Presented by 6D Helmets There was a collective groan around the world from every Supercross fan when Eli Tomac clipped the back of his left leg with his footpeg and pulled off the track during the Tampa Supercross timed qualifying sessions. The event was too reminiscent of when Tomac ruptured his Achilles tendon while leading the championship in 2023, even down to how he pulled off the track in disbelief. Unless you've been living in a cave, you know by now that Tomac raced through the pain in Tampa and finished 17th in the main event aboard his Yamaha YZ450F. Later that week, further examination revealed that he had broken his fibula, making his 17th a lot more impressive than it might be on paper. Tomac had the broken bone plated back together and is at home recovering, with plans on "returning before the end of Supercross to get some gate drops to be ready for the MX Nationals." We enjoyed a great conversation with ET3 in this installment of the 6D Helmets Midweek Podcast. Enjoy!

Herzlich willkommen zur Episode 30, einer Jubiläumsfolge und vor allem die erste Folge nach der kurzen Sommerpause. Ich hoffe, ihr habt die Sommerpause gut genutzt und seid ein paar Kilometer mit euren Vespas durch die Gegend gefahren. Das Wetter war ja gar nicht so schlecht. Was erwartet euch jetzt in der Jubiläumsfolge 30? Ich habe mich mal wieder richtig weit in den Norden begeben, und zwar auf die Insel. Leider konnten Billy und ich uns nicht live und persönlich treffen, sondern haben das Ganze per Zoom gemacht.Ein kleiner Hinweis schon mal vorab: Die Tonqualität ist diesmal auf meiner Seite nicht die beste. Ich hoffe, ihr seht es mir nach. Ich arbeite weiterhin daran. Es ist wirklich so ein Running Gag – irgendwas ist immer.Billy und ich kennen uns jetzt schon ein paar Jahre und haben uns immer mal wieder ausgetauscht. Wir haben uns allerdings nur selten live gesehen, aber Billy ist jemand, der, obwohl er so weit oben in Schottland lebt, sehr häufig auf den Vespa World Days oder anderen Veranstaltungen anzutreffen ist. Was mir an Billy immer wirklich gut gefällt, ist die gute Laune, die er verbreitet, wenn er auf den Treffen ist. Er hat immer den Schalk im Nacken, und das macht ihn für mich persönlich total liebenswert. Wir haben darüber gesprochen, wie er zu seiner ersten Vespa gekommen ist – damals noch knapp bei Kasse – und wie sich das im Laufe der letzten Jahre immer weiter entwickelt hat. Warum er eine kalifornische Vespa, eine ET3, fährt, wie er dazu gekommen ist, wie viel er gerade gesammelt hat, Und wie er es anstellt, wenn er von Schottland aus erstmal nach Mitteleuropa kommen muss – zu den Veranstaltungen in Italien, Deutschland, Ungarn, Portugal oder Frankreich. Wir haben auch darüber gesprochen, was die Vespa-Familie für ihn so besonders macht und warum er sich in dieser Gemeinschaft engagiert. Lasst euch also überraschen, was Billy uns über die schottische Vespaszene zu erzählen hat. Der eine oder andere wird vielleicht nicht alles verstehen, denn manchmal rutscht Billy ins Schottische ab, was ja doch etwas anders ist als das Oxford-Englisch, das die meisten von euch kennen. Although known for his ability to widely span many different genres, Luna Bear's love for halftime deep dub and bass music are what keep driving him forward and further into the industry. As the owner/founder of Ubuntu Productions based out of Billings, MT he is always hosting local events, and is regularly involved with other local and regional events as well. He's also been playing a monthly Ecstatic Dance event for over a year now at Studio Soul in Billings. He's played a handful of events for Foundation Entertainment and Music Lives Here in Bozeman, MT as well as sharing the stage with local and regional artists including Beautiful Existence, DAWge, ET3, Danktech, and more. In the infancy of his own personal journey as an electronic producer, he looks forward to learning even more about the industry and creating his own music. A few words from Luna Bear. I've been in the music world for most of my life, but DJing was the first musical art that gave me full control over all the different things and ideas I wanted to try going on in my head musically. Electronic producers are continually pushing the boundaries of sound design and helping drive the sound engineering behind sound system culture. I really enjoyed getting to put this mix together. In this episode of EMS One-Stop, Rob Lawrence is joined by Doug Wolfberg, Esq., of Page, Wolfberg & Wirth to discuss the immediate actions organizations should take (or should have already taken) to preserve documentation relating to the PHE, as well as adjust operational and documentation practices and procedures as we “return to normal.” Rob and Doug discuss issues such as rule changes that have become normal operating procedures over the last 2 years and the need to build a time capsule to preserve evidence. Technologist Jonathon Feit has often said that the greatest innovations serving Mobile Medicine (Fire & EMS) come from rural spaces. The most obviously reason, perhaps, is that agencies have fewer resources to begin with, and it takes longer for their neighbors to arrive on-scene, therefore they have no choice but to learn how to do more for themselves. This has always been the hallmark of those who live and work where the wild spirit of self-reliance abides. But there is something else, too: big skies, clean air, and the ability to better afford life and spend less time in traffic tend to attract those who need to be able to afford aspects of life that would be impossible where the norm is sitting in traffic going back-and-forth from long hours in an office: for example, the time to care for a child or parent who is seriously ill or disabled and need more regular attention. For this reason, while they have fewer available resources that most cities, countrysides tend to attract a higher concentration of people who themselves — or whose parents or children — are more likely to need emergency healthcare. That sets up a Catch-22 and an important risk of health inequity that rural Mobile Medical agencies navigate around the clock. Reuben Farnsworth, who spearheaded Community Paramedicine, ET3, and other specialty care programs at Colorado's Delta County Ambulance District — an agency that has won statewide honors, including as the first ambulance service to interoperate with the Quality Health Network health information exchange and local hospitals — will explain how a data — and economics-centric management style has powered his service's success, from clinical medicine, to education and policy development, to advancing legislative priorities with partners.

