I couldn't have asked for a better kick off this season on Fit As A Fiddle! I chatted with 2 of my dear colleagues and friends Leigh Welsh and Alicia Ferriere. What we have in common is that we are all Pelvic Floor Physical Therapists New York City, we are all business owners, and we all gave birth in 2023 to our first child! On this show, we talk about all things motherhood from our perspective having treated pregnancy and postpartum for years. With our combined over 22 years of experience as pelvic PTs, we share some things we would love for you to know!A bit on Leigh Welsh:Leigh is an orthopedic and pelvic health physical therapist, and she started her practice, Evexia Physical Therapy, in 2020. She does home visits, primarily seeing pregnant and postpartum people, and she also loves helping patients who are struggling with painful sex, bladder concerns, and prolapse. Leigh is also the Clinical Virtual Care Lead at Origin Physical Therapy. Origin's mission to make pelvic health PT accessible and the standard of care for those that need it. Today on the podcast I'm syncing up with Dr. Emmi Aguillard, PT, DPT, FAFS. She is a full-time physical therapist managing her own private practice that specializes in treating and training runners. She also has advanced training in pelvic health and works with women to maintain their optimal fitness during pregnancy and safely return to running after giving birth. Dr. Aguillard earned her doctorate of physical therapy from Columbia University after graduating from Tulane University, where she competed for the women's NCAA Division I track and field and cross country programs. She completed her fellowship in applied functional science through the Gray Institute and has taken numerous courses through the Postural Restoration Institute. Beyond her physical therapy practice, she coaches runners on Team Wilpers and Dashing Whippets, the 900-member running team, creating strength programs and team training plans for road races ranging from 5K to marathon. Functional training is a popular fitness phrase that can mean different things to different people. Coach Mike Boyle describes functional training as training for a purpose, but in reality, functional training simply refers to an exercise program that is designed to help you move and perform better as opposed to simply looking better. The traditional approach to physical therapy is for a therapist to work on the joint or body part that is not working or in pain. Gary Gray is a physical therapist who needed a way to help his patients without having to spend hundreds of thousands of dollars on equipment; since he couldn't afford expensive muscle-isolation machines, Gary focused on helping his clients by treating the entire body as opposed to just a particular body-part or joint as a result, the concept of functional training was born (this was the 1980s, btw). Gary Gray has evolved his approach of working with the entire body and created the Gray Institute to teach physical therapists, personal trainers and strength coaches how to help their patients and athletes move more effectively. Todd Wright is an assistant coach and Vice President of Player Performance at the Los Angeles Clippers. Todd is also Fellow of Applied Functional Science from the Gray Institute which is where I base the foundation of my training off of. Todd is someone that I have looked up to for a long time! I love how he is able blend a true functional movement program specifically for the movements of the game along with elements of strength and power development. It would be what I consider the total package for any player. Topics that I discussed with Todd are: 1.) His background and how he got into strength and conditioning. Especially the early days when he was first starting out. 2.) His Assessments on how he elevaluates players physically. 3.) Building relationships and accountability with players in order to get the best results. And a whole lot more. Matt's interest in physical therapy stems from his experiences as a young athlete. After dislocating his shoulder twice while training, he had to undergo surgery. However, this turned into a positive— his good experience with physical therapy post-surgery made him realize the career would be a good fit for him. When not working, Matt enjoys working out and competes in natural bodybuilding shows. He loves spending time with his wife and three kids, and has also coached soccer in the community for 20 years. He is working toward making his Gilbert physical therapy clinic a leading force in treating and preventing knee and ankle soccer injuries, especially among female athletes. He is also building a golf business at the clinic, and is a certified Golf Performance Specialist by Nike and the Gray Institute. His goal for Foothills in the future is to create an amazing patient experience that people enjoy coming to; he calls it the Cheers of physical therapy. “People come back to the clinic a year after they've finished physical therapy just to say hi,” he says. “We want people to enjoy working with our staff, and the staff to enjoy being there. Logan Schwartz is currently a clinical professor at the University of Texas as well as the owner of Vitality Restoration that has been running for 10 years. Prior to that he as an assistant basketball strength and conditioning coach for the Men's Basketball program at the university of Texas along with Todd Wright. For me one of the main reasons I wanted to have him on is that is also a GIFT Fellow of the Gray Institute which I draw the foundation of my training from as well. Topics that Logan and I talk about today are: 1.) Logan's background and how he got into strength and conditioning 2.) What it was like starting off as an intern to eventually a senior role for for a powerhouse program. 