ACL surgery has come a long way since the early days of acute open repair to modern-day ACL reconstruction. Still, in spite of our advancements in knowledge, technique, and technology, graft rupture as well as contralateral ACL injury remain a concern for patients and their Sports Medicine providers. Well-renowned for her body of work in ACL research, Dr. Kate Webster, PhD from La Trobe University joins us to discuss her team's recent study on the long-term survival of ACL autografts and contralateral ACL ruptures over a 10-year follow up period. These ACL re-injury rates illustrate that, while we have made great strides over the decades, there is still room for improvement.

Toxic ruptures don't just happen in a single moment of conflict—they are built over time through miscommunication, emotional wounds, and the silent grip of shame. Whether it's an argument that lingers long after words have been spoken, a past hurt that keeps replaying in our minds, or a pattern of avoidance that slowly erodes trust, these ruptures are reinforced by both our interactions and our inner world. In this episode, we dive deep into how toxic ruptures are created, not just between people, but within our own brains. We'll explore the neuroscience behind why conflict feels overwhelming, how our nervous system's response can make repair more difficult, and why shame keeps us stuck in cycles of blame, avoidance, or emotional withdrawal. You'll learn how unprocessed shame distorts our perception of others, fuels resentment, and makes us more likely to repeat the same destructive patterns in relationships. Most importantly, we'll uncover practical strategies to stop toxic ruptures before they spiral out of control. From rewiring our emotional responses and breaking free from rumination to approaching difficult conversations with presence and repair, this episode will help you shift from unconscious creation to intentional healing. If you've ever felt trapped in recurring conflict or emotional disconnection, this is your guide to understanding the deeper layers of toxic ruptures—and finally learning how to heal them. * * * Episode Links and References Psalm 41:9 The Great Divorce by C.S. Saying "no" can feel uncomfortable, but it is a crucial part of growth, both for ourselves and in our relationships. In this episode, we explore the concept of limit-setting ruptures—those moments where boundaries create tension, discomfort, and even distance, but ultimately lead to maturity and deeper connection. Drawing from psychology, neuroscience, and biblical narratives, we discuss how setting limits helps shape our character, why it's necessary for personal and spiritual development, and how it teaches us emotional regulation. Whether you're a parent setting boundaries for your child, someone navigating addiction recovery, or simply trying to manage your daily commitments, this episode will provide insights into how embracing limits can lead to greater freedom and fulfillment. We discuss how the brain undergoes "pruning" to refine skills, how children develop emotional regulation through parental boundaries, and how even Jesus modeled the power of necessary separations. We'll also tackle the emotional impact of limit-setting, from the pain of telling a parent they can no longer drive to the discomfort of quitting unhealthy behaviors. If you want to learn how to set limits in a way that fosters growth, self-control, and deeper relationships, this episode is for you. Episode Links and References Genesis (Adam and Eve) – The concept of rupture and repair is tied to Adam and Eve in the Garden of Eden, their immaturity, the test they faced, and the consequences of their choices (Genesis 2–3). Cain and Abel (Genesis 4:6-7) – God telling Cain, “Sin is crouching at your door; it desires to have you, but you must rule over it.” This is presented as a test and a moment of rupture. Numbers 34 – God defining the boundaries of Israel, in contrast to empires that constantly expand their borders. John 16 – Jesus telling His disciples, "I must go," explaining that His departure is necessary for the arrival of the Holy Spirit. Interpersonal neurobiology – While not a scripture, this scientific concept is tied to biblical themes of growth, pruning, and discipline (which could be linked to passages like John 15:2, “Every branch that does bear fruit he prunes so that it will be even more fruitful”). . . . . . Growth is often born from disruption, and in this episode, we explore how ruptures—whether in relationships, routines, or neural pathways—create opportunities for expansion and integration. We examine the physiological parallels of growth, from bone density to neuroplasticity, and dive deep into the critical role of repair in relationships. Drawing on longitudinal studies of shy children and the biblical perspective on stress, we illustrate how intentional engagement and attunement pave the way for personal and communal transformation. Curt and Pepper offer person examples to illustrate how the stress of transition, from toddler tantrums to adolescent resistance, can be a gateway to deeper resilience and connection. Trauma is more than a single event—it is the accumulation of unrepaired ruptures that we carry in our bodies and souls. In this episode, we explore how these ruptures—whether from minor conflicts or deeply wounding experiences—shape our lives and relationships. Drawing from biblical stories and contemporary examples, we discuss the ways our unresolved pain influences our decisions, interactions, and sense of self. From the silent grief of barrenness in the lives of the matriarchs to modern challenges like road rage and smartphone dependency, we examine the pervasive impact of ruptures. With thoughtful insights, we point to the hope of repair and the courage it takes to face the cost of healing. This week, Curt and Pepper invite you to reflect on your own journey and consider how God's redemptive work transforms even the most broken parts of our stories. . . . . . Questions answered this episode: 1. Did John Paul II identify the 4 ruptures? 2. My husband and I have been married for over 35 years. We've been through very difficult challenges in our married life and my body is not capable of intercourse. It's been 5 years since our last union. Why do I feel shame about myself and my inability to give what I don't have? What does the Bible mean with the words “give himself up for her” from Ephesians 5? 3. I'm a 27 yo man who has never been in a romantic relationship. What bothers me is that I've never had a desire to be in a relationship. Seems like this lack of desire for a relationship partly stems from selfishness. How can I work towards becoming less selfish and truly seek the good that can be found in union with another person? Ask Christopher West is a weekly podcast in which Theology of the Body Institute President Christopher West and his beloved wife Wendy share their humor and wisdom, answering questions about marriage, relationships, life, and the Catholic faith, all in light of John Paul II's beautiful teachings on the Theology of the Body.