In this episode, we continue our conversation with Matt Zavadsky of MedStar Mobile Healthcare about the role that Emergency Medical Services has in value-based care and how the economics come together to provide value. Transcript: How has CMS come to realize the value of the role EMS has in health care through models such as Emergency Triage, Treat, and Transport or ET3? Yeah, and Medicare for most EMS agencies is our largest payer, right. Whether it's regular fee for service or Managed Medicare. And they've been one of our audiences for a decade with us telling them, you know, if you keep paying us to transport, again, you're just incentivizing us to spend your money. So, they finally, through the CMI, the Center for Medicare and Medicaid innovation, put together a model, that we actually helped work on with them, that changes the economic model for EMS. And it now reimburses EMS agencies not just for transport anymore, but for things like nurse triage in the 911 center. If you can mitigate that 911 call and not have to even send a resource, and you can get them connected with their doctor, and you've triaged them over the phone, that's reimbursable now for select agencies that are a part of this program. If you get on scene, and you do an assessment, and you do a telemedicine connection with a physician, and that physician agrees that this patient does not need to go to the emergency department, they can be treated at home, and go see their own doctor, we're now reimbursed for that. And so, by the way, is the doctor that's doing the telemedicine. Similarly, if we transport a patient to an alternate destination. So, yep, you know what, you twisted your ankle. You might need an x-ray, but you don't need to go to an ER for that. Let's take you to an urgent care that has x-ray. It's a third of the cost and certainly much more patient-centric perhaps than going to a busy emergency department. And we transport them to an urgent care. That model revolutionizes how EMS is delivered. We've often said that, you know, Medicare does not set health care policy, but they do set payment policy. And as they change payment policy, they change the practice of health care. So now we, and about you know 40 other agencies around the country that have been approved by Medicare to test this model, are not transporting every Medicare patient to the ER anymore. We're offering them an opportunity to stay home. I did this myself with a patient a couple weeks ago and he loved it. His wife was thrilled. He was in a lot of sciatic pain. We gave him some pain medication, talked to the doc, we had an urgent care, mobile urgent care, come out to the house later in the afternoon, and we didn't have to put him on a stretcher, and take him down a bumpy road, and take him to a busy ER where he's going to languish for 2-3 hours because you know he doesn't have a high priority medical complaint. And the patients love it and it's much less expensive and everybody benefits. So those are the types of models that we're continuing to test with now other payers We just signed a in-network agreement with one of our largest commercial payers where they're paying us the same way. “Hey. We'll pay you the same whether you take somebody to the hospital or not. We prefer that you not, if you don't have to, because it's going to save us you know $5,000 cost of care at the end of the day.” So those are the types of things that Medicare, and other payers, and a lot of Medicaid offices across the country are starting to say, “Yeah. This is a much better economic model for EMS. Let's pay them for patient navigation, not just for patient transport.” How have telemedicine waivers allowed EMS to do on-scene patient navigation? The telemedicine waivers that were, that have been and they're still in place, because of the public health emergency, has really stepped up the ability for EMS to use telemedicine in the patient's home. Oregon Paramedic Maurice “Mo” Stadeli lost this fight with Cancer on February 25th, 2019. Mo was an amazing public servant who touched many lives. Mo served with multiple agencies in his long and amazing 30-year career. He spent most of his time at Salem Fire at station 6. Cheers to you, Mo….We have the watch.
