Our last poster is titled Outcomes and Return to Sport Following Arthroscopic Bankart Repair for Anterior Shoulder Instability in Contact versus Non-contact Athletes: A Systematic Review and Meta-Analysis. We've spoken a lot about the surgical treatment for anterior shoulder instability on this podcast – most recently with Dr. Brian Lau. That is episode #48 and 49 if you want to check it out.This study focused on outcomes of arthroscopic Bankart repair for the treatment of anterior shoulder instability, specifically comparing outcomes in contact athletes versus noncontact athletes. This systematic review included 18 studies with 1-year minimum follow-up.The authors found that contact and noncontact athletes had similar rates of return to sport as well as similar rates of return to preinjury level of play. However, they also found that contact athletes demonstrated significantly greater rates of recurrent instability, at 28% compared to 8% in noncontact athletes. Contact athletes also demonstrated significantly greater need for revision surgery, at 12% compared to 3% in noncontact athletes.
Brian Lau is an Acupuncture Physician and on the faculty of the Sports Medicine Acupuncture Certification program. He has an extensive background in Qigong, Structural Integration, and many other therapeutic and movement-based therapies. His knowledge of anatomy and how it interfaces with other traditions, including Chinese medicine meridian theory and fascial systems is truly inspiring. There's so much more to learn from Brian but this was a good one!
Our next poster is titled Primary Latarjet Procedure versus Latarjet in the Setting of Previously Failed Bankart Repair: A Systematic Review. We've spoken a lot about shoulder instability on this podcast, most recently with Yoni Rosenblatt last month discussing rehab tips and tricks after various shoulder stabilization surgeries. We also had Dr. Brian Lau on Episodes 48 and 49 to chat about the different surgical options for anterior shoulder instability. So, we're excited to review this poster today and add some new literature to this discussion.
And we are back for Part II of our discussion with Dr. Brian Lau. Our conversation picks back up with a discussion of surgical treatment options for more significant glenoid bone loss. There is a 2021 yellow journal article titled Diagnosis and Management of Traumatic Anterior Shoulder Instability that nicely outlines a treatment algorithm based on percentage of glenoid bone loss as well as the presence and severity of a Hill Sachs lesion. Matt Provencher and colleagues explain that good results can be expected after Bankart repair in on-track Hill-Sachs lesions with glenoid bone loss less than 13.5%. Bankart repair without additional procedures is not recommended in off-track Hill Sachs lesions, regardless of the extent of glenoid bone loss. Bone block procedures are recommended when glenoid bone loss is greater than 20% to 25% or when the Hill Sachs lesion is off-track. Then, from the April issue of AJSM this year, we discuss the study performed by our guest Dr. Lau and his team at Duke, titled Distal Clavicle Autograft Versus Traditional and Congruent Arc Latarjet Procedures. This laboratory analysis compared five different configurations of two local autograft options – coracoid and distal clavicle – using both 3D CT and 3D MRI. They looked at how much the glenoid surface area was augmented (important to address the glenoid bone loss) and the amount of glenoid apposition provided (bone-to-bone contact being important for graft healing). The authors found that the congruent arc Latarjet procedure had the largest graft surface area, the standard Latarjet procedure had the most bone-on-bone contact and the distal clavicle attached by its inferior surface had the largest graft width. This paper also found that differences between 3D CT and 3D MRI were small and likely not clinically significant.We finish up our conversation with a focus on rehab and returning to play after shoulder stabilization surgery. The last article we reference is titled Criteria-based Return-to-Sport Testing is Associated with Lower Recurrence Rates following Arthroscopic Bankart Repair. Albert Lin and colleagues and UPMC evaluated the use of a criteria-based return-to-sport testing protocol, which includes assessments of strength and function using the closed kinetic chain upper extremity stability test and the unilateral seated shot-put test. The authors found that athletes who underwent this testing protocol to guide their clearance to return to sports had a lower rate of recurrent instability than those cleared to return based on the time from surgery (5% vs. 22%).
