This week I wanted to get back out into nature while we had a break from the rain to test out the progress on the Tropaya BX #6's cure process! All parts of the process are equally as important, however the cure is where you lock in months of work and preserve what you've done! There are so many batches that have been produced that don't get a proper cure and the consumption expereince can be negatively affected. They also chat about bitcoin cresting over $100,000 in the wake of an expected crypto-friendly Washington under a Trump presidency. Later on in the show, Telis gets into what's driving people to restaurants at projected record levels, even though the cost of food and dining out have been high. He's joined by Aman Narang, CEO and co-founder of Toast, a technology company that sells software and hardware to restaurants for digital payments, online apps, takeout and delivery, kitchen management and more. The HPS Podcast - Conversations from History, Philosophy and Social Studies of Science
"It wouldn't make sense to leave the entire burden of upholding objectivity in science on the shoulders of fallible individuals, right?" Prof. Fiona Fidler Today, we return to one of our favourite episodes, with the person who first came up with the idea for our podcast – Professor Fiona Fidler. Fiona is head of our History and Philosophy of Science Program at the University of Melbourne and co-lead of the MetaMelb research initiative. In this episode Fiona first discusses her early move from psychology to HPS when she was struck by the ‘dodgy' statistical methods she found prevalent in many sciences. She has since dedicated her career to studying scientific practice to help improve confidence in scientific claims.Fiona introduces us to the concept of ‘Collective Objectivity'. Following from an earlier podcast on 'Values in Science' by Rachel Brown, Fiona discusses how contemporary understandings of objectivity have become more sophisticated.Rather than viewing objectivity in science as solely the role of individuals, today we understand there are strong social dimensions to ensuring scientific outcomes are not unduly biased. As Fiona discusses, this not only requires diversity in 'who does science' to ensure multiple perspectives are incorporated, but we also need multiple collective review mechanisms to ensure we are developing truly robust, reliable, objective outcomes. The HPS Podcast - Conversations from History, Philosophy and Social Studies of Science
“This is Holden Thorp. I'm the Editor in Chief of Science and thanks to Sam and Carmelina for all they're doing to get the word out about the history and philosophy of science”Today's guest is Holden Thorp, professor of chemistry at George Washington University and Editor-in-Chief of the Science family of journals. In April of this year, Holden published an editorial in Science with the tantalising title ‘Teach Philosophy of Science'. Holden called for more substantial teaching of history and philosophy of science across undergraduate and graduate science curricula. He argued that learning about the historical and philosophical foundations of science is crucial for improving public trust. Encouraging deeper consideration of ongoing revision in science, as well as historical and societal contexts, will better equip future researchers and professionals with a more nuanced perspective on how robust, reliable knowledge is established. In the months since its publication the editorial provoked significant discussion across social media, so we decided to reach out to Holden to join us for a more in-depth conversation. We were keen to see exactly what role Holden saw for HPS scholars in such a shift and test his willingness to engage with the more challenging insights that research across History, Philosophy and Sociology of Science bring to the surface.A full transcript of the episode can be accessed here: https://www.hpsunimelb.org/post/s4-ep-9-holden-thorp-on-teach-history-and-philosophy-of-science Relevant links:'Teach philosophy of science' | Editorial by Holden Thorp | ScienceHolden's Seminar | 'Unreasonable Doubt' | The George Washington University'Cracking the Code of Science Denial' | On Holden's Seminar | GW Today'It matters who does science' | Holden Thorp | Science | AAASHolden Thorp | Editor-in-Chief | Science | AAASWhy Trust Science? | Naomi Oreskes | Princeton University PressThanks for listening to The HPS Podcast with current producers, Samara Greenwood and Carmelina Contarino. The HPS Podcast - Conversations from History, Philosophy and Social Studies of Science
