We mark National Mental Health Awareness Month on this episode by tapping the expertise of Dr. Steve Strakowski, an internationally recognized expert in bipolar disorder, who has spent decades studying the neurobiology and treatment of mood conditions while pushing just as hard on the structural barriers that keep effective treatments out of reach for more than half the people who need them. In this conversation with Raise the Line from Elsevier host Michael Carrese, Dr. Strakowski explains why access, not science, is now the biggest obstacle to improving mental health outcomes. He also addresses the heavy toll society pays for underfunding mental health prevention and treatment programs. “The money is spent eventually, but in the most expensive places like emergency rooms and prisons, and there is the human cost of suffering and suicides." This important discussion also covers: The persistent problem of Black patients presenting with mania being misdiagnosed with schizophrenia; Why he describes bipolar disorder as a reward-processing illness; The emerging therapies he finds encouraging. The repo market is $16 trillion globally and most people have never heard of it — until the plumbing breaks. Craig Burchell of FalconX and Matteo Pandolfi of Pareto explain how it works and why bringing it on-chain is the next big unlock for DeFi. --- Heads up! It is also almost entirely absent from on-chain finance — and that gap is creating real problems for RWA liquidity, stablecoin swap desks, and DeFi protocols trying to manage redemption queues. Steve Ehrlich sits down with Craig Burchell, head of lending at FalconX, and Matteo Pandolfi, CEO of on-chain credit infrastructure provider Pareto, to map exactly how repo works, what broke in 2019, why it translates extremely well into onchain finance. We had some HYPOTHESES for this amazing episode! The main topics all revolved around fatigue resistance (often called “durability” in the literature), which we think is the most exciting area of training theory and physiology. It started with Rachel Entrekin's legendary overall win at the Cocodona 250, an event that explores some of the strangest and coolest parts of endurance. Our big theory: what we're seeing in these wild ultras is directly connected to what we see in short distances. A new study looked at fatigue resistance variations between men and women, with some shocking results. We expand the scope to talk about the literature generally before going to some ideas for how it all might relate to the central nervous system....And how it relates to fueling. You could probably guess that we were going to talk about carbs.Plus, we answer questions about the culture of fueling for young athletes and feeling good after long runs! This one starts with a description of Megan's health journey. The hottest vacation is apparently a 3-day stay in Boulder Community Hospital. Things are scary, but we're holding onto hope for the future. We love you all! New Discourses Bullets, Ep. 154 Father of Italian Fascism, Benito Mussolini, is famous for having remarked that Fascism can be summarized simply enough as "corporatism." When we hear this, many of us assume it refers to an ideology of the state being fused with its corporations. That's part of the economic program of Fascism, no doubt, but that's not actually what it means. What it means is much more concerning and creepy. The word "corporate" and thus "corporatism" come from the Latin root corpus, which means "body." Fascism as corporatism actually means that the whole of society operates as a single body or single organism! The individuals are its cells, the institutions (including corporations) are its organs, and the Fascist State is its Central Nervous System and seat of its entire soul. On this episode of Vitality Radio, Jared introduces a refined Emotional Vitality format and tackles one of the most common struggles in modern life: overwhelm. Rather than being a mindset failure or lack of willpower, overwhelm is explained as a nervous system state—specifically a freeze response driven by brain chemistry and stress signaling. Jared breaks down what's happening neurologically, why procrastination is often protection, and how clarity and motivation shut down under chronic stress. You'll learn practical, science-grounded tools to regulate your nervous system through simple physical actions, breathing, language shifts, and small, specific steps that restore agency. Supplement support like magnesium, ashwagandha, L-theanine, and B vitamins is discussed as supportive—not corrective—tools. In today's episode, I'm opening the first chapter of what I believe is the most important series I've ever created — a deep dive into progesterone and why it became the heart of my medical practice. For more than 20 years, I've watched this “simple, humble hormone” transform women's lives in ways most conventional medicine overlooks. What started in two small treatment rooms has grown into a 25,000 sq ft facility, and the core of our success comes down to understanding progesterone's impact on the female brain, stress response, and emotional resilience. In this episode, I break down: Why progesterone is far more than a reproductive hormone How it regulates the female stress response (amygdala, hippocampus, prefrontal cortex) Why anxiety, insomnia, irritability, and emotional overwhelm often map directly to progesterone decline Why so many women feel “unraveled” in their 40s — and why it's not their fault The science behind oral vs. sublingual progesterone (and why I use troches) How conventional medicine often misses the root cause The importance of physicians showing their work, their data, and their citations The lived stories and clinical outcomes that changed how I practice medicine If you've ever felt dismissed, unseen, or told that your anxiety or mood changes are “just stress,” this episode is for you. This is the beginning of a 7-part series where I break down the