The Evidence Based Chiropractor- Chiropractic Marketing and Research
Today, we dive into stabilization vs. flexion exercises for degenerative spondylolisthesis in a randomized control trial. Degenerative spondylolisthesis involves vertebral displacement due to degenerative changes. It's more common in people over 50 and significantly more in women. The L4-L5 area is often affected, causing foraminal and central stenosis. Small displacements can lead to significant compression and symptoms like chronic low back pain and radiculopathy. Conservative treatment before considering surgery is crucial. Exercise, mainly stabilization, has shown to be beneficial over time. The study examined individuals over 50 with degenerative spondylolisthesis at L4-L5.Episode Notes: Stabilization Exercises Versus Flexion Exercises in Degenerative Spondylolisthesis: A Randomized Controlled TrialJane is an online platform for health and wellness practitioners that makes it simple to book, chart, schedule, bill, and get paid. In this episode, we are joined by Bobbi Giudicelli who has an incredible story to share. For her whole life, she suffered from back pain and thought it was normal. It wasn't until she started playing tennis in her early 30s that the pain increased and became debilitating. She consulted with a spine specialist and was diagnosed with spondylolisthesis. Over the next 12 years, she had to undergo surgery three times as the condition progressed up her lumbar spine. In addition to the surgeries, Bobbi also made a significant change to her diet. She transitioned to a whole food plant-based and gluten-free diet. She found that the cleaner her diet, the easier her recovery was for each of her surgeries. Now, at 67 years old, she feels better and has more energy than she did in her 40s. She regularly rides horses and plays pickleball for several hours, three to four days a week.Join us as we explore Bobbi's journey, her experience with spondylolisthesis, and how her diet has played a significant role in her recovery and overall well-being.Bobbi Giudicelli is the author of: “Freedom From a Toxic Relationship With Food: A Journey That Will Give You Your Life Back" and founder of "Read The Ingredients Superloafs" which are available at www.rtifoods.com and on Amazon.Support the show

Dr. Chad Hackel and his athletic patient share his story of pain and recovery. Dr. Chad Hackel I have enjoyed providing chiropractic, acupuncture and nutritional care to Custer County, for the last 18 years at Backbone of Healthcare. I grew up on a farm near Ord Nebraska and then attended NE Community College and NE Christian College where I met my wonderful wife, Marci. Together we have four terrific children. In college I had an interest in natural healing and a desire to help others achieve good health. After much research it seemed like chiropractic would be a great fit. I graduated from Cleveland Chiropractic College in Kansas City Missouri in 2001. From there I moved to Lincoln to work at Conard-Love Chiropractic for about a year. Dr Conard specialized in alternative treatments such as nutrition, acupuncture and chiropractic. He taught me a lot about running a business and the treatment of hard cases. In January 2003, we opened Backbone of Healthcare in Broken Bow. Since moving here I have received post doctorate certificates in Acupuncture, Cox Technique (specialized treatment for herniated discs and arthritis) and most recently Sports Certification. I really enjoy helping people suffering from herniated discs and sports injuries and many other health conditions. Continuing my education in the latest chiropractic approaches to natural health and treatment is a priority. Also, this last year our office participated in a nationwide clinical study of failed back surgeries. Any spare time is filled with watching my kids participate in music, 4-H and sporting events. I also enjoy volunteering my time at church and church camp. If I can squeeze in a good football game or help my parents on the farm, my weekend feels complete. Low back pain, like many dysfunctions and injuries, is the result of a cause elsewhere in the body. Understanding - and applying - biomechanics is paramount in preventing, treating, and training the all-important low back. In this podcast, Dr. Gary Gray will discuss and deliver the following topics: Chain Reaction® Biomechanics of the Low Back ... "The crossroads of the body!" Strategies for Assessment and Treatment of Low Back Dysfunction ... Acute Low Back Strain, Spondylolisthesis, and Degenerative Disc Disease. Strategies for Prevention and Training of the Low Back ... "And the rest of the body!"
Today on the podcast, I'm very excited to bring to you a presentation from one of my recent students, Alea Penner, a former gymnast who has just graduated from Duke Physical Therapy School. She did a phenomenal, inservice lecture on bracing gymnasts with back-related injuries. “Discussing bracing treatment for spondylolysis and spondylolisthesis in athletes. Summarizing the current available research that supports or contradicts current treatment standards and discuss topics of future research that are demanded by lack of current literature.” The use of braces is controversial, so it's really important for us to understand the current research on bracing. Alea wonderfully took on the big task of reviewing the literature and doing an inservice on bracing versus not bracing. She discusses: Does bracing limit gross motion and/or spinal segmental motion during ADL's (activities of daily living) or sport-specific activity? Does bracing lead to decreased strength and/or changes in posture/gait with prolonged use? Does limited motion prevent the progression of a fracture or slippage? Does bracing improved bone union? And is bony union predictive of a better return to sports outcomes and subjective pain?

