Toxic multinodular goiter is a type of hyperthyroidism in which a person has a goiter in the presence of multiple thyroid nodules. In today's Save My Thyroid audit, I'm chatting with Nanette, who has been dealing with toxic multinodular goiter for over 20 years and I'll be sharing practical advice to help her improve her health. Nanette's thyroid journey began at age 16 when she had her first thyroid nodule, which was then surgically removed. Later, at age 30, she found herself in the same scenario, and she had a second surgery. In this episode of BackTable ENT, Dr. Gopi Shah and Dr. David Goldenberg, head and neck surgeon and department chair at PennState Health in Pennsylvania, discuss radiofrequency ablation (RFA) of thyroid nodules. --- SHOW NOTES First, Gopi and David review workup of thyroid nodules. Before considering RFA, patients need thyroid function tests, ultrasound imaging, and a biopsy showing benign disease. In David's opinion, radiofrequency ablation is a good option for benign nodules larger than two centimeters that are enlarging and causing compressive symptoms. RFA is not appropriate for malignant disease, with the potential application for papillary microcarcinoma, which is currently being evaluated in other parts of the world. Next, David outlines how he performs RFA. Using a combination of mild sedation, local anesthesia, and regional anesthesia, he performs this procedure in-office under continuous ultrasound guidance. Radiofrequency ablation uses heat to cause coagulative necrosis of nodular tissue. After cervical block and anesthetization of the thyroid capsule, David introduces the RFA probe through a small midline neck incision. Drawing on the “moving shock technique,” he moves the RFA probe back and forth, watching for signs of tissue necrosis on ultrasound. Complications of RFA are similar to those of thyroid surgery, but they occur at lower rates due to the less invasive nature of this procedure. Finally, the conversation places RFA in a larger context for the thyroid surgeon. RFA patients return to the clinic that same week for a physical exam. Surveillance ultrasounds occur six and twelve months post-procedure. David counsels his patients that their nodule will not shrink immediately, and may swell in the days following their procedure. However, he expects the nodule to shrink 50-80% in the year following RFA. I'm looking forward to sharing with you some of our community's questions that have come in over the past few weeks…   Cindy: I have used your Daily Level 3 protocal for a few years now, but I only use half of the multi vitamin (due to cost) because I share it with my teenage son. I do purchase additional omega 3 so that I get 2 grams daily and use 1 capsule of your B complex daily. I love your magnesium drink, healthy gut support, collagen, and thyroid support. I would like to use the mitochondrial and hormone support too! My genes do have 2 copies of the MTHFR gene mutation and I am postmenopausal. Oh, and I use about 20,000 I.U. of your Vit. D daily inorder to keep my level above 50. I can't take every supplement I would like, but how do I prioritize? If I took the full dose of all of these, would I get too much or waist vitamins?   Cindy: Do you think a microbiom could get "use to" a probiotic and therefore not work as well? A few years ago when I started taking your probiotic, it immediately helped be to have a BM daily. That doesn't happen anymore. 2 BM weekly is now the norm again. The James Cancer Hospital is the first medical facility in central Ohio to offer radiofrequency ablation to treat and destroy benign and cancerous thyroid nodules. In this episode, Barbra Miller, MD, describes this high-tech, minimally invasive treatment technique and the advantages for patients. “It can be done in the office and most patients are in and out of the clinic within 30 minutes to an hour after the procedure,” she explained. Here's how radiofrequency ablation works: The surgeon insets a very small needle into the nodule; radiofrequency waves are used to heat the tip of the needle, and this ablates (destroys) the tissue in the nodule. “We use the moving-shot technique,” Miller said, adding this technique involves moving the end of the needle to different locations with the nodule to destroy it completely. Miller also describes microwave ablation, which is similar but uses microwaves to heat the end of the needle. This podcast highlights original research published in the August official journal of the American Academy of Otolaryngology—Head and Neck Surgery (AAO-HNS) Foundation. The objective was to evaluate nodule height-to-width ratio as a continuous variable predicting likelihood of thyroid malignancy. In conclusion, our results support the association between greater height-to-width ratio and malignancy but suggest that a multilevel rather than binary variable improves prediction. The likelihood ratios at different intervals give a more nuanced view of how height-to-width ratio predicts malignancy. With continuing review of guidelines for thyroid nodule biopsy, it is important to consider these data for any point total attributed to shape.   Click here to read the article.

First, Dr. Goldenberg describes his workup for thyroid nodules, which includes palpation, ultrasound, and TSH levels. He emphasizes that a thyroid ultrasound must involve the scanning of both sides of the neck in order to make comparisons between normal and abnormal findings. After evaluating the ultrasound and having open dialogue with pathologists and radiologists, his team decides if Fine Needle Aspiration (FNA) is necessary. The doctors discuss how to counsel patients when FNA yields indeterminate results. The course of further treatment and surveillance should take into account the patient's risk tolerance, the presence/absence of compressive symptoms, and the patient's ability to return for a later biopsy. Dr. Goldenberg describes how the use of molecular testing is becoming more common in diagnosing thyroid cancers. Specifically, the BRAF mutation usually signifies thyroid cancer, and the TERT mutation signifies a very aggressive form of thyroid cancer. Finally, the doctors discuss surgical decision-making between a full thyroidectomy and a partial lobectomy.
