Kallie Fell started her professional career as a scientist in the Department of Obstetrics and Gynecology at Vanderbilt University Medical Center utilizing a Master of Science degree in Animal Sciences with an emphasis on Reproductive Physiology and Molecular Biology from Purdue University. While assisting in the investigation of endometriosis and pre-term birth, Kallie simultaneously pursued a degree in nursing with hopes of working with women as a perinatal nurse. After meeting Jennifer Lahl at a conference, Kallie became interested in the work of the Center for Bioethics and Culture (CBC) and started volunteering with the organization. Kallie is deeply passionate about women's health. She continues to work, as she has for the past six years, as a perinatal nurse and has worked with the CBC since 2018, first as a volunteer writer, then as our staff Research Associate, and now as the Executive Director. In 2021, Kallie co-directed the CBC's first documentary on “transgender medicine” titled Trans Mission: What's the Rush to Reassign Gender? In 2022 Kallie co-wrote and co-produced the CBC's latest film, The Detransition Diaries: Saving Our Sisters. Kallie also hosts the popular podcast Venus Rising and is the Program Director for the Paul Ramsey Institute. Her latest film, The Lost Boys: Searching for Manhood is set for release in January 2024 as is her new book, co-authored with Jennifer Lahl, The Detransition Diaries.

In a society that often claims to value the voices of the marginalized, one group that find themselves frequently silenced by the very people claiming to speak for them are detransitioners—men and women who have gone down the road of “gender transition,” only to change their minds, embrace their biological sex, and reverse course. We are in the midst of a transgender moral panic. Where only recently, very few people sought what used to be called a sex change, today the numbers of people seeking to “transition” to the other gender—particularly among children and teenagers—is becoming a flood. Much of the American medical establishment and the Biden administration claim that immediately yielding to children's feelings that they are not the sex they were born is medically necessary, life-saving care. But is the science really settled? Recently, the United Kingdom, France, Norway, and other European countries hit the brakes on immediate gender affirmation in children—to the point that the UK shuttered its largest gender clinic for children as unsafe for patients. Even the World Health Organization—under political pressure, to be sure—just admitted that “the evidence base for children and adolescents is limited and variable regarding the longer-term outcomes of gender affirming care for children and adolescents.” The UK's National Health Service concluded that instead of encouraging transition, “the clinical approach has to be mindful of the risks of an inappropriate gender transition and the difficulties that children may experience in returning to their original gender role.” Such “returns” are known as “de-transitioning,” a phenomenon that receives far too little attention, and when it does, too often sparks bitter denunciation of detransitioners among radical gender ideologues. My guest today has dedicated herself to raising the public profile of this important issue. Jennifer Lahl has directed, co-written, and co-produced three important documentaries on the subject. The first, Trans Mission: What's the Rush to Reassign Gender? explored the medical ethics of administering puberty blocking and cross-sex hormone in children. That film was quickly followed with the release of The Detransition Diaries: Saving our Sisters, which told the stories of three young women who transitioned to living life as if they were men—only to realize that they are, indeed, women. And. completing the trilogy, the just released, The Lost Boys: Searching for Manhood, in which five young men describe their experiences with gender dysphoria and their ultimate pursuit to find peace in their natural masculine bodies. Among her many accomplishments, Lahl is a documentarian and founder of The Center for Bioethics and Culture Network. Her writings have appeared in various publications including Cambridge University Press, the San Francisco Chronicle, the Dallas Morning News, and the American Journal of Bioethics. In the 22nd week of surrogate Brittney Pearson's pregnancy, she was diagnosed with an aggressive form of breast cancer. Because the necessary treatment could harm the baby, her doctors recommended inducing labor early and allowing the baby to be cared for in neonatal intensive care while she started chemo. However, the gay couple paying Brittney Pearson to serve as their surrogate did not want a premature baby with potential developmental or health problems. They wanted her instead to have an abortion. Pearson offered to put the baby up for adoption, but the men refused because, according to Pearson, they did not want a child who was genetically related to one of them somewhere “out there.” According to Pearson, the men threatened both her and her doctors with a lawsuit if she did not abort her child. Because of California's radical surrogacy laws, which allow financiers of a surrogacy arrangement to be granted legal parental rights of the baby before he or she is born, they likely would have prevailed. In an interview with Jennifer Lahl of the Center for Bioethics and Culture, Pearson told her story. Her son was born at 25 weeks of pregnancy, a gestational age that, thanks to advances in maternal medicine, children have survived. Though she has not publicly stated whether her son was killed before or after delivery, or whether he was given or denied the medical treatment a premature baby needs, she has confirmed that her son died the day he was born, which was Father's Day. Though, of course, not every surrogacy contract ends this way, Pearson is not the first surrogate mom pressured to kill her baby by those paying for it. However, it would be a profound mistake to think of hers as a case of “surrogacy gone wrong,” as advocates of the practice claim about stories like hers. Each and every moral violation that occurred along the way was not exclusive