On today's episode we're focusing on anterior shoulder instability with Dr. Brian Lau, orthopedic sports medicine surgeon, team physician for Duke Athletics and Director of the FIFA Medical Center at Duke.We have some great articles for you that contribute well to our conversation on the treatment of primary anterior shoulder instability. We'll start off our discussion today with a level I RCT published in the March 2020 issue of JBJS titled Primary Arthroscopic Stabilization for a First-Time Anterior Dislocation of the Shoulder, a single-center double-blinded clinical trial compared arthroscopic washout to arthroscopic Bankart repair for the management of primary anterior shoulder instability. At an average follow up of 14 years, the rate of recurrent dislocation was significantly higher in the washout group compared to the Bankart repair group, at 47% compared to 12%. The arthroscopic Bankart repair group also demonstrated significantly better clinical outcome scores, including the WOSI and DASH scores. Then, from the June issue of JSES this year, we review the publication titled Remplissage reduces recurrent instability in high-risk patients with on-track Hill-Sachs lesions. Albert Lin and Pat Denard performed a multicenter retrospective study of patients with on-track Hill Sachs lesions who underwent arthroscopic Bankart repair with or without the addition of a Remplissage procedure. We'll discuss on-track versus off-track Hill Sachs lesions, how you go about calculating this and what to do with this information in a little bit with our guest. So, for now, we'll just focus on the results of this study, which showed that the addition of a remplissage was associated with a lower rate of recurrent dislocation (1.8% vs. 11%) and revision surgery (0% vs. 6%). Remplissage protected against recurrent instability, particularly in high-risk patients.We are joined today by Dr. Brian Lau, a board-certified orthopedic surgeon dual-fellowship-trained in both sports medicine surgery and foot & ankle surgery. Dr. Lau obtained his medical degree from the University of Pittsburgh and completed his orthopedic residency at UC San Francisco. He then went on to complete two fellowships – the first in Sports Medicine and Shoulder Surgery at Duke University and the second in Foot & Ankle Surgery at Stanford University. Following training, Dr. Lau returned to Duke University, where he is a team physician for Duke Athletics and the Director of the FIFA Medical Center at Duke. Dr. Lau is the associate program director of the Duke Orthopedic Sports Medicine and Shoulder fellowship and serves on numerous educational committees in AOSSM, AANA and ASES. He is passionate about research and leads the Duke Sports Medicine Research Committee.
We welcome back Foot & Ankle Specialist and Residency Director of Loyola University Medical Center, Dr. Adam Schiff, MD to discuss surgical versus nonsurgical management of Achilles tendon ruptures. We review the study “Operative versus nonoperative treatment of acute Achilles tendon rupture: a propensity score-matched analysis of a large national dataset“ published in OJSM in February coming to us from Duke University. In the study, Dr. Brian Lau and his team found an increased risk of complications with surgical versus nonsurgical care, no difference in reoperation rates, and increased costs with surgical care initially but an equalization of costs at 5 years post-injury. The authors also report that over the study period from 2007-2015, there was little change in the rate of surgical repair, despite the widely published success of modern nonsurgical care. Dr. Schiff lends his expertise on this topic, lays out the pros and cons of each approach, the historical perspective, what situations might be best suited for each treatment pathway, and the psychology he sees in his patients as they decide whether to have surgical repair of their Achilles tendon rupture.

Il poursuit désormais sa formation dans la recherche en tant que doctorant au sein de l'équipe Neurochirurgie expérimentale de l’Institut du Cerveau et de la Moelle épinière, dirigée par Carine Karachi et Brian Lau. 4:30 Si les neurosciences, depuis les années 60, définissent simplement quelque chose de créatif comme étant utile + approprié (adapté au contexte), il est admis qu'il n'y a pas de création ex-nihilo mais plutôt une habileté plus ou moins forte de chacun à pouvoir faire des liens entre concepts notions, émotions prééxistants. Cette habilité à faire des liens se fait selon des processus contrôlés (de proche en proche - cf test des fluences verbales) ou des processus associatifs plus spontanés (idées originales, satisfaisantes, avec "effet de récompense"). 10:13 on parcourt avec David les différentes phases du processus créatif (qui ne s'enchaînent pas forcément toujours dans le même ordre) >> L'optimisation de la recherche qui est un état des lieux des données à disposition >> L'incubation dans la quelle il semble ne rien se passer mais pendant laquelle le processus se poursuit inconsciemment dans le cerveau >> La résolution - le moment où émerge une solution satisfaisante >> La vérification de la solution 12:39 On évoque plus en profondeur cette mystérieuse phase d'incubation. David nous explique qu'il existe un système qui est tout le temps actif dans le cerveau (et visible à l'IRM) et qu'on appelle le réseau par défaut. Et ce réseau est en lien avec les processus de rêverie (ou vagabondage mental / "mind wandering") et les processus associatifs inconscients. Et ce que David souligne c'est que nous ne sommes pas tous égaux vis-à-vis de la rêverie et que les parts de l'inné et de l'acquis dans cette aptitude sont difficiles à définir. 