AaNeel Infotech worked with Medstar Health to transform an isolated risk calculator into a FHIR-based app. Upendra's company helped them use the SMART on FHIR methodology. That's FHIR as in “Fast Health Interoperability Resources” and SMART as in “Substitutable Medical Applications and Reusable Technologies”. Using the SMART on FHIR approach, AaNeel Infotech helped create an app called “Mobilizing a Million Hearts”, which integrates the Million Hearts Longitudinal Atherosclerotic Cardiovascular Disease risk calculator into the MedStar Health EHR system and allows Medstar providers to get an even more comprehensive view of their patients at risk for cardiovascular disease.

In this session, I am speaking with Neel Bhanderi on the advantages of high-fidelity training within clinical practice. In the conversation, we examine the concepts of analogs of reality and how it has multifactorial benefits through the integration of handover simulation, debriefing, stress inoculation, skill acquisition under stress, communication under applied stress, and much more. To do this I have Dr. Neel Bhanderi with me. Neel is the Head of Education and Training at MedSTAR retrieval HEMS service based in Adelaide. He is also a Consultant in Emergency Medicine at the Royal Adelaide Hospital. Neel trained as an Emergency Physician and worked as a Consultant at St George's Hospital Major Trauma Centre in London for several years and for Kent Surrey and Sussex (KSS) Air Ambulance. In 2016, Neel emigrated to Australia where he works as an Emergency Specialist at the Townsville Hospital in North Queensland and is currently the head of education for MedSTAR retrieval service. In the conversation, we examine: • Why high-fidelity simulation is useful & Neels approach to multi-modal simulation. • What components of training do MedSTAR focus on (Skills based, assessment-based, comms based or all three) – templates of training outcomes used. • How Neel approaches the training debrief. • How Neel creates incremental training stressors within the operational teams. • Environmental stress training methods (operational Task and environmental noise). • Approach to micro versus macro tasks • Comms under pressure within the training environment • Handover and how MedStar simulates this (filming, use of the checklist, analyzing) • Training relationship with case review (identifying training needs from case review). • How Neel is changing training in the short term (other elements such as personality and characterological traits). In this episode of the Brawn Body Health and Fitness Podcast, Dan is joined by William "Will" Cooledge to share part 2 (of many) of Will's ongoing ACL journey. The two share insights into Will's rehab process in addition to outlining challenges which may present themselves throughout an ACL recovery. Will joined the Athlete Academy team after completing his Internship through Salisbury University, where obtained his bachelor's degree in Exercise Science. Since his time at The Athlete Academy, he's quickly moved up from a Sports Performance Coach and now heads the Sports Sciences Department. He's Coached many Athletes from Youth, to High School and College DI-DIII levels. In addition to Training at The Athlete Academy, he is a Head Travel Soccer Coach for SUSC & the Assistant Girls Varsity Soccer Coach for Stephen Decatur High School. He is a Certified Speed & Agility Coach through the National Sports Performance Association and has a passion for Sports Sciences and Sports Performance Improvement. Will experienced an acute knee injury while playing pickup basketball on 10/14/2022 with his friends. He sought eval from Dan and later an MRI which confirmed a tear of his right ACL. Will was able to work with both Dan and Dr. James Dreese at Medstar to learn about his surgical options, including graft choices, overall rehab prognosis. After extensive pre-operative PT interventions, Will underwent a lateral meniscus repair, ACL reconstruction via patellar tendon graft, and ALL procedure by Dr. Dreese on 12/8/2022. He began his post-op rehab with Dan only 18 hours after his surgery concluded. Will has been working diligently to regain use of and restore proper function of his knee, and has decided to team up with Dan to share his journey through podcast episodes and social media posts in attempt to educate others on the post-operative recovery process and inspire those who are currently rehabbing an ACL injury. Creekside residents occupy bridge

Residents at Creekside Cabins occupied the temporary bridge between the RV park and Highway 101 last night, counter-blocking an excavator that was moving into position to prevent access to the property. Residents originally had between 8:00 am Wednesday and 5:00 last night to evacuate. Now, after last night's standoff, they'll be allowed another hour this morning to get off the property that's been declared a public health emergency after a sinkhole opened up in the driveway, blocking access. By quarter after five last night, nine trailers had been hauled, most of them by a driver for United Disaster Relief of Northern California. Passenger cars and pickup trucks had been streaming over the bridge for hours, and at least one mobile home containing a family with six kids had inched onto the highway, belching smoke and smelling of bad brakes. Amid conflicting information and spotty cell phone and internet access, residents didn't fully understand the immediacy of the