In this episode, Gregory J. Tiesi, MD, FACS, FSSO, hosted a discussion about the use of hepatic artery infusion (HAI) in colon cancer and liver cancer management. Dr Tiesi is the medical director of Hepatobiliary Surgery at the Hackensack Meridian Jersey Shore University Medical Center in Toms River and Brick, New Jersey. He was joined by: Anthony Scholer, MD, FACS, FSSO, a surgical oncologist specializing in hepatobiliary surgery, at Hackensack Meridian Medical Group and Jersey Shore University Medical Center in Neptune, New Jersey Benjamin Jon Golas, MD, FACS, regional chief of Surgical Oncology for Hackensack Meridian Health's Central Region, surgical director of Oncology Services at Jersey Shore University Medical Center, vice chair of Surgery at Jersey Shore University Medical Center Cancer Surgery, and an associate professor of surgery at the Hackensack Meridian School of Medicine in Neptune and Edison, New Jersey Eric Pletcher, MD, a surgeon specializing in Complex General Surgical Oncology at Hackensack Meridian JFK University Medical Center in Edison Drs Tiesi, Scholer, Golas, and Pletcher explained that HAI is a longstanding regional therapy used for treating primary and metastatic tumors to the liver, notably unresectable colorectal liver metastases and intrahepatic cholangiocarcinoma. The physiologic mechanism of this treatment leverages the dual blood supply of the liver, capitalizing on the fact that these malignancies primarily derive their perfusion from the hepatic artery, the experts noted. They emphasized that by delivering chemotherapeutic agents, such as floxuridine, directly via the gastroduodenal artery, HAI concentrates drug exposure at the tumor site, maximizing antitumor effect and minimizing extrahepatic toxicity. They explained that patient selection requires fitness for surgery and good liver function, excluding those with cirrhosis or portal hypertension. They also noted that the procedure involves implanting a subcutaneous pump, followed by rigorous intraoperative and postoperative nuclear medicine studies to confirm the absence of extrahepatic perfusion. Evidence supports that HAI combined with systemic therapy achieves higher intrahepatic objective responses, improves local disease control, and enhances conversion to resectability, correlating with improved long-term survival, the experts reported. However, potential complications include pump pocket infections, biliary sclerosis, and gastric ulcers, they added. The experts concluded by highlighting that establishing an HAI program necessitates a robust, multidisciplinary approach involving surgical oncology, medical oncology, and interventional radiology.
BUFFALO, NY — December 8, 2025 — A new #research paper was #published in Volume 17, Issue 11 of Aging-US on September 12, 2025, titled “Infusion of blood from young and old mice modulates amyloid pathology.” This study was led by co-first authors Matias Pizarro from Universidad Adolfo Ibáñez and Ruben Gomez-Gutierrez from The University of Texas Health Science Center at Houston, alongside corresponding authors Claudia Duran-Aniotz from Universidad Adolfo Ibáñez and Rodrigo Morales from The University of Texas Health Science Center at Houston and Universidad Bernardo O'Higgins. The goal was to investigate how blood from young and old mice influences Alzheimer's-related changes in a transgenic mouse model. The findings indicate that age-dependent circulating factors can either worsen or mitigate brain changes associated with dementia, highlighting blood and its components as potential therapeutic targets. Alzheimer's disease is a progressive neurodegenerative disorder characterized by misfolded amyloid proteins, inflammation, and gradual cognitive decline, with aging as its main risk factor. In this work, whole blood from young adult or very old wild-type mice was repeatedly infused into Tg2576 mice, a well-established model of amyloid accumulation and memory impairment. Over several months, recipient mice received 30 weekly blood infusions, followed by behavioral testing and detailed neuropathological analyses. “Tg2576 mice express the human APP harboring the Swedish mutation.” Mice that received blood from old donors performed worse in both short- and long-term spatial memory tasks than mice infused with young blood, suggesting that aged blood contains factors that impair cognition. When the team examined brain tissue, they found more cortical amyloid deposits detected by a specific antibody in mice treated with old blood, while overall amyloid levels measured biochemically did not change, suggesting differences in plaque type or compactness rather than total amount. The expression of amyloid precursor protein in the brain was also higher after old-blood infusion, which may partly explain the shift in amyloid pathology. Despite these changes in plaques and memory, classical markers of astrocyte activation, a sign of brain inflammation, did not differ between groups, pointing to more subtle molecular shifts. A broad proteomic analysis of brain samples revealed dysregulation of proteins involved in synapse formation, calcium signaling, and the endocannabinoid system, pathways important for neuronal communication and plasticity. Among them, the calcium channel–related protein CACNA2D2 and the signaling protein BRAF were increased in mice that received old blood, confirming that aged blood circulation can reshape key signaling networks linked to neuronal function and degeneration. Overall, this study supports the idea that blood is not just a passive carrier but a powerful modulator of brain health during aging and disease. While young blood has been associated in previous work with improved synaptic function and reduced amyloid and tau changes, this study emphasizes the harmful impact of old blood, particularly on cortical amyloid patterns and memory. The identification of CACNA2D2 and BRAF as potential mediators of these effects suggests new avenues for targeting blood-borne factors or downstream brain pathways to slow or modify Alzheimer's-related decline. Was sind deine Stärken? Für was bist du hier? Was du willst. After more than 25 years of treatment at the same small-town hospital, Heather shares how changes in systems and processes have stripped away what once made her infusion experience bearable—the human connection. In this candid conversation, Heather and Larry unpack how rigid procedures, check-ins, and hoops designed to satisfy administrators can leave patients feeling like numbers instead of people. They talk about:The difference compassionate nurses and personal relationships make in patient care. How “systems” often prioritize efficiency and liability over humanity. Why being seen and treated as a person—not just a patient—matters so much.Practical advice on advocating for yourself when the system feels stacked against you.Whether you're a patient, caregiver, or healthcare professional, this episode is a reminder that true healing goes beyond medicine—it's about dignity, respect, and human connection.