Today our guest presenter Thomas Spiteri is joined by Professor Edouard Machery, Distinguished Professor in the University of Pittsburgh's HPS department and director of the Center for Philosophy of Science.Edouard, a leading figure in experimental philosophy (X-Phi), shares insights into the X-Phi movement, which integrates empirical methods into philosophical inquiry. He discusses the limitations of traditional philosophical methods, particularly the use of intuitions, and explains how X-Phi broadens the tools available to philosophers.Reflecting on the evolution of X-Phi, Edouard talks about its future direction and the need to balance empirical research with philosophical rigor, while cautioning against the risk of the field becoming too isolated from broader philosophical discussions.A full transcript of the episode can be accessed here: https://www.hpsunimelb.org/post/s4-ep-4-edouard-machery-on-experimental-philosophy Relevant Links:Edouard Machery | Personal WebsiteEdouard Machery | hps.pitt.eduEdouard Machery | phil.papersThe Center for Philosophy of ScienceYou can find Conversations at the Center on YouTube and SpotifyExperimental Philosophy OverviewAdditional Resources:Gettier Cases Explained Experimental Philosophy and IntuitionsThanks for listening to The HPS Podcast with current producers, Samara Greenwood and Carmelina Contarino. Today we are joined by Dr Darrin Durant a Senior Lecturer in HPS at the University of Melbourne specialising in Science and Technology studies. Darrin's research covers two seemingly distinct areas: nuclear energy and expertise. Yet nuclear energy and other contested public policy issues are informed by experts on both sides of the debate. As Darrin explains in today's episode, there are different types of expertise and we must learn to better judge who is, and who isn't an expert. Using real-world case studies, Darrin discusses the problems around creating public policy where conflicting scientific evidence or scientific uncertainty exists. By understanding how conflicting positions are treated when differing expert opinions arise and by understanding the different types of expertise at play, Darrin argues that policymakers and the public are better equipped to make active judgements about the experts involved and the contentious issues under discussion. The HPS Podcast - Conversations from History, Philosophy and Social Studies of Science
This episode is the first of two in which the celebrated Professor of History of Science, Simon Schaffer, discusses the famous HPS publication, Leviathan and the Air-Pump: Hobbes, Boyle and the Experimental Life, which Simon co-wrote with another esteemed HPS scholar, Steven Shapin, in the early 1980s. The book went on to become one of the most well-known across both HPS and STS, with next year marking 40 years since its first release. In todays episode, Simon discusses his own academic story, introduces us to the books main themes and aims, and muses on why it was this particular publication became so well known. Simon is also a delightful, scholarly communicator, so enjoy listening and remember to tune in again next week for the equally entertaining second half.Transcript available here: https://www.hpsunimelb.org/post/s4-ep-2-simon-schaffer-on-leviathan-and-the-air-pump-40-years-later-part-1 Relevant links:Leviathan and the Air-Pump | Princeton University PressIntroduction the the 2011 Edition of Leviathan and the Air-PumpSimon Schaffer (cam.ac.uk)Steven Shapin (harvard.edu)Thanks for listening to The HPS Podcast with your current hosts, Samara Greenwood and Carmelina Contarino. Dr. Emily Caffrey, a certified health physicist with the Health Physics Society, joins host Christ Stallman, CGC to talk about different types of radiation and their potential effects on a pregnancy. Links Mentioned in This Episode: Health Physics Society's Ask The Experts https://HPS.org Ep. 76 Transcript You're listening to the MotherToBaby podcast, medications and more during pregnancy and breastfeeding. Ask the experts with your host, genetic counselor and mom of four, Chris Stallman. This episode contains evidence based information that's current as of the day recorded and may change as more data becomes available. To get the very latest information about this topic or other topics in pregnancy and breastfeeding, please contact a mother to baby specialist at 866 626 6847 by text at 855 999 3525 or through our website at mothertobaby.org. Welcome to another episode of the mother to baby podcast. My name is Chris Stallman, and I'm a genetic counselor, a mom of four, and a teratogen information specialist. So what that means is that I talk to people, so patients, family members, healthcare providers, the general public, about exposures that can happen before pregnancy, during pregnancy, while breastfeeding, and in cases of adoption. Thank you And an exposure can be anything. So it could be a medication you take. It could be a vaccine. It could be a hair treatment. And in some cases it could be in the place where you work. Today we're going to talk about a very specific exposure, radiation. And we have a very special guest to talk with us today. Dr. Emily Caffrey is the program director and an assistant professor for the master's in health physics program at the University of Alabama at Birmingham. She is also a certified health physicist that specializes in calculating radiation doses from environmental sources of radiation. Dr. Caffrey, welcome to the show. Thanks for having me. Excited to be here. Great. So let's get started today. We're going to talk about radiation. Can you tell us a little bit about what radiation is and how people are exposed to it? Yeah, absolutely. Um, you know, radiation is just a form of energy. So there's two types of radiation. There's non ionizing radiation and ionizing radiation. So non ionizing radiation are lights, microwaves, your cell phone emits non ionizing radiation, things like that. There's also ionizing radiation, and that type of radiation is