Chapter 1: Meet the Microbes (02:41) John opens the episode, introducing Dr Vanessa Schmidt and inviting her to share her background. Vanessa explains her role at the University of Liverpool, her PhD in antimicrobial resistance in staphylococci, and her leadership in infection control and antimicrobial stewardship. (03:38) John asks whether staph infections in pets are usually caught or part of their natural skin flora. Vanessa replies that most infections come from an animal's own commensal microbiota, which coexist harmlessly but can cause disease when the immune system or skin barrier is compromised. (05:07) Sue asks whether humans and animals share the same commensals. Vanessa explains that while many organisms are shared across species, each host also harbours unique flora. Cross-species transfer is possible but not common. (06:16) Sue asks whether different body sites have different staph species. Vanessa says this is well-mapped in humans, while in pets we know carriage is common in the nose, mouth, and perineum, but site-specific species need more study. (07:28) John asks about coagulase-positive vs coagulase-negative staph. Vanessa explains that coagulase-positive staph are generally more virulent, while coagulase-negative species can still be important, especially in immunocompromised hosts or in association with implants. (09:37) Sue asks how to interpret a coagulase-negative result on a lab report. Vanessa advises considering clinical context, immune status, culture growth level, and cytology to judge significance. (12:34) Sue emphasises the value of cytology. Vanessa agrees, explaining it's routine in dermatology and helps identify intracellular bacteria and neutrophilic inflammation. Chapter 2: Resistance Training (14:30) John transitions to methicillin resistance. Vanessa explains MRSP carries the mecA gene, conferring resistance to beta-lactam antibiotics. It spreads clonally and is more stable than resistance in Gram-negatives. (17:40) Sue asks whether antibiotic use can switch the resistance gene on or off. Vanessa says it's about selection pressure - resistant strains survive when antibiotics are overused. (19:03) Sue shares her “rucksack” analogy. Vanessa agrees, adding that over time resistant strains adapt, carrying resistance genes more efficiently. (20:15) John asks about zoonotic risk. Vanessa confirms bacteria like MRSP can pass between pets and owners, though actual infections are rare. Chapter 3: Less is More: Treating MRS the Smarter Way (22:33) Sue asks about managing MRSP pyoderma in practice. Vanessa outlines a tiered approach: treat the underlying disease, apply barrier nursing, and use topical therapy like chlorhexidine, aiming to avoid systemic antibiotics. (27:00) Sue raises chlorhexidine use and guideline updates. Vanessa recommends 2% or above concentrations of chlorhexidine and warns that dilution reduces effectiveness and may lead to treatment failure. (30:05) Sue mentions suspected resistance. Vanessa confirms resistance is reported and linked to bacterial efflux pumps. She uses hypochlorous acid or bleach as follow-up options in certain MRSP cases. (33:15) John asks what Vanessa avoids. She avoids systemic antibiotics in superficial MRSP unless absolutely needed, and tailors treatment to the underlying disease. Immunosuppressives are avoided if the infection arose due to immune compromise. (35:00) Sue asks about treating MRSP otitis. Vanessa explains that lower chlorhexidine concentrations are potentiated by TRIS-EDTA, so she still uses them as first-line ear cleaners. (36:40) John asks about long-term carriage. Vanessa explains MRSP can persist for months or even years. Decolonisation before high-risk surgery is common, but long-term clearance is difficult and evidence is limited. (39:42) Sue summarises the discussion: in MRSP, less is more - fewer antibiotics and more topicals. New guidelines offer hope for better resistance control. May begins and stock buy backs return; earnings season has been okay, on lowered expectations. Employment report is stronger than expected, but showing slippage. Lance Roberts explores why resistance levels are challenging both bulls and bears in today's volatile market. Over the past two weeks, the market has had a furious nine-day rally, the longest winning streak in 21 years. However, there are two takeaways from such a historic advance. First, it is “bullish” as investors return to the market. However, investors should also recognize that if the rally is the longest in 21 years, then previous such rallies failed. Source: reuters.com Trump may be his own worst enemy in his fight against the Fed Trump has said he wants lower interest rates, but he may be standing in his own way. His policies, though, are an obstacle the Fed could have to navigate as it adjusts monetary policy. Fed Chair Jerome Powell sent a clear message this week that central bankers were in no rush to lower rates, with Fed officials opting to pause rate cuts in the first policy meeting of the year. That seemed to defy pressure from Trump, who said in his address to the World Economic Forum last week that he would demand interest rates be lowered "immediately." He added that his policies — including plans to levy steep tariffs on imports from China, Canada, and Mexico — could help renew the US economy. But those plans could end up being the very obstacle the Fed has to navigate this year as it decides what to do with interest rates, and they're likely the reason the Fed will hit pause until the picture becomes clearer, Wall Street forecasters say. Economists have warned that Trump's plan to levy steep tariffs on US imports could stoke inflation, a notion that Trump has pushed back on. The president has said he would lower prices, and he implemented tariffs in his first term without a significant inflation increase. His tariff plan this time around, though, is far more wide-reaching,

