He also teaches EMS law at the University of Pittsburgh, and teaches health law at the Widener University School of Law, where he is also a member of the school's Board of Overseers. Doug is a known as an engaging and humorous public speaker at EMS conferences throughout the United States. He is also a prolific author, having written books, articles and columns in many of the industry's leading publications, and has been interviewed by national media outlets including National Public Radio and the Wall Street Journal on EMS issues. From his bio on Physicians for a National Health Program (PNHP), “Dr. Phil Caper received his BA, MS and MD degrees at UCLA, and trained in internal medicine on the Harvard Medical Unit at Boston City Hospital. He has held professorships at Dartmouth Medical School and the University of Massachusetts Medical School, where he was also Vice-Chancellor for Health Affairs, chief of the medical staff, and hospital director. He has been an adjunct lecturer on health policy and management at the Harvard School of Public Health, a research associate at Harvard's Kennedy School of Government and an associate in health policy and management at the Johns Hopkins School of Public Health. This episode: At the outset of this series, we invited anyone interested to participate in a Zoom call to help us gather information on what folks saw as major challenges facing Maine people during the lifetime of those alive today. One of those challenges mentioned by several of the participants on that call was affordable and accessible health care. It's important to note right at the beginning of today's program that health care and health insurance are two different things that are sometimes conflated. Health care refers to the care that providers such as doctors, nurses, therapists, and others provide to people with health issues. How to pay for that care is a separate challenge, often provided in part by either for-profit insurance companies or government programs such as Medicaid, for those with very low incomes; or Medicare, for those age 65 and over. On today's program, we will speak with two retired physicians who, over long careers, took somewhat different paths but wound up at the same conclusion about how to provide Maine people, and all Americans, with affordable, accessible health care. Guests: Dr. Geoff Gratwick practiced medicine with a specialty in rheumatology in the Bangor area, and in clinics across Maine for over 40 years. He eventually became so concerned about what he saw as problems with Mainers getting access to quality health care that he ran for the Maine Senate after serving 9 years on the Bangor City Council. He served four terms in the Maine Senate before leaving because of term limits, and while there served on the Opioid Task Force, as well as on several legislative committees. He was a key player in establishing the state's Health Care Task Force which has been charged with determining how to make health care in Maine universal, affordable, accessible and of high quality. Dr. Phil Caper, in addition to practicing as a physician, spent a good part of his career in policy areas related to health care. From 1971 to 1976, he was a professional staff member on the United States Senate Labor and Public Welfare's subcommittee on Health. He served on the National Council on Health Planning and Development from 1977 to 1984, chairing the panel from 1980 to 1984. He has also taught at Dartmouth Medical School, the University of Massachusetts Medical School and the Harvard School of Public Health, and worked in private industry trying to improve the technology of hospital medical records. Dr. Deyana Lewis' academic career began at the University of Illinois where she earned her Bachelor of Science Degree in Health Planning and Administration. After leaving the University of Illinois, she worked at Washington University’s McDonnell Genome Institute where her analytical and computation skills played an integral part in mapping the Human Genome Project. She then continued her education at the University of Pittsburgh where she received a Masters in Public Health Genetics as well as a PhD in Human Genetics. Dr. Lewis is the first African American woman to graduate with a Ph.D from the University of Pittsburgh Graduate School of Public Health in Human Genetics with a specialization in Statistical Genetics. Currently, Dr. Deyana Lewis is serving as a Post-Doctoral Fellow at the Computational and Statistical Genomics Research Institute. She has a passion for investigating the genetic mechanisms underlying diseases that disproportionately affect African Americans in comparison to other races/ethnicities . Debra then obtained a Ph.D. in Social Policy from the Heller School at Brandeis University in August 2009. Debra has over twenty-five years of legal experience and is an active member of the Massachusetts Bar Association and was the 2018 recipient of the Powley Elder Law Award. Today she is going to be talking with us about legal remedies for families coping with long term mental health issues. She is going to speak about alternatives to guardianship as well as other topics. This is part 2 of two podcasts focused on legal remedies in mental health. Debra will be telling us more about how to integrate into long term mental health planning the use of a durable power of attorney’s, health care proxies, psychiatric advanced directives and discretionary trusts with incentives. Three main talking points: 1. A brief overview of what guardianship is and who the people are who seek this legal intervention. We will also discuss some of the challenges and limitations of guardianship. 2. What alternatives to guardianship exist and in most instances are preferable? Alternatives such as DPOA’s (or durable powers of attorney), healthcare proxies and incentivized trusts. We will also discuss the importance of using a team approach when accessing any of these legal remedies. 3. PAD’s or psychiatric advanced directives. What they are, why everyone with a mental health issue needs one, and how they are written. Virgil and Debra also talk more about the DPAs and HCPs. These are critical tools, and aside from steps we one take to minimize “revocation” in a crisis, families should know who should serve in these roles, how to use of teams (or committees), family members and social workers etc.

Jonthan Engel is Professor of Health Policy and Management at the Marxe School of Public and International Affairs at Baruch College, CUNY. He has taught previously at Seton Hall University, Mailman School of Public Health at Columbia University and the School of Public Health at the University of Massachusetts. He has been a consultant to the White House's President's Advisory Committee on Human Radiation Experiments, the lead author on multiple HIV needs assessments for the City of Newark and director of research at the Local Area Board for Health Planning for Essex and Union counties in New Jersey. His published works include: Doctors and Reformers: Discussion and Debate of Health Policy 1025-1950; Poor People's Medicine: Medicaid and the US Charity Care Since 1965; The Epidemic: A History of AIDS; American Therapy: The Rise of Psychotherapy in the US; and, Fat Nation, forthcoming. He is currently writing a book on Cold War science and policy in the US. Professor Engel received his BA from Harvard, an MBA from the Yale School of Management and his PhD in the history of medicine from Yale.
