Why Isn't Your Herniated Disc Healing? (And How to Actually Rebuild It)

Back In Shape

Your herniated disc hasn't healed because you've been treating the stiffness, not the injury — and a herniated disc is a load-bearing injury. If you've got a herniated disc (a slipped disc, disc bulge or prolapsed disc) at L4/L5 or L5/S1 and it still hasn't settled, this session explains why — and how it actually heals. A herniated disc is a loading failure: an injury to the annulus fibrosus, the tough outer ring of the disc. It does heal, automatically, the same way a graze does — but only if you stop aggravating it and start rebuilding its capacity to bear load.

Stop Stretching Your Spinal Stenosis: Why Knee-Hugs Make L4-L5 Pain Worse

Back In Shape

If you have recently received an MRI report citing lumbar spinal stenosis, facet joint hypertrophy, or spondylolisthesis, it is entirely normal to feel alarmed. You may have been told that the only way to manage your lower back pain or sciatica is through endless stretching, knee-to-chest movements, and forward bending to 'open up the spine'. However, this common advice often traps individuals in a cycle of persistent aggravation. In this video, we dismantle the 'stenosis trap' and explain why stenosis is merely a description of your spinal architecture, not the acute injury itself. We explore the crucial 'timeline paradox': degenerative changes like bone spurs and thickened ligaments take decades to form, yet your severe pain may have started only weeks or months ago. This highlights that the structural narrowing is simply the environment, whilst your pain is the result of a more recent mechanical injury or herniated disc struggling to heal within that confined space. By forcing a degenerated segment to bend and twist, you are not 'freeing up' the nerve; rather, you are repeatedly irritating the area, creating inflammation, and forcing the spinal segments above it to become hypermobile. True rehabilitation for a stenotic spine does not involve trying to stretch the hole bigger. Instead, the focus must shift to stabilising the affected segment in a neutral spine to stop the daily inflammatory cycle. We walk you through how to rebuild strength in your hips and lower limbs—utilising proper squats and hip hinges—so they can absorb the loads of daily life, sparing your lower back. Coupled with targeted contrast bathing to physically flush out congestion, you can successfully navigate your recovery and build long-term resilience without making the structural picture worse.Key Topics Covered