neurobiology, endocrinology, testing, dosing, delivery methods, breast health, perimenopause, and more. This podcast is sponsored by YARAL Pharma. In this episode, we are focusing on the management of hypothyroidism -- a treatable, but not curable condition – and will explore unique challenges for patients with hypothyroidism in long-term care – from tolerability and formulation considerations to consistent dosing and patient needs. Dr. Tamara Ruggles is not affiliated with YARAL Pharma. All views and opinions regarding hypothyroidism are solely her own and are not attributable to YARAL or the Pharmacy Podcast Network. IMPORTANT SAFETY INFORMATION for levothyroxine sodium capsules INDICATION AND USAGE Levothyroxine sodium capsules are L-thyroxine (T4) indicated for adults and pediatric patients 6 years and older with: Hypothyroidism - As replacement therapy in primary (thyroidal), secondary (pituitary), and tertiary (hypothalamic) congenital or acquired hypothyroidism Pituitary Thyrotropin (Thyroid-Stimulating Hormone, TSH) Suppression - As an adjunct to surgery and radioiodine therapy in the management of thyrotropin-dependent well differentiated thyroid cancer Limitations of Use: Levothyroxine sodium capsules are not indicated for suppression of benign thyroid nodules and nontoxic diffuse goiter in iodine-sufficient patients as there are no clinical benefits and overtreatment with Levothyroxine sodium capsules may induce hyperthyroidism. Levothyroxine sodium capsules are not indicated for treatment of transient hypothyroidism during the recovery phase of subacute thyroiditis WARNING: NOT FOR THE TREATMENT OF OBESITY OR FOR WEIGHT LOSS Thyroid hormones, including levothyroxine sodium capsules, either alone or with other therapeutic agents, should not be used for the treatment of obesity or for weight loss. In euthyroid patients, doses within the range of daily hormonal requirements are ineffective for weight reduction. Larger doses may produce serious or even life-threatening manifestations of toxicity, particularly when given in association with sympathomimetic amines such as those used for their anorectic effects. Contraindications Uncorrected adrenal insufficiency Warnings and Precautions Cardiac adverse reactions in the elderly and in patients with underlying cardiovascular disease: Initiate Levothyroxine sodium capsules at less than the full replacement dose because of the increased risk of cardiac adverse reactions, including atrial fibrillation Myxedema coma: Do not use oral thyroid hormone drug products to treat myxedema coma Acute adrenal crisis in patients with concomitant adrenal insufficiency: Treat with replacement glucocorticoids prior to initiation of levothyroxine sodium capsules treatment Prevention of hyperthyroidism or incomplete treatment of hypothyroidism: Proper dose titration and careful monitoring is critical to prevent the persistence of hypothyroidism or the development of hyperthyroidism Worsening of diabetic control: Therapy in patients with diabetes mellitus may worsen glycemic control and result in increased antidiabetic agent or insulin requirements. Carefully monitor glycemic control after starting, changing, or discontinuing thyroid hormone therapy Decreased bone mineral density associated with thyroid hormone over-replacement: Over-replacement can increase bone reabsorption and decrease bone mineral density. Give the lowest effective dose Adverse Reactions Common adverse reactions with levothyroxine therapy are primarily those of hyperthyroidism due to therapeutic overdosage. They include the following: General: fatigue, increased appetite, weight loss, heat intolerance, fever, excessive sweating Central Nervous System: headache, hyperactivity, nervousness, anxiety, irritability, emotional ability, insomnia Musculoskeletal: tremors, muscle weakness Cardiovascular: palpitations, tachycardia, arrythmias, increased pulse and blood pressure, heart failure, angina, myocardial infarction, cardiac arrest Respiratory: dyspnea Gastrointestinal (GI): diarrhea, vomiting, abdominal cramps, elevations in liver function tests Dermatologic: hair loss, flushing Endocrine: decreased bone mineral density Reproductive: menstrual irregularities, impaired fertility Adverse Reactions in Children Pseudotumor cerebri and slipped capital femoral epiphysis have been reported in children receiving levothyroxine therapy. Overtreatment may result in craniosynostosis in infants and premature closure of the epiphyses in children with resultant compromised adult height. Seizures have been reported rarely with the institution of levothyroxine therapy. Hypersensitivity Reactions Hypersensitivity reactions to inactive ingredients (in this product or other levothyroxine products) have occurred in patients treated with thyroid hormone products. These include urticaria, pruritis, skin rash, flushing, angioedema, various GI symptoms (abdominal pain, nausea, vomiting and diarrhea), fever, arthralgia, serum sickness and wheezing. Hypersensitivity to levothyroxine itself is not known to occur. Drug Interactions: Many drugs and some foods can exert effects on thyroid hormone pharmacokinetics (e.g., absorption, synthesis, secretion, catabolism, protein binding, and target tissue response) and may alter the therapeutic response to Levothyroxine sodium capsules. Administer at least 4 hours before or after drugs that are known to interfere with absorption. See full prescribing information for drugs that affect thyroid hormone pharmacokinetics and metabolism. To report SUSPECTED ADVERSE REACTIONS, contact Yaral Pharma Inc. at 1-866-218-9009, or FDA at 1-800-FDA-1088 or www.fda.gov/medwatch. Simon Green isn't the first country bloke to put off going to the Doctor. But when an extremely rare and sinister tumour began snaking its way down his spine, time was not on the father of three's side. In less than four weeks, his symptoms morphed from mild discomfort to nerve tingling to full-blown paralysis of his lower body. With Simon's condition quickly deteriorating, his Doctors arranged an emergency RFDS evacuation to Newcastle. Later, specialists from around the world would consult on the best way to salvage his spinal cord and save Simon's life - after diagnosing him with a cancer of the Central Nervous System that accounts for less than 2% of all brain and other CNS tumors.