a little bit higher energy, um, that makes charged particles. Um, and that's the kind of radiation you get when you're talking about a CT scan or an x ray, something like that. Um, that's ionizing radiation. Uh, and I, and I just want to point out that radiation is all around us. It was present when life first evolved on Earth. It was present when dinosaurs lived. It's still present today. Uh, we live in a radioactive world, and I think a lot of people don't know that. So I'd like to start with, radiation's all around you, and it has been your whole life. Um, natural radiation comes from space, and it comes from living things that are in the Earth. The Earth's crust is radioactive. You may, if you live in a place that has high radon, um, you may have heard of radon coming from Earth up into your basement. That's a really common source of exposure. Um, and our human bodies and cells have adapted over time to respond to and repair the small amounts of damage you might get from these low levels of ionizing radiation. So some of the more common forms of radiation, like you described, light, microwaves, CTs, would that also include mammograms and would that increase risks to a pregnancy? Yeah, that doesn't include mammograms. The mammogram is again a low energy type of x ray that's used to image the breast tissue to look for cancer usually. Um, and You know, just like all other types of, of diagnostic imaging procedures, and I think we'll talk a little bit more about this as we get into it, um, you really aren't at risk when you have a low, a low, a diagnostic imaging procedure, those are very low doses, your, your pregnancy is not at risk, the unborn child, not at risk from those types of exposures. And I'm really glad that we can talk about that and hear about that again, because it is the kind of thing, you know, if. certainly if you need it in pregnancy, it's a good idea to get your cancer screenings. Absolutely. Dr. Caffrey, can you tell us a little bit about the Health Physics Society? Yeah. And I like to tell people like health physics is a profession you've never heard of, but the health physics society is the professional society for health, which are radiation protection professionals. So we are the profession that is devoted to the safe use of radiation. Um, so anywhere radiation is used, whether that's a hospital, a nuclear power plant, um, the government and military installations, um, there's gonna be a health physicist, a radiation safety officer, someone there making sure that the people and the environment are safe from that ionizing radiation. So you may not see us, but we are there making sure that we're following regulations and keeping people safe. When you mentioned earlier that radiation comes from space, I got to say, I love hearing that. I think that that's such a cool and exciting thing. So let's talk about cosmic ionizing radiation. So what is that specifically? And does that increase risks to a pregnancy? Yeah, it's space is always fun, right? And we're talking about sending astronauts to Mars and things, right? Radiation. So, you know, one component of your natural radiation dose that you get just by living here on Earth is from outer space, and that's called cosmic radiation. Um, so particles and electromagnetic magnetic waves from outer space, um, come and hit and impact Earth. So you may have heard of solar flares and things like that. And the sun flings off all these charged particles. So those come and hit Earth. Now, Earth is actually pretty well protected. We have this magnetic field that's around Earth that deflects a lot of those particles. Think of it as like a shield around Earth. And then our atmosphere, like a literal atmosphere above us, also protects us from a lot of that radiation, but not all of it. So some of that radiation is going to come in and actually get into Earth, like where you and I are sitting here right now. Um, and, and, and, There's more at the north and south poles and as you go higher in elevation because at the north and south pole the magnetic field that protects us is weaker and at higher altitudes you're just physically closer to the radiation coming from space. So if you're, um, your background radiation dose, if you're at the North Pole or if you're standing on top of a 14 year in Colorado is going to be higher than me sitting here in my office in Huntsville, Alabama, which is basically at sea level, right? So it's just a small difference, but it does change a little bit. Um, but you know, cosmic radiation contributes Um, about 5 percent of our average total background dose. So that's a very low, low number. You and I just living on Earth, which is radioactive, get about 3 millisieverts per year. And a millisievert is just a unit of radiation, uh, dose that we, that we use to count, you know, when we're talking about radiation doses, that's just a number we, a unit that we use. Um, so you and I get about 3 millisieverts per year, and about 5 percent of that 3 millisieverts is from cosmic space radiation. Um, and I, and I want to add to that background radiation doses are too low to increase your risk of pregnancy or cause any harm to your unborn child. We are not worried about background radiation doses or doses that are lower, um, in, in than, especially not