to Pearson's unique circumstances. Rather, they are violations endemic to surrogacy itself, a practice that denies children the right to their mother and, at times, their father, and denies a mother the right to her own child. Children are treated as products to be purchased and arranged, subject to property laws and other legal realities long used to dehumanize certain individuals. It's jarring to hear these men talk of Brittney Pearson's baby as if he were a lamp ordered off Amazon that was delivered broken. However, it is the surrogacy contracts signed ahead of time that treat human babies as commodities. If it seems like an obvious violation of human rights and God's moral order for two men to demand that a woman have her baby killed, what should we make of the legal contract governing the baby's creation, gestation, and delivery in the first place? If babies are treated like products and pregnancy like the means of production at the beginning of the surrogacy process, ought we not to expect things to be different at the end? Human beings are made in the image and likeness of God. Thus, they should never be treated like any other thing. Like the social research that shows how extended exposure to violent video games and media can desensitize people to actual violence, surrogacy is among those cultural realities that reveal how much our view of children has been desensitized by all that constantly reduces them to “things.” As one gay man who sued his employer last year for refusing to pay for him and his husband to hire a surrogate put it, children are just one of the modern “trappings” of “marriage,” like a “house, children, [and] 401k.” The willful death of Brittney Pearson's son is a tragic, but logical, escalation of the moral errors fundamental to surrogacy. As marriage and family are increasingly deconstructed, reimagined, and replaced in law, the demand for surrogacy will only increase, and more babies will face the same kind of danger as Pearson's baby. If we don't stand up for them, who will? And if we do, it will require standing against this practice, which is fundamentally disordered and always wrong, and not merely against selective cases. In this week's episode, Kallie Fell and I discuss the nuances of informed consent in reference to maternity care along with the in's and out's of risk mitigation during pregnancy, physician concerns and pressures, the what may be best for children in the long run. This is an especially sensitive topic for families and Kallie Fell does an amazing job of providing context and understanding for the various decisions that parents are faced with during pregnancy. Kallie began her professional career as a scientist in the Department of Obstetrics and Gynecology at Vanderbilt University Medical Center utilizing a Master of Science degree in Animal Sciences with an emphasis on Reproductive Physiology and Molecular Biology from Purdue University. While assisting in the investigation of endometriosis and pre-term birth, Kallie simultaneously pursued a degree in nursing with hopes of working with women as a perinatal nurse. Kallie has worked with the CBC since 2018, first as a volunteer writer, then as a staff Research Associate, and now as the Executive Director. Kallie is, and always has been, passionate about women's health. She continues to work, as she has for the past 6 years, as a perinatal nurse. Jennifer Lahl, author, documentarian, and founder and president of the Center for Bioethics and Culture Network returns to the podcast today! Jennifer has been fighting assisted reproductive technology for 20 years. Her interest was peaked when her college-aged daughters were targeted for egg selling. It was during the height of the stem cell research craze, and she realized that this profit-driven medical research was being built on the backs of vulnerable young women. Her first film, Eggsploitation earned her the wrath of Big Fertility then, but they've hardly celebrated her work since. In her most recent article she asks, “Who Owns the Human Body?” examining the ethical implications of using a brain dead woman's body as a surrogate. If a woman has agreed to be an organ donor, does it follow that her body could be kept on life support in a vegetative state in order to gestate a child for an infertile couple? What about the wellbeing of the fetus, which will grow surrounded by the sound of beeping monitors instead of the sound of its mother's voice? In our discussion with Jennifer, she puts forward her theories about what she calls a “superhighway” that confidently shuttles people towards risky medical interventions; this happens, according to Jennifer, both in the realm of fertility and gender. Fertility is an area that Stella and I know very little about so it was interesting to hear Jennifer share her experiences as a nurse and filmmaker who's been following this topic very closely. She holds some very strong, but thought-provoking views on the medicalization of fertility that some are sure to find controversial. Ultimately, Jennifer's biggest concern is that any patient, whether they are pursuing interventions in either fertility or gender medicine, should be fully informed about the risks involved, and the outcomes. She also highlights that sometimes a lack of evidence underlies the model of informed consent. This poses the question: how can you consent to something if we have very little or no evidence about it? In this episode we talk about the truth behind surrogacy, why a surrogate birth is so much riskier than a natural birth and the mental health impact on surrogates who have to give up the children they give birth to, back to the parents who have paid for it. We ethically explore whether there can ever be a morally correct path for someone to be a surrogate, are arguments against surrogacy homophobic and the statistics behind surrogate deaths post-childbirth. For Jennifer's mental health, Jennifer spent an entire year in hospital as a 10 year old which had a huge impact on her life. We discuss the negatives of it and the positives in how it inspired her to take this healthcare journey, her parents divorce as an adult and the toll that working in critical care took on her mental health.