3.) Logan's training methods for basketball players and how it differs to the majority of other basketball programs in the country. 4.) How much does maximal strength actually matter to the game of basketball, or are their other qualities that are equally as important? 5.) Finally advise to new strength coaches looking to one day be in the same position that Logan is today. I connected very well with Logan on and off air. In this episode of the Female Athlete Nutrition podcast, I talk with physical therapist and Ironman triathlete Dr Caitlin Alexander. Having grown up as a dancer since the age of 3 and getting a taste of triathlon as a teenager, we hear how Caitlin was drawn back into endurance sports post college and fell in love with Ironman triathlons. She shares her quick progression in the sport as an amateur age group athlete, from total beginner to now preparing the upcoming Ironman Age Group World Championships in Kona, Hawaii, in just a few years. She reminds listeners that early specialization is not always best: it's never too late to try something new and reach a high level. Caitlin talks about the training it takes to complete an Ironman: multiple sessions a day, swimming, biking and running. We explore the nutrition required to handle high training volumes, as well as on race day, based around carbohydrates and electrolytes. Practicing fueling and hydration in training is key to race day performance, and this often requires a lot of trial and error with different products because everyone's needs and preferences are unique. Caitlin highlights how some nutrition mistakes prompted her to really dial in her approach to fueling and hydration. We talk more about hydration, understanding your sweat, and preventing GI distress. Caitlin shares how personal experiences with injury led to a major career change, going back to school to become a physical therapist after initially working in the music and performing arts industry. We discuss how varied her role as a physical therapist is and touch on some of her specialities: bike fitting, running gait analysis, and dry needling, We discuss how our own athletic experiences allow us to better relate to clients; Caitlin explains how her background in triathlon makes her more successful as a PT. We dive into running gait analysis for injury prevention and performance, centering around optimizing cadence and stride length. Caitlin and I discuss dry needling and electrical stimulation, what it is, and how it can play a role in injury recovery. Caitlin emphasizes that manual therapies like dry needling are not permanent injury fixes because they don't address the root cause, load the tissue, or re-educate the body on proper movement patterns: effective recovery requires a multidimensional approach that addresses both the mental and physical sides. Dr. Jennifer Lee received her Doctorate in Physical Therapy from Washington University in Saint Louis, Missouri and is the Owner of L2E Physical Therapy in Peoria, Arizona. She began her career in an outpatient orthopedic physical therapy clinic, focusing on injuries and imbalances of the musculoskeletal system. This led her to completion of a Fellowship in Applied Functional Science through the Gray Institute, where she studied the 3-dimensional connection of the human body specific to the musculoskeletal system and learned to better ask and answer the question, “WHY” someone would experience pain and dysfunction, compared to our traditional medical model which focuses on the “WHAT” is hurting and symptom resolution. Later studies at the Barral Institute taught her that the question, “Why” cannot always be answered by addressing the musculoskeletal system, as the true root cause of some patients' pain stems from restrictions or previous trauma to the nervous system, cranium, visceral organs/fascia, or is rooted in emotional trauma stored in the body. The work to determine the true root cause of someone's suffering, particularly chronic pain, anxiety or emotionally-rooted pain often requires more time and a softer, quieter environment than is typical of the outpatient orthopedic clinical setting. Each of her patients receives an hour of 1-on-1 care, with the goal of letting the patient's body be in a safe environment where it can communicate what it needs in order to function at optimal efficiency. Her passion is to create healing and to incorporate the totality of an individual's physical, mental, and emotional experience and to allow each patient the time and care they deserve in order to understand the story of their bodies and find the answers as to why they are experiencing pain.

Episode 012: As ACL injuries continue to rob us of our season, Brian shares with us his best practices for safely returning athletes to sport after an ACL rupture. He tells us what the process is like, what options athlete's have when it comes to the type of repair they choose, and why sports performance is key for a safe transition to returning to sport. Brian earned his BA from the University of Washington and a Master of Science in Physical Therapy from Eastern Washington University. Brian specializes in orthopedic and sports rehabilitation. Brian combines his unique sports medicine education, his sports background as a player and coach, and his passion for the game to provide cutting-edge therapy and performance training. Brian has been an adjunct professor teaching in EWU's Physical Therapy Department since 2007. He was elected a board member for the Private Practice SIG in 2016 and he also proudly serves on the U-District Foundation Board.