background, but lower than three is definitely not a concern. And we'll talk a little bit more about what levels you might want to start thinking about, about, you know, where, at what levels you might want to be concerned, but definitely not at background levels. Great. Okay. So thinking about space occupation, what's out there? What about radiation exposure for someone who's working as a flight attendant? I got to tell you, I got this question for the very first time, maybe six or seven years ago. And until then, it never even occurred to me. I was like, yeah, of course, there's radiation in the atmosphere. So I'm going to stop talking and let you give us this answer. Yeah, and I'm really glad you asked that question because we hear this concern a lot, both from flight attendants, pregnant women that travel for work a lot, uh, pilots, right? We hear this question a lot. And I think people, uh, I certainly didn't before I got into this field, didn't think about flight attendants being, uh, people that are exposed to COVID. More than average, right? Um, but when you fly up high, especially people that fly transcontinental flights that go over those poles where you have a higher radiation dose, they get a higher radiation dose in the background. They actually, in some cases, get higher radiation doses than workers at nuclear power plants, but higher does not mean there's a problem. So let's back up for just a second. Um, so the amount of exposure that you get from flying is significantly lower than, um, The exposure needed to cause any harmful effects to your, to your unborn child or to you, the pregnant, pregnant person, rightly, that we're not concerned. Um, and just to give you some context, right, we talked about 3 millisieverts as our background radiation dose. Um, the radiation dose needed to cause harmful effects is roughly, and again, there's a lot of research on this that's ongoing all the time as we improve our models, but we're talking roughly 100 millisieverts. Um, and again, millisieverts just being a unit of radiation dose, so 3. Is your background radiation dose, and 100 is what we're talking about for harmful effects. Um, and then to talk a little bit more specifically about flight, so how much radiation do you get in a flight? Well, your radiation exposure during a commercial flight is about 0. 01 millisieverts per hour of flying time. So to get to 100 then right? That means you need if you do some quick division. That means you need about 10, 000 hours during your pregnancy to reach the amount of radiation that might be harmful. I don't think anyone is even flight attendants, right? You're not flying in 10, 000 hours. You really aren't quite safe from as a as a flight attendant or a pilot or even just someone that travels a lot while you're pregnant. That's it's not of concern. Excellent. And I'm so glad that you mentioned, you know, it could be higher than 100. wherever we start out. But that alone doesn't necessarily mean that there is a problem. We have to have more information. We have to have all the pieces and that's so important for all exposures. But again, certainly for one like this, where you may not have known or you may not have remembered, it's like, oh, there's radiation all around us. Oh, wait, it also depends on how much we're being exposed to. Now, for my favorite thing to ask all of the guests on the show, when I get the opportunity, what is the most interesting question about radiation and pregnancy that you or the Health Physics Society has received? Yeah. And I'm, and again, this is a great question. We get hundreds of questions from pregnant women and thousands of questions from the general public. Yeah. Um, and you know, it's kind of, I didn't pick something like when I was thinking about this, I didn't pick something light hearted because this, this particular question that I'm going to throw is one that has stuck with me and it's the one I that just reminds me of why talking to people and using my knowledge to explain why things are okay or not okay or what you need to be concerned about and what you shouldn't be concerned about is so important. Um, and so about, about a year ago, we had a woman right in, um, an Italian woman and she had had a diagnostic x ray of her pelvis before she knew she was pregnant. And her, her gynecologist told her the pregnancy was probably more risky because of the x ray. And she specifically asked us if she should terminate her pregnancy. And under Italian law, you know, that was very time sensitive because she only had a week to make such a like, insanely impactful life decision. Um, and so I was so glad that she found us because I just want to be really clear here. The answer to that question is absolutely not. A diagnostic x ray, even to the pelvis, um, is not going to cause any harm to your unborn child. And just the relief that we were able to provide, um, that, that soon to be first time mom was just like overwhelming. She wrote back to us, um, several times over the course of, you over the course of her pregnancy just to check in and she told us she sent us an email saying that her son was born happy and healthy and she was just so grateful and so relieved because she didn't know right and and so being able to offer that service and being able to help people