Another week, another episode where we somehow go from broccoli discourse to self-driving cars to limb regeneration technology and then cap it all off with rogue timestreams on a college campus. Just a normal day for The Science Faction Podcast. Real Life Ben opens the show with an important culinary clarification: broccoli is the green one. Not the other green one. Also maybe "broccolini" exists? Science remains divided. Meanwhile, Ben's household has become a temporary kitten sanctuary. Tiny baby cats are everywhere, and while Ben is trying his best, he freely admits his wife appears to be significantly more qualified in the art of keeping tiny creatures alive. On top of that, his son has started developing an actual social life, which Ben correctly identifies as a direct threat to traditional family hanging-out time. The family also continues debating the orbital mechanics of For All Mankind, with Ben's 12-year-old officially unconvinced by the show's space logistics. Devon reports back from a Dallas anniversary trip with his wife celebrating ten years of marriage. The trip included visits to the Perot Natural History Museum, multiple Waymo sightings, an improv show with front-row seats, and a self-driving Uber ride that still included a human technician nervously supervising the robot future. Steven survived a busy week while his wife was out of town and also got some bonus hangout time with Devon during the visit. Naturally, this somehow led to new miniatures for Cyberpunk Red: Combat Zone entering the house. The crew also stumbles into Texas voter registration statistics, discovering that as of August 2025 there were reportedly more registered Democrats than Republicans in Texas, which sparks discussion about perception versus raw registration numbers. According to reporting from Independent Voter News, Democrats accounted for approximately 46.52% of registered voters compared to 37.75% registered Republicans. Future or Now (~10 min ea) Devon brings in one of the wildest science stories of the week: researchers may have identified a key genetic pathway involved in limb regeneration. Scientists studying axolotls, zebrafish, and mice uncovered a family of "SP genes" connected to regeneration. By disabling these genes, proper bone regrowth stopped entirely. Researchers then used zebrafish-inspired gene therapy techniques to partially restore regeneration in mice. The long-term dream? Moving beyond prosthetics and eventually regrowing living tissue and limbs in humans. Tiny salamanders may once again be carrying the future of medicine on their weird smiling backs. Read more from ScienceDaily. Ben follows that up with a double nostalgia feature. First up is The Thirteenth Floor, the underrated 1999 sci-fi film that had the misfortune of arriving alongside The Matrix. Decades later, removed from direct comparisons, Ben argues the movie absolutely holds up and deserves a second look. Then comes a glowing recommendation for Mixtape, a coming-of-age game centered around three teenage friends spending one final night together before life changes forever. Ben describes it as emotionally sincere, genuinely hilarious, visually stunning, and powered by an incredible soundtrack. The animation style apparently evokes Spider-Man: Into the Spider-Verse energy, while the tone lands somewhere between Dazed and Confused, The Perks of Being a Wallflower, and High Fidelity. Ben strongly recommends it even for non-gamers, suggesting that simply watching a playthrough could still deliver a great experience. Check it out at Mixtape Official Site. Steven unfortunately runs out of time this week, proving once again that reality remains the greatest enemy of podcast scheduling. Book Club  Next Week's Story Next week the crew will be reading: Narcissus Meets the Ghost of AI in a Dark Alley Behind a Fusion Restaurant by Lesley Hart Gunn "I suppose you want my wallet. No? My body then." This Week's Story This week's discussion focused on: Update on Rules for the Spatiotemporal Use of Campus Spaces by Andrea Kriz The story presents a university campus slowly unraveling under the pressure of a rogue timestream, delivered through increasingly absurd administrative announcements and policy updates. "Dear Members of the Community, As we begin yet another fall semester in the throes of the rogue timestream unleashed on our campus…" The crew spends a lot of time trying to piece together exactly what catastrophic event caused the university to devolve into bureaucratic temporal chaos. Everyone agreed the story was fantastic, weird in exactly the right ways, and surprisingly effective at balancing humor with unsettling implications. Ever feel like your legs get tired way too quickly when you walk—even though you're otherwise healthy? In this episode, we break down one of the most overlooked causes of leg fatigue: spinal stenosis—a condition where the space around the nerves in your lower spine becomes narrowed. Many people assume it's a muscle or circulation issue, but in many cases, it's actually a nerve compression problem. We explain why symptoms like heaviness, burning, or weakness in the legs often show up with walking, why leaning forward or sitting brings relief, and how structural changes in the spine—including conditions like Spondylolisthesis—can reduce the space available for your nerves. We also talk about what can be done. While spinal stenosis isn't always something that can be completely reversed, the right approach—combining adjustments, traction, postural correction, and exercises—can help reduce pressure, improve function, and keep you active. If your legs don't feel the way they used to when you walk, this episode will help you understand why—and what you can do about it.

Bed bound After A Minor L5 S1 Disc Bulge: Why?!

It is a common and incredibly frustrating scenario: a relatively minor L5-S1 herniated disc spirals into years of debilitating lower back pain. In this session, we explore exactly how this happens by looking at Maya's story. After receiving the standard, well-meaning advice to stop all resistance training and focus purely on flexibility through yoga and Pilates, she systematically lost the muscle mass required to support her spine. Years of avoiding load and relying on deep spinal flexion stretches completely eroded her resilience, ultimately leaving her bedbound from an injury that should have been highly manageable. To understand the full picture, we dive deep into the clinical reality of spinal alignment by reviewing Maya's MRI and X-rays. Despite being repeatedly told by practitioners that she had an anterior pelvic tilt and excessive lumbar lordosis, the objective imaging proves her alignment is actually perfectly normal. This highlights the severe inaccuracies of basic physical examinations and why you should be cautious of practitioners who blame back pain on unverified postural flaws. Ultimately, a back problem requires an active solution. We discuss why transitioning away from passive treatments and focusing on a structured program of neutral-spine stability and progressive resistance training is the only dependable route to long-term recovery.Key Topics Covered