situation until very close to the deadline. At 5:13, with sunset approaching and a line of cars creeping toward the exit, a crewmember began to move the excavator onto the bridge. Shaylene Harvey walked towards him with her dog, a four-year-old Australian shepherd named Lilly. We can't repeat most of what she had to say, but before she sat down on the bridge, she implored the crew to, “Just let the last few people out.” “Let us be homeless on this side of the bridge!” she screamed, as Randy and Mitzi Feta began to cross in their car, fully loaded with personal belongings, houseplants, and two huskies. Their trailer, packed to capacity, was left behind as the huskies howled and Mitzi sobbed into her hands. “I'm trying to enforce the order,” the crewmember said, after backing the excavator away from the bridge to the side of the road and disembarking. Another resident wept as he described his situation. “They paid a quarter million to put this bridge in,” he said. “And they could have put a permanent one in for way less than that…I don't know why they did this, and now they gave us two days…I have three children, a five-year-old, a twelve-year-old, and a seventeen-year-old, that I have to find a house for.” Everyone on the premises was committing a misdemeanor, but no was cited, as a Highway Patrol Officer arrived on the scene and asked frantic residents to explain what was going on. “Why do you want to get your trailers out?” he asked, amid the sound of heavy equipment backing up, people screaming, and the dogs voicing their displeasure. “Because we're all going to be homeless,” Harvey exclaimed. “Do you not know what's going on?” He did not. Residents briefed him at the scene, as more highway patrol arrived, quickly reinforced by sheriff's deputies. Shortly after 6:00 pm, a MedStar ambulance and Willits fire truck arrived, responding to a woman experiencing an oxygen-related medical emergency inside the park. We spoke with Randy Feta just as the sirens subsided and firefighters spilled out of the truck and ran across the road, over the bridge, and into the campground. “The thing is, they gave us some directions to get out of here, otherwise they're shutting the place down, that's fine and dandy,” he said. “We followed the directions, we packed, we're ready to go, now we got no place to sleep. My car's packed, my trailer's packed, there's no room, no nothing. We're ready to go. Now they won't let us go…I want to get out,” he exclaimed. “I packed all day. I got a bad back, a bad neck, I'm disabled, I'm elderly, and I packed hard to get ready to go. I'm just following the directions. I did everything.” The Fetas had packed so tightly that the bed in their trailer was inaccessible, so they slept in their car last night. On the bridge, occupying residents moved out of the way to allow Manny, another resident we spoke with earlier this week, to cross over and get out. He told us yesterday he spent the whole day getting his trailer registered, but he wasn't hauling it with the pickup truck he'd been working on. When we walked into the park in near-total darkness hours later, his trailer was still there, with the lights on. One former resident was on scene with a one-ton truck, offering to haul people out for whatever they could pay. Aaron Rusty Deeson observed that, “Unfortunately, most of the residents out here have not been able to afford to purchase or maintain a one-ton vehicle to haul these large trailers they live in with. About half the residents still have all of their belongings here. Most of the residents spent the last 24 or 48 hours getting everything ready. At the last minute, they were ready to go, but there were just not enough vehicles available to get them out of here in a timely manner. I mean, it's been chaos.” There is one road in the campground, heavily potholed and barely wide enough to be called a one-lane thoroughfare. At the back of the property, so far from the road it was impossible to hear the commotion, 62-year-old John Fails, who has been at Creekside Cabins for 25 years, stood in the dark and sounded stunned that he'd been unable to get a hauler's attention. “Basically, I'm being put homeless,” he said. “And it's not right. I've been ready to go. I actually stopped him two or three times today and said, yeah, I'm ready. And he hooked up this guy, and then he went and hooked up the girl at the end, and the whole time, I'm waiting. And he just drove right on by and never came back.” Sheriff Matt Kendall was fairly sure there would be a chance to get out first thing in the morning. “If they can get lined up at the bridge in the morning, they're going to give them about an hour, is what I was told,” he said. “But you have to remember, this contractor has gone into a contract and has very specific permits about what has to be done for the fish and game remediations,” he said. “Nobody wants to get on the wrong side of those…And we have to remember, these road workers out here, they have a safety plan, and they have to do these things, because this work is absolutely incredibly dangerous…Every one of these guys deserves to go home to their wives and families.” In the meantime, Aaron had gotten the guarantee he was waiting for: that he could get back out if he drove deep into the property to pick up one old man who had been left behind. Everyone got out of the way as John Fails' trailer trundled over the bridge spanning the sinkhole, then slowly crept off into the night.