understand and make massive decisions is just really amazing. Uh, it gives me the warmth, like, that is amazing. I am not only so glad that the society exists and that folks like yourself are out there giving this information. I'm so glad they found you. I'm so glad that, you know, it seemed like a good outcome for them. That's wonderful and so important. And it is one of those things where, you know, you don't know what you don't know. The good news is in a lot of cases, there are people, um, again, like you and the health physics society that are out there assisting with some of this information that isn't as commonly available. So thank you so much. What a great story. So Emily, if someone has a question specific for the health physics society, so radiation. Or even, you know, concerns in the pre pregnancy or after pregnancy planning stage. How did they get to your organization to find this information? Yeah, thanks for asking that. Um, so you can find us at hps. org, hps. org. That's the website of the Health Physics Society. And you'll see a little Ask the Experts button. And you just click that button and you'll see our webpage. Um, there's a webpage specifically for pregnancy and radiation. And on that web page are a couple of videos that the Health Society has produced. There are fact sheets about radiation and pregnancy. There are Q& A's from, uh, not only some, some basic Q& A's that we have written, but also, um, some stories that other people have, other questions that people have written in with. We, we anonymize them, of course, and we post some of those to our website. And we cover everything from, um, background radiation, to pregnancy and flying, to what if I need a medical procedure, to I'm a lactating mom that's breastfeeding, you know, and I have to have this nuclear medicine procedure, you know, if, am I okay, what should I do? Um, I think the medical industry does a good job of providing information, but I think people sometimes still want, you're still overwhelmed, right? When you, you have a child and you are doing something in the medical realm, like those are two very overwhelming things. In and of themselves. And so when you have another question that didn't get answered, please reach out to us and we're always happy to help. That's awesome. And we are going to put information for the Health Physics Society in the show notes. Something else, um, that I was just thinking of, I, you know, talk to people all day. That's my job. But if I forget to ask my doctor or healthcare provider something, or if I'm traveling or busy, I might not always have the opportunity to connect with them, and I will say that sometimes I turn to Google. So it's great to know where they can find this information. Providers are wonderful. They give good information. Sometimes I do not have it in me to wait 30 minutes on the phone. You know, life is busy. So it's great to know that we can find this information online as well. Dr. Caffrey, before I let you go for this episode, um, cause I would love to have you come back and talk more about radiation. Is there a final thought that you would like to leave our audience with? Yeah, I think, you know, I think the thing I want to emphasize is that radiation can be scary. You can't see it. You can't feel it. You don't know it's there. Like anything you can't see and you don't know is there. Can be scary, but I just I want to emphasize that our bodies evolved in this radioactive world. They have repair mechanisms for low doses. And we know from population studies from epidemiological studies. That the amount of radiation used in these diagnostic procedures, so x ray scans, mammograms. Um, even to the pelvis area are just not are way too small to cause harm to your unborn child. So you don't need to worry about those types of procedures. You know, and if you are concerned or you had a specific procedure or you had a nuclear medicine procedure, you know, those are things that experts can talk to you about and counsel you through, um, to make sure that you're getting up to date information. And it's so important to remember to ask those questions because diagnostic procedures can prevent a lot of issues that are harmful to your baby, right? And, or could be harmful to your, to your unborn child and, or yourself, right? As the pregnant woman, like, um, There are actual harms that can be done by not having a CT scan when it's clinically indicated, and the radiation dose should, should be something that you think about, um, and, and be aware of and educated about, but it should not stop you from getting a diagnostic procedure that could potentially save your life or your unborn child's life. So radiation's scary, but we, um, myself, the Health Physics Society, and all of my topic editors and experts are here to help. Hps. org or through the mother's baby website, there are links, um, and you can find us online and we are, please reach out. We are happy to help. Absolutely. And thank you. Thank you for the reminder that, you know, these procedures, MRIs, CTs, and so forth are not done lightly. They are done because someone needs it, especially during pregnancy and you, you probably do need it. So please feel free to contact. The health physics society or mother to baby with any questions that you