***Thanks so much for listening to this episode of the Flying Doctor Podcast. Poor diet, lack of exercise, toxins, alcohol, trauma, or even other peoples energy can negatively affect our nervous system. Constantly worrying about things beyond our control, striving to meet expectations, being in unhealthy relationships, living in a toxic environment, or engaging in people-pleasing behaviors can take a toll and overwhelm your nervous system. Left unchecked, a dysregulated nervous system can make it harder for your body to function optimally! Your body isn't broken - it's disconnected. The key to reconnecting? It's not in a pill bottle or another fad diet. It's your vagus nerve, and it's time we talk about it. You may have your diet and nutrition dialed in, but you can't truly heal without addressing this crucial piece of the puzzle. Why? Because you're likely stuck in fight-or-flight mode, and your body needs to learn how to take its foot off the gas pedal. Unless you work on this, you'll never get better. Period. Your chronic symptoms? Your "incurable" condition? That unexplained fatigue? They might all trace back to one overlooked nerve. This isn't just another health hack - it's essential in understanding your body. Your brain-body connection is REAL, and it's called the vagus nerve. It's time to tune into your vagus nerve and turn up your vitality. Don't miss this game-changing episode dropping next week. No more chasing symptoms - learn how your vagus nerve could be the key to unlocking your body's true healing potential. It's time to stop accepting "good enough" and start demanding optimal health. I get a little giddy when I come across a novel mitochondrial-boosting strategy; particularly, when its mechanisms of how it impacts the mitochondria aren't redundant with other powerful modalities, such as red light therapy, methylene blue and C60. Of course, it must also be safe, efficacious and backed by research for it to even be a consideration. Well, ladies and gentleman, the amino acid taurine fits that bill to a tee!If you're like me, taurine may have a knee-jerk negative connotation due to its association with energy drinks. But if that's the case, I politely ask you to table that association as we head into this episode. The more I have learned about this mysteriously overlooked amino acid, and its science-backed efficacy for improving mitochondrial function in a myriad of way, the more I have gotten excited to report this information to you. Taurine is the real deal!In this episode, I will be providing "knowledge snacks" from a textbook I recently read, "Taurine and the Mitochondrion: Applications in the Pharmacotherapy of Human Diseases." The chapters and information covered in part one include the synthesis, dietary sources, and mechanisms of action of taurine; taurine and the mitochondrion; and applications of taurine in the central nervous system disorders linked with mitochondrial impairment. Subsequent parts of this series will cover other diseases and body systems impacted by mitochondrial dysfunction and how taurine may play a pivotal role in mitigating, preventing and/or reversing aforementioned conditions. If you found the information in today's episode particularly interesting and/or compelling, please share it with a family member, friend, colleague and/or anyone that you think could benefit and be illuminated by this knowledge. In this era of highly effective treatments, early intervention for multiple sclerosis can be life changing. However, the wide range of initial symptoms often leads to many people being undiagnosed or misdiagnosed, leaving them without the proper care for years. In this podcast episode, we'll discuss the early signs of MS and the key tests, such as specific MRI and spinal fluid results, that are crucial for a timely and accurate diagnosis. Criteria to diagnose MS continues to evolve to help make a diagnosis more swiftly and correctly. Specific MRI findings in the brain and spinal cord can allow for an immediate diagnosis of MS with the first attack. Brand-new updates highlight the role of the optic nerve and even offer a path to diagnosing MS in people who show no neurological symptoms. With so much at stake, a timely and accurate diagnosis of MS is more important than ever. Barry Singer MD, Director of The MS Center for Innovations in Care, interviews: Xavier Montalban MD PhD, Chair of Neurology, Director of CEMCAT (Multiple Sclerosis Center of Catalonia) at Vall d'Hebron University Hospital, Barcelona, Spain Léorah Freeman MD PhD, Assistant Professor, Dell Medical School Department of Neurology, UT Health Austin Multiple Sclerosis and Neuroimmunology Center

Maybe you've tried supplements, diet changes, exercise, and other healing protocols, but haven't made much progress toward healing. Or maybe you've made progress, but don't feel optimal. It could be that you haven't addressed past trauma, stress, or simply your mindset. On this episode of Emotional Vitality on Vitality Radio, Jared talks with Juanique Grover from Gutsy Health about this critical first step in healing the body and achieving vitality. You'll learn why nervous system dysregulation and limiting beliefs can keep your body from healing, even when you're doing “all the things”. Juanique shares her journey of healing from Graves' disease, losing her husband to cancer, and the cascade of health conditions she experienced following this traumatic time in her life. Juanique gives a beautiful explanation of how our bodies can only be healthy if we care for them and why self care is a gift to ourselves and everyone around us.