Transcript: Tell us a little bit about MedStar, what your organization does, and how it has impacted the communities in which it operates. Thanks for asking. MedStar is the trade name for a public authority called the Metropolitan Area EMS Authority. We are a regional, governmental administrative agency that is created by 15 member jurisdictions to provide emergency medical services across all 15 of those cities, irrespective of city boundaries. It's a, again a regional public authority. The challenge was that the Metropolitan Area EMS Authority was way too many letters to try and put on the side of an ambulance. So, we when the, when the Authority was formed the community had a naming contest back in 1986 and MedStar was the name that was chosen. So we are that public authority. We provide 911 and non-emergency medical services, emergency medical services, to about 430 square miles with 1.1 million population. Fort Worth is our largest member jurisdiction. There are 14 others. And we do so without any tax subsidy which is a little bit unusual being a public authority much like you might think about a transportation authority or an airport authority, but we receive no tax dollars. So, it's a very high-performance EMS system. What is mobile integrated healthcare and how does it reduce utilization? What are some of the proactive measures being done? Mobile integrated healthcare is a term that has been used by the EMS profession to categorize services that we are able to provide that may or may not be the result of a 911 response. Most people think of an EMS agency as you know group of experts who hang around a station, wait for a 911 call to occur, and then we respond, mitigate the emergency, and then shlep people off to an emergency department. What we've learned over time is that there's a certain portion of our population who could benefit from some proactive education, medication management, connection with other resources in the community, maybe re-connection with their primary care network, to actually prevent a 911 call. So, the term mobile integrated healthcare is really that umbrella term that refers to all of the things that EMS agencies can really do to improve the health of populations, to reduce the expenditures of the health care system, and to most importantly improve the patients experience of medical care. What are some of the healthcare roles that EMS has transformed? So that's a great question. And if you think about it, one of the major transitions and transformations has been that prevention of the 911 call. We have always been reactive as a profession. EMS agencies, sort of by nature, react to a 911 call. But what we're doing now, and at MedStar and a number of other agencies across the country, is working with partners, payers, hospital systems, home health agencies, hospice agencies, ACOs, to fill a gap that still exists in our health care system and those gaps are different depending on the population and the partner that we're working with. So for example, a hospital system has a bunch of frequent flyers that come to the emergency department for ambulatory care sensitive conditions. Those things that really had they seen their PCP or their primary care system, that ER visit would have been avoided. They identify those patients, refer those patients to us, we go visit them with specially trained community paramedics who are trained in things like motivational interviewing and social determinants of health, in addition to doing the typical things that paramedics do, 12-lead EKGs, medication administration, vital signs assessment, following protocols. So now you've got these community paramedics who can reach out to the person who...

In that role she was the acting Program Director for Georgetown's Executive Master's in Clinical Quality, Safety and Leadership program.  Prior to that Crystal has held various roles at MedStar where she has skillfully led the development and implementation of system-wide strategies for high reliability and patient safety, including the development and implementation of the Care for the Caregiver program, the Patient Communication Consult Service, and an event review process after adverse events. Crystal is exceptionally passionate about her work and enjoys making connections and working in diverse teams. Welcome back to The Sports Docs Podcast with Dr. Catherine Logan and Dr. Ashley Bassett.   In episode 16, we're going to continue our discussion with Dr. Anand Murthi and focus on the surgical treatment options for irreparable rotator cuff tears.We'll start with the article titled “Minimum 2-year Clinical Outcomes After Superior Capsule Reconstruction Compared With Reverse Total Shoulder Arthroplasty for the Treatment of Irreparable Posterosuperior Rotator Cuff Tears in Patients Younger Than 70 Years” from December 2020 issue of JSES. Peter Millett and colleagues at the Steadman Clinic in Vail reported significant improvements in clinical outcome scores, high patient satisfaction and high rates of return to sport at 2-years post-op, with no significant differences between the two procedures. The authors concluded that SCR is a reliable alternative to arthroplasty for younger patients with irreparable cuff tears in the absence of glenohumeral arthritis. SCR has the benefit of being an arthroscopic procedure that preserves native anatomy, allowing for easier revision surgery in the future. Then, from the June 2019 issue of JBJS, we review the publication “Biomechanics of a Biodegradable Subacromial Balloon Spacer for Irreparable Superior Rotator Cuff Tears.” Authored by our guest Anand Murthi and his team at MedStar, this laboratory study found that placement of a subacromial balloon spacer in the setting of a massive irreparable supraspinatus tear restored glenohumeral contact pressures and the center of glenohumeral contact to that of an intact rotator cuff. Additionally, placement of the balloon increased the deltoid load and lowered the humeral head, increasing the acromiohumeral interval and decreasing subacromial contact pressures. The authors concluded that the subacromial balloon spacer may help overcome the biomechanical effects of a deficient cuff and thereby postpone the need for arthroplasty.Lastly, we finish up with a systematic review from the July 2020 issue of JSES titled “The Lower Trapezius Transfer: A Systematic Review of Biomechanical Data, Techniques and Clinical Outcomes.” Two tendon transfers have been described for treatment of irreparable posterosuperior rotator cuff tears: latissimus dorsi and lower trapezius. Compared to traditional latissimus dorsi transfer, the lower trap transfer is more anatomic and better restores shoulder abduction and external rotation. One drawback of the lower trap transfer highlighted by this paper is limited tendon excursion requiring the addition of a graft. In the setting of a cuff tear, direct transfer to the tuberosity without graft extension increases muscle tendon strain and risk of failure, and therefore is not recommended. Lower trap transfer can be performed through an open or arthroscopically-assisted approach while latissimus dorsi transfer can also be performed entirely arthroscopically.
