I live in Northern NY State. We cannot get any at home lab test except hair sample tests. How accurate are these tests? We also do not have ANY functional medicine practices near us. I have lupus and hashimotos, and my husband and my daughter both have digestive issues. I don't know where to start but I want my family healed. We do eat mostly whole food and organic diet and strength train regularly.     Lina: Hi Dr. Cabral. Thank you so much for all you do to help us maintain good health. I have two questions: 1) After fasting for more than 24 hours, when I eat my first meal, my throat literally hurts when I swallow food. This happens every single time. What could be causing this and what can I do to avoid this discomfort? The pain does goes away after the first meal after fasting. In this episode of The PODdoctors podcast, Dr. Damien Dauphinee, a board-certified foot and ankle surgeon, and Dr. Raafae Hussain, a fellowship-trained foot and ankle surgeon, discuss the excision of a lipoma, a benign fatty nodule that can cause pain in the foot and ankle. They explain that lipomas are commonly found in the subcutaneous layer of the foot and ankle, and while they are usually benign, they can become painful. The doctors emphasize the importance of clinical diagnosis, as an MRI may not show whether the lipoma is causing pain. They also mention that lipomas are often misdiagnosed as underlying ankle sprains. The surgical procedure involves making an oblique incision along the relaxed skin tension lines, carefully dissecting the lipoma, and removing it as a solid piece. The doctors highlight the need to identify and protect the lateral branch of the superficial peroneal nerve during the procedure.QUOTES:"You can get something called lipidema, and that's a different animal where a known normal fatty deposit can become painful. A lipedema can be a real problem in different parts of the body, and that's a normal place for adipose tissue. But if it becomes painful, sometimes we have to consider removing it. I've not appreciated that as much over the years as I do now. I think I'm seeing that that can become a source of pain all by itself in some patients. The reason we do that is so the knot stays buried. It's not close to the skin. So when she ends up tying this, you'll see that the knot will stay nice and low and hopefully the patient won't ever feel it. And then as time goes on, the weeks go on, the stitches will dissolve away, and hopefully no more score. I’m looking forward to sharing with you some of our community’s questions that have come in over the past few weeks… Let’s get started!    Jamie: Hi Dr. Cabral, thanks for all you do! I would like to ask you about how to be healthy after bariatric surgery. I had the gastric sleeve on March 1st 2019 and found your podcast later that same month. Had I found you earlier I likely would NOT have had the surgery. However, I believe things happen for a reason so maybe this is my path. Now, my question is I can't eat as much at one serving and so I get hungry faster/need more meals throughout the day and I am also worried I'm not getting enough nutrition. I am starting the 21 detox then I will go into the CBO protocol and finally heal & seal. I have been drinking the DNS smoothie every morning for about 4 months. What advice can you give me & others like me to be very healthy after bariatric surgery. Thanks! Sahsa: Hello Dr. Cabral, thank you for the wisdom you share with the community everyday. I know that I need to start detoxes, but there is something in the daily nutritional support shakes that is exacerbating headaches. I’ve tried it on 2 separate occasions 4 months apart, and both times my headaches began. It makes me nervous to use it or any of the detox pills. I’m able to use some of your other products like the balanced zinc and sinus support without any issues. How would you suggest doing a detox without the shakes or the detox capsules? Thanks for all you do. Laura: I am a healthy, active 31 year old female recently diagnosed with a schwannoma or neurofibroma tumor, 3cm big, under my right collar bone near my brachial plexus, likely nerve C6 or C7. Surgery to have it removed is Dec 2. I’ve had shoulder/arm pain in certain yoga postures for the past year and could never figure out where it was coming from until a failed lymph node biopsy pointed the doctors in the right direction. For the past year, I’ve easily tolerated the minimal nerve pain, but ever since the biopsy a month ago, I’ve been in moderate pain and haven’t been able to do yoga. I’m very worried about permanent nerve damage from this - since they aggressively poked a nerve that they thought was a lymph node. Is that possible? Is there anything you can recommend that will help? Seeing as how you will likely see this after my Dec 2 surgery, do you have any tips on recovery from nerve surgery? Not being able to do yoga (I practice Bikram yoga daily) brings my anxiety and depression back in full swing. Thank you for all you do!! Nicole: Hello, I have a question I didn’t want to ask on the Facebook group as my daughter would be so horrified if anyone knew. So I have a 15 year old daughter who for most of her life has really struggled with pinworms . We can seem to get a handle on them. We use the combantrin and do the second and even have done third doses. They seem to come back. Yes we have done all the things, washing, nails short, washed bedding everyday even for a week. They examine, diagnose, and treat patients autonomously and as part of a team in all various specialties of medicine.   