have. Dr. Caffrey, it was my pleasure to speak with you today. Morgan and Jesse just returned from Glass House Brands' first analyst and institutional investor day and were joined this week by Glass House CEO Kyle Kazan and President Graham Farrar. The group recapped the day and shared some of the highlights from the presentations and tours. Glass House is a single-state operator focused on the California market. We walked through the state of California cannabis and the challenges and opportunities it presents. Kyle and Graham helped us better understand the retail climate and CPG opportunities and explained their continued success in the wholesale market. The company is in the process of building out Greenhouse Two, which is a special greenhouse with almost 11,000 HPS lights supplementing the power of the sun. Glass House is considering using this facility to grow Farm Bill flower, or what some might consider THCA flower, that is compliant with the 2018 Farm Bill. We cover the potential to enter new states with Farm Bill flower, direct-to-consumer options, and how pricing compares to California cannabis. The team also explained the risks of the dynamic political environment and the risk of potential changes to the Farm Bill language. For this reason, they are weighing all options before making a final decision to enter this market. Finally, it would not be a cannabis conversation if we didn't discuss politics, and Kyle and Graham shared their thoughts on Schedule III timing and the path to higher exchanges.
Buying Cheap Computers Hi. I'm Moss Bliss. You may have heard me on mintCast, Distrohoppers' Digest, or Full Circle Weekly News. I keep hearing people complain that they can't find a decent computer under $2,000. My response is: lower your expectations, and help end digital waste. Do you really need a top-end gaming machine? Most games will run on older computers just fine. And there are LOTS of 2016 models (and a few newer years) coming off office leases. My suggestion is the Lenovo ThinkCentre M700 Tiny; I will have other suggestions later in the podcast, but this is what I went with an have personal experience. The M700 Tiny is a box about the size of an older external DVD drive, with your choice of an i3, i5 or i7 processor, up to 64 Gb RAM, and a power supply sipping away at up to 35 watts. If you go on eBay or your local equivalent, you can find these for ridiculously low prices. My recommendation is to go for a box with an i5, 8 or 16 Gb RAM, with or without a drive. You can find them even cheaper if you search for "no OS", which frequently still includes the drive, but SSDs are quite cheap these days so it hardly matters. I recommend the i5 rather than the i7 because, in the 2016 models, the i7 wasn't much more than a heat producer compared to the i5 but no faster, and the i5-based machines are often $100-150 cheaper. Of course I recommend putting the Linux of your choice on these machines when you get it. You will need a monitor and keyboard for this. If you don't already have one, I would suggest looking locally for a used 26" LCD/LED TV. You will also want a cable for it, and you can either get a DisplayPort cable while you're at eBay, or a DP-to-HDMI adapter to use the HDMI cable which probably comes with the TV. For a keyboard, you should go to your local big box store and get one you like, if you don't want to buy used. You can find them from $5 to $20. I apologize for my US-centric prices, but you should have similar prices in your local currency if you have eBay or something like it. I just did a quick lookup for "M700 Tiny No OS" on eBay, and saw i5 machines which were purported to work from between $80 and $120 with 8 Gb RAM. Be careful to watch for shipping prices, I see a particularly good-looking refurbished model with 16 GB for under $120 -- but the shipping is over $35 from Australia! If you want an AMD chip, you'll need to research the model number... but they do make them, just not as many. If you don't like Lenovo, you can find Dells and HP EliteDesks in the same range. One good thing about the HPs is that they label which generation they are -- G2, G3, G4. Newer generations cost more, but will be more future-proof. There is currently an HP EliteDesk 800 G2 with 8 Gb RAM and No OS on eBay for $50! Again, if you buy a G3 or G4 it will cost more, but be a newer machine as well. These are all 64-bit quad-core computers, some over 3 GHz, with low power demands. Cheap to buy, cheap to use. Any version of Linux will run on them, and if you're desperate you can run Windows 10 on them as well. That should be enough to get you started. If you have questions, write me at bardmoss@pm.me. Hello Hidden Pearls Fam!What a year it has been so far. We hope you all are well and staying present for all of the magic that is moving around us. This year has already brought some of our highest highs and our hardest lessons. It has asked us to spend a lot of time looking inward and has offered opportunities for us to shift our perspective and priorities. All in all, it has left us feeling so grateful for our community and so aligned with the work we are creating in Hidden Pearls Studio. How WILD that we are already in May. So many months away from the Super Bowl and already so many months closer to the start of a new