When I first spoke with Dr. BJ Miller, I felt the weight of his experiences transform into something unexpectedly hopeful. BJ is a palliative care physician and he's also a storyteller of life's hardest truths, a triple amputee who turned personal tragedy into a mission to redefine how we approach suffering, dignity, and mortality. Years ago, I watched BJ Miller's TED Talk on What Really Matters at the End of Life, I was struck by his bold challenge to healthcare's design—how it was built around diseases rather than people. He spoke with a clarity that cut through the clinical detachment of modern medicine, emphasizing that suffering is not just something to be erased but an intrinsic part of being human. He invited us to see death and illness not as failures but as natural, even meaningful, parts of life. When my husband, Dan Rosenman, died suddenly of a heart attack while mountain biking 11 years ago, BJ's words became a lifeline. His reflection on the difference between necessary and unnecessary suffering helped me shift my perspective: rather than trying to “fix” my grief, I learned to live alongside it and open my eyes to see the beauty of grief. That's why I invited him here—to share how we might reshape our approach to life's most difficult realities with creativity, presence, and compassion. Known globally for that groundbreaking talk, he co-founded Mettle Health, a telehealth service providing support for patients and caregivers. His journey began with a life-altering accident at Princeton, where he lost three limbs—an event that reshaped his understanding of vulnerability and resilience. Through his work at UCSF, the Zen Hospice Project, and now Mettle Health, he has challenged medicine to prioritize humanity over cure rates, integrating art, spirituality, and raw honesty into care. David Cohn, MD, described one of his goals for patients of the James. “When a patient and a family say we didn't want to be diagnosed with cancer and we didn't want to be at the James Cancer Hospital, but after this experience there's nowhere else we'd rather be and that is the end result of empathetic care and compassion in the delivery of that care,” said Cohn, the interim chief executive officer and the medical director of the James. In this episode, Cohn and John Schaffner, MBA, the director of coaching at Ohio State's Fisher College of Business, described a new coaching program for James physicians that's part of the Physician Wellness Program. This coaching focuses on compassion and helps physicians better understand their careers and goals and utilize compassion when working with their colleagues and patients. “It's not that our physicians had problems, they're exceptional, it was rather, how can we support our people so they can flourish,” Cohn said, adding this type of leadership initiative was welcomed by his team. Schaffner, who leads the James coaching program, explained his definition of empathy. “There's cognitive empathy, that's noticing someone else's emotions; there's empathetic concern, which is an emotional reaction to someone else; and this empathy plus action equals compassion.” There are currently 15 certified coaches who work with the 240 attending physicians of the James. “I love working with physicians,” Schattner said. “They're smart and ambitious, understand the idea of flourishing and they do the work.” Some of these physicians, he added, are new to leadership positions and coaching can help them communicate better and build comradery. The coaching program is new and funded through a philanthropic donation to the James. Cohn said the program is designed to help each physician be the best version of themselves they can be with the overreaching goal of delivering the best-possible care to James patients. “Everything we do is driven by the delivery of care to our patients and this is another process to help us get there,” he said.

Research suggests that LGBTQ adults have higher rates of substance use and mental health issues, compared to their heterosexual cisgender peers. These health disparities are often related to minority stress—the chronic psychological and emotional strain that people experience due to being part of a marginalized social group. For example, some LGBTQ folks may use alcohol and drugs to cope with internalized homophobia or to self-medicate for anxiety and depression. On the other hand, some people use drugs in the context of social settings and connecting with others in their community. Since not all substance use is unhealthy, how do providers know how to identify when substance use is problematic? And what can physicians do to best support their patients who may be at greater risk for harm or even overdose? On this episode, Antonio Urbina, MD, Medical Director for CEI's HIV Primary Care and Prevention Center of Excellence, speaks with Eric Kutscher, MD, MSc. Dr. Eric Kutscher is an Assistant Professor of Medicine at the Icahn School of Medicine, where he practices as a primary care and addiction medicine physician at Mount Sinai's Internal Medicine Associates and REACH Program. He currently serves as the Medical Director of the Mount Sinai Hospital Opioid Overdose Prevention Program and Primary Care Lead for the Mount Sinai Center for Transgender Medicine and Surgery. His clinical work and research focus on harm reduction techniques for patients with substance use disorders, particularly within the LGBTQ+ community. Drs. Urbina and Kutscher talk about how providers can identify when substance use is a problem and how to help patients manage substance use disorder, including treatment options and harm reduction strategies. Throughout my 11 year nursing career, I focused on oncology and hospice and education; but, lost faith in the medical system knowing that pharmaceutical medicine was only a quarter of the puzzle. I dove into personal development through the work of transformation in 2019, nervous system regulation teaching, death doula work, and alternative cancer treatment all as my gifts and life's work.