I also answer the question, "How can you tell if it's the person with BPD or yourself as a Codependent?" In other words, is it my BPD partner and his or her BPD or is it me, the Codependent? Why do so many with Codependency believe that they are to blame for BPD relationship ruptures and BPD relationship impossibility? https://ajmahari.ca/sessions
Probably the most inspired and provocative conversation I've ever had on the nature of time and human creativity.Joining me for this trialogue are Eric Wargo, author of From Nowhere: Artists, Writers, and the Precognitive Imagination (previously on FF episodes 117 and 171), and J.F. Martel, author of Reclaiming Art in The Age of Artifice and co-host (with Phil Ford) of Weird Studies podcast (previously on FF episodes 18, 71, 126, and 214). Our discussion centers on the concept of precognition — the ability to perceive future events — as the mechanism of all human creative activity. Both Eric and J.F. argue that art, like shamanistic practices, acts as a means of accessing and expressing precognitive experiences, often manifesting as seemingly coincidental events or uncanny correspondences between art and reality. We talk about the role of trauma and dissociation in stimulating creative breakthroughs — why there seems to be a direct biological and psychological link between suffering, displacement, and the discovery of radical new insights and modes of being. Can we create without destroying, or are rupture and connection one thing? We also examine how emerging media through the ages have shaped our experience of time. Starting with the earliest Paleolithic artifacts and the role of cave art in facilitating or encoding ecstatic experience, we trace the evolution of art through to how the development “the cut” in modern cinema led to new ideas of causality. Each new medium provides novel ways of thinking about leaps across space and time, and their study offers new points of entry into a unifying philosophy of rupture and discontinuity.Lastly, we explore some of my own most potent and disquieting precognitive experiences in light of Eric's argument that the UFO phenomenon may actually be the braided precognitive experiences of future human beings and symbiotic artificial intelligences — a thesis that sheds new light on everything from the lives and work of Philip K. Dick, Jacques Vallée, Carl Jung, Andrei Tarkovsky, to The Book of Ezekiel.Where we're going, we won't need roads…Speaking of art, UFOs, psychedelic experience, and time machines, here's the standalone music video for the song we discuss in this episode that was inspired by my UFO (or were they time machine) experiences in 2007. I threw it back in as a coda to the episode but in case you want to view it in its original resolution and in the context of the entire album, here you go. What to do when supervision goes bad? A guide to supervision ruptures and repair Curt and Katie chat about what to do when there is a loss of trust in supervision. We explore what has already been researched (and the limitations there). We also look at what can make ruptures more likely, different types of ruptures, and how supervisors can mitigate the risk of rupture and then repair when ruptures inevitably happen. This is a continuing education podcourse. Transcripts for this episode will be available at mtsgpodcast.com! In this podcast episode we talk about how to address ruptures in clinical supervision Clinical supervision has the potential for errors and requires management from the supervisor to acknowledge, deal with, and repair from mistakes that could rupture the supervisory alliance. This workshop helps to identify the types of ruptures that can occur and offers a strategy on how to deal with them so that they are no longer a problem. What is the focus within the research base on clinical supervision? · Most of the research on supervision is focused on clinical supervision without a consideration of the employment element of the relationship · There is not a lot of research on supervision, much less ruptures in supervision · Much of the research has been from the supervisee perspective, but that is shifting · Without supervisors being involved in research, there is a risk of supervisors dismissing it What can make ruptures more likely in supervisory relationships? · Power differential within the clinical supervision relationship may lead to supervisees feeling unable to talk about mistakes that supervisors make · Expectations of safety and confidentiality that are not discussed or are assumed to be greater than they are, with a lack of transparency · Supervisors are responsible to address ruptures, but may not know about them · Supervisors can fall into risk management and punishment or not allowing supervisees to do anything that may challenge their skill level What can supervisors do to avoid supervisory ruptures? · Acknowledge the fallibility of both the supervisor and the supervisee · Supervisors share when they don't know something and learn what they need to learn to support their supervisee · Understanding the impact of each perspective on expectations (i.e., supervisors in agencies are also impacted by the expectations that they have on them from their own supervisors) · Identify confrontation versus avoidance rupture risks · Being more transparent on the process of supervision and about the supervisor's competence and emotional responses · Understanding and assessing for cultural ruptures, microaggressions, etc. Can supervisory ruptures also happen based on actions of the supervisee? · Yes! · Supervisees can misunderstand their role and not complete what is expected of them · Supervisees can fail to develop as a clinician · Supervisees may ask peers rather than their supervisor for what to do and do the wrong things How can supervisors repair the ruptures that happen in clinical supervision? · Setting the stage for the most positive supervisory experience · Cultural humility · Pause, Consider, Reflect, Act (C.E. Watkins, Jr.'s work) Receive Continuing Education for this Episode of the Modern Therapist's Survival Guide Continuing Education Information including grievance and refund policies.