Barak Swarttz is a leading strength and conditioning coach and former college basketball player. As an athlete, he leverages his experience and adversity to help others reach their full potential. As a trainer, Barak's goal is to inspire everyone that he works with to find their own GRIT (gain, resilience in Time.) He also uses fitness as a vehicle for enhancing and improving every aspect of his clients' lives. Barak Swarttz trains NBA, WNBA, and Euroleague basketball players, as well as anyone who wants to reach their fitness goals! His family moved to Israel when he was six; that was when he first picked up a basketball. Shortly after Barak returned to the states and basketball became his life. Training, playing, and competing were the only things he cared about. At 16 years-old, Barak was diagnosed with 2 hairline fractures and Spondylolisthesis in his back. He was put in a back brace and shut down from activity for 6 months. He pushed through his pain and continued to play eventually playing college ball at Elmira College and then transferred for his last two years at Curry College. Barak spent countless time in surgery and rehab. He knew he wasn't on his way to the NBA, but he never allowed his work ethic be the reason he wasn't getting better. Barak thought that being injured and never reaching his potential was one of his life's biggest failures, but now realizes that it's actually his life's biggest asset. Barak's experience of being an injured athlete gave him a window of opportunity to help athletes become so healthy, fit, elite and strong that they don't have to endure what he did. After eight years of pain, physical therapy with Tim Fearon changed Mike's life. Now, Dr. Mike Heywood runs his practice, DASH Physical Therapy, in Queen Creek, Arizona. Dr. Mike, as Trent affectionately refers to him, had initial desires to be a sports physical therapist, as he states that many PTs do. Through schooling and performing his rotations, he narrowed his focus on manual intervention and dry needling. From Pain to Gain, Schooling and more Schooling• Mike shares a story from Milwaukee University and working with some athletes of the Brewers. • His rotations in PT school helped him to evolve to what he looked forward to the most. • Tim Fearon, the guru of PT, was a guiding light for Mike's drive.• Undergraduate at ASU in Exercise and Wellness, Doctorate at A. T. Still University• The 'aha' moment of applicable knowledge in PT• One of the most challenging questions to answer is, "how long will it take for me to get better?"• Many people are finally shifting out of the quick-fix mindset, the quick medicine or surgery fix, and into the healthy long idea to work for the fix.Success, What Does It Mean To You? • "Simply put, I like helping people." • Personally, Mike just wants to be involved in his kids' life and be a good example for them. • Defining success in work starts with defining a patient's goal. DASH Physical Therapy• How long does a typical healing case take, from ACL to rotator cuff tear?• Case study, from a fender bender to discovering a brain tumor, to an inability to walk.• The most uncomplicated cases are those with no pain but with restrictions.• The repetitive movement problems are often related to stress relief activities or passion - it's hard to move a patient away from recurring pain. Therapy Tech and Procedure• Dry needling is one of the newer techniques used by Mike besides being a manual intervention.• Spondylolisthesis is probably one of the most challenging conditions to deal with (and words to spell).• Plans with DASH Physical Therapy?• How do you manage your business strategy today?• One of the most challenging business aspects is employees taking time off. • The ever-growing sedentary lifestyle has been a large contributor to the pervasiveness of back pain. COVID-19, the Impact and Opinion• What were the impacts of coronavirus on your business?• What's your take on COVID-19"For eight years, I had pain in my chest and lats and just hurt. I had a herniated disc, and Tim Fearon fixed me. So I've been there. Let's talk if you have any issues."CONTACT: Dr. Mike Heywooddashphysicaltherapy.net480.677.8202All Social Media: DASH Physical Therapy Support the show

Prominent Nashville spine specialist, Dr. Casey Bearden shares a story of a Marine veteran suffering with lower back pain and leg pain who also had had two previous neck surgeries. Dr. Bearden's Story Several years ago I developed a severe low back condition known as a “bulged or herniated disc.” It progressed quickly and the pain I felt at certain times was so piercing that I couldn't tie my own tennis shoes. I feared my future career and active family life would be at risk if the disability continued. I definitely considered my options, with surgery being the only one presented by the surgeon I saw. I decided against surgery. It just didn't make sense to me to undergo back surgery without trying other treatments first. A friend encouraged me to try some treatments from a chiropractor. I took his advice and went for my very first chiropractic examination. The doctor examined me, took some photos, and then “adjusted” my spine. I anticipated the adjustment would be painful, but it was not. It actually felt good in that it provided some relief. I was able to bend over and utilize my back once again. This experience triggered my interest in studying learning more about how chiropractors help with so many different injuries and chronic conditions. I soon enrolled in chiropractic school. That is how it all started… It is strange how life is. It is hard to believe that my herniated disc treatment resulted in me having the opportunity to help others with back pain, neck pain, headaches, carpel tunnel syndrome, migraine headaches, chronic pain, shoulder/arm pain, whiplash from car accidents, neuropathy and numbness in limbs, athletic injuries, just to name a few.