Why Your Back Rehab is Failing: The "Vespa vs. Dodge Ram" Analogy

In this session, we dive deep into the mechanics of how specific exercises actually facilitate the healing of a herniated disc and relieve chronic sciatica. Many people are led to believe that a therapist "fixes" them, but the reality is that your body is constantly trying to heal itself every single day. The role of a structured rehabilitation programme is to provide the optimal environment for that healing to occur. We discuss the critical distinction between "relief-based" movements—which often involve bending and twisting that provide momentary comfort but can aggravate the underlying injury—and "stability-based" exercises that protect the lumbar spine and allow the damaged tissues to recover.Understanding your "load tolerance" is the key to long-term recovery. We use the analogy of a 50cc Vespa trying to pull a one-ton trailer to describe a weak, injured back struggling with the demands of daily life. To stop the "engine" from screaming—or your back from flaring up—you must upgrade your vehicle to a Dodge Ram or a heavy-duty truck. This means committing to a progressive resistance training programme that builds bone mineral density, muscle coordination, and spinal resilience. By mastering the technique of the squat and the hip hinge, you aren't just doing "gym moves"; you are learning life skills that allow you to navigate the world without constantly re-injuring your spine.Key Topics Covered

Spondylolisthesis & Lower Back Pain: X-Ray Analysis and Strengthening

In this live session, we conduct a detailed X-ray analysis for a member named Desmond to explore the mechanical realities of spondylolisthesis and lower back injuries. A common misconception in clinical settings is the idea that a practitioner can simply "put a joint back in place." We dismantle this myth, explaining why structural shifts like spondylolisthesis cannot be manually reversed and why focusing on "alignment" without objective measurements can be misleading. Instead of chasing a temporary fix, we focus on the importance of stabilising the lumbosacral junction (L5/S1) and building a "database of reasons" why your back is resilient rather than fragile. We also address the psychological hurdles of recovery, specifically the anxiety and fear-avoidance behaviours that often follow a herniated disc diagnosis or a microdiscectomy. By shifting the focus from how you "feel" to how you "perform" in foundational movements like the squat and hip hinge, you can objectively measure your progress. Whether you are returning to a demanding job like bricklaying or managing the daily load of a young family, the goal of a professional rehabilitation programme is to ensure your body is conditioned for more strain than your daily life requires. We discuss why "not lifting weights" is often lazy advice, as every daily action—from putting on a coat to picking up a child—is a form of loading the spine that requires preparation and strength.Check out the article: https://backinshapeprogram.com/2026/02/stop-trying-to-pop-your-back-a-strategic-guide-to-spondylolisthesis-and-spinal-stability/Key Topics Covered

Is It Safe to Squat with a Herniated Disc?

Back In Shape

Play Episode Listen Later Jan 20, 2026 71:04

One of the most common fears for those suffering from lower back pain, sciatica, or a herniated disc is the idea of performing squats. The concern is understandable; when you are in a dark place with debilitating pain, adding load to the spine seems counterintuitive. However, it is vital to recognise that you are already squatting every single day—whether getting off the toilet, standing up from a chair, or getting out of bed. The question, therefore, is not whether you should squat, but whether you should continue to do so with poor mechanics that aggravate your injury, or learn to perform the movement with a neutral spine to build resilience and support recovery. Many people fall into the trap of "waiting to heal" before starting rehabilitation, particularly when dealing with nerve damage or post-surgical recovery. While peripheral nerves heal slowly, the structures of the lumbar spine (discs and ligaments) require gradual exposure to stress and strain to remodel effectively. Avoiding movement often leads to further deconditioning and weakness, leaving the spine vulnerable to the next minor incident. A structured programme that progresses from stability to strength is the only long-term solution to breaking this cycle. In this session, we also dive deep into the nuances of spinal mechanics, addressing questions on loss of lumbar lordosis (straightening of the spine), spondylolisthesis, and the often over-complicated topic of muscle imbalances. We explain why focusing on "glute firing" is often majoring in minors compared to mastering the fundamental compound movements. Finally, we discuss specific considerations for pregnancy-related back pain and why advanced variations like the 'airplane' hip hinge should be reserved for those who have already built a solid foundation of strength in the later phases of the Back In Shape Program.Key Topics Covered

