A las puertas de la celebración de los cincuenta años de vida del Hospital Nacional de Parapléjicos, Documentos RNE dedica este capítulo a profundizar en el conocimiento de este centro público especializado en lesión medular, inaugurado en 1974, y que se ha convertido en una referencia internacional en su tratamiento. Nuestros compañeros Juan Ballesteros y Samuel Alarcón han realizado una visita a este hospital, ubicado en Toledo, para hablar con profesionales y pacientes y trazar una completa fotografía sonora de su vida; tanto de su actividad, como de las historias personales de superación y esperanza. Recorremos con nuestros micrófonos las distintas áreas que lo integran: desde la UCI a actividades complementarias como las deportivas, pasando, entre otras, por la fisioterapia, la terapia ocupacional, el apoyo psicológico o la investigación. Y, además, recordamos cómo fueron los primeros pasos de este centro en el que, precisamente, se sueña con caminar. Lo hacemos con médicos que son una auténtica institución en la lesión medular, como los ya jubilados, Alberto de Pinto (creador del área de Rehabilitación Sexual y Reproductiva del hospital y parapléjico); o Jesús Mazaira, paraplejista que se formó junto al prestigioso doctor Ludwig Guttmann y que implantó, en Toledo, el primer marcapasos diafragmático de Europa. Abstract: Gap closure is a common morphogenetic process. In mammals, failure to close the embryonic hindbrain neuropore (HNP) gap causes fatal anencephaly. We observed that surface ectoderm cells surrounding the mouse HNP assemble high-tension actomyosin purse-strings at their leading edge and establish the initial contacts across the embryonic midline. The HNP gap closes asymmetrically, faster from its rostral than caudal extreme, while maintaining an elongated aspect ratio. Cell-based physical modelling identifies two closure mechanisms sufficient to describe tissue-level HNP closure dynamics; purse-string contraction and directional cell crawling. Combining both closure mechanisms hastens gap closure and produces a constant rate of gap shortening. Purse-string contraction reduces, whereas crawling increases gap aspect ratio, and their combination maintains it. Closure rate asymmetry can be explained by embryo tissue geometry, namely a narrower rostral gap apex. At the cellular level, our model predicts highly directional cell migration with a constant rate of cells leaving the HNP rim. These behaviours are reproducibly live-imaged in mouse embryos. I know I’ve said this a million times and I’ll say it again. This course is responsible for me being able to quit my day job. And it cost me $27 because I wanted the fast track. I have no idea how I found her, I’m sure it was from a Facebook group because I found the Uncaged Life and I was like, holy sh%t, this is the kind of business I want to run. Like, I had all the what and how and why and proof from Nathalie, Pat, and Chalene, but Becca Tracey showed me how to SAY it, and how to BE it, and how to OWN it, and how to be myself, and how to package that up into a product and sell it. I’m not an edgy person, you guys. I’m a plain vanilla basic midwest girl who wishes she was edgier, so at first I wanted to be like Becca, but through her Hey Nice Package course and HNP live workshops and just her blog and webinars, I just figured out how to be me, and that’s a huge relief, because if I had to pretend I was super cool and trendy all the time I would not be able to fake it. I already feel like I’m not in the right job, that would make it worse! AND IF I THOUGHT I KNEW IT ALL FROM TAKING IN AND TAKING ACTION ON EVERY LAST PIECE OF INFORMATION I COULD FROM THOSE GUYS, I’M INTRODUCED TO THE BEING BOSS PODCAST. OMG. This is the not-so-distant future, you guys. Talk about some real people who have taken massive, focused action toward success. Emily Thompson and Kathleen Shannon’s podcast is like hanging out every day with people who GET you – your dreams, your goals, your fears, but also who celebrate your successes. Because let’s face it… When I tell my best friend or my husband about how I finally landed on a name for my course, they smile and are happy for me but they just don’t GET it. It’s a huge dose of validation and it makes me feel like I’m on the right track. And they dig in, you guys, just like Pat Flynn does. It’s not fluff, it’s action. AND FOR CHRIS GUILLEBEAU AND HIS SIDE HUSTLE SCHOOL PODCAST AND SIDE HUSTLE BOOK. This one… this one is just mind-blowing. I heard Chris on Lewis Howe’s School of Greatness podcast, and his message about struggling with depression even with all of his business success really resonated with me, like, this guy is a real, vulnerable person, not just some “bro” entrepreneur out to make bank. I replied to a call for side hustle stories, and I didn’t even know it was for Chris or his podcast – and he featured my story!! A New York TimesBestselling Author told my story on his podcast. And then… I find out my story is part of his book. Are you kidding me right now? Like my name is printed in a book, and my side-hustle story is in a book. That other people read. And then they find me online and I help them build websites to support their side-hustles. It’s really hard to wrap my mind around that… So I’m grateful to Chris not only for the opportunity, the amazing people he’s connecting me with but also for helping me expand my mind around what’s POSSIBLE. ON THAT NOTE, I’M SO GRATEFUL FOR PODCASTS. You guys, if you’ve ever thought you don’t