Dr. Richard L. Oehler, Professor of Medicine at the Division of Infectious Diseases, Morsani College of Medicine, reviews two important clinical syndromes caused by Spirochetes. Dr Oehler begins by presenting an actual clinical case of Leptospirosis in a returning traveller from the Caribbean. He then discusses the epidemiology of Leptospira, a widespread zoonotic pathogen that favors temperate and topical climates worldwide. He then relates the life cycle, means of transmission, microbiology, clinical manifestations, and most relevant diagnostic studies. He concludes the leptospirosis section by discussing treatment and prevention. The second case he discusses is a febrile illness in a returning traveler from Jordan. Dr. Oehler goes on to discuss the epidemiology and classification system for relapsing fever (RF), and its broad categorization into tick-borne (endemic) and louse-borne (epidemic) forms. He further describes the characteristics of the soft-shelled Ornithodoros ticks and the Human Body Louse, and differentiates the Hard Tick Relapsing Fevers such as Borellia miyamotoi and B. lonestarii. He then reviews the clinical manifestations, diagnostic criteria, and treatment options for each form of RF. Lastly, Dr, Oehler breaks down treatment and prevention strategies for each form of relapsing fever.

Dr. Richard Oehler, Professor of Medicine at the Division of Infectious Diseases at the University of South Florida Morsani College of Medicine, presents a review of bacteremia. Dr. Oehler begins by reviewing the epidemiology, personal and institutional costs of bacteremia. He then discusses blood culture collection and how false positive blood cultures occur. With the recent blood culture bottle shortage of 2024, he emphasizes the new concept of “blood culture stewardship”–when it is most appropriate to order a blood culture versus when it may be unnecessary. Next, Dr. Oehler reviews automated blood culture systems and other related identification technology, with a focus on new methods of blood culture isolate identification such as multiplex PCR and Maldi-TOF. The speaker then contrasts gram positive and gram negative bacteremia in general and by organism. The management of a positive blood culture with regard to treatment and source control is discussed. Lastly, ways to reduce the incidence of bacteremia are shared.

Manson's work has been shown in group shows at institutions such as Ballroom Marfa in Texas, and the Center for Craft, Asheville, NC, and at Tribeca Park in New York. Elisabeth takes a holistic approach to athlete development in her coaching to make her athletes stronger, faster, and more powerful. Besides the physical performance aspects, she helps her athletes to develop mental toughness, integrity, the ability to work in a team, discipline, respect, and leadership skills on and off the field or competition floor. Elisabeth is currently undertaking a Professional Doctorate in Elite Performance at Dublin City University.

In his presentation, Dr. Oehler initially covers Nocardiosis, followed by Actinomycosis, and compares and contrasts both and how they produce human infection. Topics covered for both infections include their taxonomy, epidemiology, types of patients infected, infectious syndromes, potential complications, recommended therapies, and duration of treatment. At the end of the talk both Nocardiosis and Actinomycosis are directly compared to help summarize the important learning points presented.

Dr. Richard L. Oehler, Professor of Medicine at the Division of Infectious Diseases, Morsani College of Medicine, reviews two important clinical syndromes caused by Spirochetes. Dr Oehler begins by presenting an actual clinical case of Leptospirosis in a returning traveller from the Caribbean. He then discusses the epidemiology of Leptospira, a widespread zoonotic pathogen that favors temperate and topical climates worldwide.He then relates the life cycle, means of transmission, microbiology, clinical manifestations, and most relevant diagnostic studies. He concludes the leptospirosis section by discussing treatment and prevention . The second case he discusses is a febrile illness in a returning traveler from Jordan. Dr. Oehler goes on to discuss the epidemiology and classification system for relapsing fever (RF), and its broad categorization into tick-borne (endemic) and louse-borne (epidemic) forms. He further describes the characteristics of the soft-shelled Ornithodoros ticks and the Human Body Louse. He then relates the clinical manifestations, diagnostic criteria, and treatment options for each form of RF. Lastly, Dr, Oehler breaks down treatment and prevention strategies for each form of relapsing fever.

Dr. Oehler begins by reviewing the epidemiology, personal and institutional costs of bacteremia. He then discusses blood culture collection and how false positive blood cultures occur. Next, Dr. Oehler reviews automated blood culture systems and other related identification technology. The speaker then contrasts gram positive and gram negative bacteremia in general and by organism. The management of a positive blood culture with regard to treatment and source control is discussed. Lastly, ways to reduce the incidence of bacteremia are related.

In this episode I sat down with Matt Oehler again for a part 2 because in the first recording, Matt shared with me the intense experience he had as a young man in New York actually witnessing 911 as it happened. Then, offering aid as a trained Marine reserve attend to the injured and the dead that day, to going full time into the Marines to serve our country in Iraq surviving one of the areas most deadly attacks on his group to now, years later having served our country again as a law enforcement officer putting his life on the line year after year and what we can learn from that. We ran out of time in the first session and part two needed to be heard. As I sat and listened to Matt unfold his story of law enforcement, I realized even more clearly that he is a good man, a husband, father, and son. He loves to serve others. He would do anything to help another in trouble. And we sit back and hear judgement against these good men and women to serve and this behind the scene needed to be heard by not only me but anyone out there wanting to know some truth about these heroic people! Matt had many close calls with death in these years of Laws enforcement and he saw death take place all around him for years. That changes ones perspective on life and how precious it is. When a retired librarian in Oregon started corresponding with a man in a Louisiana prison, neither of them could have imagined it would lead to his release and exoneration. Yutico Briley was sentenced to 60 years without parole at the age of 19 for an armed robbery he didn't commit. He wrote to journalist and author Emily Bazelon, pleading with her to cover his case and the many injustices that lead to his conviction. His letter remained unread until Briley's longtime penpal, Karen Oehler, emailed Bazelon and urged her to look at it. Bazelon chronicled the years-long struggle that followed in a recent New York Times Magazine article. After he was released in March, Briley visited Oehler in Oregon. We hear from Briley and Oehler about how their ongoing friendship has affected both of their lives.