16:50 On se penche sur l'évolution de la créativité de l'enfance à l'âge adulte. A la fin de la vingtaine, notre cerveau fait une sorte de ménage dans ses connexions pour cristalliser nos notre façon de penser et d'associer les idées. Cet élagage a lieu dans le lobe frontal qui est le siège de nos fonctions exécutives. C'est le lobe frontal qui a la lourde responsabilité de nous faire faire les choix les plus appropriés dans notre vie de tous les jours, et d'inhiber les réponses préétablies pour nous permettre de nous adapter. Cet élagage indispensable à la structuration de la connaissance nous fait cependant perdre un peu de notre capacité à faire des associations lointaines. Et on le constate : Les plus experts d'un domaine ne sont pas forcément les plus créatifs dans ce domaine là car pour eux les choses sont simples, elles sont comme ça et leur capacité à faire des associations avec d'autres domaines est réduite. Au sein du lobe frontal il y a cependant un réseau de plusieurs régions (notamment une zone antérieure dans le lobe frontal) qui nous aide à rouvrir la porte à nos rêveries pour faire émerger des solutions innovantes. C'est Emmanuelle Volle qui avait entrepris, par méta-analyses, via l'étude de cas de lésions cérébrales, de cartographier les zones cérébrales impliquées ensemble en réseau dans les processus de créativité : "Un hub, comme une grand aéroport" 26:00 David nous décrit une autre étude réalisée sur les processus d'association d'idées, par évaluation de la "distance sémantique" entre des concepts. Les participants identifiés comme les plus créatifs par l'étude étaient ceux capable de trouver les liens entre les mots à première vue éloignés. Et dans cette étude, la zone du cerveau qui s'active est bien la même que celle identifiée par les méta-analyses. 29:36 Le "flow". Il y a une récompense cognitive via un bonus de dopamine à la recherche d'information. Et il est possible qu'il y ait aussi une récompense similaire dans le processus de création et d'innovation. 33:08 On a ensuite questionné David sur la créativité dans son travail de neurologue et de chercheur. Si les études de médecine en général n'invitent pas à la créativité mais plutôt à l'application de méthodes strictes il cite en revanche de nouvelles techniques d'apprentissage plus créatives. Il cite en exemple la sémiologie neurologique dont l'apprentissage peut être facilité par du mime (technique développée par Emmanuel et Constance Flamand-Roze). Dans l'exercice de son métier au quotidien la créativité est sollicitée cependant face à des diagnostics difficiles pour aller chercher une solution, et aussi lorsqu'il n'y a pas de médicament à proposer, pour trouver des moyens d'aider les patients. Il cite en exemple un atelier de "danse therapy" pour des patients parkinsoniens (qui subissent une baisse de dopamine justement) pour qui la stimulation de la créativité via la danse a eu un effet réellement épanouissant. 40:40 Pour cette seconde partie rituelle à la découverte des étoiles de notre invité, David joue avec nous le jeu du voyage dans l'infiniment grand et l'infiniment petit. Ensemble nous avons discuté de structures grandioses et de connexions entre les cerveaux, mais aussi d'échangeurs neuronaux, synapses tricolores et vaisseaux nanométriques comme ceux du dessin animé La Vie que David aime beaucoup. Si on lui parle d'une grève des neurones David répond "on est plus biologique, on redevient minéral". Et pour le feu d'artifice physiologique de l'"eureka", un bouquet final à l'unisson qui donnerait la solution, comme la dernière note d'une symphonie. 49:40 On a redemandé des exemples de patients dont une lésion cérébrale avait modifié la créativité (autre que celle de la légende urbaine du réveil de coma en parlant une autre langue!) et il nous a donné celui de Maurice Ravel et sa composition du Boléro concomitante de son atteinte au lobe temporal, qui provoque justement une tendance à la répétition. D'autres patients avec des lésions au lobe frontal ont semblé devenir plus créatifs en peinture mais aussi avec des formes très répétitives. Des premières audacieuses trépanations du néolithique jusqu'aux méta-analyses de données sur patients lésés la découverte du cerveau a parcouru un long chemin. Les zones où siègent les processus de la créativité se précisent mais il reste encore beaucoup à explorer pour comprendre comment ils fonctionnent en réseau. On a réalisé avec David que la structuration de notre connaissance s'était fait au détriment d'une partie de la créativité de notre enfance. Heureusement la créativité peut être favorisée par le vagabondage mental propice à l'incubation des idées. Pour David c'est le calme de la nuit qui est propice, probablement car la fatigue provoque un relâchement de certains processus de contrôle et ainsi favoriser la rêverie. The textbooks tell us that sinew channels are important in the treatment of pain and musculoskeletal issues and they are accessed through the jing-well points. But beyond that not much else is said. In this conversation we investigate the channel sinews from both the Chinese medicine and western functional anatomy and physiological perspectives. This gives us a more nuanced look into how structure influences function and it further helps to illuminate channel theory and its profound impact on both organ function and channel based issues.