Will Google searches soon become a thing of the past? Will apps become extinct? Decades ago, Tim Kitchens served in the Air Force as a Russian intel analyst, but after the military he trained to become a Senior Software Engineer and expert programmer. We sat down with him to get an in depth look at how the explosive growth of AI technology is poised to revolutionize the way we work and interact with the Internet.Kitchens begins by explaining the basics of AI technology; What is it? How does it work, etc. He later expands on how ChatGPT can be incorporated into our daily lives and how other AI driven tech is transforming online content and digital video. He reflects on the dark side of AI too, ie; the dangers it presents when used to promote greed, misinformation and other dark desires. But it's not all dark. He shares how the traditional Internet search will eventually be replaced by “Prompt” technology, allowing for AI to perform search, research and other mundane tasks within minutes. He also sheds light on how AI is responsible for light-speed advances in medicine; the COVID vaccine and hopefully someday for diseases like cancer. We also discussed how to “future proof” your career with “prompt skills”, and the surprising places to find training. The battle for the top position in the EPL constricted this weekend as Arsenal were able to defeat Liverpool at the Emirates Stadium. Victory for the Gunners saw their gap to the league leaders shrink to two points and with a Manchester City win on Monday, the Citizens also went within two points; though having played one less game. Did this Arsenal victory signal the impending Manchester City title charge or can either Liverpool or Arsenal dethrone the thrice consecutive champions? Wolves earned three points away to Chelsea that saw them eclipse the London outfit for tenth position in the league. With less refereeing controversies this year, could Wolves be sitting closer to the European places? While Tottenham failed to secure victory against Everton, the leading man for the Spurs now has 9 goals in 8 appearances. Check out this weeks review of match week 23. Today's meditation comes from The Book Of Ecclesiastes in The Old Testament and from the writings of Lao Tzu, with music by Outside The Sky.Passage: The race is not to the swift or the battle to the strong, nor does food come to the wise or wealth to the brilliant or favor to the learned; but time and chance happen to them all. —Ecclesiastes 9:11-18Perspective: Life is a series of natural and spontaneous changes. Don't resist them; that only creates sorrow. Let reality be reality. Let things flow naturally forward in whatever way they like. ―Lao TzuMusic: “Open To Receive” by Outside The SkyNarrator: Ryan Phipps
The term “dharma” has been used generically in the West to refer to any real teaching of contemplative spirituality. “Doo-doo” has to do with being stuck in our spiritual endeavors. A quote attributed to St. John of the Cross is, “In order to come upon that which you know not, you must go by a way in which you know not.” There are many things to benefit from in the domain of the known, but the spiritual path is about the domain of the unknown. The “now” that is spoken about by Realizers is outside of time, outside of the known. Peace comes and goes in the known, but there is a Peace that surpasses understanding. Enlightenment is a questionable term; it is not anything that we might imagine happens to an individual in the domain of the known. Anything we do to get there can take us away from it. The personality does not disappear when the condition of no-self is realized—it is just seen through. The known includes anything we've read, been conditioned into, or been told. It is imprisoning to embrace any belief or handed-down structure at the expense of finding our own way through the forest of the unknown. A realized person, awakened to no-self, is simply himself or herself, freed of all handed-down beliefs. Consciousness accepts by its nature and so trying to accept may only muddy the water. Recognizing our helplessness, that we are in doo-doo, can be a good sign. No practice can bring about total surrender, yet many sages have recommended them. We can hold any teaching, teacher, or practice in high regard but stand on our own two feet. The talk includes discussion about whether a Guru is needed in this day and age versus the direct path. Peter Cohen was the drummer for the Western Baul rock band, Liars, Gods, and Beggars from 1988 to 1994. He has followed the nondual path and rhythm of life in Alaska and Idaho as a nurse and a musician.