Is Your Back Actually Tight? Measuring True Lumbar Motion

Back In Shape

Play Episode Listen Later Dec 22, 2025 67:25

Many sciatica and back pain sufferers feel a constant urge to "stretch out" a tight lower back. However, using clinical tools like a dual inclinometer reveals a critical misunderstanding of spinal mechanics. When you perform a toe touch or a forward bend, the vast majority of that movement comes from your hips, not your lumbar spine. The lumbar spine has a finite range of flexion at specific segments. Attempting to push past this biological limit does not help muscles; it strains the ligaments and discs, potentially worsening herniated discs or instability. The sensation of "tightness" in the lower back is rarely a result of short muscles that need lengthening. Instead, it is often a neurological protective mechanism—a "brake" applied by the nervous system to stabilise a spinal segment that it perceives as vulnerable or injured. By aggressively stretching (such as pulling your knees to your chest), you may be stretching the very ligaments that are trying to heal, engendering further hypermobility and perpetuating the pain cycle. The solution is not to mobilise the spine, but to stabilise it through core engagement and proper hip mechanics.Key Topics Covered

Worried you'll make a herniated disc or sciatica worse, so you do… nothing? Today's live explains why “doing nothing” usually means doing the wrong things all day (and staying stuck), and what to do first: education → strategy → tutorials → then careful practice. We show why daily life (chairs, shoes, kids, work) often provokes more than a controlled squat, how to troubleshoot towel vs bed decompression (including getting in/out safely), and simple gym setup wins (hello safety bars) that spare your back. We also cover why the Jefferson curl isn't a smart rehab driver, plus Q&A on AS/spondylo, post-microdiscectomy progress, spondylolisthesis goals, isometric holds, “can a disc bulge fully heal?”, and best sitting/posture tweaks.Start here → https://backinshapeprogram.com/start/Highlights:

Sitting often makes sciatica feel instantly better because a posterior pelvic tilt opens the nerve exit holes—but that short-term relief can reinforce the problem. In today's live, we explain what's really going on at L4/5–L5/S1, why repeated flexion (knee-hugs/child's pose) “feels good but harms,” and how to sit smarter (reclined, supported neutral) while you rebuild capacity. Then we show how to prevent those “getting up = lightning bolt” moments by learning neutral-spine transitions and using the Core 6 (Dead Bug, Marching Bridge, Squat, Hip Hinge, Step-Up, SLHH) to restore load tolerance. Member Q&A covers spondylolisthesis, night flare-ups, “squat on the toes,” deficit deadlifts, flights, Roman-chair/back extensions, and more.Start here → https://backinshapeprogram.com/start/Highlights:

Today, we dive into stabilization vs. flexion exercises for degenerative spondylolisthesis in a randomized control trial. Degenerative spondylolisthesis involves vertebral displacement due to degenerative changes. It's more common in people over 50 and significantly more in women. The L4-L5 area is often affected, causing foraminal and central stenosis. Small displacements can lead to significant compression and symptoms like chronic low back pain and radiculopathy. Conservative treatment before considering surgery is crucial. Exercise, mainly stabilization, has shown to be beneficial over time. The study examined individuals over 50 with degenerative spondylolisthesis at L4-L5.Episode Notes: Stabilization Exercises Versus Flexion Exercises in Degenerative Spondylolisthesis: A Randomized Controlled TrialJane is an online platform for health and wellness practitioners that makes it simple to book, chart, schedule, bill, and get paid. In this episode, we are joined by Bobbi Giudicelli who has an incredible story to share. For her whole life, she suffered from back pain and thought it was normal. It wasn't until she started playing tennis in her early 30s that the pain increased and became debilitating. She consulted with a spine specialist and was diagnosed with spondylolisthesis. Over the next 12 years, she had to undergo surgery three times as the condition progressed up her lumbar spine. In addition to the surgeries, Bobbi also made a significant change to her diet. She transitioned to a whole food plant-based and gluten-free diet. She found that the cleaner her diet, the easier her recovery was for each of her surgeries. Now, at 67 years old, she feels better and has more energy than she did in her 40s. She regularly rides horses and plays pickleball for several hours, three to four days a week.Join us as we explore Bobbi's journey, her experience with spondylolisthesis, and how her diet has played a significant role in her recovery and overall well-being.Bobbi Giudicelli is the author of: “Freedom From a Toxic Relationship With Food: A Journey That Will Give You Your Life Back" and founder of "Read The Ingredients Superloafs" which are available at www.rtifoods.com and on Amazon.Support the show

In college I had an interest in natural healing and a desire to help others achieve good health. After much research it seemed like chiropractic would be a great fit. I graduated from Cleveland Chiropractic College in Kansas City Missouri in 2001. From there I moved to Lincoln to work at Conard-Love Chiropractic for about a year. Dr Conard specialized in alternative treatments such as nutrition, acupuncture and chiropractic. He taught me a lot about running a business and the treatment of hard cases. In January 2003, we opened Backbone of Healthcare in Broken Bow. Since moving here I have received post doctorate certificates in Acupuncture, Cox Technique (specialized treatment for herniated discs and arthritis) and most recently Sports Certification. I really enjoy helping people suffering from herniated discs and sports injuries and many other health conditions. Continuing my education in the latest chiropractic approaches to natural health and treatment is a priority. Also, this last year our office participated in a nationwide clinical study of failed back surgeries. Any spare time is filled with watching my kids participate in music, 4-H and sporting events. I also enjoy volunteering my time at church and church camp. If I can squeeze in a good football game or help my parents on the farm, my weekend feels complete. Today on the podcast, I'm very excited to bring to you a presentation from one of my recent students, Alea Penner, a former gymnast who has just graduated from Duke Physical Therapy School.   She did a phenomenal, inservice lecture on bracing gymnasts with back-related injuries.    “Discussing bracing treatment for spondylolysis and spondylolisthesis in athletes. Summarizing the current available research that supports or contradicts current treatment standards and discuss topics of future research that are demanded by lack of current literature.”   The use of braces is controversial, so it's really important for us to understand the current research on bracing.   Alea wonderfully took on the big task of reviewing the literature and doing an inservice on bracing versus not bracing.    She discusses:   Does bracing limit gross motion and/or spinal segmental motion during ADL's (activities of daily living) or sport-specific activity?   Does bracing lead to decreased strength and/or changes in posture/gait with prolonged use?   Does limited motion prevent the progression of a fracture or slippage?    Does bracing improved bone union? And is bony union predictive of a better return to sports outcomes and subjective pain?

Dr. Grant Elliott with RehabFix Online Low Back Program: In today's episode, I discuss how much misinformation there is regarding spondylolisthesis and how to go about your rehab with this "diagnosis". We have had many clients around the world who have received this diagnosis and were told to avoid so many things because of it. Most of the time, we find doing the very things they were told to avoid largely contributed to their success in our program. You are not your diagnosis! Betsy is a prime example of someone who could have gotten lost in the medical model of unnecessary surgery and medication with her diagnosis of Spondylolisthesis (like me!) We are similar in that we rejected those options, but were still in despair. Luckily for both of us, we discovered mindbody medicine. Betsy's a beautiful story of pain and healing, and serves as a powerful example of the way this work "works"! Join us for an inspiring interview. XOOX n.