season. Our intention with this episode is to share the practices and tools we are both using to stay present and awake with all that is moving around us. For those of you who are already on HPS and working through the classes with us - thank you for your support! And for those of you who have yet to give it a try, this episode shares insight into how the classes are structured and organized and why we are creating this content now. Episode 167: Aspirin in PregnancyDr. Marquez explains the use of aspirin during pregnancy to prevent preeclampsia. Dr. Arreaza adds comments and questions and clarifies that aspirin is not used for the treatment of preeclampsia.Written by Verna Marquez, MD, and Hector Arreaza, MD.You are listening to Rio Bravo qWeek Podcast, your weekly dose of knowledge brought to you by the Rio Bravo Family Medicine Residency Program from Bakersfield, California, a UCLA-affiliated program sponsored by Clinica Sierra Vista, Let Us Be Your Healthcare Home. This podcast was created for educational purposes only. Visit your primary care provider for additional medical advice.Introduction to the topic: Pregnancy is a special time in a woman's life, and we want to make sure that both the mother and the baby are safe and healthy. 1. What is aspirin? Aspirin is one of the most ancient medications in history, it is known as acetyl-salicylic acid (ASA) and it belongs to the family of non-steroidal anti-inflammatory drugs (NSAID), and it is also an anti-platelet, among other properties that may be unknown. It is widely used for pain, fever, and inflammation, but due to adverse effects during viral illness (i.e. Reye Syndrome), it is used less frequently during viral infections. As we know, aspirin is widely used to treat myocardial infarction and ischemic stroke, and especially for secondary prevention. The use of aspirin for primary prevention of cardiovascular disease has become less popular, but we are going to leave that discussion for another episode because today we will talk about the use of aspirin in pregnancy!2. Why should we use aspirin in pregnancy?Low-dose aspirin in pregnancy is most commonly used to prevent or delay the onset of preeclampsia. Aspirin lowers the risk of preeclampsia by 10% and its consequences (such as growth restriction and preterm birth). Several organizations have agreed on the risk factors we will mention briefly. These organizations are ACOG (American College of Obstetricians and Gynecologists), USPSTF (US Preventive Services Task Force), and SMFM (Society for Maternal-Fetal Medicine).3. Who should we start on aspirin in pregnancy? Aspirin is not for every pregnant patient, for example, a healthy nulliparous or any patient who had an uneventful, full-term delivery previously, is considered low risk and should NOT be started on aspirin because there is no benefit in preventing any condition. Low-dose aspirin is recommended for women who have at least a high-risk factor because the incidence of preeclampsia is about 8% in these patients. The risk factors are:•Previous pregnancy with preeclampsia (especially early onset and with an adverse outcome)•Type 1 or 2 diabetes mellitus.•Chronic hypertension.•Multifetal gestation.•Kidney disease.•Autoimmune disease with potential vascular complications (antiphospholipid syndrome, systemic lupus erythematosus).Your patient only needs 1 high-risk factor to be put on aspirin in pregnancy. 4. What are the moderate risk factors?A patient needs to have more than 1 moderate risk factor to meet the criteria for prenatal aspirin.•Nulliparity.•Obesity (BMI >30).•Family history of preeclampsia in mother or sister.•Age ≥35 years.•Sociodemographic characteristics (Black persons, lower income level [recognizing that these are not biological factors]).•Personal risk factors (for example, previous pregnancy with low birth weight or small for gestational age newborn, previous adverse pregnancy outcome [such as stillbirth], interval >10 years between pregnancies). However, low-dose ASA prophylaxis is not recommended solely for the indication of prior unexplained stillbirth in the absence of risk factor for preeclampsia.•In vitro conception.USPSTF/ACOG may also suggest aspirin in selected patients with only one moderate risk factor, but it would require consultation with a specialist in obstetrics. 5. When should we start aspirin? After 12 weeks of gestation, ideally before 16-20 weeks of gestation. If a patient is more than 16 weeks pregnant, aspirin can be started but most of the benefit has been noted when initiated before 16 weeks because many of the abnormalities that cause preeclampsia are developed early in pregnancy. It is not recommended to start before 11 weeks. It is important to mention also that low-dose aspirin appears to have little or no benefit in patients who already have developed preeclampsia. Starting aspirin in preeclampsia can even cause damage such as bleeding in cases of thrombocytopenia. 6. What is the dose? The dose is between 75 to 162 mg daily. Conveniently, we have an 81 mg presentation in the United States, and it falls within the recommended range. It can be taken in the morning or at night, and adherence of >90% is associated with better prevention.7. When do we stop aspirin?Expert opinion recommends stopping aspirin at the time of delivery. 