2017 Median Pay: $104,860 per year ($50.41 per hour)   Educational Degree: Masters Degree   Number of US jobs in 2016:106,200   10 Year Job Outlook: 37% growth, much faster then avg.     *Bureau of Labor Statistics, U.S. Department of Labor, Occupational Outlook Handbook, Physician Assistants, (visited August 5, 2018).     Terms Covered in Episode Navy A School   Corpsman - Enlisted medical specialist   Lipoma - Overgrowth of adipose tissue (fat) creating a benign tumor often located between skin and muscle layer.   Eugene Stead Jr. MD   CT Surgery - Cardiothoracic surgery deals with issues of the thorax, generally the heart and lungs.   Trauma Surgery - Surgical field dealing with acute traumatic injuries such as falls, motor vehicle crashes, gunshots, blunt and penetrating injuries, etc.   ICU (Intensive Care Unit, Critical Care Unit, or Intensive Therapy/Treatment Unit) - Part of the hospital with the sickest patients requiring the most intervention from both staff and equipment.  May consist of intubated, sedated, and ventilated patients.   Call - To be available on your “off time” for phone calls, patient visits, surgeries, emergencies, etc that varies by job.   Open Surgery - Traditional way of surgery going through the layers of the body to operate.   Endoscopic Surgery - Surgery utilizing cameras, smaller openings and temporary ports in the body to pass through instruments in a tight space allowing for less pain and quicker recovery.   Vein Harvest - Generally endoscopic removal of a vein to replace a coronary artery (cardiac bypass)   Bypass - Surgical procedure to restore blood flow to the heart after an obstruction occurs.   Cardiac Graft   BP (Blood Pressure) - Force of blood against the arterial walls.   Recovery - Phase after surgery when anesthesia is wearing off and patient is being monitored before being sent home with family or admitted to the hospital.   Punch Biopsy - Small round biopsy (cut) taken to identify the cause of abnormal tissue.   Hernia - Bulging of an organ (intestines) through an abnormal opening.   ENT Surgery - Ear, Nose, and Throat.  Various procedures involving the head.   MBA - Masters of Business   UCSF Neuroscience   Medicare - Government provided healthcare insurance to those over 65, young people with disabilities, and people with end stage renal disease.   401k - Retirement plan.   Western Governors University   Bedside Manner - approach or attitude towards a patient.   Neurosurgery - Surgery dealing with the Nervous System (brain and spine).   Trauma LVL 1 Center - Hospital able to provide total care for every aspect of injury.  Large facility with all types of staff including Residents.   Cerebral Stenosis - When artery inside the brain that becomes blocked by plaque or disease.   TPA (Tissue Plasminogen Activator) - Protein involved in the breakdown of blood clots.   Aneurysm - Ballooned and weaker area of an artery.   CVA (Cerebral Vascular Assault) - Stroke.  Damage to the brain from a loss of blood flow.   Ischemic CVA - loss of blood flow from a blockage.   Hemorrhagic CVA - loss of blood flow from a blockage a ruptured aneurysm.   Elective Surgery - Nonemergent scheduled surgery   Discectomy - Surgical removal of whole or part of a intervertebral disc.   Fusions - Surgery to join two or more vertebrae together.  Done for fractures, deformities, instabilities, slipped vertebrae, or herniated disk.   ACDF (Anterior Cervical Discectomy & Fusion) - Discectomy of the cervical spine (neck) with fusion to decompress the spinal cord and nerve roots of the cervical spine.   Neuroaxis (Neuraxis) - Central Nervous System   Tumor - Abnormal growth of cells   Resection - to surgically remove   Spine Stabilization - Surgery to stabilize the spine with more flexible materials then traditional fusion.   Angio - technique used to visualize inside blood vessels.   MRI (Magnetic Resonance Image) - Medical Imaging using magnets to see deep layers of the body.   Space Occupying Lesion - abnormal mass in the brain usually due to cancer, but may be abscess (infection) or hematoma (blood).   Glioblastoma - Rare very aggressive and fatal cancer of the brain or spine.   Code Yellow - Phrase used over PA system of hospital when Trauma Patient is coming to the ER.  To alert the hospital to assemble the Trauma Team and support staff to receive the patient.   Hypotensive - lower then normal blood pressure.   Chest Tube - tube placed in the chest cavity to evacuate blood, pus, or air.   Central Line - Larger then an traditional IV placed into a main vein of the body. Background Retroperitoneal lipomas are an extremely rare condition with only 17 cases described in the literature since 1980. They can reach enormous size and cause significant abdominal symptoms. The most important differential diagnosis is the well-differentiated liposarcoma, which preoperatively often may not definitely be ruled out. Case presentation We present the case of a 73 year-old Caucasian patient with a giant retroperitoneal lipoma of 9 kg measuring 55 cm in diameter. The patient presented with abdominal pain and swelling that had been slowly progressive for the last 15 years. On computerized tomography an immense retroperitoneal tumor was revealed. Intraoperatively, the tumor did not show any signs of infiltrative growth, therefore sole tumor extirpation was performed. Conclusion Retroperitoneal lipomas are not clearly distinguishable from well-differentiated liposarcomas on imaging and even biopsies may be misleading. Moreover, abdominal symptoms, i.e. pain, obstipation and dysphagia may occur due to mechanical displacement. Therefore, surgical exploration with complete oncological resection is the therapy of choice if malignity cannot be ruled out.