Diabetic ketoacidosis can be life-threatening but there's some variability in the way it's managed between health settings. Intervention involves intravenous insulin administration, hydration, electrolyte replacement and treatment of the underlying precipitant. In a survey of practitioners from 31 different hospitals in Australia there was an even split between those organisations which followed a fixed rate insulin infusion protocol, usually based on bodyweight, or a variable rate infusion protocol, titrated against blood glucose concentration. Three quarters of survey respondents had worked at another hospital that had different DKA management protocols raising concerns about the cognitive load on junior health staff moving between institutions. In Europe there has been some normalisation towards fixed rate protocols, despite there being no good quality evidence for superiority.

We've all heard stories about someone who went in for surgery and came out...different. A grandmother who struggled with names after hip replacement, or an uncle who seemed foggy for months following cardiac bypass. But why does this happen to some people while others bounce right back? This week, we explore this question with Dr. Martin Angst, a professor of anesthesiology at Stanford who's studying the biological factors that determine cognitive outcomes after surgery. With support from the Knight Initiative for Brain Resilience, Martin and his team are following hundreds of cardiac surgery patients, tracking everything from blood biomarkers to cognitive performance both before and after their procedures.Their findings are revealing fascinating insights about what makes some brains more resilient than others when faced with the significant stress of major surgery - insights that could help physicians better advise patients and potentially lead to interventions that enhance resilience.Read MoreUnder the Lights: What Surgery Reveals About Brain Resilience (Knight Initiative for Brain Resilience, 2025)Infusion of young donor plasma components in older patients modifies the immune and inflammatory response to surgical tissue injury: a randomized clinical trial (Journal of Translational Medicine, 2025)Blood test predicts recovery after hip-replacement surgery, study finds (Stanford Medicine, 2021)Can major surgery increase risk for Alzheimer's disease? I'm Stacey Simms and every other Friday I bring you a short episode with the top diabetes stories and headlines happening now. XX Tandem gets FDA clearance for a new infusion set. The company shared in an SEC filing that its Capillary Biomedical (CapBio) subsidiary won clearance for its SteadiSet Infusion Set. SteadiSet features an integrated inserter with a hidden needle designed for one-handed insertion. It received clearance to deliver insulin for up to three days of use. However, Tandem plans to submit a separate request to extend the indicated use time to up to seven days. It doesn't expect to begin commercial activities for SteadiSet until after it receives clearance for the extended use time. Many of us have been following this since Tandem acquired CapBio back in 2022.. it's been pretty hush hush – CapBio has never accepted my requests for interviews, so I'm excited to see this approval, mostly because my son really really needs it! https://www.drugdeliverybusiness.com/tandem-capbio-fda-clearance-insulin-infusion-set/ XX Insulet just announced via an email to health care providers that Dexcom G7 compatibility with the Omnipod 5 iPhone app will be released IN THE COMING WEEKS! Great news for people with #t1d and XX Sernova announces positive interim data from its ongoing Phase 1/2 clinical trial in patients with T1D. this is their Cell Pouch Bio-hybrid Organ as a functional cure for type 1 diabetes (T1D), today Interim data from 12 patients with transplanted human donor islet cells show patients achieving insulin independence, islet cell engraftment in Cell Pouch, islet function, islet survival, improved glycemic control, improved patient reported quality of life (QOL) and improved awareness of hypoglycemia and increased sensitivity to severe hypoglycemic symptoms. Improvement in patient outcomes was correlated with a cumulative increase in the quantity of transplanted islets. Based on these findings, the study is on track to meet its primary and secondary endpoints, and the confirmatory Cohort C is expected to initiate in H2 2025. Clinical trials with iPSC islet-like clusters from Sernova's partner, Evotec, are anticipated to begin in 2026 after completion of Cohort C of the ongoing trial. These interim findings, with 8 of 12 patients achieving insulin independence so far, support the thesis that Sernova's high volume ten channel Cell Pouch, used in Cohort B, plus an optimised immune suppression regimen, has the potential to achieve insulin independence, without portal vein transplant, in our planned clinical trial with Evotec's high quality iPSC islet-like clusters. Measured HbA1c (a blood test that shows the average blood sugar levels over the past 2-3 months) in patients with Cell Pouch alone, showed that 9 of 12 patients had a reduced value within the American Diabetes Association (ADA) recommended range of