8. What are the contraindications to ASA use during pregnancy?Absolute contraindications to aspirin: -Patients with a history of ASA allergy (urticaria) or hypersensitivity to other salicylates are at risk of anaphylaxis and should not receive low-dose ASA. -Because of significant cross-sensitivity between ASA and other NSAIDs, low-dose ASA is also contraindicated with known HPS to NSAIDs. -Exposure to low-dose ASA in patients with nasal polyps may result in life-threatening bronchoconstriction and should be avoided.Relative contraindications are history of GI bleed, active peptic ulcer disease, other sources of GI or GU bleeding, and severe hepatic dysfunction.Aspirin is an excellent way to prevent preeclampsia in patients who are at high or moderate risk. Remember to think about the high-risk factors, and if your patient has only 1 positive, then aspirin needs to be started. Mainly, previous preeclampsia, diabetes, hypertension, multifetal gestation, and kidney or autoimmune disease. Look for moderate risk factors and start aspirin if the patient has 2 or more of those risk factors. _________________Conclusion: Now we conclude episode number 167, “Aspirin in Pregnancy.” Dr. Marquez explained that aspirin is started between 12-16 weeks of gestation to prevent preeclampsia in patients with at least 1 high-risk factor or patients with 2 or more moderate-risk factors. Dr. Arreaza also mentioned that aspirin is not for low-risk patients or for patients who already developed preeclampsia. As you know, preeclampsia can result in severe consequences for the fetus and the mother, but by preventing it, we can improve the chances of having a positive outcome in pregnancy. GK HPP WK 20 POST-WILDCARD WEEKEND & NINER BYE WEEK / 2023 – We enter week 20 following an exciting lineup of games over the “Wildcard Weekend”. The playoff bracket is filled out for the Divisional Round this weekend, and after resting for a week the Niners spent this week preparing to host the 7th Seed Packers after they demolished the Cowboys on the road (48-32 - and it was not even that close!). Packers looked good and owned the game from start to finish. Another disappointing end of season for the Cowboys (sorry Micha, laugh now - cry later I guess?), but we shall not feel a twinge of pain for them as we look to this most important game as playoff football comes back to Levi's on Saturday!! It is now win or go home and Niners are right where they want to be. !2 Seed, homefield through playoffs, and a healthy and loaded roster. Pull up a chair and a favorite beverage as we review the playoff bracket, the road to the Superbowl, and the Niners game against the Packers!! An unexpected stop-ride notice from SRAM, a new bike from Lotus, and thoughts on tyre widths and pedal choice gave our tech team plenty to discuss in this week's Geek Warning podcast.Rumours of a Lotus road bike have been circling for some time, but we didn't expect it to be an aero-e-road bike. The Type 136 takes design concepts from current Lotus-designed track bikes, adding a lightweight HPS mid-drive e-bike system for a complete claimed weight of just 9.8 kg. Lotus has launched the bike with a limited edition model with just 136 available at a cool price of £20,000 / €25,000, ouch.Ratio Technology – the company best known for bringing new life to old SRAM DoubleTap shifters – have done it again. The company's latest release is a new ratchet and pulley wheel kit for SRAM 10 and 11-speed mechanical road groupsets, allowing you to use a Shimano 12-speed cassette and chain.Speaking of SRAM, the American company has announced a stop-sale notice (for North America) related to its aftermarket 12-speed road levers. Set to become a voluntary recall, the notice applies to all SRAM 12-speed road shifters manufactured prior to July 2023 (it's a long list). And before you run to your local store, know the issue is simply that too much thread locker is applied to the clamp bolt, meaning the levers aren't clamping as tightly as what a torque wrench says. If you're already riding a bike with SRAM 12-speed then this one doesn't apply to you, but we'll keep you updated if things change. The biggest news in the bicycle industry has been related to the loss of funding for Signa Sports United, the parent company to Wiggle, ChainReactionCycles, and countless other brands. Now, Wiggle/CRC is in administration with reports of widespread redundancies in business segments responsible for future product and growth. The first mover in producing truly great kids bikes, Isla Bikes, announced it's ceasing production. It's a brand that James has fond memories associated with the first Isla bike he purchased, and it's a brand that will surely be missed by many.POC Sports have launched an LED-equipped version of its Omne road helmet. Integrated lights have been done before and are often somewhat underwhelming in terms of brightness, it'll be interesting to see how POC's new integration goes. After the news, we get into the recurring segments of the Geek Warning. We think that SRAM's Transmission gearing favourability seems to be largely related to geography and riding style. We discuss our preferences related to road tyre width. And then our new ‘Pick One' segment returns, this time we each share and justify our favourite road pedals.
