, a medical oncologist from the Cross Cancer Institute in Edmonton, AB, Canada. He brings his area of expertise to explore the medical aspect of radon induced lung cancer, including chances of survival and treatment options. This episode continues my series on consciousness—are we just biological robots? Following on from my exciting interview last week with quantum biophysicist Dr. Luca Turin, I dig further into competing quantum theories of consciousness. The famous Sir Roger Penrose and his partner Stuart Hameroff have proposed the Orchestrated OR theory. This theory posits a modification of the incomplete theory of quantum mechanics that underlies half of physics. They argue that consciousness arises from moments of wavefunction collapse that are objectively linked to certain aspects of quantum superpositions. They further argue that biology has evolved a mechanism to orchestrate these wavefunction collapses in tiny subcellular structures called microtubules. My guest today has been doing experiments in the lab to determine if physical processes in microtubules respond to anesthetics. Professor Jack Tuszyński obtained his Ph.D. in condensed matter physics in 1983 from the University of Calgary. From 1983 to 1988 he was a faculty member at the Department of Physics of the Memorial University of Newfoundland in St. John's. He moved to the University of Alberta in 1988 as an assistant professor, between 1990 to 1993 he was an associate and then full professor at the Department of Physics. Between 2005 and 2020 he has held the prestigious Allard Chair in Experimental Oncology at the Cross Cancer Institute where he leads an interdisciplinary computational drug discovery group. He is also a Fellow of the National Institute for Nanotechnology of Canada. He has held visiting professorship and research positions in China, Germany, France, Israel, Denmark, Belgium and Switzerland. He is widely published in the scientific literature providing frequent invited talks on his ground-breaking work. Acute myeloid leukemia (AML) is a cancer of the blood that begins in the bone marrow and progresses quickly if left untreated. AML can occur both in adults and children and is often treated with allogeneic hematopoietic stem cell transplantation (allo-HSCT). Allo-HSCT is a procedure that replaces stem cells that were damaged or destroyed after radiation and/or chemotherapy treatment with stem cells from healthy donors. While allo-HSCT provides a high rate of curability in AML patients, the success of this procedure is partially dependent on the efficacy of pre-transplant treatment regimens. Researchers have identified an urgent need to determine new therapeutic approaches that provide better cytotoxicity in AML cells, without jeopardizing patient safety. To improve AML patient outcomes after allo-HSCT, researchers from the University of Texas MD Anderson Cancer Center and the University of Alberta's Cross Cancer Institute conducted a new study investigating the combinations of the BCL-inhibitor ABT199/venetoclax with two alkylating agents and a nucleoside analog. He is a Fellow of the Royal College of Physicians and Surgeons of Canada (RCPSC) in Internal Medicine and Gastroenterology having completed medical residency at McMaster University in Hamilton and the University of Alberta. During his training, Dr. Wong was awarded research fellowships from both the Alberta Heritage Foundation for Medical Research and the Canadian Association of Gastroenterology for translational research in Experimental Oncology. He has also completed a fellowship in endoscopic ultrasound and is a therapeutic endoscopist focusing on gastrointestinal cancers. He is the medical director of the Edmonton Endoscopic Ablation Program which treats Barrett’s esophagus and early upper gastrointestinal tract cancers. He is also the provincial medical director of the Alberta Colorectal Cancer Screening program (ACRCSP). His research interests include clinical and laboratory innovations in colon cancer screening and Barrett’s esophagus. He received funding from AHFMR and Alberta Innovates. Dr. Wong is a Past-President of the Alberta Society of Gastroenterology. He has received regional and national teaching awards for excellence in medical education including Endoscopy & Teacher of the Year from the UofA GI Residency Training Program, the University of Alberta Medical Students’ Association Teacher of the Year Award, the Canadian Association of Medical Education (CAME) Certificate of Merit, and the University of Alberta Rutherford Award for Excellence in Undergraduate Teaching.
Dr. Todd McMullen is a scientist and endocrine surgeon at the University of Alberta in Edmonton, Alberta. In this episode we talk to Dr. McMullen about a topic that is sometimes a pain in the neck for trainees: thyroid nodules. Having received his PhD and additional training in Physiology, Dr. McMullen went on to complete medical school at the University of Toronto and a residency in General Surgery at the University of Alberta. He further specialized in Endocrine Surgery at the Royal North Shore Hospital in Sydney, Australia, where he finished a TS Reeve Fellowship in Endocrine Surgery. Since that time, Dr. McMullen has been an active member of the University of Alberta staff as part of the Faculty of Medicine and Dentistry, where he now holds positions in Surgery and Oncology (Division of General Surgery), an Adjunct appointment in the Division of Endocrinology (Department of Medicine) and also serves as the Director of the Division of Surgical Oncology (Department of Oncology) based at the Cross Cancer Institute. In this episode, Mark and Curtis are in Edmonton Alberta with archaeologist and author Jack Brink. Jack was instrumental in helping uncover and tell the story of Aboriginal communal bison hunting at Head-Smashed-In Buffalo Jump, a UNESCO World Heritage Site that is situated in the Porcupine Hills in southern Alberta. Jack discusses how bison were coaxed over the deadly jump using highly sophisticated hunting methods involving trickery, mimicry and an unrivaled understanding of bison behavior. Jack explains how the animals were processed at the butchering grounds and what a monumental task that was. We chat about what the best season was for driving bison to the buffalo jump, what specific animals were preferred as food, how long the jump was in use and why it came to an end. Since it began in 2003, the annual Bryan Mudryk Golf Classic has raised more than $1 million for cancer treatment, equipment, and research.