have TIME to learn what it takes to follow your dreams, then I’d like to introduce you to this thing called PODCASTS. It’s the only way to learn where you can be doing other things, like driving, working out, working your day job, making dinner, cleaning your house… Seriously, if you are not taking advantage of learning from some of the smartest people out there who are actually successful online, make it your mission in 2018 to start listening to podcasts. I’M ALSO GRATEFUL FOR HAVING A JOB THAT I HATED BECAUSE I FINALLY LEARNED THAT I’M IN CONTROL OF HOW I FEEL. That my job never did anything to me to make me hate it, it was my bad attitude and negativity that made it unbearable. If I hadn’t figured that out, there’s no way I’d be able to create a successful business of my own, I’d carry the same bad attitude with me. And once I changed my attitude, everything changed. In fact, I’m having a hard time leaving this place now that it’s time because all of my coworkers are amazing people who have gone out of their way to make it know how much they want me to stay. Wow. I’M GRATEFUL FOR HAVING A BOSS WHO IS A TRUE MENTOR. One who believes in me, who wants to see me succeed, who supported my side hustle from the moment I told her about it, and who knows that an employee who is fulfilled outside of work is a better employee at work. Thanks, Lori! There should be more CEOs in the world like Lori Chovanak, and if anyone can make that happen, she can. AND I’M GRATEFUL FOR THE COACHING INDUSTRY, A THING I DIDN’T EVEN KNOW EXISTED UNTIL A FEW YEARS AGO. I’M GRATEFUL THAT I HAVE A SUPER SUPPORTIVE, LOVING FAMILY WHO IS PROUD OF ME. They encourage me when I share that I’m afraid to quit my job to follow my dreams, who tell me that I HAVE to do it, that I will be fine and give me perspective by reminding me that I’m not the first person to do it and that both my mom and Grandpa have owned their own businesses, who read every single blog post (hi Mommy!!) and by telling me that I can come live with them if I need to or that they have a job I’d be perfect for if it all goes to shit. They make it so that I can’t “fail”, and I love the crap out of them for it. I’M GRATEFUL THAT I BELIEVE IN MYSELF ENOUGH TO TAKE ACTION. To try. To embarrass myself. To fail. Although a commonly recognized condition in adults, herniated nucleus pulposus (HNP) is often an unrecognized cause of back and leg pain in the pediatric/adolescent population. This often leads to a delay in diagnosis and/or inappropriate treatment. HNP is distinct from a "bulged disc", an MRI finding often cited as a cause of pediatric /adolescent back pain. With HNP, patients generally have some element of back pain, but it is the leg pain (radiculopathy) or sciatica that often leads to their presentation to a health care provider.It is not uncommon for children to present with "sciatic scoliosis", a spinal deformity that is a response to the patient's nerve root irritation. This condition can have a sudden presentation from a traumatic event of occur spontaneously with no known cause. In the presented thesis, matrix-assisted laser desorption/ionization (MALDI) imaging mass spectrometry was used for the proteomic analysis of gastric cancer tissue samples, with the aims of 1) identifying proteins that predict disease outcome of patients with intestinal-type gastric cancer after surgical resection, and 2) generating a proteomic classifier that determines HER2-status in order to aid in therapy decision with regard to trastuzumab (Herceptin) administration. In the first study, a seven-protein signature was found to be associated with an unfavorable overall survival independent of major clinical covariates after analyzing 63 intestinal-type primary resected gastric cancer samples by MALDI imaging. Of these seven proteins, three could be identified as CRIP1, HNP-1, and S100-A6, and validated immunohistochemically on tissue microarrays of an independent validation cohort (n=118). While HNP-1 and S100-A6 were found to further subdivide early (UICC-I) and late stage (UICC-II-III) patients into different prognostic groups, CRIP1, a protein previously unknown in gastric cancer, was confirmed as a novel and independent prognostic factor for all patients in the validation cohort. The protein pattern described here serves as a new independent indicator of patient survival complementing the previously known clinical parameters in terms of prognostic relevance. In the second study, we hypothesized that MALDI imaging mass spectrometry may be useful for generating a classifier that may determine HER2-status in gastric cancer. This assumption was based on previous results where HER2-status could be reliably predicted in breast cancer patients. Here, 59 gastric cryo tissue samples were analyzed by MALDI imaging and the obtained proteomic profiles were used to create HER2 prediction models using different classification algorithms. Astonishingly, the breast cancer proteomic classifier from the previous study was able to correctly predict HER2-status in gastric cancers with a sensitivity of 65% and a specificity of 92%. In order to create a universal classifier for HER2-status, breast and non-breast cancer samples were combined, which increased sensitivity to 78%; specificity was 88%. This study provides evidence that HER2-status can be identified on a proteomic level across different cancer types suggesting that HER2 overexpression may constitute a widely spread molecular event independent of the tumor entity.
